Binding of Cu(II), Tb(III) and Fe(III) to chicken ovotransferrin. A kinetic study.

The kinetics of binding of Cu(II), Tb(III) and Fe(III) to ovotransferrin have been investigated using the stopped-flow technique. Rate constants for the second-order reaction, k+, were determined by monitoring the absorbance change upon formation of the metal-transferrin complex in time range of milliseconds to seconds. The N and C sites appeared to bind a particular metal ion with the same rate; thus, average formation rate constants k+ (average) were 2.4 x 10(4) M-1 s-1 and 8.3 x 10(4) M-1 s-1 for Cu(II) and Tb(III) respectively. Site preference (N site for Cu(II) and C site for Tb(III] is then mainly due to the difference in dissociation rate constant for the metals. Fe(III) binding from Fe-nitrilotriacetate complex to apo-ovotransferrin was found to be more rapid, giving an average formation rate constant k+ (average) of 5 x 10(5) M-1 s-1, which was followed by a slow increase in absorbance at 465 nm. This slow process has an apparent rate constant in the range 3 s-1 to 0.5 s-1, depending upon the degree of Fe(III) saturation. The variation in the rate of the second phase is thought to reflect the difference in the rate of a conformational change for monoferric and diferric ovotransferrins. Monoferric ovotransferrin changes its conformation more rapidly (3.4 s-1) than diferric ovotransferrin (0.52 s-1). A further absorbance decrease was observed over a period of several minutes; this could be assigned to release of NTA from the complex, as suggested by Honda et al. (1980).

Positive indium-111 leukocyte imaging in post myocardial infarction syndrome.

The diagnosis of post myocardial infarction syndrome (PMIS) is sometimes difficult because of the absence of a specific test. We report a 68-year-old man with PMIS who had a persistent accumulation of indium-111 oxine labeled leukocytes in the infarcted myocardium for 1 month. The uptake of leukocytes preceded the appearance of the main symptoms and disappeared with the clinical improvement after the therapy with steroids. Leukocyte imaging has a potential as a useful tool for early diagnosis, evaluation of therapy and assessing the mechanism of PMIS.

Possible involvement of a novel protease in neurite outgrowth of PC12 cells.

A tripeptide aldehyde protease inhibitor (Ac-Leu-Leu-Nle-al, ALLNal), among many other protease inhibitors, initiates neurite outgrowth in PC12h cells. The neuritogenesis induced by this inhibitor is different in some aspects from that induced by NGF; one or two long neurites are initiated from the cell body and differentiation is transient. The results provide evidence that the protease inhibitor and nerve growth factor elicit neurite initiation by different mechanisms and suggest the existence of a novel protease which modulates neurite initiation in PC12 cells. To identify the target protease, Leu-Leu-Leu-al was immobilized and used as a ligand for affinity chromatography of the protease. A protein with an apparent molecular weight of 33 kDa was isolated specifically from the cytoplasmic fraction of PC12 cells using the affinity column. The same protein was identified in the brains of 1-day postnatal rats, but the amount was much less in the brains of adult rats. Thus, we suggest that the 33-kDa protein regulates neurite initiation in nervous systems, possibly as a protease which degrades membrane proteins or the cytoskeletal framework.

Marked cardiac involvement in limb-girdle muscular dystrophy.

A 42-year-old man with limb-girdle muscular dystrophy who showed prominent cardiac involvement is described. He was treated for and diagnosed with dilated cardiomyopathy before admission. Initially, no overt feature of muscular dystrophy was evidenced, but serum enzymes were elevated. During the 2-year follow-up period, distinct features of muscular dystrophy appeared, and muscle biopsy confirmed the diagnosis. He was regarded as experiencing a sporadic occurrence. Because some patients diagnosed with and treated for dilated cardiomyopathy may have similar results, this form of muscular dystrophy should be recognized as occasionally associated with cardiac muscle disease.

Plasma and serum G4 isoenzyme of acetylcholinesterase in patients with Alzheimer-type dementia and vascular dementia.

We developed a new assay for plasma or serum G4 acetylcholinesterase (AChE) activity. There was no difference in G4 AChE activity between serum and plasma. Plasma AChE activity in control subjects (8-92 years) was independent of age. Serum G4 AChE activity was significantly increased in patients with vascular dementia (P less than 0.01), compared with age-matched control, and significantly decreased in those with probable Alzheimer-type dementia (P less than 0.02). The difference between VD and ATD was significant (P less than 0.001), and thus the measurement of plasma or serum G4 AChE activity would be helpful in the differential diagnosis of dementia.

