A novel epidermal cell differentiation inhibitor (EDIN): purification and characterization from Staphylococcus aureus.

A factor inhibiting the calcium-induced terminal differentiation of cultured mouse keratinocytes was purified to homogeneity from the extracellular products of S. aureus E-1 and designated 'epidermal cell differentiation inhibitor' (EDIN). EDIN activity was sensitive to trypsin and heat-labile, suggesting that EDIN is a protein. EDIN gave a single band with a molecular weight of 27,000 on sodium dodecyl sulfate-polyacrylamide gel electrophoresis and was found to be a single chain polypeptide, having an isoelectric point higher than 9. The N-terminal amino acid sequence of EDIN was determined as A-D-V-K-N-F-T-D-L. EDIN inhibited the differentiation of not only mouse but also human keratinocytes in culture.

[Malignant ovarian tumor invasion to the uterus and bladder: assessment with magnetic resonance imaging].

Twenty-nine patients with ovarian malignant tumor were studied by means of magnetic resonance (MR) imaging. The findings were compared with surgical findings concerning tumor invasion to the bladder and to the uterus. The bladder invasion was evaluated with MRI and classified as follows: (1) Huge amount of fat, colon or small intestine between the tumor and the bladder. (2) Thin fat layer between the bladder and the tumor, (3) No fat layer, and the bladder was smoothly compressed by the tumor, (4) No fat layer with local deformity of the bladder caused by the tumor existed, or tumor protruded into the bladder. In group (4), 4 of 5 cases were proved to have bladder invasion. However in group (3), half of the 14 cases showed bladder invasion and the others showed no invasion. Criteria used in MRI analysis of uterine invasion was as follows: (1) Fat layer between the uterus and the tumor, (2) Fat layer between the tumor and the bladder, and the uterine contour was indistinct, (3) No fat layer and deformity of the uterus was apparent. In group (3), many cases showed uterine invasion, but 4 cases out of 12 cases did not. In the group (2), 3 out of 11 cases showed uterine invasion. The diagnostic ability of uterine invasion was not as accurate as in cases of the bladder invasion. The MRI sagittal plane can directly show the relationship between the uterus, the bladder and tumor, so that diagnosis of tumor invasion by MRI is superior to CT. However MRI could not differentiate between adhesion and invasion.

Mechanism of growth inhibition of melanoma cells by 4-S-cysteaminylphenol and its analogues.

Our previous studies have shown that 4-S-cysteaminylphenol (4-S-CAP) causes a significant inhibition of in vivo melanoma growth and a marked depigmentation of black skin and hair follicles. These studies have suggested a role of tyrosinase in the manifestation of these in vivo effects. In this study 4-S-CAP and its analogues were examined for their effects on the growth of human melanoma cells in vitro. 4-S-CAP and 4-S-HomoCAP exhibited strong cytotoxicity with effects much greater than those of alpha-methyl-4-S-CAP and N,N-dimethyl-4-S-CAP. The cytotoxicity of the former two amines was completely prevented by semicarbazide, an inhibitor of plasma monoamine oxidase, while that of the latter two was not prevented by semicarbazide, catalase, and phenylthiourea, a tyrosinase inhibitor. In culture medium 4-S-CAP was rapidly converted by the action of monoamine oxidase present in fetal bovine serum to the aldehyde which was then metabolized to the alcohol and the carboxylic acid when cells were present. alpha-Methyl-4-S-CAP was found to exert higher cytotoxicity to cells with higher tyrosinase activity and melanin content. These results suggest that the in vitro cytotoxicity of 4-S-CAP and 4-S-HomoCAP is mediated through conversion to the aldehydes while that of alpha-methyl-4-S-CAP appears to be dependent on tyrosinase activity to some extent.

Characterization and anticancer activity of the micelle-forming polymeric anticancer drug adriamycin-conjugated poly(ethylene glycol)-poly(aspartic acid) block copolymer.

Adriamycin (ADR), an anthracycline anticancer drug, was bound to the poly(aspartic acid) chain of poly(ethylene glycol)-poly(aspartic acid) block copolymer by amide bond formation between an amino group of Adriamycin and the carboxyl groups of the poly(aspartic acid) chain. The polymeric drug thus obtained was observed to form a micelle structure possessing diameter of approximately 50 nm, with a narrow distribution, in phosphate-buffered saline and to show excellent water solubility despite a large amount of ADR introduction. Further, it was able to be stored in lyophilized form without losing its water solubility in the redissolving procedure. Increased stability of the bound Adriamycin molecules in phosphate-buffered saline and elimination of binding affinity for bovine serum albumin due to the micelle formation were further advantages of this polymeric drug. In vivo high anticancer activity of this micelle-forming polymeric drug against P 388 mouse leukemia was obtained with less body weight loss than that seen with free ADR, due to low toxicity as compared with free ADR.

