The traditional office diagnosis of prostatitis has been based on clinical history, rectal palpation of the gland, and a microscopic examination of the expressed prostatic fluid. In light of present day information this appraoch appears to be incomplete. We have examined 41 consecutive patients with complaints of prostatitis along with 17 controls. In each case expressed prostatic fluid was collected under sterile conditions. On each specimen, we performed a bacterial culture, microscopic examination for white blood cells, and assay for zinc. The intent of this protocol was to classify clearly those with bacterial prostatitis from those with nonbacterial prostatitis and prostatodynia. We believe this approach is simple to perform even in the setting of a busy office practice.
Since the prostate gland secretes specific substances, several investigators have suggested that measurement of these products may be useful for the evaluation of prostatitis. In our laboratory we have measured seminal zinc as a marker of prostatic secretion to establish normal values and define the confidence limits for this assay. Zinc assays were performed on whole semen, fractions of the split ejaculate, and expressed prostatic fluid from among a population of 456 men. We hope that these data will be useful when compared with data from patients with prostatic disease.
A previous report showed that following spinal cord transaction well below the C7 level in cats, later components of the cortical somatic evoked potential (SEP) (40 msec or longer) produced in response to median or radial nerve stimulation were abnormal (Katz et al., 1977). Evidence presented here shows that early components of the radial nerve SEP are also altered following spinal cord transection. Latencies of both early and late components were increased as higher functional transections of the spinal cord were made. This monotonic increase in latency of the primary components supports the hypothesis that the results may be due to increasing loss of excitatory input. Correlation of latency change with level of injury may serve as a useful diagnostic tool.
The composition and structure of attitudes toward the physically disabled were studied using Fishbein's theoretical framework and methodology in which an individual's attitude toward any object is assumed to be (1) a function of his beliefs about the object, and (2) the evaluative aspects of these beliefs. This is stated algebraically as: [Formula: see text]. The subjects for the study were 52 workers employed in a large public construction company in Israel. They were asked to state their beliefs about the characteristics of physically disabled persons. Using the Semantic Differential Technique, the most frequent beliefs expressed were used to construct (a) a scale which measured the evaluative aspect of the belief, and (b) a scale to measure the probability dimension of each belief. In addition, an obtained measure of attitude towards disabled persons was elicited, and in order to test the validity this scale the Attitude Towards Disabled Persons was also given. The estimated attitude score for each subject was obtained using the above formula. A correlation of r = .91 (p < .01) between the Fishbein attitude score and the obtained attitude score was found. The correlations between the Fishbein attitude score and the ATDP was r = .78 (p < .01). These correlations provide support for the main hypothesis. In addition, the study demonstrated the advisability of assessing attitudes towards the physically disabled as they relate to specific roles that the disabled person has in various life areas and not as a general attitude.

Two long-term care settings not now covered by Medicare--adult day care and homemaker services--were studied in a randomized experiment to test the effects on patient outcomes and costs of using these new services. This article reports findings for day care. Patients' physical, psychosocial and health functions were assessed quarterly, and their Medicare bill files were obtained. Medicaid data were obtained on most patients, but few used many Medicaid-covered long-term care services. Multistage analysis was performed to mitigate effects of departures from the randomized design. Day-care patients showed no benefits in physical functioning ability at the end of the study, compared with the control group. Institutionalization in skilled nursing facilities was lower for the experimental group than the control group, but the factors other than the treatment variable appeared to explain most of the variance. There was a possibility that life was extended for some day-care patients. the new services averaged $52 per day or $3,235 per year. When costs for existing Medicare services used were added, the yearly cost of the experimental group was $6,501, compared with $3,809 for the control group--an increase of $2,692 or 71 per cent.

The possible presence of histamine H2 receptors in dog ventricle was investigated by determining the effect of histamine on adenylate cyclase prepared from that tissue. Epinephrine produced a dose-dependent increase in adenylate cyclase activity but histamine had no effect. Histamine H2 receptors probably do not exist in dog ventricle.
Sarcoidosis rarely involves anterior mediastinal nodes, pancreas, or uterine cervix. A 47-yr-old woman with sarcoidosis was found to have enlargement of the anterior mediastinal lymph nodes with "eggshell" calcifications on a lateral chest roentgenogram. This finding was confirmed by lateral tomography and thoracic computerized tomography. A review of 100 lateral chest roentgenograms of patients with documented sarcoidosis failed to reveal other examples of anterior mediastinal node enlargement. At laparotomy for total abdominal hysterectomy, a mass in the head of the pancreas and diffuse, nodular enlargement of this organ was found. A pathologic study of the pancreas and uterine endocervix demonstrated noncaseating granulomas. Sarcoidosis in these 3 unusual sites is discussed.
The coronary arteriographic anatomic findings in 491 male patients clinically symptomatic for less than six months are described. Single and multiple vessel disease was noted, respectively, in 40 and 60 percent with 6.5 percent also manifesting left main coronary disease. Our findings indicate that severe coronary artery disease is present early in the symptomatic phase and that patients presenting with initial inferior myocardial infarctions may represent a select group having an excessive frequency of severe coronary disease suitable for early angiographic detection and surgical bypass therapy.

The effect of aprotinine, a natural non-specific protease inhibitor, on cyclic AMP accumulation in a dog heart washed particle preparation was studied. Aprotinine inhibited GTP-stimulated cyclic AMP accumulation at concentrations equal to or higher than 40 KIU/ml (8 x 10(-7) M). Aprotinine inhibited epinephrine (10(-4) M)-stimulated cyclic AMP accumulation to a greater degree than fluoride (10(-2) M)-stimulated activity. Trypsin stimulated cyclic AMP accumulation at low concentrations (0.5 to 2.5 microgram/ml) and inhibited at higher concentrations. Aprotinine at low concentrations (4 KIU/ml) inhibited the trypsin (1 microgram/ml) stimulation of cyclic AMP accumulation. We conclude that aprotinine may act on the regulator rather than on the catalytic sub-unit of adenylate cyclase and that its mechanism of action is not due only to its antiprotease activity.