Studies on sustained-release suppositories. III. Rectal absorption of morphine in rabbits and prolongation of its absorption by alginic acid addition.

Rectal absorption of morphine from various kinds of suppository bases was investigated. The extent of bioavailability of morphine by rectal administration varied with the bases used (30.5-97.5%), but every value was higher than that in the case of oral administration (13.5%). Witepsol bases were preferable to macrogol base for the rectal absorption of morphine. In particular, Witepsol S-55 or W-35 gave a higher plasma peak level than H-15 or E-75, whereas the difference in the mean residence times obtained from these bases could not be regarded as significant. Sustained-release suppositories of morphine could be prepared simply by mixing alginic acid (Alg) with morphine in a suppository base. Further, prolonged rectal absorption could be obtained by using these sustained-release suppositories, and the absorption rate was controlled by the amount of Alg added. It seems likely that the sustained release was due to the binding of morphine to Alg from the results of partition coefficient and binding ratio measurements in aqueous solution. The rapid initial absorption and the subsequent prolonged absorption of morphine simultaneously obtained from the morphine-Alg suppository may be useful in the clinical context.

Effects of intermittent coronary sinus occlusion on experimental myocardial infarction and reperfusion hemorrhage.

The effects of intermittent coronary sinus occlusion (ICSO) on the size of myocardial infarction and reperfusion hemorrhage was evaluated. In Protocol 1, 8 dogs with ICSO and 8 controls underwent 4h of occlusion of the left anterior descending coronary artery. The same number of dogs underwent 4h of occlusion followed by 1h reperfusion in Protocal 2. The ICSO was started 1h after the ligation and continued through the occlusion period. There was no difference between the ICSO and the control group in hemodynamics and regional myocardial blood flow using hydrogen clearance method. However, ICSO did accelerate the rate of decline in intramyocardial CO2 tension. The half life of CO2 tension was 256 +/- 106 min in the control group but 139 +/- 34 min in the ICSO group (p less than 0.01). Lactate extraction rate showed the improving tendency during ICSO period. The ICSO resulted in a 50% and 80% reduction on an average in the size of infarct and reperfusion hemorrhage, respectively. We conclude that ICSO has prospective effects on myocardial ischemia with promise for clinical application.

Effects of PRL and FSH on LH binding and number of Leydig cells in hypophysectomized mice.

The effects of PRL and FSH on testicular LH receptors were studied in hypophysectomized (hypox) mice. From the eleventh day after hypophysectomy, they were given 100 micrograms ovine (o) PRL and/or 2 micrograms oFSH in two injections per day for 10 days. Hypophysectomy reduced the weight of testis, epididymides and seminal vesicles, and the LH binding to the testis. Treatment with oPRL and/or oFSH in hypox mice resulted in an increase in the weight of testis and epididymides, and the LH binding per testis. There was no difference between the testicular LH binding in oFSH- and oPRL-treated mice. Histological examination showed that oPRL and/or oFSH treatment in hypox mice restored normal spermatogenesis. Administration of oFSH to hypox mice led to an increase in the number of typical Leydig cells, whereas oPRL was not effective. These results suggest that either PRL or FSH stimulates the LH binding to the testis, but that the action of PRL and FSH on the increase in testicular LH binding is different.

Hypothalamic luteinizing hormone-releasing hormone (LHRH) and LH secretion in aging female rats.

Age-related changes in hypothalamic luteinizing hormone-releasing hormone (LHRH) and luteinizing hormone (LH) secretion were studied in young (6 months), middle-aged (12 months) and old (18 months) female rats. The LHRH levels in the mid-hypothalamic area were higher in intact middle-aged and old females than in young ones. Additionally, there was no age difference in the hypothalamic LHRH levels in male rats. In order to clarify the significance of this age-related increase in female rats, we examined the effects of progesterone treatment in estrogen-primed ovariectomized young and old rats on the LHRH levels in the median eminence (ME) and on plasma LH levels. We found phasic changes in ME-LHRH and plasma LH levels in estrogen-primed rats following progesterone treatment in rats of both ages, but the progesterone-induced change in ME-LHRH levels tended to be delayed in old rats compared with young females. This delay may correspond to the delayed onset, slow and low magnitude of plasma LH increase in old females. The ME-LHRH levels were generally higher in old rats than in young rats. Nevertheless, we found that the increase in plasma LH in response to progesterone treatment in estrogen-primed ovariectomized females was smaller in old rats than young rats. These results suggest that the LHRH secretory mechanism changes with age in female rats. Such alterations may result in the accumulation of LHRH in the mid-hypothalamic area and an increase in ME-LHRH.

