An investigation of N-terminal pro-opiocortin peptides in the rat pituitary.

Extracts of neurointermediate lobe (NIL) and anterior lobe (AL) of the rat pituitary, and material released from perfused rat pars distalis (PD) and pars intermedia (PI) cells were gel chromatographed and monitored using three antisera, each recognizing different regions of the non-corticotropin (ACTH)-lipotropin (LPH) portion of pro-opiocortin (POC). Two peaks (termed N-POC I) which emerged close to the elution position of rat beta-LPH were detected. The first peak was reduced significantly in the PI. Two smaller N-POC fragments which eluted near beta-endorphin were detected only in extracts and secretions of intermediate lobe tissue. One peak cross-reacted in the gamma 3-melanotropin (MSH) assay (N-POC III) whereas the other peak possessed amino (N)-terminal N-POC immunoreactivity (N-POC II). The results demonstrated differences in the distribution and nature of N-POC peptides released and extracted from the PD and PI of the rat pituitary, and suggest that the enzymic processing of N-POC is different in the two pituitary lobes.

Pro-enkephalin peptides possessing Met-enkephalin-Arg6-Gly7-Leu8-immunoreactivity in rat CSF, striatum and adrenal gland.

Cerebrospinal fluid (CSF) taken from rats implanted with chronic cisternal cannulae and extracts prepared from rat adrenal gland and striatum were subjected to Sephadex G-50 chromatography and HPLC. Fractions were monitored using specific radioimmunoassays (RIA) for the pentapeptide methionine enkephalin (Met-Enk) and methionine enkephalin-Arg6-Gly7-Leu8 (Met-EnkRGL). In rat CSF, striatum and adrenal gland, three Met-EnkRGL-immunoreactive (IR) peaks of Mrs 8000, 5000 and 1000 daltons were detected. The same peaks were also found to possess Met-Enk-immunoreactivity after enzyme digestion of Sephadex G-50 fractions with trypsin and carboxypeptidase B (CPB), suggesting their derivation from proenkephalin. HPLC of the 8K and 5K peaks on a column of Ultrapore RPSC showed them to elute discretely with similar retention times, indicative of hydrophobic peptides of large molecular weight. Their similar hydrophobicities yet significant separation during gel filtration would suggest that the 8K and 5K peptides are structurally closely related yet different with respect to their molecular weights. HPLC of the small molecular weight material from rat striatum and adrenal gland revealed the presence of Met-EnkRGL and Met-EnkRGL sulphoxide in both tissues. In rat striatum Met-Enk and its sulphoxide were also detected. The oxidised pentapeptide was found to be present in rat CSF, together with two unidentified small molecular weight Met-Enk-IR peaks detected without prior enzyme digestion of fractions. The small molecular weight Met-EnkRGL-IR material in rat CSF was found to be comprised of two unknown peptides which were less hydrophobic than Met-EnkRGL and its sulphoxide derivative.

Regulated progression of a cultured pre-B-cell line to the B-cell stage.

The variable (V) regions of heavy and light immunoglobulin chains are encoded by multiple germline DNA elements which are assembled into complete variable-region genes in precursor(pre-) B lymphocytes. The heavy-chain V region (VH) is assembled from three separate germline DNA elements, the variable (VH), diversity (D) and joining (JH) segments; whereas light-chain variable regions of either the kappa or lambda type are assembled from two elements, the VL and JL. Analysis of tumour cell lines or sorted cell populations which represent early and late pre-B cells has suggested that heavy-chain assembly and expression generally precedes that of light chains; but, primarily because of the lack of appropriate model systems to study the phenomenon, the mechanism and significance of this apparently orderly differentiation process are much debated. Here we describe for the first time a transformed cell line, 300-19, which sequentially undergoes all of the immunoglobulin gene rearrangement and expression events associated with the differentiation of pre-B cells to surface immunoglobulin-positive B lymphocytes. Analysis of the in vitro differentiation of 300-19 cells provides direct evidence for distinct differentiation phases of first VH and subsequently VL assembly during B-cell differentiation. Furthermore, these analyses suggest that the mu heavy chain, resulting from a productive VHDJH rearrangement, has both a positive and a negative regulatory role in mediating this ordered differentiation process, that is, signalling the cessation of VH gene assembly and simultaneously signalling the onset of VL assembly.

