Angiography in colonic carcinoma.

Angiography of the mesenteric arteries was performed in 17 patients with adenocarcinoma of the colon and rectum. At angiography, the marginal artery was encased in 13 patients and 10 of these were surgically unresectable while three could be resected radically. Encasement of the mesenteric arterial branches was found in seven of 11 unresectable tumors and in two of five resectable tumors. The resectability of colonic carcinoma cannot be established by means of angiography but in two patients with recurrent colonic carcinoma, barium-enema examinations were negative, while infiltrated vessels were demonstrated by angiography and the carcinomas were verified at surgery. Four patients received intra-arterial chemotherapy with mitomycin C. In one of these, a slight decrease in tumor size was noted, while in the others no change was noted.

Glycoproteins in cystic fibrosis: a lectin binding study.

Ten lectins have been used to detect glycoproteins, after SDS polyacrylamide gel electrophoresis and gel isoelectric focusing, in fibroblasts, red cell membranes, urine, and plasma of patients and obligate heterozygotes with cystic fibrosis. No disease specific changes were detected but considerable individual variation was observed, some of which was attributed to known genetic polymorphisms unrelated to cystic fibrosis.

Reversible renal insufficiency after percutaneous transluminal angioplasty (PTA) of renal artery stenosis.

Three patients developed reversible renal insufficiency after successfully performed PTA at the same time as their hypertension was beneficially influenced. The main cause of this complication probably is toxic effects of contrast medium in combination with the temporary occlusion of the renal artery during the PTA of kidneys with previously damaged parenchymal and impaired perfusion. During the phase of renal insufficiency contrast medium retention in the renal cortex was noted at computed tomography.

Percutaneous transluminal angioplasty of arteriosclerotic lesions in the pelvis and lower extremities.

The experience of percutaneous transluminal angioplasty (PTA) in 66 patients (71 stenoses) with arteriosclerotic lesions in the pelvic and lower extremity arteries is reported. The PTA was technically successful in 70 stenoses. Complications were seen in eleven patients. The follow-up time was 19 months (2-44). The frequency of recurrencies was clearly related to the location of stenosis, the more distal the lesion the higher the recurrency rate. The immediate roentgenologic result had no prognostic value to protect recurrencies. To conclude PTA is an alternative to surgery in patients with short stenoses in the iliac arteries, but very strict indications should be used when lesions are localized below the inguinal ligament.

Denture mobility and bolus position during mastication in patients wearing conventional and pelotte-retained full upper dentures: a cineradiographic study.

With cineradiographic technique an assessment was made of the denture mobility of conventional and pelotte dentures in the upper jaw and of the position of the bolus during chewing of meat and bread. Five randomly selected edentulous patients with moderate alveolar ridge resorption rehabilitated with dentures of conventional design, and another five with dentures furnished with subnasal retentive pelottes, were incorporated in the study. Cineradiographic registrations were made in lateral and frontal projections with a picture frame speed of 38/s. Analysis of the cineradiographic films revealed only small differences for the denture mobility for the two patient groups. This indicated in the patients with severe ridge resorption, an obvious reduction of denture displacement through the use of the nasal pelotte or prong. The displacement was of similar magnitude as for those dentures where an alveolar ridge creates effective retention and limitation of horizontal movements. The analyses of the bolus position during chewing gave the information that in all individuals wearing pelotte dentures, the bolus was positioned more distally, than for patients with conventional dentures. Presumably this was in order to reduce discomfort in the frontal region, i.e. the region of the pelottes. Patients with conventional dentures showed a more even distribution of the bolus to all regions.

A genetic polymorphism of a human urinary mucin.

We report here a novel genetically determined polymorphism of a human urinary mucin which is demonstrable by the separation technique of SDS polyacrylamide gel electrophoresis, followed by detection with radio-iodinated lectins. The mucins are demonstrable using various lectins but the polymorphism is most easily recognized using peanut agglutinin and we therefore propose to designate this new genetic locus PUM (peanut-reactive urinary mucin). Four common alleles have been identified and an autosomal codominant mode of inheritance has been found in the families studied so far.

Gc subtypes in Icelanders.

Isoelectric focusing was used to determine the frequencies of Gc subtypes in Icelanders. The gene frequencies observed were Gc1F = 0.107, Gc1S = 0.631 and Gc2 = 0.262. An Icelandic Gc variant allele (Gc Iceland) is shown to have a different isoelectric point from the variant allele Gc-Norway. Gc-Iceland has been detected in 3 unrelated individuals giving the allelic frequency of 0.004 in the Icelandic population. Studies of 65 mother-child pairs confirm the assumed three-allelic mode of inheritance.

Gc subtypes in Northern Indians.

Isoelectric focusing was used to determine the frequencies of the Gc subtypes in a population sample from The North Indian subcontinent (now living in Birmingham, UK). The gene frequencies observed were as follows: Gc1F = 0.191, Gc1S = 0.519 and Gc2 = 0.290.243 individuals were typed and no variant alleles were detected.

