Differential assay of salivary and pancreatic alpha-amylase in serum and urine, with use of monoclonal antibody to human salivary amylase immobilized on bacterial cell wall.

We previously reported (Clin Chim Acta 1986;159:89) that bacterial cell wall chemically coated with a monoclonal antibody specific to human salivary (S) amylase (EC 3.2.1.1) could be successfully used to separate S and pancreatic (P) amylase in solution. We have now applied this method to serum and urine samples and found that the activities of S and P amylases so measured correlated well with those measured by the isoamylase inhibitor method. The present method is simple and reliable for routine clinical tests.

[Diagnosis and treatment of secondary spine tumors].

The surgical results of the 63 patients with metastatic cancer of the spine, treated in Chiba University Hospital and Chiba Cancer Center in 1977-86, were analyzed. The location of the primary tumor were lung (30.2%), thyroid gland (14.3%), kidney (14.3%), breast (12.7%) and gastro-intestinal tract (9.5%). All patients had intractable pain, and 47 (76%) had neurologic deficit. 46 patients were treated with posterior procedure, 7 with anterior procedure and 10 with combined anterior and posterior procedure. Anterior and posterior stabilization using implants (instrumentation, ceramics, cement) were undertaken together with tumor resection and surgical decompression in past 10 years. In 21 patients treated with laminectomy only, 53% had pain relief, and 48% had improvement of neurologic deficit. In 42 patients treated with surgical decompression and stabilization, 78% had pain relief, and 54% had improvement of neurologic deficit. Surgical decompression should be undertaken before complete paralysis develop (degree B on Frankel classification). With multi-disciplinary treatment patients with metastatic cancer of the spine survived for a significantly longer time. In patients with neurological deficit but few visceral metastasis and in those with slow-growing tumor (thyroid cancer, breast cancer and kidney cancer) in which a survival time of longer than 6 months can be expected, operative intervention using advanced surgical technique the quality of the patient's life and let patients support themselves in ADL.

[Multiple submucosal cysts of the stomach associated with IIc + III type early gastric cancer--report of a case].

A 59-year-old man was admitted with a complaint of epigastric discomfort, under the diagnosis of gastric cancer, subtotal gastrectomy was performed. In the resected specimen IIc + III type early gastric cancer were observed at the lesser curvature of the corpus and multiple submucosal cysts in almost entire area of the stomach. Histologic examination confirmed early adenocarcinoma and an atypical epithelium, which was present separately. Submucosal cysts consisted of a pseudo-pyloric-gland-type of epithelium, containing some fundic glandular and foveolar epithelium. Frequent erosion and regenerated epithelium and extensive intestinal metaplasia were observed in the gastric mucosa. There was no carcinomatous change in the submucosal cysts.

[A case of renovascular hypertension due to chondrosarcoma arising from lumbar vertebrae].

A 35-year-old man visited our clinic with the chief complaint of headache and an upper abdominal mass. Renal vein renin activity, drip infusion pyelogram, GT scan and selective renal angiogram demonstrated a retroperitoneal tumor and right renovascular hypertension. Thus tumor extirpation and right nephrectomy were performed. After nephrectomy, blood pressure was corrected within the normal range. Histologically the tumor was diagnosed to be chondrosarcoma and there was no lesion in the wall of the right renal artery. This case had secondary chondrosarcoma arising from the first and second lumbar vertebrae and produced renovascular hypertension due to compression of the tumor.

Scanning electron microscopic study on the angioarchitecture of glomeruli in experimental toxemia of pregnancy in the rabbit.

Experimental toxemia of pregnancy in late gestation in rabbits was induced by constricting the aorta below the renal arteries on day 21 of gestation. Light and transmission electron microscopic examination of glomeruli on day 29 revealed only a slight swelling of endothelial cells. The angioarchitecture was also investigated using scanning electron microscopic examination of methyl methacry corrosion casts of the glomeruli. The kidney in experimental toxemia was characterized by a reduction of glomerular vessel diameter compared to that of normal pregnant rabbits. This probably resulted from enlargement of the endothelial cells, suggesting a reduced blood supply to the glomerulus. These findings suggest that interference with the blood supply to the pregnant uterus is an important factor in the development of the renal lesion of experimental toxemia of pregnancy in the rabbit.

[Pulmonary resection by microwave tissue coagulator (MTC)--clinical application].

A 55 year old male with metastatic lung and liver cancer was subjected to pulmonary and hepatic resections using Microwave Tissue Coagulator (MTC). No adverse effects were observed in this patient after operation. The resected specimen of the lung had the bronchioli partially accompanied with cartilages. Coagulation for the bronchioli of at least lmm in size, by MTC, could be carried out resulting in no air leakage after operation.

Arginine sensors in the hepato-portal system and their reflex effects on pancreatic efferents in the rat.

To investigate the possible existence of arginine sensors in the liver and their reflex effects, afferent discharges from the hepatic branch of the vagus nerve and efferent discharges from the pancreatic branches of the splanchnic and vagus nerves were recorded. Intraportal injection of arginine increased afferent discharge rate of the hepatic branch of the vagus nerve, slightly depressed the efferent discharge rate of the pancreatic vagal branch, and increased the efferent discharge rate of the pancreatic splanchnic branch. The results suggest the existence of arginine sensors in the hepato-portal system that exert reflex regulation of the pancreatic neuroendocrine system.

Enhancement of insulin and glucagon secretion by arginine after hepatic vagotomy.

The hepatic vagus nerve consists of mostly afferent fibers in the rat, and is a major afferent pathway between the liver and the medulla. The present study was carried out to examine the role of the hepatic branch of the vagus nerve in secretion of insulin and glucagon after intraperitoneal injection of arginine (1 g/kg body wt.) in rats. Measurements were made one week after section of this branch. Intraperitoneal arginine enhanced both plasma insulin and glucagon concentrations; more in hepatic-vagotomized than in sham-vagotomized rats. The results suggest that inhibition of the secretion of insulin and glucagon after arginine stimulation is mediated by the hepatic branch of the vagus nerve. The existence of 'sensors' in the liver for arginine, or its derivatives, is proposed as an explanation for the inhibition of the secretion of insulin and glucagon by the hepatic vagus nerve.

Entrapment of phenylalanine ammonia-lyase in silk fibroin for protection from proteolytic attack.

Phenylalanine ammonia-lyase was entrapped in silk fibroin. The entrapped enzyme showed a similar Km for Phe and pH optimum to the free enzyme. It was resistant against chymotrypsin and trypsin in vitro. To assess the activity in vivo, the free or entrapped enzymes and then Phe were injected into rat duodenum, and cinnamate, a product, in plasma was determined as the most direct evidence of the enzyme activity. The entrapped enzyme but not the free form caused a marked raise of plasma cinnamate. It declined with a half life of about 45 min, which was significantly longer than that (10-15 min) observed upon i.v. administration of cinnamate. These results indicated that the entrapped enzyme was actively degrading Phe in the intestinal tract. Entrapment of phenylalanine ammonia-lyase in fibroin thus provides a new prospect for oral enzyme therapy of phenylketonuria.
