Reduction of blood loss by triglycyl-lysine-vasopressin during early excision of burns in pigs.

In a burn model using anesthetized piglets, the circulatory and renal effects and the blood loss during and after excision were compared in two groups, one treated by triglycyl-lysine-vasopressin (TGLVP) given as intermittent bolus injections and the other serving as control. The third degree burn covered approximately 33% of the body surface area, and a solution containing 100 mmol NaCl/l in 2.5% glucose was infused at a rate of 2.4 ml/kg/% burn. Regional blood flows were determined before burn, 5 and 24 h after burn with the aid of radioactively labeled microspheres. Cardiac output (CO) decreased about 30% after burn and there was a slight further decrease of CO after the first TGLVP dose. After the excision there were similar restitutions of CO in the two groups. TGLVP induced an increase of blood flow to the hepatic artery, did not change flows to the brain, heart and kidneys and decreased flows to the preportal organs, carcass and skin 5 h after burn. After 24 h the perfusion of the skin and pancreas remained low but there were no differences in other organs. There was a significant decrease of blood loss associated with burn excision performed 5 h after burn in the TGLVP treated group, mean blood loss 39 g/25 kg versus 145 g/25 kg in the control group. There was a temporary increase in diuresis following TGLVP and also increased excretions of sodium and potassium. The results obtained in this study and possible clinical applications are discussed.

Analytical, occupational and toxicologic aspects of the degradation products of polypropylene plastics.

Thermooxidative degradation of polypropylene (PP) at close to the industrial processing temperatures was studied with thermogravimetric analysis, infrared spectroscopy, and gas chromatography-mass spectrometry (GC-MS). GC-MS allowed identification of 47 volatile degradation products. Formaldehyde, acetaldehyde, alpha-methylacrolein, acetic acid, and acetone were the major products. Antioxidants markedly slowed down the degradation of polypropylene and the evolution of the degradation products. The relative amounts of the oxidized products were mostly independent of the degradation temperature or the type of antioxidant. Measurements in the plastics industry revealed low airborne concentrations of individual volatile products. The concentration of aerosols, which infrared analysis showed to resemble paraffin fumes, was significant and probably the most important hygienic hazard. The biochemical parameters (the consumption of protecting sulfhydryl groups and the effects on foreign compound metabolism) indicated that the degradation products are biologically reactive, affecting the balance of normal metabolism in exposed animals.

Roentgen stereometry with the aid of metallic implants in hemifacial microsomia.

Displacement of the mandible and the maxilla with growth in relationship to the frontal bone was recorded in eleven children with hemifacial microsomia, aged 3 to 14 years at the initial examination. The goals were to assess the efficiency of the method for accurate recording of articular growth and to obtain information on facial development in each of these children. Correction of the skeletal asymmetry was not attempted during observation periods of from 707 to 1,484 days. In continued observations three of the children were treated with functional appliances. Roentgen stereometry with the aid of metallic implants proved to be an efficient method for highly accurate recordings of articular facial growth in relationship to the frontal bone. Displacement of the jaws was asymmetrical in all of the examined children as determined in the frontal and transverse planes of the head. In about 50 percent of the children the degree of asymmetry increased in one plane while it decreased in the other plane. No correlation was found between the extent of the mandibular deformity, as seen on orthopantomograms, and the displacements of the mandible. Generally, the displacement of the maxillary bones corresponded with the displacement of the mandible. Articular growth of the jaws was redirected toward a more favorable pattern in two of the children during the time they used a functional appliance. In the third child the use of a functional appliance had no such effect.

Laser Doppler flowmetry and transcutaneously measured carbon dioxide tension for observing changes of skin blood flow in fingers.

Laser Doppler flowmetry (LDF) and transcutaneous carbon dioxide tension measurements (PtcCO2) were performed on fingers of healthy volunteers subjected to a sequence of hydrostatic and thermal stimuli. LDF response using an unheated probe agreed well with measurements made with Xenon133 clearance and strain gauge plethysmography in a previous work. The PtcCO2 results, however, were found not in any way to reflect changes in skin blood flow.

Posteroanterior traction in maxillonasal dysplasia (Binder syndrome). A roentgen stereometric study with the aid of metallic implants.