Expression of lipoprotein receptor and apolipoprotein E genes by perinatal rat lipid-laden pulmonary fibroblasts.

Lipid-laden interstitial fibroblasts (LIFs) are abundant during alveolar septal formation in rats and accumulate droplets of neutral lipids. The mechanisms controlling lipid acquisition by LIFs are incompletely understood and accumulation varies during postnatal development, because lipid droplets are usually a transient phenotype. We hypothesized that plasma lipoproteins may be an important source of lipids and that the cells may alter their acquisition of lipoproteins by changing the expression of lipoprotein receptors and apolipoprotein E. We quantified the accumulation low-density lipoproteins (LDLs) and very-low-density lipoproteins (VLDLs) by LIFs and the expression of LDL and VLDL receptors mRNA and protein at various perinatal ages and found no significant age-related differences. Apolipoprotein E mRNA was maximal at postnatal day 15, whereas immunoreactive apolipoprotein E protein was maximal at gestational day 21, suggesting complex regulation. Our findings indicate that the age-related difference in the lipid droplet contents of LIFs is not primarily related to differences in LDL or VLDL receptor expression. They suggest that changes in the quantities of plasma lipoproteins, which are presented to LIFs in the lung at various perinatal ages, are more likely to be responsible for age-related alterations in lipid droplet size and abundance.

Apparent absorption efficiencies of nectar sugars in the Cape sugarbird, with a comparison of methods.

Nectarivore sugar preferences and nectar composition in the Cape Floristic Kingdom (southern Africa) differ from trends reported for analogous systems in America and Europe in that sugarbirds and sunbirds show no aversion to sucrose, which is the dominant nectar sugar in many of their food plants. To elucidate the physiological bases (if any) of nectarivore sugar preferences, we determined apparent sugar absorption efficiencies in a passerine endemic to this region, the Cape sugarbird Promerops cafer. Apparent absorption efficiencies for the three major nectar sugars, sucrose, glucose, and fructose, were extremely high (> 99%), as in other specialized avian nectarivores. Xylose, a pentose sugar recently reported in the nectar of some Proteaceae, was absorbed and/or metabolized inefficiently, with a mean of 47.1% of ingested sugar recovered in cloacal fluid. We did not measure the proportions of xylose that were absorbed and/or metabolized. We also compared three methods of estimating absorption efficiency: (1) measurements of total sugar in cloacal fluid with refractometry, without correction for differences between volumes of ingesta and excreta; (2) the same measurements combined with correction for volume differences; and (3) HPLC analyses quantifying individual sugars in cloacal fluid, with correction for volume differences. Refractometry has been frequently used in previous studies. For all sugars except xylose, method 1 yielded results similar to those obtained with method 2, but the convergence was artifactual, and we do not recommend use of this method. Apparent absorption efficiencies calculated with method 2 underestimated true absorption efficiency, because refractometry measures nonsugar solutes, but this error is biologically significant only when efficiencies are low.

Women's health centers and minority women: addressing barriers to care. The National Centers of Excellence in Women's Health.

New models of care delivery have been developed to better coordinate and integrate healthcare for women. In the United States, one of the challenges is to incorporate the needs of racial and ethnic minority populations into these newer care paradigms. This paper begins with a brief historical review of the experience of racial and ethnic minorities in the American healthcare system to provide a context for discussing barriers and limitations of more traditional models of women's healthcare. Specific approaches used by National Centers of Excellence in Women's Health are presented as examples of strategies that may be implemented by other communities to address these barriers.

Prevention and management of pressure sores.

Continuity of care is needed when transferring patients at risk of developing pressure sores. The tools used to assess such patients must be consistent. A turning clock system has been developed for use with the patient.

Drug administration in day centres for people with learning disabilities.

