[Pre-, intra- and postoperative allergic manifestations in hepatic echinococcosis].

Data obtained from 502 patients operated for echinococcosis (hydatid disease) are analyzed. In 61 of them (12,15 per cent) a variety of allergic manifestations are observed before the operation. Serious allergic shocks intraoperatively, and lethal outcome due to allergic shock in the postoperative period are described. Inferences are reached about the necessity of performing mandatory examination of all allergic patients for echinococcosis as an eventual underlying cause of allergy, as well as mandatory preoperative tests for allergy to drugs and anesthetics along with undertaking antiallergic prophylaxis of patients in the postoperative period to obviate eventual allergic accidents, regardless of their presence or not in the previous history.

[Occupational hygiene in industrial assembly work].

Data on occupational hygienic characteristics of industrial-assembly work are reported, which have not been subject upto now to such studies. Complex hygienic evaluation of the basic physical and chemical factors of the working environment is performed as well as physiological and ergonomic characteristics of the principle professional groups, analysis of the occupational diseases and diseases with temporary disability and clinic and laboratory examinations on the workers. The following peculiarities of occupational hygienic aspect are established: 1. dynamic and multiform professional itinerary of the industrial-installing workers; 2. large variability in the characteristics of the physical and chemical factors of the working environment, most frequently surpassing the admissible norms and requirements, as well as the duration of exposure to them; 3. considerable working engagement (from 63 to 78% operation time), connected with heavy physical loading (dynamic and static), exceptionally unfavourable, compulsory and fixed working posture, possibilities of prolonged working day, high rate of involvement of the psychomotor sphere, analyzers and attention, great professional responsibility and nervous and emotional stress; 4. occupational and general morbidity of workers, adequate to the already established by the authors occupational noxa--acute poisoning with smoking gases, injury of the hearing, diseases as a result of overpressure of the locomotor apparatus and the nervous system.

[Toxic lesions in workers in the pharmaceutical industry].

Clinical studies are performed on 15 workers from the chemical and pharmaceutic plant "Stanke Dimitrov". Seven of the workers are with acute poisoning from aniline and phenylhydrazine, and eight are exposed to a chronic combined effect of a large group of organic solvents. The clinic and laboratory examinations aim at clarifying the liver and kidney functions, the state of the blood and nervous systems. The tests embrace also blood test morphology and osmotic resistance of erythrocytes, iron, correlation of haemoglobin types, electrolytes, transaminase, GGTP, lipid metabolism, iron-binding capacity. Moderately increased transaminase activity is found in 2/3 of the examined. In four of the workers is registered expressed anaemic and lung syndrome. The following diagnoses are accepted: chronic combined effect of organic solvents--in 3 workers; chronic poisoning with aniline and other solvents--in 1; toxic hepatitis--in 4, with no data for poisoning--in 7. The leading clinical syndromes in acute and chronic combined effect of organic solvents are discussed.

[Dust levels and dust-induced diseases in the main dust-producing industries].

The data on airborne dust and occupational disease in the main dust-related industries of the People's Republic of Bulgaria are generalized and analyzed for a period of 15 years. In the ore producing industry is established a stability in the concentration of total and respirable dust 1-2 TWA concentrations and decrease of silica content in dust. The highest occupational hazard is registered in diggers. The levels of TWA total and respirable dust and silica concentrations in the other underground mines as well as in the overground industries are strongly variable and high. Silicosis is still in the first place among dust-related diseases, but mainly among miners of advanced age. The newly detected other pneumoconiosis, asbestosis, mixed pneumoconiosis are already more than the cases of silicotuberculosis. An essential part is taken by the newly detected diffusive pneumosclerosis and dust bronchitis. The distribution of the newly detected occupational lung diseases is studied according to branches of industry.

[In vitro cytotoxicity of melanoidins isolated from cooked meat and fish samples].

A study has been undertaken to investigate the biological activity of melanoidins isolated from meat samples baked according to Kuntscheva (electro-baking oven, 200-300 degrees C, 60-210 min) and from semi-finished meat products baked in swimming fat (200 degrees C, 3 X 50 min, repeated use of the oil) as well as from semi-finished fish products (190 degrees C, 60 min). A weak toxicity of the melanoidins formed within a longer thermal treatment, but no mutagenic effect was stated. The cytotoxic alterations showed no specific character.

[Correlation of the cytologic, endoscopic and echographic findings in the diagnosis of pancreatic cancer].

