[Effects of new dental amalgam fillings in pregnancy on Hg concentration in mother and child. With consideration for possible interactions between amalgam and precious metals].

Amalgam-tooth fillings are suspected to be cause of many different diseases. With special consideration of the problem of newly made fillings, blood samples were taken from 45 affected pregnant women. After birth, newborns and placenta tissue were tested for Hg-contamination. One hundred and twenty one women with older amalgam-fillings were compared to 19 women without amalgam-fillings. The surfaces of the amalgam-fillings were measured in each case in mm2. In 58 women the tooth fillings contained additional precious metal. While in the placentas a retaining function were found of up to ten times higher Hg-values but low correlations to the amalgam surfaces, the Hg-concentrations in the blood samples of mother and child did not correlate to the amalgam-fillings. Newly made fillings during pregnancy had no influence on the Hg-concentration, neither had the contact to precious metals in the approximal and the antagonistic range. The wide-spreading of Hg-values without correlation to the filling-surfaces speaks for a great influence of other environmental factors.

[Thermal stress of the inner ear during laser stapedotomy. I: Continuous-wave laser].

As a consequence of perforating the footplate during laser stapedotomy, direct radiation to the inner ear will warm perilymph and adjacent structures. To determine the possible thermal dangers to cochlear structures from different laser parameters, heat transport mechanisms, temperature increases and temperature fields were investigated in a model system approximating caloric and physiologic changes in the inner ear. The temperature-time course of local cochlear warming showed a rapid convection-dependent increase that reached a peak at about the end of the laser impulse. An increase in power density caused an elevation of the temperature in all laser systems used. Maximum temperatures varied widely for CO2 lasers at the same wave-length, but a different beam-time behavior was found at a distance of 2 mm behind the perforation by using low-power densities. Heat values were lowest at a pulse duration of 50 ms in the superpulse (< 5 degrees C) and continuous wave (cw) modes (< 9 degrees C), while the highest value was found in the pulser mode (to 21 degrees C). After argon laser irradiation at high-power densities, temperatures were nearly independent of location (5.5-13 degrees C). When considering risks of possible inner ear damage from thermal stress during laser stapedotomy, application of the CO2 superpulse and cw laser appears to be safe over a large power-density range. Low energies using a small-beam diameter and short pulse durations (50-100 ms) are recommended. In contrast, use of the CO2 laser in a pulser mode may result in inner-ear damage because of the high temperatures produced. Structures located at a greater distance can be endangered by direct irradiation with the argon laser.

Technique and indications of extended sublabial rhinotomy ("midfacial degloving").

Midfacial degloving is an extended sublabial rhinotomy, which permits good bilateral access to the nasal and paranasal cavities, the base of the skull, and the clivus. The incisions leave no visible scars. The access is suitable for large benign tumours (such as nasopharyngeal fibromas or inverted papillomas), but can also be used for malignant tumours. It can be enlarged by supplementary incisions to meet the demands of tumour surgery.

Autogenous auricular concha cartilage transplant in corrective rhinoplasty. Practical hints and critical remarks.

Numerous materials have been suggested for correction of nasal defects, especially for the saddle nose. We present here our experience with the autogenous cartilage transplant from the auricular concha in our collective of 32 patients. The concha cartilage is used for saddle nose corrections and reconstruction of the alar cartilages. Auricular concha cartilage is an almost ideal transplant material for corrective rhinoplasty because: 1. Harvesting of the material is a low-risk procedure that is not time-consuming and can be performed under local anaesthesia. 2. Concha cartilage is stable enough for support and elastic enough for contouring. 3. It can easily be shaped as desired. 4. Concha cartilage shows little tendency towards dislocation. 5. Resorption is negligible and thus plays no role in connection with these transplants. 6. Rejection or infection rarely occurs.

Diagnosis of nasal allergy to the house dust mite.

Twenty-five patients with perennial rhinitis and a positive skin prick test (SPT) for Dermatophagoides pteronyssinus (Dp) and Dermatophagoides farinae (Df) were submitted to nasal provocation and the radioimmunosorbent test (RAST) for specific IgE-antibodies. We found a significant correlation in the reaction to both allergen extracts for all parameters examined. In addition, there was a significant correlation among the SPT, the RAST and the nasal provocation for Dp and between the SPT and the RAST for Df. In patients with perennial rhinitis we recommend the combination of all three methods to differentiate unspecific rhinitis from an allergic rhinitis. Only the patients with proved allergic rhinitis could benefit from a specific hyposensitisation.

Adamantiades-Behçet's disease with inner ear involvement.

Adamantiades-Behçet's disease is a chronic recurrent inflammatory disorder involving the small and large vessels. Typical loci of manifestations are the mucous membranes, skin and eyes, as well as the joints and central nervous system. Other organs are not commonly involved. We present two patients, one with ocular and the other with mucocutaneous manifestation of Adamantiades-Behçet's disease. In addition, the first patient reported three episodes of sudden hearing loss while under immunosuppressive therapy for his eye involvement. The second, therapy-naive patient complained of tinnitus in his left ear. Careful examination revealed vestibular involvement in the first patient and retrocochlear involvement in the second. Inner ear involvement is an uncommon manifestation of Adamantiades-Behçet's disease. In case of relevant signs or history, such as hearing disturbance, tinnitus and/or vertigo, patients should be examined for inner ear involvement.

[Possible applications of a method for 3D reconstruction of CT image data. OP-planning, finishing of individual alloplastic implants for replacement of cranial and maxillofacial bone structures].

A method for the three-dimensional reconstruction of CT-image data permits manufacturing of natural size anatomical models thus enabling preoperative measurements of relevant parameters (length, areas, and volumes). The methods described allow for new perspectives in preoperative planning of dental implants, particularly for the maxillary problem area. The use of 3D image data for manufacturing individual alloplastic implants simplifies the exact reconstruction of symmetrical bone structures, which up to now was impossible to achieve with conventional methods. Applications in the fields of orthodontic surgery and in neurological surgery are suggested. Refinement of available techniques is possible by means of additional segmentation of soft tissues, and by implementation of more sophisticated software for better visualisation and smoothly shaped surfaces. Future applications may even include aesthetic surgery.
