Proliferative glomerulonephritis in monoclonal gammopathy.

A 62-year-old white male suffering from plasmacytosis with monoclonal gammopathy developed gastrointestinal bleeding, microscopic hematuria, proteinuria and progressive azotemia. A renal biopsy demonstrated capillary wall thickening, mesangial hyperplasia, crescent formation and subepithelial humps. Serum immunoglobulin analysis showed IgG of lambda type. Serum complement, ANA, ASO titer and cryoglobulin were normal. It is suggested that proliferative glomerulonephritis with subepithelial humps should be added to forms of renal pathology that can be seen in plasma cell dyscrasias.

Renosonogram in diagnosis of renal failure.

Twenty patients (10 males, 10 females age 20-60 years) with normal renal function underwent standard IVP and renosonogram. Length of kidney in its longest axis on IVP and sonogram was 13.6 +/- 1.43 cm, and 11.76 +/- 1.61 cm respectively. The ratio of the length of kidneys on IVP and sonogram respectively. There was significant correlation between these two factors (r=.95, P less than .001). When these ratios were analyzed individually for right and left slides it was found that ratios on IVP for the right kidney were 4.05 +/- .29, and for the left kidney 4.21 +/- .37. The ratios on the sonogram were for the right kidney 3.45 +/- .33 and for the left kidney 3.67 +/- .42. Using these ratios expected length of renal shadows on sonogram were calculated in 15 patients with severe renal failure (mean creat. 13.79 mg/dl). In ten patients in whom the infusion IVP visualized the renal shadows, the sonographic findings were confirmed. In 5 patients where infusion IVP was not helpful renosonogram revealed hydronephrosis in three patients, a retroperitoneal mass in another and cross-ectopia in a third patient. The results of this study indicate renosonogram is a useful procedure in evaluation and management of patients with renal failure with no demonstrable toxicity and requiring little preparation.

Biliary excretion of copper, manganese, and horseradish peroxidase in Eisai hyperbilirubinemic mutant rats (EHBRs) with defective biliary excretion of glutathione.

A mixture of copper (Cu) (0.38 mg/kg), manganese (Mn (0.038 mg/kg), and horseradish peroxidase (HRP) (5.0 mg/kg) was injected intravenously (i.v.) into mature Eisai hyperbilirubinemic rats (EHBRs) and Sprague-Dawley rats (SDRs). Bile was collected at 10-min intervals before and after the injection, under anesthesia. The liver, kidneys, and blood were removed 40 min after the injection. The serum conjugated bilirubin concentration was 0.85 mg/dL in the EHBRs, but was below detection limits in the SDRs. The bile-reduced glutathione (GSH) concentration was much lower in the EHBRs (0.04 mg/mL) than in the SDRs (1.30 mg/mL). However, the hepatic GSH concentration was about 1.6 times higher in EHBRs (2.26 mg/g liver) than in SDRs (1.43 mg/g liver). The low excretion of biliary GSH was not caused by the activity of GGT in the liver, since there was no significant difference in the activity between the two groups (5.8 +/- 3.4 and 4.6 +/- 2.4 mumol p-nitroaniline/g protein/30 min in SDR and EHBR groups, respectively). There was a delay of initial biliary excretion of Cu in EHBRs compared to SDRs. The biliary concentration of Mn was slightly lower in EHBRs than in SDRs. Forty min after the injection of metals, however, there was no difference between hepatic concentrations of the two metals in the two groups. Our results suggest that abnormal deposition of the two metals is not observed naturally in EHBRs. Injected HRP was excreted rapidly and notably in the EHBRs compared to SDRs. Furthermore, the biliary concentration of beta-N-acetyl-D-glucosaminidase (beta-NAG) was significantly higher in EHBRs than in SDRs, Rapid biliary excretion of Cu, but not of Mn, may be related to the hepatobiliary transport of GSH, but the transport and lysosomal function do not originally regulate the biliary excretion of Cu.

Ribosomal DNA fragments enhance the stability of transfected DNA in Entamoeba histolytica.

Genetic manipulation of Entamoeba histolytica is limited by the inability to express foreign genes at high levels. We tested whether sequences from the E. histolytica rDNA episome, present in 200 copies per cell, could act to stabilize the episomal transfection vector pTCV1. Ligation of the rDNA transcription unit, or sequences downstream of the rDNA transcription unit, increased pTCV1 copy number and stability and conferred additional zones of DNA replication. Sequences upstream of the rDNA transcription unit dramatically destabilized pTCV1. These experiments give additional insights into the mechanism of DNA replication and provide for E. histolytica a set of transfection vectors with unique properties.
