Initial phase of infection with Tyzzer's organism in cultured mouse hepatocytes.

The initial phase of infection with Tyzzer's organisms in cultured mouse hepatocytes was observed using indirect immunofluorescence (IF) and a plaque assay. The organisms adhered poorly to aldehyde- or acetone-fixed cells, but once adhered to host cells, whether methanol-fixed or unfixed, they were not removed by methanol or acetone. By the IF as well as the plaque assay, both of which discriminated intra- and extracellularly located organisms, the number of cell-associated organisms increased linearly up to 3 h post-inoculation. The number of intra-cellular organisms increased rapidly in the first 1 h, followed by linear increase at a much lower rate. Similar results were obtained by the plaque assay.

Somatic mosaicism for DNA repair capacity in fibroblasts derived from a group A xeroderma pigmentosum patient.

A female Japanese xeroderma pigmentosum (XP) patient with severe skin lesions and various neurologic abnormalities was assigned to complementation group A by conventional cell fusion studies. Ultraviolet (UV)-irradiated skin fibroblasts showed a biphasic survival curve, as measured by colony-forming ability. The surviving fraction decreased rapidly up to 2 J/m2 of UV, with a steep slope of D(O) (mean lethal dose) = 0.95 J/m2. At much higher doses it decreased more slowly, with D(O) = 3.5 J/m2. To èlucidate the cause of this unique survival response, we isolated a large number of independent clones from single colonies and measured their responses to UV. Of 81 clones analyzed, ten showed a marked resistance to killing by UV, which was only slightly more sensitive than normal cells, and these clones had a rate of unscheduled DNA synthesis (UDS) that was about 45% of normal cells. By contrast, the remaining 71 clones were extremely sensitive to UV, typical of XP group A strains, and had a UDS level 1%-3% of normals. Analysis of restriction fragment length polymorphism using seven polymorphic DNA probes indicated that the UV-resistant clones were derived from the same individual as the UV-sensitive clones. These results clearly demonstrate that this patient's fibroblast cells consist of two types with differing responses to UV, and provide direct evidence of somatic mosaicism for DNA repair capacity in an XP patient.

The faecal flora of the giant panda (Ailuropoda melanoleuca).

The faecal floras of two adult (male and female) and one infant (male) giant panda kept at the Ueno Zoo, Tokyo, Japan were examined and shown to be quite different from those of other animals. The predominant bacteria in the adults were Streptococcus (including Enterococcus) and Enterobacteriaceae, while obligate anaerobes had minor populations. Fastidious anaerobes were not detected. The predominant bacteria in the suckling infant were Lactobacillus and Streptococcus, followed by Bifidobacterium. After the infant began to eat bamboo leaves the number of Lactobacillus decreased and Bifidobacterium became undetectable, whereas Enterobacteriaceae became one of the most predominant flora. The most dominant streptococcus isolated from the female panda was identified as Streptococcus bovis, but those from the male adult and the weaned infant were not identified as any known species.

Development of Babesia ovata in the midgut of the tick, Haemaphysalis longicornis.

Studies were made on the development of Babesia ovata in the midgut of the nymphal tick vector, Haemaphysalis longicornis. In 12 hr post-repletion, merozoites were observed outside of erythrocytes infected with B. ovata in the contents of the midgut of the tick. After that, these merozoites were transformed into ring-forms which were comparatively large ring 2-3 microns in diameter. Within 48-72 hr post-repletion, ring-form protozoa developed into spherical form 4-5 microns in diameter. Within 3-4 days post-repletion, fission-forms were transformed into fission-bodies 2-3 microns in diameter. Within 4-6 days post-repletion, fission-bodies developed into bizarre-forms 6-7 microns in diameter. At this time, elongated form protozoa which were considered as microgametes, 6-8 microns in length, are also seen. Within 6-8 days post-repletion, round-formed protozoa which were considered as zygotes in 9-10 microns in diameter were observed in the gut. About 10 days after repletion, those round-formed protozoa were transformed into vermicule-formed and round-formed protozoa, 13-15 microns in length, appeared again in the gut epithelial cells.

An eight year experience with gradually longer interval postremission therapy for adults with acute leukemia.