A new sialic acid analogue, 9-O-acetyl-deaminated neuraminic acid, and alpha -2,8-linked O-acetylated poly(N-glycolylneuraminyl) chains in a novel polysialoglycoprotein from salmon eggs.

A new polysialoglycoprotein, designated PSGP(On), was isolated from the unfertilized eggs of the kokanee salmon, Oncorhynchus nerka adonis. 400-MHz 1H NMR analyses showed the O. nerka adonis PSGP contained alpha -2,8-linked oligo- and polysialic acid (polySia) chains that were made up of 4-O-Ac-, 7-O-Ac-, and 9-O-Ac esters of N-glycolylneuraminic acid (Neu5Gc) residues. The presence of a new sialic acid derivative, identified by 1H NMR as 9-O-acetyl-2-keto-3-deoxy-D-glycero-D-galacto-nononic acid (trivial name, 9-O-acetyldeaminated neuraminic acid; 9-O-Ac-KDN), was also shown to be present as a minor component. The O-acetylated KDN residues appear to cap the nonreducing termini of the O-acetylated poly(Neu5Gc) chains. The O-acetylated polySia chains were resistant to depolymerization by bacterial exosialidases and a bacteriophage-derived endo-N-acylneuraminidase that is specific for catalyzing the hydrolysis of alpha -2,8-linkages in polySia containing either N-acetylneuraminic acid or Neu5Gc residues. After de-O-acetylation by mild alkali, the polySia chains were sensitive to digestion by endo-N-acylneuraminidase, yet partially resistant to exosialidase. These data confirm the alpha -2,8-ketosidic linkage in these chains and the nonreducing terminal location of the KDN residues. These results extend further the range of structural diversity in polySia-containing glycoconjugates, and in the family of naturally occurring sialic acids. They also suggest that the O-acetylated Neu5Gc and 9-O-Ac-KDN residues may have an important role during oogenesis.

Elemental analysis of hair among hydrofluoric acid exposed workers.

The concentrations of fluorine (F), sodium (Na), magnesium (Mg), aluminum (Al), potassium (K), phosphorus (P), calcium (Ca), iron (Fe), nickel (Ni), zinc (Zn) and iodine (I) in the hair of 142 hydrofluoric acid (HF) exposed workers and 273 control subjects were analyzed by wavelength dispersive X-ray fluorescence spectrometry. Postshift urine and serum specimens were also collected for the determination of fluoride. To determine whether external contamination influences hair analysis, the control hair samples were kept in the work environment for one week. With the exception of lower K and Fe values, the levels of F and other elements in the hair of HF workers were higher than in the control subjects. The concentrations of F in hair and in postshift urine were in good correlation (r = 0.65). In the hair of HF workers F and Ca, F and Mg, F and P, F and Al were well correlated with each other (r greater than 0.6). F concentration in the hair increased after the sample was retained, however, it decreased to the reference value after washing. The levels of Ca, Mg and other elements did not change during the experiment.

Lipoma of the esophagus--report of a case and review of the literature.

We report herein, a rare case of esophageal lipoma and review the Japanese literature on this subject. Lipoma of the alimentary tract is relatively uncommon but that of the esophagus is extremely rare with only 17 cases having been reported in Japan. The majority of these cases occurred in the cervical esophagus with the most serious symptom being regurgitation of the pedunculated tumor which lead to asphyxia and death in one case. Only 2 cases occurred in the thoracic esophagus and these tumors were small in size and resected endoscopically. This is the first reported case of an esophageal lipoma being located in the thoracic esophagus which was resected through a thorocotomy. The clinical features of esophageal lipoma are also described herein.

Does H2-receptor antagonist alter the renal function of cyclosporine-treated kidney grafts?

Histamine-type 2 antagonists (H2-blockers) as represented by cimetidine have been shown to adversely affect renal allograft function, particularly when coadministered with cyclosporine, currently a major immunosuppressant. To determine whether or not a newer and more powerful H2-blocker, famotidine, would produce similar adverse effects, we assessed seven cyclosporine-treated renal allograft recipients with regard to changes in their renal function on or off the H2-blocker over a one-week period. Neither the administration nor withdrawal of famotidine (20-40 mg/day) resulted in any significant changes in serum creatine, BUN, urine output or cyclosporine trough levels, suggesting that famotidine can be safely administered as an H2-blocker to cyclosporine-treated renal allograft recipients.

Thymic carcinoid--a case report and review of the literature.