The study was undertaken to examine the effect of posteroanterior traction in an 11-year-old boy with maxillonasal dysplasia. Movement of the maxillary bones and of the mandible was recorded, by means of roentgen stereophotogrammetry, in relationship to the frontal bone during and after treatment. In the examined child face-mask therapy mainly influenced the position of the mandible while the recorded advancement of the maxillary bones was slight (0.6 mm.). It is possible that the limited maxillary response to traction may be due to insufficient growth capacity of the circummaxillary sutures in a child with maxillonasal dysplasia. While movement of the maxillary bones and of the mandible during traction conformed with the pattern of treatment effect described by Delaire a total maxillary relapse occurred in the posttreatment observation period (no retention), possibly in adaptation to the retropositioned mandible. In our patient, posteroanterior traction accomplished improvement of the maxillary retrusion in relationship to the mandible only.

Bacteriologic and serologic investigation in diffuse sclerosing osteomyelitis (DSO) of the mandible.

An attempt was made to clarify the obscure origin of diffuse sclerosing osteomyelitis (DSO) of the mandible. Bacteriologic investigations of specimens from diseased mandibles were performed with special attention to anaerobic culture technique. Propionibacterium acnes and Peptostreptococcus intermedius were in some patients found to be of etiologic importance. Sampling techniques and associated problems are discussed. An examination was made for antibodies in the patients' sera against antigens prepared from bacterial isolates. ASTA, IgM, IgG, and IgA were determined. Lymphocyte-stimulation tests were performed. The inflammatory events and chronicity of the disease could not be explained by the immunologic findings.

On the relation between transcutaneous oxygen tension and skin blood flow.

Transcutaneous oxygen tension (pTcO2) varies with pAO2, circulatory parameters, skin metabolism and oxygen diffusion. The necessity to heat the skin beneath the electrode to allow transcutaneous oxygen diffusion limits the use of pTcO2 in the study of skin blood flow. The oxygen measured probably emanates from the capillaries of the superficial dermis. A large number of arteriovenous shunts in the skin may modify the pTcO2 response. The heat-dilated vessels beneath the electrode behave passively in most circumstances. Differences in local blood temperature with secondary shifts in the release of oxygen from oxyhemoglobin may affect pTcO2 at high and low flow rates respectively. pTcO2 may vary with the perfusion pressure and possibly also with the resistance of the blood vessels outside the area heated by the electrode, but only at low flow rates.

Growth in hemifacial microsomia studied with the aid of roentgen stereophotogrammetry and metallic implants.

Displacement of the mandible and of the maxillary bones with growth was studied in 11 children with hemifacial microsomia. The subjects were from three to 14 years of age at the initial examination and were observed over a period of 350 to 1218 days. Roentgen stereophotogrammetry with the aid of metallic implants proved to be an efficient method for highly accurate recordings of growth relative to the frontal bone. Complicated movements of the jaws occurred and either increased or decreased the degree of facial asymmetry. No correlation could be found between the extent of the mandibular deformity as seen on orthopantomograms and the displacement of the mandible with growth. Generally, the displacement of the maxillary bones corresponded with the displacement of the mandible.

Movement of maxillary segments after expansion and/or secondary bone grafting in cleft lip and palate: a roentgen stereophotogrammetric study with the aid of metallic implants.

The study was undertaken to examine the effects of expansion treatment and secondary bone grafting in cleft lip and palate. Movement of the lateral maxillary segments in four patients with complete bilateral cleft lip and palate (BCLP) was complicated and asymmetric, and a greater relative widening of the dental arch was observed. The least expansion was found in the oldest patient. Bone grafting did not bring about a stable relationship between the segments. The width of the dental arch was mostly maintained despite segmental movement. In one patient with unilateral cleft of the lip and primary palate (UCL) growth in the midpalatal suture followed the pattern observed in noncleft patients but was less extensive. Accurate and comprehensive recording of the relatively small maxillary movements requires that implant stability is checked and that a reference is established outside the maxilla.

Treatment and prognosis of diffuse sclerosing osteomyelitis (DSO) of the mandible.

Twenty-one patients with diffuse sclerosing osteomyelitis (DSO) of the mandible were investigated and treated in Göteborg. Sixteen of these patients ( twelve women and four men) participated in a long-term follow-up study in order to evaluate different therapeutic procedures. Cortisone was found to be a valuable aid to the therapy arsenal. Clinical and radiographic observations are presented. Etiology, therapy, and prognosis of DSO are discussed.