Between January 1980 and March 1983, a study was conducted into the effects of postremission therapy on 20 patients with acute leukemia who had achieved complete remission through induction therapy. Postremission therapy consisted of cyclic administration of six combination therapies given at gradually longer intervals. Postremission therapy used DCMP (D, daunorubicin; C, cytosine arabinoside; M, 6-mercaptopurine; P, prednisolone), DCyMP (Cy, cyclocytidine), DCVP (V, vincristine), BHAC-DMP (BHAC, behenoyl-ara-c), BHAC-AMP (A, aclarubicin) and ACM-MP (ACM, aclacinomycin). Six combinations were given sequentially starting at one month interval, and then at 2, 3, 4, 5 and eventually 6 month intervals until 5 year survival was reached. The median remission duration was 38 months for acute myelogenous leukemia (AML), and 17 months for acute lymphoblastic leukemia (ALL). The median survival was 66 months for AML, and 33 months for ALL. The survival rate at 5 years was 60% for AML., 40% for ALL, and 50% in all 20 patients. Methotrexate and prednisolone were administered intrathecally for prophylaxis of CNS leukemia on Day 4 of each stage of postremission therapy. There was no CNS leukemia. This postremission therapy was shown to be effective in improving the prognosis of adults with acute leukemia.

[The effect of food consistency on conditioned avoidance response in mice and rats].

The purpose of this study was to examine the effects of chewing a hard and soft diet on acquisition of conditioned avoidance response in mice and rats. 76 C57BL mice and 38 Wistar rats were divided into 2 groups respectively 20 days after birth. One group was fed a solid diet and another was fed a powder diet containing the same components as the solid one (Nihon Crea Company, CE-2). Using the avoidance conditioning system of Gunma University, conditioned avoidance experiments started at the age of 8 weeks and the rate of avoidance (number of avoidance/number of trials) was measured. The mean percentage of avoidance rates in the solid diet group were significantly higher than those of powder diet group at level of 1% in mice and 5% in rats. These results suggest that mastication of a hard diet in younger animal may facilitate their brain function more than that of a soft diet.

[A case report of retroperitoneal leiomyoma and review of the Japanese literature].

A 76-year-old woman presented with a history of pain on urination and urinary frequency. The sonogram incidentally demonstrated a mass in the area of the upper right kidney, and CT scan revealed a retroperitoneal tumor. Operation was carried out, and the tumor measured 4 X 4 X 5.5 cm (35 g). The pathology report was consistent with leiomyoma. The patient had undergone surgical excision of a right retroperitoneal tumor before 27 years, and it is suspected to have been an ectopic retroperitoneal tumor judging from operative findings. Retroperitoneal leiomyoma is rare and a review of the Japanese literature is presented.

[Conservative surgery of renal cell carcinoma associated with renal (artery) aneurysm in solitary kidney--a case report].

A rare case of renal cell carcinoma associated with renal artery aneurysm in solitary kidney is reported. The patient is a 63-year-old woman with asymptomatic macrohematuria as CC, who previously received left nephrectomy (for which underlying disease is unknown). Excretory pyelography, abdominal CT and right renoarteriography revealed a 6 X 6 cm-tumor at the center of the right kidney, comprising the middle calices and a 16 X 18 mm-saccular aneurysm at the first bifurcation of the renal artery. No remote metastasis was revealed. Aneurysmectomy and partial nephrectomy (three-quarter of right middle and lower calices) were simultaneously performed in situ. The renal cell carcinoma was pT2b, of clear cell subtype and grade 1, and no calcification of the aneurysmal wall was observed. Anuria lasted for 21 postoperative days, but thereafter diuresis appeared. Renal function after 3 post-operative months remains kept as follows: BUN, 26 +/- 7 mg/dl; serum creatinine, 26 +/- 0.6 mg/dl; creatinine clearance, 20.1 ml/min. Now, after 18 postoperative months, the patient is already returning to work, with neither relapse nor need of dialysis.

[A case report of Cushing's syndrome due to adrenocortical nodular dysplasia, with remission period of 6 years after unilateral adrenalectomy].

A 29 y.o. female had been associated with Cushing's syndrome, gradually, from 1977. She was advised to get further examination at our hospital on September 1980. A pre-operative diagnosis of Cushing's syndrome due to bilateral adrenocortical tumors was made and right adrenalectomy was performed on December 22, 1980, as the 1st of two stage operation. The right adrenal, 1.8 cm in diameter, had a pigmented nodule and multiple pigmented micronodules, which were diagnosed pathologically as primary adrenocortical nodular dysplasia. It was not necessary for this patient to maintain steroid therapy, but after 3 months she complained of the withdrawal syndrome and was administered dexamethasone. Since July, 1981, she had no symptom without adrenocortical steroid administration. In November 1986, she became cushingoid and on June 1, 1987, we performed left adrenalectomy on her. The appearance of the left adrenal was similar to the right. We have found no reports of the cases of adrenocortical nodular dysplasia such as delayed appearance of withdrawal syndrome after unilateral adrenalectomy. In this case, plasma ACTH had relatively poor correlation with cortisol. Her plasma ACTH was not always suppressed and sometimes within normal range. Her adrenals had not only autonomy but also pituitary dependency. It is concluded that in this case there may have been a pituitary-adrenal dual control.

[Acute myelomonocytic leukemia with inv (16) (p13 q22) disappeared abnormal karyotype during complete remission].

