[Effect of endoscopical administration of OK-432 in 4 patients with advanced esophageal or gastric cancer].

OK-432 has been injected endoscopically into 4 patients with an unresectable esophageal or gastric cancer. Three months after the initial injection, a remarkable improvement in the symptoms and a reduction of the tumor size was recognized in all 4 patients. This study indicates that an OK-432 injection provides considerable benefits to patients with an advanced esophageal or gastric cancer.

Antitumor activity of 7-N-(2-((2-(gamma-L-glutamylamino) ethyl) dithio) ethyl) mitomycin C (KW2149) against human tumor xenografts serially transplanted into nude mice.

The antitumor activity and toxicity of 7-N-(2-((2-(gamma-L-glutamylamino) ethyl) dithio) ethyl) mitomycin C (KW2149) were evaluated using a human tumor xenograft--nude mouse system, and compared with those of the maternal compound, mitomycin C. The maximum tolerated dose of KW2149 was estimated to be 15 mg/kg by bolus intraperitoneal or intravenous injection, at which a remarkable reduction of spleen weight was observed, suggesting bone marrow suppression by this agent. A bolus injection of KW2149 seemed to be more effective than a divided injection schedule, when a total of 15 mg KW2149/kg was administered to mice bearing breast (MX-1) and colon (Co-4) carcinomas. The antitumor activity of KW2149 was dose-dependent, and the difference in antitumor effect according to route of administration was minimal. The antitumor spectrum of KW2149 was essentially identical to that of mitomycin C administered intraperiotoneally as a bolus at a dose of 6 mg/kg.

[Acute and chronic effects of bunazosin in patients with congestive heart failure].

The acute hemodynamic effect (right atrial pressure, mean pulmonary artery pressure, pulmonary capillary wedge pressure, cardiac index, heart rate, blood pressure) and neurohumoral response (alpha-ANP, plasma renin activity, aldosterone, angiotensin II) of Bunazosin, oral alpha 1 blocker, was investigated in 28 patients with congestive heart failure at rest and immediately after exercise. Bunazosin reduced alpha-ANP, but, other neurohumoral factors did not change. Bunazosin produced significant hemodynamic improvements both at rest and after exercise. Its chronic effect was also investigated in 11 patients in 28 days after taking oral Bunazosin. Improvement of hemodynamics at acute phase was also preserved at chronic phase without deterioration of neurohumoral factors. It is concluded that Bunazosin may be an effective Balanced vasodilator both at acute and chronic phases in patients with congestive heart failure.

Comparative clinical study with iotrolan versus metrizamide in lumbar myelography.

In order to examine the radiopacity, safety, and usefulness of iotrolan, a new dimeric, nonionic, water-soluble contrast medium for myelography, a multicenter clinical study was performed in lumbar myelography using metrizamide as the reference drug. Both media were injected at a strength of 190 mg I/ml in 90 patients each. Both contrast media showed good opacification; there was no significant difference between the two media. The incidence and severity of side effects were significantly lower with iotrolan than those with metrizamide. No severe side effects, such as convulsion and shock, were observed with either medium.

Correlation between urinary melanin-related metabolites and tumour weight in melanoma-bearing mice.

Melanoma tissues produce not only 5-S-cysteinyldopa (5-S-CD), a pheomelanin precursor, but also 5,6-dihydroxyindoles, eumelanin precursors. We compared 5-S-CD and eumelanin-related metabolites regarding the correlation of their production in melanoma and excretion in urine, versus the weight of B16 mouse melanoma. It was found that B16 melanoma tissues produce 5,6-dihydroxyindole-2-carboxylic acid and its two O-methyl derivatives, the latter of which are partly conjugated with sulphuric acid and then excreted. These indolic metabolites are referred to as total indoles. Both total indoles and 5-S-CD in melanoma and in urine correlated well with each other and reflected the tumour weight. Total indoles had a four-fold greater level of excretion than 5-S-CD, suggesting that the urinary excretion of total indoles is a quantitatively more significant marker of melanoma progression in this melanoma.

[Clinical study of large doses of vecuronium; duration of initial and additional doses].

