Treatment of femoral neck fractures with a cancellous screw and fibular graft.

Twenty-six patients with femoral neck fractures were treated by open reduction, cancellous screw fixation and free fibular grafting. The patients were between 14 and 50 years of age. There were 16 old and 10 fresh fractures. Four patients had radiological signs of avascular necrosis before the treatment was instituted. Bony union was achieved in all patients except one, where the failure occurred because of a technical error. The patients were followed up for at least two years. No new case of avascular necrosis was detected after treatment, and there was clinical and radiological improvement in all four patients with pre-operative avascular necrosis.

Acquired male urethral diverticulum.

Four cases of acquired urethral diverticula in male subjects are reviewed. All patients presented with complications such as urinary tract infection, urethral fistulas and stone or sequestrum formation in the diverticulum. The recommended treatment is excision of the diverticulum and repair of the urethra. The relevant literature has been reviewed.

Selective approach to repair of ureteropelvic junction obstruction: results of 126 pyeloplasties assessed on clinical and radiological bases.

Record-files of 126 pyeloplasties of different types performed on 119 patients (7 bilaterally), between 1966 and 1989, have been reviewed in an attempt to illustrate the selective approach to hydronephrosis due to ureteropelvic junction obstruction. Clinical success was achieved in all cases. A new grading system for assessing urographic impairment has been suggested. Radiological success was obtained in 123 (97.6%) renal units. The near total success is attributed to the theme of selective approach.

Fine needle aspiration cytology of a lipoblastoma.

A lipoblastoma, an uncommon tumor of childhood that can be mistaken for a liposarcoma, was preoperatively diagnosed by fine needle aspiration cytology. The characteristic features on the cytologic smears were the presence of immature fat cells in the form of spindle-shaped cells, stellate cells and vacuolated lipoblasts along with lipocytes. The cytologic diagnosis was confirmed by histologic study of the excised tumor.

Comparative study of knee function after patellectomy and osteosynthesis with a tension band wire following patellar fractures.

In order to assess the relative merits of patellectomy and osteosynthesis with tension band wires in patellar fractures, 66 patients given patellectomy and 30 given tension band wiring for almost identical fractures of the patella have been analysed. Following patellectomy only about half the patients achieved excellent results while after osteosynthesis excellent results were in 80%. We, therefore, recommend that whenever possible patellar fractures should be treated by accurate reduction and tension band wiring. When needed patellectomy should be performed without hesitation as the operation produces a negligible number of poor results, although excellent results will be achieved in only about half the cases.