A study was made of the problems associated with the need to administer medicines at day centres for people with learning disabilities. In Leicestershire, 167 (13%) of 1317 people with learning disabilities who attended day centres required the administration of medicines. The percentage of attenders requiring drug administration varied from day centre to day centre (minimum 4%, maximum 23%). Antiepileptics were the most commonly administered drugs, followed by antipsychotics. The administration of medicines took up a significant amount of time for senior members of the day centre staff and had a great potential for errors. In only 32 cases was it deemed essential that the drug administration occurred at the day centre.

Polyethylene glycol precipitates of serum contain a large proportion of uncomplexed immunoglobulins and C3.

High pressure liquid chromatography (HPLC) was used to fractionate redissolved polyethylene glycol (PEG) precipitates isolated from the sera of normal volunteers and from patients with IgA nephropathy (IgAN) and systemic lupus erythematosus (SLE), 2 diseases characterized by elevated levels of circulating immune complexes. The individual fractions were analyzed by solid phase ELISA for IgA, IgM, C3, IgG, and complexes of IgG-IgA and IgG-C3. Although PEG precipitates were enriched for high molecular weight IgA and IgG (presumably bound within CIC), significant amounts of IgM, unbound IgG and C3 were also present. The quantities of the PEG-precipitable proteins did not correlate with their serum concentrations. IgG-IgA and IgG-C3 complexes were found in all precipitates examined, but the levels of complexes were higher in both patient groups. These results indicate that PEG precipitates a considerable quantity of proteins not bound in immune complexes. There appeared to be greater protein precipitation from sera of the patient groups compared to the amount precipitated from the normal sera. These results suggest that an understanding of the mechanism of PEG precipitation may be important in defining abnormalities in IgAN, SLE and perhaps other diseases characterized by elevated levels of CIC. In addition, the possibility of undetected CIC in PEG precipitable material must be considered.

Late expression of tumor necrosis factor-alpha is markedly depressed in patients with IgA nephropathy.

In this study we demonstrate that peripheral blood mononuclear cells (PBMC) of patients with IgA nephropathy (IgAN) express mRNA for tumor necrosis factor alpha (TNF-alpha) at a significantly lower frequency after stimulation for 24 hours with pokeweed mitogen than do PBMC of a control population. These differences were not seen in patient samples stimulated for only 3 hours. To identify the cells responsible for TNF-alpha expression after overnight mitogen stimulation, we performed reverse transcriptase polymerase chain reaction analysis after preparative flow cytometry of PBMC labelled with fluorescent monoclonal antibodies. After stimulation for 24 hours, PBMC of all patients tested displayed no detectable TNF-alpha mRNA; in the control PBMC, TNF-alpha mRNA was present in CD4+ and CD8+ T cells, as well as in CD19+ B cells in some samples. These results suggest that diminished expression of TNF-alpha by T cells may play a role in the pathogenesis of IgAN.

A persistent hindlimb ischemia model in the rabbit.

The study of new approaches for the treatment of limb-threatening ischemia has been hampered by the lack of a suitable animal model of persistent limb ischemia. We describe the development and evaluation of an animal model of persistent hindlimb ischemia, in which ischemia was induced in the left hindlimb of 28 rabbits by ligation of the distal external iliac artery and excision of the common and superficial femoral arteries. The severity of the ischemia and its relief in each animal were evaluated every 10 days postoperatively until day 40 (all animals) or day 90 (five animals). Nine animals developed superficial tissue necrosis in the foot, but no deaths were attributable to the ischemia-inducing procedure. Angiography demonstrated minimal collateralization and sluggish filling of distal vessels up to postoperative day 90. This was accompanied by a decrease at rest in the calf blood flow ratio (p < 0.005 vs day 0), an increase in lactic acid in the femoral venous blood (left vs right side, p < 0.002) up to postoperative day 40, and a decrease in the calf blood pressure ratio (p < 0.0001 vs day 0) up to day 90. Histologic study of the gastrocnemius muscle demonstrated evidence of atrophy and fibrosis in the left hindlimb. This model can be used to evaluate direct and indirect approaches to the treatment of chronic limb ischemia.

The gift of time from the friendly professional.

This article presents the preliminary findings from a qualitative study of what users of mental health services need from psychiatric and mental health nurses. The study has found that, ideally, nurses need to give time and energy, emotional commitment and knowledge to clients.