Neuromuscular blockade by large doses of vecuronium was investigated clinically and the duration of action of initial doses (0.2 mg.kg-1 and 0.3 mg.kg-1) and additional doses (0.01 mg.kg-1 and 0.02 mg.kg-1) were measured under enflurane-nitrous oxide anesthesia using a neuromuscular transmission monitoring system (Accelograph). In group A (initial dose = 0.2 mg.kg-1, additional dose = 0.02 mg.kg-1), the time of spontaneous recovery to 25% of control twitch height (T25) and 50% of control (T50) were 63.1 and 82.0 minutes respectively. The T50 interval of the two adjacent added doses (given at the time point of 50% recovery from the previous dose) was 31.1 minutes. Reversal time from TOF ratio = 25% to 75% after administration of edrophonium was 6.1 minutes. In group B (initial = 0.3 mg.kg-1, add. = 0.01 mg.kg-1), T25, T50, T50 interval and reversal time were 122.6, 159.4, 39.2 and 4.6 minutes respectively. In group C (initial = 0.3 mg.kg-1, add. = 0.02 mg.kg-1), above values were 119.2, 145.8, 48.4 and 6.7 minutes respectively. In this study there was no obvious side effect associated with administration of large doses of vecuronium. These methods will be very useful for long surgical procedures.

[The effect of bunazosin vs captopril on hemodynamic and neurohumoral parameters in patients with congestive heart failure].

The hemodynamic parameters (right atrial pressure, mean pulmonary artery pressure, pulmonary capillary wedge pressure, cardiac index, heart rate, blood pressure) and neurohumoral responses (alpha-ANP, plasma renin activity, aldosterone, angiotensin II) of Captopril, oral ACE inhibitor, and Bunazosin, oral alpha 1-blocker, were investigated in 28 patients with congestive heart failure at rest and after exercise. These data were analysed in both acute and chronic phases. 1) Acute effect. Captopril produced significant improvement of neurohumoral factors at rest and also after exercise. Bunazosin reduced alpha-ANP, but other neurohumoral factors did not change. Bunazosin produced significant hemodynamic improvement both at rest and after exercise. 2) Chronic effect. Captopril produced significant hemodynamic improvement both at rest and after exercise. Improvement of neurohumoral factors in acute phase was also preserved at chronic phase. On Bunazosin, improvement of hemodynamics at acute phase was also preserved at chronic phase without deterioration of neurohumoral factors.

[CT image of primary ovarian cancer seeding to the peritoneal cavity].

Early diagnosis of primary ovarian cancer is difficult, because of a few symptoms and clinical signs, this cancer has many pathological findings and CT images. Therefore, initial CT Scan documents often the seeding to peritoneal cavity. We evaluated the seeding pathway to peritoneal cavity for 59 primary ovarian cancer by CT, especially ascites, omental metastasis, peritoneal metastasis and scalloping.

[Significance of decreased washout rate in hypertrophic cardiomyopathy by exercise Tl-201 myocardial scintigraphy].

UNLABELLED: A clinical significance of decreased washout rate (WR) in hypertrophic cardiomyopathy by exercise thallium-201 myocardial scintigraphy was evaluated. Thirty-six patients with hypertrophic cardiomyopathy and normal coronary angiogram were categorized into 3 groups according to initial distribution (ID) and WR: decreased ID group (17 patients, ID group), normal ID but decreased WR group (9 patients, WR group) and normal ID and normal WR group (10 patients, N group). RESULTS: (1) Cardio-thoracic-ratio and left ventricular end-diastolic pressure were higher in WR group than in other 2 groups. (2) Resting ejection fraction and left ventricular end-diastolic volume index did not differ in three groups. (3) Exercise ejection fraction was increased in N group, whereas it was not increased in ID or WR groups. It was increased in 47% of ID group, but were decreased or not changed in all patients of WR group. In conclusion, decreased WR in patients with hypertrophic cardiomyopathy may strongly suggest left ventricular myocardial damage.

[The effect of fat tissue autotransplantation on the changes in tissue surrounding the dura mater following laminectomy].