Movement of the cleft maxilla in infants relative to the frontal bone. A roentgen stereophotogrammetric study with the aid of metallic implants.

Four implants (tantalum balls 0.5 mm in diameter) were inserted in the frontal bone, and three implants (tantalum pins 1.5 x 0.5 mm) were inserted in each lateral segment of the cleft maxilla of ten infants aged 13 to 42 months. Motion of the maxillary segments was studied relative to the frontal bone by means of roentgen stereophotogrammetry during observation periods from 371 to 868 days. The results were presented by means of computer drawings of the implant triangles projected on the three cardinal planes. With this technique, movement of the maxillary segments could be visualized with a high degree of accuracy. No pattern of movement related to time or type of surgery, to type of cleft (UCLP/BCLP), or to cleft or non-cleft segment could be found.

Mucosal melanoma. Immunological findings in a rare case treated with BCG vaccine, autologous tumor cells, and cytarabine.

A case of mucosal melanoma involved the bronchus, maxillary sinus, and small intestine. Treatment with a combination of autologous tumor cells, BCG vaccine, and cytarabine was given, and the immunological system was investigated regularly. After each treatment episode, there was a transient improvement of the cell-mediated immunity to melanoma antigens. The cutaneous delayed hypersensitivity reaction to PPD was converted to a permanently positive one. In the final stage of the disease, the number of T cells was decreased. Difficulty occurred in interpreting the immunological findings according to the clinical condition.

Diffuse sclerosing ostemyelitis of the mandible.

Diffuse, sclerosing osteomyelitis of the mandible is a disease which requires a synthesis of clinical, radiographic and histologic findings for a correct diagnosis. The biopsy technique is important. 99mTc-scintigraphy and enzyme histochemistry can assist the diagnosis. Antibiotics and cortisone are the treatments of choice. Decortication should be reserved for recalcitrant cases.

A roentgen stereophotogrammetric study of implant stability and movement of segments in the maxilla of infants with cleft lip and palate.

Three implants (tantalum pins 1.5 x 0.5 mm) were inserted in each lateral segment of the cleft maxilla of ten infants aged three to 33 months. To check the stability of the implants in the bone the distances between the three implants within each segment were measured by means of roentgen stereophotogrammetry (accuracy 0.05 mm) at intervals of 7, 35, 63, 147, and 287 days after initial examination and at varying intervals thereafter (maximal observation time 833 days). No implants were lost and 18 of 60 measured distances were stable (final changes less than 0.2 mm). The three implants were regarded as a rigid-body model which represented the segment in the calculations of motion. In all infants with complete clefts, transverse narrowing occurred immediately after primary closure of the lip, or primary palate repair, while subsequent movement followed individual patterns.

5 years' experience of 5-S-cysteinyldopa in melanoma diagnosis.

Determinations of the urinary excretion of 5-S-cysteinyldopa were performed in 571 patients previously treated by surgery for melanoma or melanoma metastasis. 90% of the 161 patients with metastases showed values exceeding 0.15 mg/24 h, and 9% of the 410 patients without metastases had such values. The increase in 5-S-cysteinyldopa excretion was generally more pronounced in men with metastases than in women, 98% of the men and 77% of the women with metastases showing values exceeding 0.15 mg/24 h. High levels of 5-S-cysteinyldopa are of grave prognostic significan4% died within one month, and only 3% survived for more than a year. In Sweden, determination of 5-S-cysteinyldopa in patients operated on for melanoma gives maximum information in the winter (October--March), when sun exposure does not influence the excretion levels.

Chronic sclerosing osteomyelitis of the mandible. Scintigraphic and radiographic findings.

Fourteen patients with chronic sclerosing osteomyelitis of the mandible were investigated with 99mTc-labeled bone-seeking agents. Scintigraphic findings were compared with radiographic features. High uptake of diphosphonate and polyphosphate was demonstrated in diseased areas. Good agreement was found between radiography and scintigraphy concerning the extent and activity of each process. The use of bone-seeking radiopharmaceuticals in other types of skeletal lesions and differential diagnostic aspects are discussed.