We report herein a case of a patient with thymic carcinoid who was operated on twice and has been followed for 9 years. In 1979, the original tumor was removed through an emergency left thoracotomy incision done for a hemothorax caused by an anterior mediastinal biopsy. Tumor recurrence was found 6 years later and removal carried out through a median sternotomy. The patient has been working and enjoying life following radiation and chemotherapy for a total of 9 years after his first operation. Recurrent thymic carcinoid has been thought to carry a poor prognosis, but this successfully treated case has been followed up for a long time after the removal of his recurrent tumor. We present this case and discuss other such cases reported in the Japanese literature.

Androgen metabolism in rat L6 myoblast cells; high formation of 5 alpha-androstane-3 alpha,17 beta-diol from testosterone.

We have studied androgen metabolism in L6 rat myoblasts. 4-androstene-3,17-dione (Adione), testosterone, 5 alpha-dihydrotestosterone (DHT), and 5 alpha-androstane-3 alpha, 17 beta-diol (3 alpha-diol) were used for substrates and the amounts of metabolites formed from the respective substrates in the medium were determined. Conversion of Adione to testosterone was dominant over the reverse conversion. DHT formation from testosterone was low and did not change with the duration of incubation, whereas 3 alpha-diol formation increased in a time-dependent manner. Major metabolite of testosterone was not DHT but 3 alpha-diol. A large amount of 3 alpha-diol was formed from DHT, however, DHT formation from 3 alpha-diol was very low. These data indicate that L6 cells have high 5 alpha-reductase activity and suggest that DHT formed from testosterone is rapidly metabolized to 3 alpha-diol in these cells.

Very-low-calorie diet-induced weight reduction reverses impaired growth hormone secretion response to growth hormone-releasing hormone, arginine, and L-dopa in obesity.

To determine whether impaired growth hormone (GH) secretion in obese subjects is a consequence of obesity or a pre-existing pituitary-hypothalamic disorder, we measured (1) plasma GH response to growth hormone-releasing hormone (GRH; 1 microgram/kg body weight [BW]), arginine (0.5 g/kg BW), and L-dopa (500 mg); and (2) plasma glucose, insulin, and free fatty acids (FFA) in obese subjects before and after weight reduction due to very-low-calorie diet therapy using Optifast (240 kcal/d for 8 to 12 weeks). Body weight and body mass index (BMI) values before and after weight reduction were 87.2 +/- 4.1 kg and 34.5 +/- 0.9 kg/m2, and 67.8 +/- 2.7 kg and 27.0 +/- 0.4 kg/m2, respectively. GH response to GRH, arginine, and L-dopa in obese subjects was markedly impaired before weight reduction, whereas significantly increased responses were noted after weight reduction (P less than .01). Impaired integrated GH response to GRH, arginine, and L-dopa in obese subjects was significantly restored after weight reduction (P less than .01). Plasma glucose levels did not change, while plasma insulin and FFA levels decreased significantly after weight reduction (P less than .01, P less than .05). There was no significant correlation between integrated GH response to these three stimuli and plasma levels of glucose, insulin, and FFA, respectively. The reversibility of GH response to all three stimuli after weight reduction suggests that impaired GH secretion is a consequence of obesity rather than a pre-existing pituitary-hypothalamic disorder.

Amino acid sensors sensitive to alanine and leucine exist in the hepato-portal system in the rat.

We reported the existence of vagal arginine sensors in the liver which modulate arginine-induced pancreatic hormone secretion. The present study was carried out to examine the possible existence of other amino acid sensors such as L-alanine and L-leucine in the hepato-portal system in rats using an electrophysiological approach. Afferent discharges were recorded from fine filaments dissected from the peripheral cut end of the hepatic branch of the vagus nerve. Administration of 0.1, 1, and 10 mM L-alanine and L-leucine solution (0.1 ml) into the portal vein caused an increase in the discharge rate of hepatic vagal afferents in a dose-dependent manner. The results suggest the existence of vagal amino acid sensors which are sensitive to alanine and leucine in the hepato-portal system.

Efficiency of skin sterilization for a venipuncture with the use of commercially available alcohol or iodine pads.