The cytologic, endoscopic and echographic findings in 28 patients with pancreatic cancer are compared. The echographic findings indicated the presence of pancreatic cancer in 4 patients only, in 9 patients they were considered as suspicious for cancer and in 15 patients they did not discover the tumor. The endoscopic examinations revealed a pancreatic tumor in 8 patients and in 10 patients only indirect data for tumor were found. The cytologic examination of the pancreatic juice sediment found cancer cells in 26 out of the 28 patients studied. In 4 patients the diagnosis was determined by cytologic examination only. The simultaneous application of all three methods led to the correct diagnosis of pancreatic cancer in all 28 patients while they were still alive.

[Autonomic innervation of the heart of young patients with insulin-dependent diabetes].

The autonomic innervation of the heart has been studied in 32 young patients with insulin-dependent diabetes mellitus and 28 controls by measuring the intervals PP of electrocardiograms at rest, deep breathing, stimulation of n. vagus according to Ashner method and after suppression of vagus tone by atropine. The orthostatic test was performed. The patients with diabetes were established to have a higher heart rate and less manifested variations of the intervals PP at rest. During deep breathing, the patients with diabetes also showed a poorer variability of the intervals PP as compared with the controls. The reactions, however, in both groups with Ashner test and orthostatic test, were similar. Asymptomatic sinoatrial block was recorded in a patient with diabetes by the method of Ashner. The study revealed that the autonomic innervation of the heart was oft disturbed in case of diabetes. There was a correlation between the severity of that disorder and diabetes duration, diabetic peripheral polyneuropathy established in all cases. The other manifestations of diabetic polyneuropathy are more rare and later observed. The disturbed autonomic innervation of heart activity could be responsible, in some cases, for the sudden cardiac death among the patients with diabetes.

[Comparison of x-ray and radionuclide studies of the joints in patients with Touraine-Solente-Golé syndrome].

The thesis of primariness of the disease and of its osseous tissue manifestations is discussed on the grounds of the results from a complex study on 3 patients. The periosteal ossifying hyperplasia and disturbances in the osseous tissue structure develop with no other organ symptomatics and in the absence of any changes in the available laboratory and immunological indices. The macromorphological X-ray characteristic corresponds to the radionuclide tests of the study with 99MTc-pertechnetate, 99MTc-pyrophosphate and 99MTc-sulfocolloid, informing about abnormal vascularization index, growth of bone-marrow tissue and intensified joint-tissue anabolism.

[Amino acid composition of white poultry meat contaminated with psychrophilic microorganisms].

Studied was the effect of psychrophilic organisms of the Acinetobacter, Aeromonas, Alcaligenes, and Flavobacter genera on the amino acid composition of white poultry meat stored at 0 degrees to 4 degrees C in the course of 48 hours, and at 18 degrees C in the course of 6 to 12 months. The quantitative determination of individual amino acids was carried out by means of an automatic amino acid analyzer, Hitachi type, after the method of Speckman, Stein, and Moore. No changes were established in the amino acid composition of white poultry meat that was treated with psychrophilic organisms as against the composition of untreated meat. Proteins preserved their biologic value.

[Amino acid composition of white poultry meat contaminated with psychotropic microorganisms of the genus Pseudomonas].

Investigated was the amino acid content in white poultry meat contaminated with four species of organisms of the Pseudomonas genus and kept for 48 hours at 0-4 degrees C and for 6 and 12 months at -18 degrees C. The content of the individual amino acids was expressed in g/100 g of protein, giving the figures of the essential amino acids. Results showed that there were no differences in the qualitative and quantitative composition of the amino acids both with regard to the various Pseudomonas species and with regard to the different duration of storage.

[Contamination of butchered poultry with psychrophilic and psychrotrophic microorganisms].

Studied were the ways in which slaughtered birds were contaminated with psychrophilic and psychrotrophic microorganisms as well as the effect of the various technologic stages in the yield and processing of the poultry meat on the extent of its contamination and the occurrence of such organisms in alive and slaughtered broilers in dependence on the season. It was found that the individual stages of production of poultry meat influenced the rate of contamination of slaughtered birds. There was a difference between the psychrophilic microflora in alive birds and that found after slaughter and processing. In the first group highest was the percent of the Acinetobacter (100%) genus, followed by the Flavobacter (80%), Corinebacter (60%) and other genera, while in the slaughtered birds prevailing were the Pseudomonas (50-75%) and Flavobacter (17-30%) genera. Following scalding the psychrophile count dropped 12 times as compared to the count established at stunning, and the count of psychrotrophic organisms dropped 7 times. Greatest was the drop following the process of singeing--8 times for the psychrophiles and 2.5 times for the psychrotrophic organisms. During the following stages of the technologic process no rise was observed of the count of both types of organisms. The count of psychrotrophic organisms was found to be lower in the spring and autumn seasons, and it was higher in the summer. Psychrophiles showed highest number of strains isolated in the winter season--as many as 102.