Age-related changes in growth hormone and prolactin messenger RNA levels in the rat.

Growth hormone (GH) secretion declines with age, while prolactin (PRL) secretion increases in rats. The age-related changes in the pituitary GH and PRL mRNA levels in the Wistar Tw rat were studied at 6, 12 and 18 months of age by cytoplasmic dot hybridization. Hormone and mRNA concentrations were expressed as values per micrograms GH-cell or PRL-cell DNA. Concurrent decrease in GH concentrations and GH mRNA concentrations with age was observed in female rats, but not in male rats. GH mRNA concentrations (per microgram GH-cell DNA) in male rats at 12 and 18 months of age were 88 and 66% of those at 6 months of age, but there was no difference. GH mRNA concentrations in female rats at 12 and 18 months of age were 51 and 53% of those at 6 months of age. A concurrent decrease in PRL concentrations and PRL mRNA concentrations was observed in both male and female rats. PRL mRNA concentrations (per microgram PRL-cell DNA) in male rats at 12 and 18 months of age were 37 and 18% of those at 6 months of age. PRL mRNA concentrations in female rats at 12 and 18 months of age were 51 and 31% of those at 6 months of age. Pituitaries in 12- and 18-month-old females contained more PRL and PRL mRNA than those at 6 months of age. These increases in PRL and PRL mRNA contents may result from the increase in the number of PRL cells, although each PRL cell had less PRL and PRL mRNA. These results suggest that the age-related changes in pituitary GH and PRL levels occurred at the transcription level of GH and PRL syntheses, and that the age-related changes in GH-cell and PRL-cell populations are responsible for the changes in GH and PRL secretions.

Immunohistochemical evidence of the extracellular localization of calcium-activated neutral protease (CANP) in rabbit skeletal muscle, lung and aorta.

The subcellular localization of calcium-activated neutral protease requiring a millimolar calcium concentration (m-CANP) was examined by light and electron microscopy in various tissues of the rabbit, using an immunoperoxidase method with a monoclonal antibody (1C6D1). In skeletal muscles, specific staining for m-CANP was recognized on collagen fibrils (ca 40 nm in diameter) with a periodic banding pattern. In the lung, dense reaction products were precipitated on elastic fibers under the bronchial epithelia. In the aorta, tunica intima and adventitia were intensely stained. Dense reaction products were observed on collagen fibrils and elastic fibers. Basal laminae, on the other hand, were not stained by anti m-CANP antibody in these tissues. These findings suggest that m-CANP may be involved in the regulation of the structure and function of the extracellular matrix.

[The management of drooling in patients with cerebral palsy--submandibular duct relocation].

Drooling is a significant problem demonstrated by individuals of all ages with a variety of etiologies such as neurological disorders, cerebral palsy, mental retardation and others. Persistent drooling not only creates troublesome hygienic problems for patients, teachers, nurses, and playmates because of the constant soiling of clothes, toys, and work materials, but also causes an odor from their clothing and bibs. The older patient with normal cognition is disturbed by the drooling and may become depressed and reclusive. Successful management of drooling alleviates these problems, improves appearance and self-esteem and significantly reduces the time involved in the care of the sufferers. The author, who had studied at the University of Toronto for 2 years, acquired the technique of submandibular duct relocation under the guidance of Dr. Crysdale. We carried out the surgical procedure of submandibular duct relocation for drooling on six patients who had cerebral palsy. The surgical procedure resulted in a dramatic decrease in drooling and odor levels. All of them improved in appearance, and the time involved in the care of the suffers was significantly reduced. The complication of ranula, however, appeared in two cases, which suggested a much higher frequency than that in the report of Crysdale. It seems that a detailed explanation to parents and teachers about the advantages and disadvantages of submandibular duct relocation is important.

[Oral findings in severely handicapped patients participating in the periodic dental check-up system for five years--dental caries, gingival recessions and hyperplasias, periodontal diseases].

