[Clinical studies with a new non-ionic contrast medium (iopamidol) in intravenous pyelography in aged patients].

Iopamidol (IOPAMIRON 300) was examined for excretory urography on 21 patients, between 70 and 94 years old, with reference to visualization, safety and urinary excretion. The results about visualization and safety were compared with those obtained with 60% Urografin. Iopamidol was demonstrated visualization nearly equivalent to that with Urografin, and 19% better pyelogram. Especially, calix showed a clearer contrast than those obtained with Urografin. None of the 21 old patients experienced any side effects or adverse reactions. No abnormalities were noted in serum chemistries. Iopamidol was not quickly excreted into the urine, only 19% during the first 2 hours, in elder patients.

[Clinical study of primary carcinoma of the fallopian tube: experience with 14 cases].

Between 1950 and 1986, 14 cases of primary carcinoma of the fallopian tube were treated and diagnosed at the Cancer Institute Hospital. These cases constituted 0.13% of the total number of gynecologic malignancies at the hospital during the period. The clinical/pathological findings and prognoses were described. Of the 14 cases, the average age was 56.0 years. The most frequent symptom was atypical genital bleeding, seen in 11 cases (79%). Massive watery discharge was seen in four cases (29%). In preoperative cytologic examination of vaginal smears, six cases (43%) were positive for cancer. All cases underwent operation as therapy. Postoperative irradiation, adjuvant chemotherapy, and/or second-or third- look operation was also used. Histopathologically, all materials were found to be adenocarcinoma. Four cases were well differentiated, seven were moderately differentiated, and three were poorly differentiated. Two patients with stage III and IV cancers died of the disease. Nine patients were still alive at the end of this study. The five-year survival rate was 57% (4/7). In stage I cancers, the five-year survival rate was 80% (4/5). The prognosis of stage I cancer patients was estimated as rather good.

[Optical internal urethrotomy after traumatic disruption of the urethra].

We report 4 cases of complete traumatic disruption of urethra treated by endoscopic management, core-through optical internal urethrotomy. A direct vision urethrotome was used to incise the totally obliterated urethra with transvesical endoscopic guidance and digital rectal or perineal control. Postoperatively, the patients required an average of 1.3 subsequent urethrotomies. The results were satisfactory in three cases and one case was under observation. All patients were continent and three of them were potent. We suggest that the endoscopic approach should be considered as an initial procedure for restoring urethral continuity prior to more extensive surgical repair.

[Staging pelvic lymphadenectomy for carcinoma of the prostate: review of 23 cases].

Between June, 1982 and May, 1985, 37 patients underwent pelvic lymphadenectomy for carcinoma of the prostate, 23 patients as an independent procedure. Of these 23 patients 7 had pelvic node metastasis. Node involvement occurred in 60% with clinical stage C tumors. Correlation with tumor grade revealed nodal metastasis in none of the latients with well differentiated tumors, 33.3% of those with moderately differentiated lesions, and 75% of those with poorly differentiated tumors. Metastatic disease most commonly involved the obturator-hypogastric lymphnodes (85.7%). Of the 23 patients, 14 underwent conventional node dissection, and 9 limited node dissection. Complications of lymphocele, lymphfistula, penoscrotal edema and lower extremity edema occurred mostly in patients with conventional node dissection. Limited staging pelvic node dissection provides information similar to that identified after more extensive dissection and can be accomplished with little morbidity.

[Clinical statistics on inpatients and operations in the Department of Urology, Toyooka Public Hospital (from January 1982 to December 1985)].

Statistical observations on inpatients and operations in our department from 1982 to 1985 were reviewed in comparison with the statistics for the preceeding three years. More elderly persons comprised the inpatients at our hospital than at other facilities. Urogenital tumors and urolithiasis were the predominant disease of the inpatients, as they were in the preceeding three years. Number of prostatic cancer has remarkably increased. Transurethral operations, TUR-P and TUR-Bt, were the most representative. Operations for the malignant disease, especially for prostatic cancer, were more frequent in this period than in the preceeding three years. As to urinary diversion, ileal conduit was the main procedure used during this four years. Kock continent ileal reservoir was adopted in 1985. In July 1985, open operations for urolithiasis were dramatically replaced by percutaneous and transurethral procedures.

[Computerized tomography in the diagnosis and follow-up of retroperitoneal fibrosis].

We report 4 patients with surgically proved idiopathic retroperitoneal fibrosis. Retroperitoneal fibrosis was diagnosed correctly in 3 of the patients by computerized tomography. On the computerized tomogram, retroperitoneal fibrosis demonstrated a characteristic soft tissue mass enveloping the vessels and ureters. In 2 patients, the follow-up computerized tomogram revealed spontaneous resolution of the mass in the retroperitoneal space. Computerized tomography is one of the most useful methods in the preoperative evaluation and postoperative follow-up of this disease.

Cold hardiness in various organs and tissues of Rhododendron species and the supercooling ability of flower buds as the most susceptible organ.