Computer-analysed movements in three dimensions recorded by light-emitting diodes. A study of methodological errors and of evaluation of chewing behaviour in a group of young adults.

Mandibular movements recorded by means of intraorally placed light-emitting diodes (LEDs) were described and tested. By using a reference system attached to the test subject, the influence of head movements on the registration of the mandibular movements could be avoided. The errors of the method were tested. The stability of the reference system during function was found to be greater when the light impulses did not have to penetrate the lip. A limited clinical test was performed and the following observations were made: the mean cycle duration was, after the first cycles, stable through the whole chewing phase, 0.52 s when chewing crispbread; the duration of the occlusion phase appeared to be shorter than that of the opening and closing phases; the velocity of the mandible was greater in the opening phase than in the closing phase, with a mean velocity of 84 and 70 mm/s respectively; the clinical test showed that the method was easy to handle in the clinical situation, thereby creating an instrument for studies in larger groups of patients. By adding a simple computer program, the manual analysis could be reduced considerably and a number of variables could be measured with a high degree of precision.

Intravenous arteriography in the preoperative evaluation of advanced atherosclerosis.

Intravenous arteriography may be used as an alternative to translumbar or transaxillary angiography in cases with occlusion or severe stenosis of the aorta or iliac arteries, to determine the extension of the disease. During a 10-year period the method was used in 117 patients. For the examination, 80 to 100 ml of contrast medium was injected into the right atrium and after a delay of 10 to 20 seconds the lumbar aorta, iliac and femoral arteries were demonstrated. The quality of the examination was excellent or good in 92 per cent. No complications occurred. In 19 patients a continuous pressure recording was made in the main pulmonary artery. The pulmonary artery pressure increased to a significantly higher degree after contrast medium injection than after injection of the same amount of isotone saline. In all patients the pressure returned to normal within 4 minutes.

Vascular tumours of the hand.

The series comprises 74 patients with benign and 2 with malignant vascular tumours located in the upper extremity and treated at our hospital during the period 1962-80. In 9 instances the tumour was associated with skeletal hypertrophy of the involved extremity (Klippel-Trénaunay syndrome), whereas in another 7 patients a hypotrophy of the skeleton of the affected extremity was seen. Preoperative angiography was performed on 39 patients and characteristic features for the various types of tumours are discussed. Fifty-nine patients underwent surgical treatment. The observation period ranged from 1 to 18 years (mean 7 years). Twenty-six patients were cured, 19 improved, and 14 had recurrence. The cure rate was found to depend on the presence of arteriovenous shunts, the size of the tumour and its localization. Tumours with diameters of less than or equal to 2 cm located on the fingers or on the dorsum of the hand had the best prognosis.

Angiomatosis osteohypotrophica.

Based on a clinical and radiological study of seven patients, a syndrome consisting of congenital hemangiomas with arteriovenous fistulae engaging the soft tissues and the bones, and associated with shortening of the involved extremity, for which we suggest the name angiomatosis osteohypotrophica, is reported. Angioma, varicose veins and/or arteriovenous fistulae with resulting increase in limb have been widely known since the beginning of this century as Klippel-Trénaunay syndrome. Although the patients in this series present all other features of Klippel-Trénaunay syndrome, thinning and shortening of the long bones occurred, resulting in an overall reduction in limb length. Intraosseous hemangiomas in the metacarpal, respective metatarsal bones were common characteristic findings. The arteriograms undertaken in this study showed, in addition to hemangiomas, presence of arteriovenous shunts within the area of the tumour. Their influence on skeletal growth is discussed.

Computer tomography of ovarian dermoid cysts.

Five patients with ovarian dermoid cysts (benign cystic teratomas) were examined by CT. The appearance of these tumours at CT as well as at conventional radiography is discussed. It is concluded that CT adds valuable information in the preoperative evaluation of cystic teratomas.

Denture adhesives - their effect on the mobility of full upper dentures during chewing. A cineradiographic study.

Using cineradiographic technique an assessment is made of the denture mobility during function in the upper jaw for full denture wearers without and using two types of denture adhesives. One of them with a new formula. Ten randomly selected edentulous patients with moderate alveolar ridge resorption took part in the study. They were rehabilitated with dentures of conventional design in the upper jaw. Cineradiographic registrations were performed in lateral projection without and with the two types of adhesives supplied to the denture base. The analysis of the cineradiographic films disclosed no significant differences for the denture mobility. Neither between the situation without and with denture adhesive nor between the two adhesives. This indicates an obvious limited effect of the denture adhesives, at least for denture wearers with moderate resorption of their alveolar ridges. The only positive effect of the adhesives discovered was a limited number of vertical loosening drops of the distal parts of the denture when the seal was broken.
