Detection of (3'-hydroxy)-3,N4-benzetheno-2'-deoxycytidine-3'-phosphate by 32P-postlabeling of DNA reacted with p-benzoquinone.

Cytidine-3'-phosphate was reacted with p-benzoquinone under neutral aqueous conditions, and the fluorescent product formed was isolated and characterized. The structure of the covalent adduct was identified as (3'-hydroxy)-3,N4-benzetheno-cytidine-3'-phosphate by high-resolution MS and 1H NMR spectroscopy. A similar product was isolated from the reaction of 2'-deoxycytidine-3'-phosphate with a hydroquinone-p-benzoquinone mixture. 32P-Postlabeling of calf thymus DNA reacted with p-benzoquinone detected several adducts, the principal adduct being (3'-hydroxy)-3,N4-benzetheno-2'-deoxycytidine-3'-phosphate. Our studies demonstrate that the reaction of DNA with p-benzoquinone in vitro leads to multiple DNA adducts. 32P-Postlabeling may allow detection of benzene-DNA adducts in vivo.

Dexamethasone-cyclophosphamide pulse therapy in pemphigus.

Fifty patients with pemphigus (45 pemphigus vulgaris, 5 pemphigus foliaceus) were treated with dexamethasone-cyclophosphamide pulse therapy. The pulse consisted of 136 mg dexamethasone dissolved in 5% dextrose given in a drip over a period of 1-2 hours on 3 consecutive days. In addition, 500 mg cyclophosphamide was added in the drip on the first day. Such pulses were given at monthly intervals. In between the pulses patients were given 50 mg cyclophosphamide orally each day. The results were encouraging, the chief advantage being freedom from side effects of corticosteroid therapy. The lesions healed in 3-4 days and the patients were able to resume their work within one week. Further scope of such therapy in pemphigus is discussed.

Childhood psoriasis: an epidemiologic survey of 112 patients.

One hundred and twelve cases of childhood psoriasis, constituting 5% of all psoriasis patients registered in our clinic, were studied over a period of nine years (January 1980 to December 1988). There were 57 male and 55 female patients. Age of onset ranged from 1 month to 13 years, with a mean and median age of 7 years. A positive family history of psoriasis was elicited from 9.8% of patients. The commonest form of psoriasis found was plaque (69.6%), followed by guttate (25.9%). Infection was the most common triggering factor, occurring in 15.2% of cases. Many of these findings are at variance with observations reported elsewhere for this age group.

Contact sensitization by pesticides in farmers.

30 farmers with contact dermatitis and 20 controls were patch tested with a series of locally used pesticides. Allergic reactions to 1 or more pesticides were seen in 11 patients but none of the controls. Carbamates (maneb, carbofuran, carbaryl) were the most frequent sensitizers (7 patients), followed by organophosphorus compounds like: malathion and oxydemton methyl (4 patients); 2,4-dichlorophenoxyacetic acid and fenvalerate (3 patients each); streptocycline (2 patients). Irritant reactions to captaf were seen in 4 patients, thiobencarb weedicide in 3, and organophosphorus compounds in 3. Pesticides should be patch tested in all farmers with contact dermatitis.

Renal brushborder membrane vesicle. Study of marker enzymes and uptake of nutrients in Mycobacterium leprae infected mice.

The renal brush border membrane vesicles (BBMV) were used to elucidate the early biochemical functional status during the course of experimental M. leprae infection in mice. The activities of the characteristic brush-border enzymes viz: alkaline phosphatase, leucine amino peptidase and gamma-glutamyl transpeptidase were found to be significantly decreased (p less than 0.001) at 3 and 6 months after infection. The transport of nutrients viz: D-glucose, L-alanine, L-lysine and L-aspartate across BBMV showed similar pattern. The activity of brush border enzymes and transport of nutrients across the membrane returned to normal at 9 months post-infection suggesting regeneration of the brush border membrane.

HIV-I antibodies in health and disease.

Antibodies to human immune deficiency (HIV) virus were studied in 2000 individuals including cases of non-Hodgkin's lymphoma, systemic lupus erythematosus (SLE), leprosy, chronic renal failure on haemodialysis and patients attending STD clinics. A group of blood donors was also screened, ELISA kits provided by Wellcome Diagnostics were used. Results indicate that the ELISA values were far above the cut off figure in all except in a couple where the husband who had stayed in Uganda for several years, and had features of full blown AIDS died 4 months after the diagnosis. The spouse contacted AIDS within a relatively short incubation period and died within 6 months of diagnosis. The North Indian population thus appears to be free of this virus so far. This observation will be an important lead mark in the future epidemiology of HIV infection in India.

Acid-fast bacilli in semen; correlation with bacterial index.

Thirty multibacillary patients (12 LL, 18 BL) were historically reviewed and clinically assessed for evidence of testicular involvement. The bacterial index (BI) of the patients ranged from 1+ to 6+; the morphological index (MI), from 0% to 3%. Nine (30%) patients had oligospermia, while acid-fast bacilli (AFB) were demonstrable in the semen of 3 (10%) patients (2 LL, 1 BL). There was a significant correlation between the BI and the demonstration of AFB in semen (p less than 0.01).