Problems, probably caused by the adhesion of scar tissue to the dural sac or nerve roots, may appear after a laminectomy in which resection was required. In our department, free auto-fatty tissue transplantation has been clinically performed since 1979 in an effort to prevent this phenomenon, and favorable therapeutic results have been obtained. In the present study, the possible influence of extradural tissue on the dura mater over certain periods was investigated in post-laminectomized adult rabbits. Additionally, the inhibitory effect of transplanted fatty tissue on adhesion was examined. In the laminectomy-only subject group, a remarkable development of granulating tissue was observed. This tissue subsequently adhered to the dura mater, often resulting in a delayed repair of the dura mater and its surrounding tissue. In the fatty tissue transplanted group, on the other hand, the transplanted fat preserved its composition morphologically and rapid degeneration was deterred even in free grafts, so scar formation was inhibited at the site of the laminectomy. In comparison with the laminectomy-only group, furthermore, the repair of the tissue surrounding the dura mater was accelerated by the transplanted fatty tissue, beginning in the early postoperative stage.

Amino acid sequence of nerve growth factor purified from the venom of the Formosan cobra Naja naja atra.

Nerve growth factor (NGF) was isolated from the venom of the Formosan cobra (Naja naja atra). The amino acid sequence was determined by a combination of conventional methods. The total number of amino acid residues was 116, giving a molecular mass of 13,057 Da. The sequence was identical with that deduced from the nucleotide sequence of an NGF cDNA from the venom gland of Naja naja siamensis, reported by Selby et al. [J. Neurosci. Res., 18, 293-298 (1987)].

[Modified Fontan operation of single ventricle with D-TGA].

An 8-year-old boy who had previously received two systemic pulmonary shunts, underwent successfully direct anastomosis between SVC and right pulmonary artery (bidirectional Glenn shunt), and proximal side of SVC and main pulmonary artery. Post operative course was uneventful with pleural effusion during a shot period. Post operative right atrial pressure was 9/5 mmHg without evidence of live swelling.

[Therapeutic implication of plasma C3 rich fraction against bacterial infection in surgical patients].

The depleted plasma opsonic activity is a major provoking cause of severe postoperative infection in patients who underwent extended surgical procedures. In order to establish the therapeutic use of C3 rich fraction (C3RF) for these patients in place of large amount of fresh frozen plasma (FFP) replacement, a basic study was done dealt with this fraction which was extracted from FFP with polyethyleneglycol. The results were as follows. (1) Addition of minimum amount of C3RF in vitro sufficiently revived the opsonic activity of opsonin exhausted guinea pig's plasma with heating or treating with cobra venom factor (CVF). (2) The clearance of E. coli 075 from peritoneal cavity and from blood stream in CVF treated guinea pigs was much delayed. However, in vivo administration of C3RF remarkably improved the clearance of bacteria and survival rate of opsonin depleted animal in E. coli peritonitis (p less than 0.05). (3) Opsonin lowered plasma samples in postoperative, septic, or cirrhotic patients were revived by not only replacing FFP, but also by adding minimum amount of C3RF. These results suggest that transfusion of C3RF should have beneficial effect in surgical patients with severely depleted plasma opsonic activity.

[Study of carcinoembryonic antigen (CEA) in the epidermis of psoriasis vulgaris].

By immunohistochemical methods, we detected CEA in the epidermis of psoriatic skin in 10 out of 20 cases with the plaque type of psoriasis vulgaris. The CEA detected in the psoriatic skin was limited in area to the intra-squamous cell cytoplasm, cell membranes, and intercellular space just beneath the markedly parakeratotic layer. It was not found in the orthokeratotic layer. At the same time, serum CEA value was measured in 7 cases. No increased CEA value was found in any case with or without positive CEA in the epidermis. The origin of CEA in the psoriatic skin might be: 1) squamous cell just below the rapidly proliferating parakeratotic cells. 2) CEA contained in eccrine sweat and deposited by dishidotic procedure. It can not involve permeation of CEA in serum into the epidermis. Of the 1) and 2) theories we prefer the first, but further study is needed. This is the first report of the existence of CEA in the psoriatic epidermis.

[Severe hyponatremia associated with captopril therapy in dilated cardiomyopathy: a case report].

A 47-year-old man, with dilated cardiomyopathy developed severe hyponatremia and hypoosmolarity during captopril therapy. He also had an inappropriate elevation of antidiuretic hormone and urine osmolarity, but no evidence of dehydration, renal or suprarenal disturbances. The hyponatremia and hypoosmolarity improved after withdrawal of captopril alone, and recurred after readministration of captopril. We conclude that the hyponatremia may be caused by high secretion of prostagrandin and bradykinin associated with captopril therapy.
