[Paraurethral cyst in a female neonate: a case report].

A seven-hour-old female neonate visited our hospital because of the anormaly of external urethral meatus. An interlabial cystic mass, the diameter of which was about 1 cm, was observed and an external urethral meatus could not be found. Direct needle aspiration of the cystic mass showed a small quantity of milky fluid and an urethral meatus was then identified.

Investigation of morphological changes in absorptive cells in young adult and infant mice fed different amounts of iron for a long-term.

The changes in fine structure of the intestinal tract in young adult (4 week-old) and infant (2 week-old) mice fed a diet containing different amounts of iron salt (Fe-0, Fe-2.5, Fe-25: 0, 2.5 and 25 mg Fe/100 g diet, respectively) for a long-term (1 or 2 weeks) were investigated. The hepatic iron levels in infant mice fed Fe-25 for 2 weeks were significantly higher than those observed after 1 week of feeding, but there was no such increase in young adult mice during the feeding period. Observations of fine structure indicated typical signs of impairment of enterocytes due to excess iron such as the opening of intercellular junctions between adjacent epithelial cells and the marked appearance of eosinophilic leukocytes outside the basement membrane in young adult and infant mice fed Fe-25. The frequency of the opening in intercellular junctions increased in young adult mice fed Fe-25 for 2 weeks, but decreased in infant mice. On the contrary, under iron-deficient conditions, the frequency in infant mice was higher than that in young adult mice. The appearance of eosinophilic leukocytes indicated that some immunological reaction was elicited in both groups of mice fed Fe-25 for 2 weeks.

Hyaluronan oligosaccharides induce tube formation of a brain endothelial cell line in vitro.

In remodeling tissues the formation of new blood vessels is an essential process which is regulated by different factors. During such processes an increase in hyaluronan synthesis and turnover is often seen and recent observations have suggested that hyaluronan can both promote and inhibit neovascularization depending on its molecular mass. In this work we show that a brain capillary endothelial cell line forms tubes in a collagen gel after stimulation by hyaluronan oligosaccharides. Ultrastructural examination of the tubes by transmission electron microscopy revealed that the cord-like outgrowths consisted of 2-10 tightly packed cells containing a continuous lumen. The tube formation in response to hyaluronan oligosaccharide was not mediated by activation of receptors for fibroblast growth factor. The endothelial cell line, which does not synthesize hyaluronan, exhibited specific hyaluronan-binding sites, with about 3000 hyaluronan molecules (M(r) 3.85 x 10(6)) bound per cell at saturation and Kd was 0.05 x 10(-9) M. Furthermore, the cell line was stained with mAb IVd4 that recognizes a novel class of hyaluronan-binding proteins and mAb IM7 which recognizes CD44 molecules.

Phosphatidylinositol 3'-kinase-independent p70 S6 kinase activation by fibroblast growth factor receptor-1 is important for proliferation but not differentiation of endothelial cells.

p70(s6k) has a role in cell cycle progression in response to specific extracellular stimuli. The signal transduction pathway leading to activation of p70(s6k) by fibroblast growth factor receptor-1 (FGFR-1) was examined in FGF-2-treated rat L6 myoblasts. p70(s6k) was activated in a biphasic and rapamycin-sensitive manner. Although phosphatidylinositol 3'-kinase was not activated in the FGF-2 treated cells, as judged from in vitro and in vivo analyses, wortmannin and LY294002 treatment inhibited p70(s6k) activation. Inhibition of protein kinase C (PKC), by bisindolylmaleimide or by chronic phorbol ester treatment of the FGFR-1 cells, suppressed but did not block p70(s6k) activation. In cells expressing a point-mutated FGFR-1, Y766F, unable to mediate PKC activation, p70(s6k) was still activated, in a bisindolylmaleimide- and phorbol ester-resistant manner. The involvement of S6 kinase in FGFR-1-dependent biological responses was examined in murine brain endothelial cells. In response to FGF-2, these cells differentiate to form tube-like structures in collagen gel cultures and proliferate when cultured on fibronectin. p70(s6k) was not activated in endothelial cells on collagen, whereas activation was observed during proliferation on fibronectin. In agreement with this finding, rapamycin inhibited the proliferative but not the differentiation response. Our results indicate that FGFR-1 mediates p70(s6k) activation by a phosphatidylinositol 3'-kinase-independent mechanism that does not require PKC activation and, furthermore, proliferation, but not differentiation of endothelial cells in response to FGF-2, is associated with p70(s6k) activation.