The freezing resistance of various organs and tissues was determined in 24 Rhododendron species (mainly Subgenus Tsutsutsi) having different ecological distributions. The order of hardiness for organ or tissue is as follows: leaf bud greater than wood greater than or equal to bark greater than flower bud, and the flower bud is characterized as the most cold-susceptible organ. The relationship of killing temperature (KT) to northern distribution was the most significant in leaf buds compared to other organs and tissues. KTs of leaf buds for the most hardy species were -45 degrees C (or below) and those for the most tender species were about -23 degrees C, while KTs of flower buds were about -28 degrees C for the former and -16 degrees C for the latter. Although KTs of flower buds native to southwestern Japan were well correlated with the exothermic temperature distribution (ETD) of florets, those in the more northern species were generally lower than ETDs. The supercooling ability of flower buds appears to be sufficient to avoid the freezing stress since the extreme minimum temperature (EMT) at the northern limit of natural distribution for each tree species examined was not lower than the KT and ETD of the flower buds.

[The effect of prostaglandin F2 alpha on the gastrointestinal movement after urological surgery].

To facilitate postoperative flatus, Prostaglandin F2 alpha (PGF2 alpha) was given intravenously to 23 patients who underwent urological operations. The patients were 14 males and 6 females aged from 20 to 77 years old. Patients with hypertension or cardiovascular disease were not included. Twelve operations were performed under general anesthesia, and 8 under epidural anesthesia. Thirteen operations were performed for the upper urinary tract or adrenal gland, and 5 were for the lower urinary tract. In 2 cases, the peritoneal cavity was opened and operations were performed on the intestines. PGF2 alpha 2000 micrograms was added to the postoperative drip infusion and administered in 2 to 3 hours. Until the first flatus was recognized, PGF2 alpha was given once a day in the same manner. Twenty-six patients, 10 of whom were given either vagostigmine or pantothen postoperatively, served as the control group. PGF2 alpha accelerated the postoperative flatus by 8.7 hours (mean) compared with the control group, but it was not significant. The onset of flatus was significantly promoted under epidural anesthesia. Gastrointestinal movement tended to be facilitated in the PGF2 alpha group after lower urinary tract surgery and in the patients over 50 years old. Three patients complained of severe abdominal pain as a side effect; and, injection of PGF2 alpha was stopped. In 7 patients, mild stomachache , vascular pain, nausea, vomiting or elevation of blood pressure were observed.

[Total cystectomy for bladder tumor: eight-year experience].

Between 1975 and 1982, 39 patients underwent total cystectomy and urinary diversion at our hospitals. The type of urinary diversions were ileal conduit (32 cases) and ureterocutaneostomy (7 cases). Preoperative irradiation was used in 10 patients. The postoperative mortality rate was 2.6%. Early complications occurred in 38.5% and included wound infection, acute pyelonephritis, intestinal obstruction, pelvic infection, intestinal leakage and/or medical complications. Ureteroileal stricture was most frequent in late complications. Over-all relative 1-, 3-, and 5-year survival rates were 92.4, 56.6, 41.7%, respectively. Survival was dependent on the stage and the grade of the tumor. No significant difference was found between the older (greater than or equal to 65 years old) and the younger (less than 65 years old) groups.

Prolapsed mitral cusps in atrial septal defect. An erroneous radiological interpretation.

Forty patients with simple atrial septal defect had left ventriculograms using cineangiography and/or Elema or Sircam still films. All had open heart surgery and careful scrutiny of the mitral valve by experienced surgeons. Prolapse of the posterior cusp was diagnosed from angiography in 28 patients but was confirmed at operation in only 5. Another 5 had a different mitral valve abnormality and the remaining 18 had no clinical signs to suggest mitral valve dysfunction after operation. It is concluded that though true ballooning or prolapse of the mitral cusps may coexist with simple atrial septal defect, its presence is overdiagnosed from the appearances of left ventriculography on anteroposterior, lateral, and right anterior oblique views. Left ventriculography was equally unreliable in the exact recognition of other anatomical abnormalities of the mitral valve in secundum defect, but accurate when the mitral valve was pronounced to be structurally normal.

[Health effect of long-term diet mercury contaminated tuna. Part II. Accumulation and retention of mercury and selenium in organs and clinical symptoms (author's transl)].

Health effects of long-term tuna diet in cats were examined. The cats were fed daily with three kinds of tuna, containing different concentrations of mercury and selenium. The following results were obtained. 1) A high correlation was noted between the amount of fish intake and the mercury level in hair and blood samples of cats. 2) Mercury levels in organs were elevated corresponding with the amount of tuna consumption, but the distribution of mercury in organs of cats exposed to mercury in tuna flake differed from that in tuna fresh meat. In the liver, the ratio of methyl mercury to the total mercury was very low with an average of 11.5% of the ratio. 3) With regard to selenium, there was no correlation between intake of selenium and selenium levels in the cerebrum, celebellum, and kidneys, exept in the liver (r=0.766). 4) None of the cats had definite methyl mercury poisoning, but some slight disturbances were noted in several cats.
