Anaesthetic management of insulinoma.

The anaesthetic management of two cases of insulinoma treated by distal pancreatectomy is presented. Stress is laid on pre-operative and intra-operative glucose administration in sufficiently high quantity. Pre-operative corticosteroid therapy is better avoided. Dextrose infusion should be stopped soon after removal of the tumour to prevent excessive hyperglycemic response in the postoperative period. Methyoxyflurane was used because it tends to raise the blood sugar level. Pancuronium bromide was preferred because of the stable cardiovascular conditions it produces.

Further studies of the anti-recall effect of lorazepam: A dose--time--effect relationship.

The time of onset and duration of the anti-recall action of lorazepam were assessed under clinical conditions by measuring recall and recognition of visual stimuli 24 hours after intravenous administration of lorazepam. The visual stimuli were first presented 5-240 minutes after 2 mg and 5-360 minutes after 4 mg lorazepam. Retrograde amnesia was not produced. Lorazepam, 2 mg, produced a short anti-recall effect (anterograde amnesia) in 50 per cent of the cases, with a latency of 30 minutes and a duration of less than half an hour. Duration and frequency of the anti-recall effect were greater after 4 mg, while the latency was shorter. More than 70 per cent of the individuals tested were amnesic for the visual stimuli 15 minutes to 4 hours after 4 mg lorazepam. Sedation was satisfactory and long-lasting following both doses of lorazepam, but was not related to the anti-recall effect.

Anterograde amnesic effects of pethidine, hyoscine and diazepam in adults.

1. The intravenous administration of 5 and 10 mg of diazepam caused anterograde amnesia in 50 and 90% of adults, the peak effect occurring in 2-3 min and action persisting for 20-30 minutes.2. Hyoscine (0.4 and 0.6 mg) caused amnesia in 35 and 50% of patients with peak effect not occurring until 50-80 min after injection and action persisting for at least 120 minutes.3. With neither drug was there any relationship between the incidence of amnesia and the degree of drowsiness.4. Amnesia was not observed after saline or after 50 mg pethidine.5. On questioning 6 h after a short operation many patients had no memory of an object which they clearly recognized and described 1 h after surgery.