Size Determination of Catena-&mgr;-2,5-dihydroxy-p-benzoquinonatonickel(II) Coordination Polymer Colloid and Its Photochemical Reaction

Electric birefringence and streaming dichroism measurements of catena-&mgr;-2,5-dihydroxy-p-benzoquinonatonickel(II) [dhqNi(II)] aqua sol suggested that the average length of the sol particles was ca. 380 nm and the axial ratio was 2.0-2.3 as the prolate spheroid. The average particle length in the scanning microscopic images was 370 ± 60 nm and the axial ratio 2.3 ± 0.6. This aqua colloid was decomposed by UV irradiation under an O2 gas atmosphere, but under a N2 gas atmosphere its decomposition was depressed. Furthermore, addition of sacrificial reagents such as l-ascorbic acid depressed the decomposition. These facts suggested that the photogenerated electron originating from the irradiated dhqNi(II) colloidal particle reduced O2, while the positive hole on the colloid oxidized dhqNi(II) itself. It was found that the reduction product from O2 reduced Cr6+ (in Cr2O72-) to Cr3+, and the oxidized dhqNi(II) oxidized I- to I2/2. Therefore, H2O2 formation was proposed as the photochemical reduction product. Copyright 1997Academic Press

Diltiazem derivatives modulate the dihydropyridine-binding to intact rat ventricular myocytes.

To examine whether the modulation of the 1,4-dihydropyridine-binding by diltiazem derivatives, which has been shown in cardiac and skeletal muscle membranes, takes place in intact cardiac myocytes, effects of diltiazem on the specific binding of [3H](+)-PN200-110 to freshly isolated adult rat ventricular myocytes were investigated in normal Tyrode solution at 37 degrees C. Diltiazem consistently potentiated the [3H](+)-PN200-110-binding in a concentration-dependent manner, while DTZ323 (3-(acetyloxy)-5-[2-[[2- (3,4-dimethoxyphenyl)ethyl]-methylamino]ethyl]-2,3-dihydro-2(-4 methoxyphenyl)-1,5-benzothiazepin-4-(5H)-one), a potent diltiazem derivative, inhibited it in a non-competitive manner. In saturation studies, 100 microM decreased the Kd value of the 3[H](+)-PN200-110-binding (control, 0.102 +/- 0.008 vs. diltiazem, 0.074 +/- 0.004 (nM, n = 6), P < 0.05) without significant effect on Bmax (control, 65.7 +/- 6.4 vs. diltiazem, 76.7 +/- 4.4 (fmol/mg protein, n = 6). Moreover, membrane-impermeant quaternary diltiazem also potentiated the [3H](+)-PN200-110-binding in intact myocytes. These results suggest that diltiazem modulates the 1,4-dihydro-pyridine-binding even in intact cardiac myocytes, and the binding site of diltiazem is accessible from the extracellular side of the L-type Ca2+ channels.

Idiopathic osteosclerosis of the jaws: panoramic radiographic and computed tomographic findings.

OBJECTIVE: The purpose of this study was to determine the frequency and anatomic location of idiopathic osteosclerosis in the jawbones and to present computed tomographic findings of this lesion in Japanese patients. STUDY DESIGN: Panoramic radiographs of 1047 patients were examined for the presence of idiopathic osteosclerosis in the jawbones. Computed tomography was performed in 11 patients of this series. RESULTS: A total of 64 patients (6.1%) showed this radiopacity. The highest occurrence was in the mandibular first molar region. There was no statistically significant difference in the prevalence of idiopathic osteosclerosis between males and females. On computed tomography images, these radiopaque areas were divisible into two types: enostosis (five cases) and central sclerosis (six cases). CONCLUSION: This is the first report of computed tomographic findings on idiopathic osteosclerosis in the jawbones, although the results on panoramic radiographs are similar to other investigators.

Reliability of ultrasonography and sialography in the diagnosis of Sjögren's syndrome.