Skin sterilization for a venipuncture is routinely done with commercially available alcohol or iodine pads. Selection of the antiseptics, alcohol or iodine, however, in most situations has been made on the basis of very little scientific data. With many patients with granulocytopenia who are undergoing venipunctures, the choice of antiseptic may be an important factor in preventing infections. We investigated two widely and commercially available disinfectant pads, alcohol and iodine, in the efficacy of skin sterilization. Seventy subjects (35 adults and 35 children) were randomly selected for this study. A designated area of the right and left forearm was sterilized either with alcohol or with an iodine pad in a predetermined uniform fashion. Specimens were obtained for cultures before and after sterilization. The bacterial cultures were performed with the use of blood agar plates and trypticase soy broth. For data analysis growth of any organisms on agar plates or trypticase soy broth after sterilization was interpreted as a sterilization failure. The iodine swab was significantly more efficient than the alcohol swab; the former yielded an 80% sterilization rate whereas the latter resulted in a rate of 61% (p less than 0.02). If, however, the growth only in agar plates was compared, the alcohol wipe yielded no growth in 83% and iodine in 84%, virtually identical success rates. Bacillus spp. predominated the residual organisms after either the alcohol or the iodine wipe. A variety of other organisms, however, including Staphylococcus and Corynebacterium spp., grew after alcohol but not after iodine sterilization.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of vitamin A in enteral formulae for burned guinea-pigs.

A burned guinea-pig model (30 per cent body surface area) was used to study the effects of dietary vitamin A. Sixty-five female guinea-pigs were infused enterally via gastrostomy feeding tubes with identical formulate (175 kcal/kg/day, 20 per cent of calories as protein) containing varying amounts of vitamin A. Groups I, II, III and IV received formulae containing 0, 10,000 iu (approximately equivalent to the guinea-pigs' RDA), 50,000 iu (5 x RDA) and 250,000 iu (25 x RDA) of vitamin A per litre, respectively. After 14 days of tube feeding, the animals were killed. Group I animals had evidence of vitamin A deficiency including low haemoglobin levels, lower red blood cell counts and lower caecal mucosal weight. Findings of hypervitaminosis A were observed only in animals given the highest dose of vitamin A (25 x RDA). These were elevated serum alkaline phosphatase and complement C3 levels and enlarged adrenal glands. Group IV also showed defective cell-mediated immunity as reflected by reduced delayed cutaneous response to dinitrofluorobenzene. In a second experiment groups I, II, III and IV were given formulas containing 0, 1 x RDA, 5 x RDA, and 10 x RDA of vitamin A respectively for 14 days. Through postburn days 12 to 14 they were injected subcutaneously with 3 x 10(8) of Staphylococcus aureus once daily. On postburn day 15 the animals were killed and the numbers of viable bacteria at each injection site were counted. No significant differences were observed in viable bacterial numbers between the groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of intraluminal antibiotics on translocation of Candida albicans in burned guinea-pigs.

Guinea-pigs were pretreated orally with various antibiotics and given 3 x 10(10) Candida albicans by gastric lavage followed by a 40 per cent TBSA full skin thickness burn. The mesenteric lymph nodes, liver and spleen were cultured for the presence of viable organisms. Caecal contents were quantitatively cultured for aerobic bacteria and C. albicans. Clindamycin and penicillin G were the greatest promoters of translocation followed by the combination streptomycin/bacitracin. The mechanism for antibiotic-induced translocation is multifactorial centering on intestinal flora, anaerobic spectrum of the antibiotic and host defense as well as microbe virulence. The systemic use of broad-spectrum antibiotics, particularly those with strong anaerobic activity, should not be taken too lightly. A severely immunocompromised patient on this type of therapy may be prone to a severe fungal infection. This study reaffirms the concept that translocation from the gastrointestinal barrier is a potential source of life-threatening nosocomial infection.

Purification of staphylococcal exfoliative toxin by high pressure liquid chromatography.

Exfoliative toxin (ET) isolated from a clinical strain of Staphylococcus aureus was purified to homogeneity, using a 3-step HPLC system. NH2-terminal 20 amino residues of purified ET was found to be identical with ETA of S. aureus TA (7), S. aureus TC16 (9) and S. aureus ZM (10), but stability of purified ET was completely different from that of ETA. This purification system gave a high yield of pure ET, which exhibited higher purity than specimens purified by more complicated and time-consuming procedures. It is useful for small-scale purification for the comparative study of ET and easy to scale up for preparative purification.

Mandibular reconstruction using electrically stimulated periosteum.

Electrically stimulated periosteum (ESP) grafting to a 12-year-old female patient who had had a segmental mandibulectomy in the anterior region following a diagnosis of Ewing's sarcoma was tried for the mandibular reconstruction. One year after the ESP transplantation, examination of the patient showed that fine, radio-opaque bone formation had occurred. The new technique consists of 1) electrical stimulation of the tibial periosteum with 20 microamperes for five weeks, and then 2) ESP transplantation into a titanium mesh tray placed in the mandibulectomized region. In the present report, these new procedures are detailed, and the process of calcification in the transplanted ESP is discussed.