A 21-year-old man was admitted to our hospital because of anorexia and general malaise in July, 1988. On admission, the white blood cell count of 18,600/microliters with 72% leukemic cells. The bone marrow aspirate showed 76.8% immature monocytes, 10% mature and immature eosinophils. Leukemic cells were 66.6% myeloperoxidase positive cells, and 20.6% naphthylbutyrate esterase positive cells. The lysozyme activity in urine was high. Cytogenetic analysis revealed the presence of 46 XY inv (16) (p13 q22). Under the diagnosis of acute myelomonocytic leukemia with eosinophilia (M4Eo) associated with inv (16) (p13 q22), one course of DCMP induction therapy was performed. After complete remission, the bone marrow aspirate showed disappearance of inv (16) (p13 q22), and associated with decreased residual leukemic cells.

Development of Babesia ovata in the haemolymph of the tick, Haemaphysalis longicornis.

The haemolymph of adult tick Haemaphysalis longicornis (H. longicornis) infected with Babesia ovata (B. ovata) was examined at regular intervals. As a result kinetes could be detected in the haemolymph 15 days after engorgement. The kinetes detected could be classified into three types on the basis of their morphological characteristics. They increased gradually in number, reaching a maximum 23 days later. Then they tended to decrease in number, disappearing almost completely from the haemolymph 38 days after the engorgement of ticks.

[A renal biopsy study in pre-eclampsia: clinical-pathological correlations in 20 cases].

Twenty cases of pre-eclamptic toxemia were examined clinicopathologically. Twelve primipara and 6 multipara underwent renal biopsy at 1-24 (3.5 +/- 5.4, mean +/- SD) weeks after delivery. Specimens were examined by light and electron microscopy and immunofluorescence. Their mean blood pressure at delivery was 177.0 +/- 19.4/116.3 +/- 10.2 mmHg, proteinuria was 9.5 +/- 8.4 g/day. Follow-up period was 23.0 +/- 19.3 months after delivery. The severity of double contour of the glomerular basement membrane and mesangial interposition were correlated with the amount of proteinuria at biopsy (r = 0.40, p less than 0.05, r = 0.51, p less than 0.05, respectively). Proteinuria disappeared in all cases after delivery. The length of the hypertensive period after delivery correlated with the severity of glomerular lesions (r = 0.63, p less than 0.05), but did not correlated with the severity of vascular lesions. All patients became normotensive within three months after delivery. No patients showed microhematuria during pregnancy or after delivery.

[An intensification therapy of adults acute leukemia].

Between January 1980 and March 1983, a study was conducted on the effects of intensification therapy in 20 adult acute leukemia patients who had achieved complete remission with induction therapy. Intensification therapy consisted of cyclic administration of six combination therapies given at gradually longer intervals, using daunorubicin, cytosine arabinoside, 6-mercaptopurine and prednisolone (DCMP), cyclocytidine (DCyMP), vincristine (DCVP), behenoyl-ara-c (BHAC-DMP), aclacinomycin (BHAC-AMP) and (ACM-MP). Six combinations were given sequentially at one-month intervals, at 2-, 3-, 4-, 5- and eventually 6-month intervals, until 5-year survival. The median remission duration was 38 months for AML, and 17 months for ALL. The median survival was 66 months for AML, and 37 months for ALL. The five year survival rate was 50%. Nine of the 20 patients are still alive. Methotrexate and prednisolone were administered intrathecally for prophylaxis of CNS leukemia on Day 4 for each intensification therapy. There was no CNS leukemia. This intensification protocol was shown to be effective in improving the prognosis of adults acute leukemia.

Influence of gestation on renal function in gravida with IgA nephropathy.

Nineteen pregnancies in 17 women with IgA nephropathy (IgAGN) were studied in terms of the influence of gestation on the natural course of renal function in IgAGN. We performed serial examinations of the serum creatinine (S-Cr) levels before and during pregnancy and after delivery. Group I comprised 11 pregnancies in 10 gravida who revealed proteinuria (1.1 +/- 0.7 g/day; mean +/- SD) and microhematuria before pregnancy. The S-Cr before pregnancy averaged 0.86 +/- 0.13 mg/dl. Group II comprised 8 pregnancies in 7 gravida who showed isolated microhematuria. The S-Cr before pregnancy averaged 0.81 +/- 0.06 mg/dl. In group I, S-Cr did not decrease during pregnancy and was elevated at delivery and at 1-4 weeks after delivery (1.03 +/- 0.29 mg/dl, p less than 0.05) in comparison with the level before pregnancy. In group II, S-Cr decreased significantly during pregnancy and was not elevated at delivery or after delivery. The above results suggest that gestation had a slight and transient adverse effect on renal function in IgAGN with proteinuria.