OBJECTIVE: The purpose of this study was to evaluate quantitatively observer performance with ultrasonography and sialography in the diagnosis of parotid gland involvement of Sjögren's syndrome. STUDY DESIGN: Sixty-four parotid gland sialograms and 65 ultrasonograms were prepared for observer performance experiments. They included both modalities in 24 Sjögren's syndrome and 19 nonspecific parotitis cases, 21 normal parotid gland sialograms, and 22 normal ultrasonograms in healthy volunteers. The images were randomly sequenced and presented to five observers who were asked to describe several findings and finally to determine the imaging diagnosis by ranking the abnormal features and the diagnosis on a five-point-rating scale. Observer performance was evaluated on the basis of the reliability of findings interpreted and the diagnostic accuracy of each modality from observers' rating scores. RESULTS: The diagnostic accuracy of sialography was very high, nearly perfect. The diagnostic accuracy of ultrasonography was lower than that of sialography, resulting from the lower incidence of characteristic findings in the disease groups and lower sensitivity on ultrasonography. In the differentiation of Sjögren's syndrome from the normal, however, the diagnostic accuracy of ultrasonography increased to 80% for all cases, and up to nearly 90% in the advanced sialographic stages. CONCLUSION: Ultrasonography is useful for the diagnosis of Sjögren's syndrome in the advanced stages. Taking the noninvasiveness of this technique into account we recommend first applying ultrasonography to the diagnosis of Sjögren's syndrome and performing sialography when no positive findings are detected on ultrasonography.

Submandibular gland duct endoscopy. Diagnostic value for salivary duct disorders in comparison to conventional radiography, sialography, and ultrasonography.

The purpose of this study is to evaluate the usefulness of endoscopy as a procedure for the diagnosis of submandibular gland duct disorders. Endoscopy of the submandibular glands was performed on 12 patients with symptoms of obstructive sialoadenitis to identify the cause of obstruction. The endoscopic findings were then compared to those of diagnostic procedures such as conventional radiography, sialography, and ultrasonography. Six normal subjects also underwent endoscopy to better understand the normal findings of the duct system. Endoscopy demonstrated salivary gland calculus in 5 of 12 patients, which was revealed as filling defects on sialograms and as strongly echogenic structures on ultrasonograms in 4 of the patients. Endoscopy revealed secretion plugs, secretion plaques, and/or stenosis, which could not be seen by any other diagnostic procedures in 5 patients, as the cause of recurrent swelling in all 7 patients not demonstrating sialolith. Abnormal findings of the duct wall such as vasodilatation, fibrosis, edema, or erythema were seen in four patients, three of whom exhibited dilatation of the duct system on sialograms. In four patients, a decreasing internal echo level of the gland was seen on ultrasonograms. Our initial results for submandibular gland duct endoscopy thus appear to be promising.

Imaging findings of lipomas in the orofacial region with CT, US, and MRI.

OBJECTIVE: The aim of this study was to document retrospectively the imaging findings of lipomas with the use of computed tomography, ultrasonography, and magnetic resonance imaging. STUDY DESIGN: Thirteen patients with 11 lipomas and 2 lipomatoses were evaluated. Eleven cases were examined by computed tomography, 9 by ultrasonography, and 3 by magnetic resonance imaging. RESULTS: Lipomas had a density ranging from -134 to -83 Hounsfield units, (mean-108) on the computed tomography images. The margins were ill defined in 9 of 10 cases. The superficial muscles were displaced externally in 8 cases and internally in 2 cases. With ultrasonography, 8 lesions were hypoechoic, and one was hyperechoic. All three lesions had a high signal intensity on both T1- and T2-weighted images. CONCLUSIONS: Lipomas had a specific range of computed tomography Hounsfield unit values and also displaced the surrounding soft tissue. Although some variation in the ultrasonographic appearance was observed, the lesions tended to be hypoechoic. These findings may be useful for diagnosing lipomas in the orofacial region.

Morphologic analysis of odontogenic cysts with computed tomography.