In March 1987, we investigated the quality of oral hygiene, and the incidence of dental caries and periodontal diseases in 140 severely handicapped patients who had been hospitalized in two national sanatorium. All of the patients had participated under the Matsumoto Dental College Hospital Periodic Dental Check-up System for five years. The results were as follows: 1. The teeth of all the subjects had been brushed by the sanatorium nursing staff twice per day. The prevailing brushing technique was the horizontal method. 2. 37.5% of the subjects showed complete adaptability to the tooth brushing by the nursing staff. However, 2.1% showed no adaptability whatsoever. 3. The mean value of the OHI-S was 1.53. The labial surfaces of the maxillary anterior teeth showed the lowest OHI-S value, while the mandibular left posterior teeth showed the highest. 4. The DMF-T was estimated to be 12.51 (DMFT ratio = 48.2%). The average D-T was 2.07 +/- 4.03, and the average F-T was 8.43 +/- 7.22. 5. 19.3% of the patients were diagnosed with gingival hyperplasia, and 10.7% had local gingival recessions. 1) Of the patients who had taken Phenytoin daily, 35.5% were diagnosed with hyperplasia. 2) Most local gingival recessions were found on the labial gingiva of the mandibular anterior teeth. A relationship was determined to exist the local recession and the horizontal brushing method. 6. 89.2% of the patients suffered from periodontal diseases.(ABSTRACT TRUNCATED AT 250 WORDS)

[Atrial septal defect with idiopathic thrombocytopenic purpura: a case of open-heart surgery].

A 40-year-old woman of atrial septal defect associated with idiopathic thrombocytopenic purpura (ITP) was successfully operated upon under cardio-pulmonary bypass. Steroids had been given preoperatively, resulting in an increase of the platelet count from 0.7 X 10(4)/mm3 to 20.6 X 10(4)/mm3. Steroids could be withdrawn before the operation. No bleeding tendency was encountered during and after surgery. This is the 6th case of open-heart surgery associated with ITP in the literature.

[The optional usage of blood cardioplegia through eleven-year experience of open-heart surgery].

We propose two clinical studies in this paper for the optimal usage of cold blood cardioplegia (CBC) to evaluate 1) the effects of increasing the volume of CBC, and 2) the optimal use of verapamil, on postischemic myocardial recovery. First, patients with valvular heart surgery were divided into three groups according to the initial dose of CBC: Group I, 5 ml/kg in 368 patients; Group II, 10 ml/kg in 41; and Group III, 12 ml/kg in 86. In all groups CBC was infused every 25 to 30 minutes. Although the preoperative conditions of Group III patients tended to be more serious compared to Groups I and II, there was no difference in cardiac-related hospital mortality among the three groups. Second, 45 patients who had undergone valvular or coronary heart surgery were divided into three groups according to the usage of verapamil: Group A, infusion of verapamil of 1 mg/L into CBC; Group B, infusion of verapamil of 0.2 mg/kg into a perfusate; and Group C, the same method as Group B with additional administration of verapamil of 0.1 mg/kg into a perfusate at 60 minutes after the commencement of cardiopulmonary bypass and a continuous drop of 0.5 micrograms/kg/min for the succeeding 24 hours. Each group consisted of nine patients who had valvular surgery and six patients who had aorto-coronary bypass surgery. There was no difference in the severity of the patients' conditions between the groups. The optimal concentration during and after the operation and good clinical results were obtained in Group C. We conclude that the initial optimal dose of CBC is 12 ml/kg, and that the combination of an infusion of verapamil into a perfusate and a succeeding continuous drop is the appropriate method.

Three-dimensional analysis of averaged electroretinograms for the evaluation of rhegmatogenous retinal detachment and related pathological conditions.

Retinal functional imaging in patients with rhegmatogenous retinal detachment and related pathological conditions was undertaken in 62 eyes of 44 cases by means of three-dimensional analysis (X-Y plane and time) of electroretinogram (ERG) topography. The analysis revealed that the area of maximal amplitude in both dominantly photopic and scotopic conditions deviated to the skin area closest to the location of the retinal detachment (hereafter referred to as paradoxical localization). In temporal retinal detachment, for example, the location of the maximal amplitude of the a- and b-waves deviated toward the temporal side on surface topography. Flicker ERG with a stimulus frequency of 30 Hz was especially successful for disclosing the existence and the location of posterior involvement of retinal detachment within the area surrounding the temporal vascular arcades. The mechanisms of paradoxical localization were considered to be changes in the axis of the equivalent dipole in detachment eyes. These abnormalities disappeared after the retina was reattached. Although the buckling procedure and argon laser retinopexy had little effect on the topographical distribution of the a- and b-waves, it was markedly distorted by cryoretinopexy. No remarkable changes in the topographical distribution of the waves were detected in pathological conditions predisposing to rhegmatogenous retinal detachment. This new method for functional imaging of the retina promises to be valuable for objective clinical evaluation of retinal detachment.
