Dimorphism-associated changes in intracellular pH of Candida albicans.

Intracellular pH (pHi) was monitored during pH-regulated dimorphism of Candida albicans using two different methods: (1) by steady-state distribution of propionic acid and (2) by use of polyene antibiotic, nystatin. There was no significant change in pHi during the first 120 min in either bud- or germ tube-forming populations. However, there was a rapid increase around 135 min which also coincided with the time of evagination. The magnitude of increase in pHi was different in the two populations; being 0.44 and 0.14 pH units in bud- and germ tube-forming populations, respectively. In the two diverging populations, the transient increase in pHi was followed by a rapid drop. The sharp rise in pHi of the population destined to form buds was sensitive to orthovanadate and to the depletion of K+ from the medium while this was not the case with germ tube-forming cells. The results suggest that pHi may play an important role in the phenotypic divergence of C. albicans.

Pityriasis (tinea) versicolor in infancy.

Pityriasis (tinea) versicolor is a common disorder of adults. We cared for five infants (four males and one female) with the disease. Diagnosis was confirmed by potassium hydroxide preparation demonstrating the filaments of Malassezia furfur and/or Pityrosporum orbiculare, the yeast form. Three patients had lesions in the neonatal period. The mother of one baby had pityriasis versicolor. Two patients were siblings. One baby had associated atopic dermatitis and two had a positive family history of atopy.

Abdominal epilepsy misdiagnosed as psychogenic pain.

The authors present the case report of a child with abdominal epilepsy who had suffered from abdominal pain for several months under the label of psychogenic pain. The important historical clues were pallor and cold sweating during the paroxysm, followed by lethargy and prolonged sleep. An abnormal electroencephalogram and a remarkable response to anticovulsants confirmed the diagnosis. This condition must be considered in a child with undiagnosed recurrent abdominal pain.

Effect of preoperative iodine in patients with Graves' disease controlled with antithyroid drugs and thyroxine.

Thirty-four patients with Graves' disease, first rendered euthyroid with antithyroid drugs (ATD) then given supplementary thyroxine (T4), were randomly allocated to three treatment groups. In Group I ATD and T4 were stopped 10 days before partial thyroidectomy and the patients were treated with Lugol's iodine alone. In Group II the patients were treated up to the time of operation with ATD and T4 alone. In Group III ATD and T4 were continued until the day of operation, but the patients also received 10 days' treatment with Lugol's iodine. Analysis of the results showed that pre-operative iodine therapy in patients with Graves' disease, already rendered euthyroid with ATD and T4, made no difference to the vascularity of the gland, operative blood loss or thyroid follicular size. Over a third of patients in Group I, treated with Lugol's iodine alone for 10 days pre-operatively, had subnormal levels of thyroid hormones at the time of operation and this was also the Group in which the complications of partial thyroidectomy tended to occur. It is concluded that the addition of iodine preoperatively is unnecessary in the patient who is already euthyroid on ATD and T4.

Involvement of palate and cheek in leprosy.

22 Cases of bacillary positive leprosy with no apparent lesion in the oral cavity, soft or hard palate were studied for any evidence of pathological involvement. Granulomata were present in 11 (65%) out of 17 cheek biopsies studied. M. leprae were identified in four specimens only. 9 specimens (64%) out of 14 palate biopsies showed definite granulomata. M. leprae were seen in six specimens.

Bacillaemia in leprosy: correlation with slit-skin and nasal smears.

Twenty five multibacillary patients (BL/LL) were studied for bacillaemia. Majority (76%) showed acid fast bacilli in peripheral blood. There was good correlation between bacillary load in peripheral blood and bacteriological index (BI) but poor correlation with morphological index (MI) of skin slit smear and BI/MI of nasal smear.

Effect of in vitro formed immune complexes on macrophage functions of Mycobacterium leprae infected mice.

Nonspecific macrophage functions were studied in Mycobacterium leprae infected and preformed immune complex (IC) administered normal (NI) and thymectomized/irradiated (TRI) mice at different time periods. Uninfected controls given IC were also included. Significant decrease in the chemotaxis, phagocytosis and bactericidal activities of macrophages obtained from infected groups compared to their controls were observed. Phagocytic and chemotactic activities of macrophages were normal but intracellular killing was seen to be depressed in studies conducted in normal and thymectomized immunosuppressed groups (Vaishnavi et al., 1985, Kumar et al, 1987) which were not administered with preformed IC.

Drug use in dermatology.

Prescription of drugs was studied for 197 outpatients and 93 inpatients seeking treatment under the specialized services of the Dermatology Department of the Nehru Hospital attached to the Postgraduate Institute of Medical Education and Research, (PGIMER) Chandigarh. Disease patterns in both settings and drugs prescribed for them were analyzed from prescriptions, retrospectively. Brand drugs were prescribed to the tune of 60-65%, in both settings, some lacunae in prescription are discussed.

Plasma dapsone and its metabolite monoacetyldapsone levels in leprotic patients.

Dapsone (DDS) is a drug of choice in the treatment of leprosy. The DDS and MADDS levels and different pharmacokinetic parameters after a single dose of dapsone and at steady state in leprotic patients have been studied. At steady state, all the patients showed plasma DDS and MADDS levels above 0.5 micrograms/ml throughout the 24-h duration. There was no significant difference in the elimination half-lives of DDS and MADDS after a single dose as compared to at steady state, but AUC0-alpha for both DDS and MADDS were significantly increased at steady state. From these results, it could be concluded that 100 mg daily dose is sufficient to maintain plasma therapeutic concentration in leprotic patients in Indian population.

Effect of multidrug therapy on the levels of antibodies to Mycobacterium leprae glycolipid-1 in the leprosy spectrum.

Sera from one hundred and forty five untreated leprosy patients, ten proven cases of active pulmonary tuberculosis and twenty five healthy volunteers not exposed to M. leprae infection were assayed for PGL-1 antibodies. All available follow up samples after multidrug therapy were also assayed. A decline in the level of PGL-1 antibodies were seen in many of the post-treatment samples, giving an indirect assessment of the bacterial load and the prognosis of the disease.