PURPOSE: The purpose of this study was to analyze the effects of lesion site and epithelial keratinization on the morphologic characteristics of odontogenic cysts and clarify determinate factors for cyst morphology. MATERIAL AND METHODS: Computed tomographic images of 92 odontogenic cysts were analyzed: 31 primordial, 31 dentigerous and 30 radicular. Thirty-four cysts were located in the maxilla (6 primordial, 10 dentigerous, and 18 radicular) and 58 in the mandible (25 primordial, 21 dentigerous, and 12 radicular). Histologically, 31 cysts showed epithelial keratinization (18 primordial and 13 dentigerous). No keratinization was seen in radicular cysts. The morphologic features of cysts were assessed by measuring long length parallel to dental arch and short length vertical to it and calculating the long/short ratio. In addition, the computed tomography pattern of the cyst was classified into unilocular, lobulated, and multilocular patterns. Appearance of the sclerotic rim and surrounding cortex were classified into three and four patterns respectively to evaluate the developmental features of the cyst. RESULTS: As a whole, the long length of the primordial cysts was statistically larger than the other two cyst groups and resulted in a larger long/short ratio. Statistical differences of CT pattern were also seen among cyst groups. There was no preference in any cyst group for the appearance of the sclerotic rim and cortex. There were statistical differences between maxilla and mandible in short axis and long/short ratio. The maxillary cysts generally showed round shapes irrespective of their histologic characteristics. A multilocular pattern was more frequent in the keratinized group of mandibular primordial cysts. In dentigerous cysts, a multilocular pattern was seen only in the keratinized group and the long/short ratio was statistically larger; cyst shape was elliptical along the long axis. CONCLUSION: Our results demonstrated morphologic differences of odontogenic cysts caused by lesion site and keratinization. The dentigerous cyst with predominant keratinization should be included in the primordial cyst (odontogenic keratocyst) group.

Computed tomography appearance of marked keratinization of metastatic cervical lymph nodes: a case report.

A case of squamous cell carcinoma of the tongue is reported with emphasis on an atypical finding of cervical lymph node metastasis visible on computed tomography. Multiple cervical lymph nodes revealed a high computed tomographic value of about 330 HU, lower than that of calcification. The lymph nodes revealed well-defined elliptical hyperechoic masses with an echogenic line and posterior shadow on ultrasonography. The appearance of a high density mass on computed tomography with a computed tomographic value lower than that of calcification may be a reliable finding of metastasis because it demonstrates the presence of marked keratinization produced by squamous cell carcinoma.

Application of fuzzy reasoning in an expert system for ultrasonography.

OBJECTIVES: To apply fuzzy reasoning in a computer-assisted diagnostic system for ultrasonography and to evaluate its clinical utility as a diagnostic aid for the unskilled clinician. METHODS: A computer-assisted diagnostic system for ultrasonography was constructed to differentiate metastatic from inflammatory lymph nodes. The pattern of a skilled clinician's considerations was simulated with the fuzzy expert system. Three fuzzy production rules were set up according to the diagnostic criteria for lymphadenopathy. Each rule was transformed into a membership function. The max-min composition method was used for inference and the centroid method for defuzzification. The system was tested with 20 clinicians who were one to three years after graduation and inexperienced in ultrasonography. Accuracy, sensitivity and specificity were evaluated. RESULTS: The average increase in accuracy was 8.5%, and the sensitivity and specificity 10.7% and 6.4% respectively. These scores were significant for accuracy (P = 0.005), sensitivity (P = 0.02) and specificity (P = 0.036). CONCLUSIONS: Application of fuzzy reasoning in an expert system for ultrasonography improves the diagnostic performance of inexperienced clinicians.

Two-dimensional gray-scale clustering for texture analysis.

OBJECTIVES: To develop a new quantitative method for the visual discrimination of image texture. METHODS: Two kinds of image phantoms were prepared, one for evaluating the effects of change in size and gray values of individual pixels (primitives) on perceived coarseness and the other for evaluating changes in groups of pixels (clusters) on perceived heterogeneity. The phantom images were displayed on a CRT and presented to 11 observers who assessed heterogeneity and coarseness on a 10-point scale between -5 and +5. On the basis of the observers' results, a new texture analysis method termed two-dimensional gray-scale clustering analysis was developed and applied to measure quantitatively the texture of the phantoms. The results obtained were then compared with those of the visual evaluation. RESULTS: The size of the primitives and the clusters greatly affected the visual evaluation of heterogeneity and coarseness. Changes in the gray value had only a slight effect. The intra-observer variation for heterogeneity was significantly larger than that for coarseness. Two-dimensional gray-scale clustering analysis could differentiate heterogeneity from coarseness. A high correlation was obtained between the visual evaluation and the quantitative data. CONCLUSION: Quantitative two-dimensional gray-scale clustering analysis appears to be a useful means of texture analysis.

Computed tomography of carcinoma of the upper gingiva and hard palate: correlation with the surgical and histopathological findings.

OBJECTIVES: To clarify the diagnostic utility of CT in the evaluation of carcinoma of the maxillary gingiva and hard palate. METHODS: The CT scans of 27 patients with squamous cell carcinoma of the upper gingiva and hard palate were reviewed. Tumor extent and bone destruction were compared with the surgical and histopathological findings to estimate the sensitivity, specificity and accuracy of CT. RESULTS: The primary tumor was detected by CT in 89% of patients. The grade of bone destruction determined by CT correlated well with that verified at surgery or by histopathological examination. The sensitivity, specificity, and accuracy of CT for invasion to the buccal mucosa were 64%, 89% and 75% respectively. For maxillary sinus invasion, CT showed low specificity but high sensitivity. The accuracy was 86% when the criterion for sinus invasion was the presence of an enhanced mass continuous with the primary tumor. CONCLUSION: CT is helpful for the evaluation of tumor extent in the upper gingiva and hard palate carcinoma. However, invasion of the maxillary sinus should be considered carefully, because CT findings suggestive of destruction of the floor of the maxillary sinus are not always consistent with sinus invasion.

Ultrasonographic texture characterization of salivary and neck masses using two-dimensional gray-scale clustering.

OBJECTIVES: To evaluate the clinical usefulness of texture analysis in ultrasonography (US) of head and neck masses. METHODS: Sixty-one cases with salivary or neck masses were examined by US with a linear 7.5MHz array. Images were digitized and quantitatively evaluated by two-dimensional gray-scale clustering using two texture indices, M and SD, as measures of coarseness and homogeneity respectively. RESULTS: Coarseness of Warthin's tumors was significantly higher than that of the other salivary tumors. Malignant lymphomas had finer texture values than other lymph node lesions. CONCLUSIONS: Quantitative texture analysis appears to be a useful complement in diagnostic US.

Activity of keratinocyte growth factor-like substance extracted from rabbit liver on renal tubular cell growth during compensatory renal hyperplasia.

BACKGROUND: In response to unilateral nephrectomy, the rabbit liver transiently produces 2 growth regulators for cultured renal cortical tubular cells: a tubular cell growth factor and a growth inhibitor. We report on the effects of the tubular cell growth factor on a variety of cell lines. METHODS: The tubular cell growth factor activity was partially purified from the rabbit liver by using gel filtration and ion-exchange chromatography. The activity was monitored by the incorporation of iododeoxyuridine into DNA of cultured cells. Expression of the fibroblast growth factor receptor-2 in the rabbit kidney was determined by the immunoblot analysis. RESULTS: This growth factor stimulated the DNA synthesis in LLC-PK1 cells, LLC-RK1 cells, and human keratinocytes. It did not affect the growth of BS-C-1 cells, MDCK cells, BALB/c 3T3 fibroblasts, or rat parenchymal hepatocytes. The additive effect of this factor on the DNA synthesis of cultured tubular cells maximally was stimulated by insulin-like growth factor-I, basic fibroblast growth factor, and epidermal growth factor, but was not stimulated by keratinocyte growth factor. The amount of this activity also increased in the liver after sham operation. In the days after surgery, expression of fibroblast growth factor receptor-2, which includes the keratinocyte growth factor receptor, was down-regulated in the kidneys of both uninephrectomized and sham-operated rabbits. CONCLUSION: These findings indicate that tubular cell growth factor in the liver seems to be a keratinocyte growth factor, and acts in an endocrine manner in renal tubular hyperplasia.

Basic fibroblast growth factor stimulates phosphatidylcholine-hydrolyzing phospholipase D in osteoblast-like cells.

We examined the effect of basic fibroblast growth factor (bFGF) on the activation of phosphatidylcholine-hydrolyzing phospholipase D in osteoblast-like MC3T3-E1 cells. bFGF stimulated both the formations of choline (EC50 was 30 ng/ml) and inositol phosphates (EC50 was 10 ng/ml). Calphostin C, an inhibitor of protein kinase C (PKC), had little effect on the bFGF-induced formation of choline. bFGF stimulated the formation of choline also in PKC down regulated cells. Genistein and methyl 2,5-dihydroxycinnamate, inhibitors of protein tyrosine kinases, significantly suppressed the bFGF-induced formation of choline. Sodium orthovanadate, an inhibitor of protein tyrosine phosphatases, enhanced the bFGF-induced formation of choline. In vitro kinase assay for FGF receptors revealed that FGF receptor 1 and 2 were autophosphorylated after FGF stimulation. bFGF dose-dependently stimulated DNA synthesis of these cells. These results strongly suggest that bFGF activates phosphatidylcholine-hydrolyzing phospholipase D through the activation of tyrosine kinase, but independently of PKC activated by phosphoinositide hydrolysis in osteoblast-like cells.
