Morphological classification of atrial muscle in the atrioventricular junctional area--3-dimensional reconstruction of serial sections of the human heart.

To trace anatomical structures that might be associated with the dual atrioventricular (AV) nodal pathway and to investigate the morphologic characteristics of the cells that form these pathways, we examined serial sections of the AV junctional area with a light microscope and reconstructed them 3-dimensionally with a computer. Twelve hearts were obtained at autopsy from patients who had not shown AV conduction disturbances or supraventricular tachycardia before death. The method of Lev et al was used to prepare serial sections. Fascicles of atrial muscle contiguous with the AV node were examined with a light microscope and were classified into 3 groups, on the basis of morphologic characteristics and myocyte diameter. A computer was used to reconstruct 3-dimensionally the course of the fascicles and surrounding structures. At the border of the AV node and bundle of His relatively large myocytes extended directly into the AV bundle from the anterosuperior interatrial septum. Morphologically, the course was considered to be consistent with the fast pathway. In contrast, small cells that entered the AV node from the inferoposterior interatrial septum resembled sinus node cells with few myofibrils and a winding shape. These cells extended from the coronary sinus ostium to the tricuspid valve annulus and are thought to make up the slow pathway.

[Antihypertensive effects of a novel calcium antagonist, AE0047, in various hypertensive models].

The antihypertensive effects of oral or intravenous administration of AE0047, a novel 1,4-dihydropyridine-type calcium antagonist, were investigated in spontaneously hypertensive rats (SHR/crj), one kidney-one clip renal hypertensive rats (RHR), deoxycorticosterone acetate (DOCA)-salt hypertensive rats (DHR) and two kidney-one clip renal hypertensive dogs (RHD). AE0047 (1, 3, 10 mg/kg, p.o.) caused a dose-related reduction of systolic blood pressure (SBP) with low reflex tachycardia in SHR/crj and RHR. The effect reached its maximum at 2-4 hr after administration and was sustained for a long time. In DHR, AE0047 (0.3, 1, 3 mg/kg, p.o.) similarly showed the antihypertensive effects at 2-7 hr with no significant changes in heart rates (HR). The doses (ED30) of AE0047 required to decrease SBP by 30% were 2.6, 3.4 and 0.68 mg/kg in SHR/crj, RHR and DHR, respectively. In RHD, and AE0047 capsule (GJ-0956: 4, 8, 16, 32 mg/body, p.o.) produced dose-dependent and long lasting effects with a transient and slight increase in HR. Furthermore, the intravenous administration of AE0047 (10, 30, 100 micrograms/kg) produced the antihypertensive action slowly, reached a plateau 10 min later and then maintained for many hours. In contrast, nitrendipine (3-100 mg/kg, p.o., 3-30 micrograms/kg, i.v.) and nicardipine (1-30 mg/kg, p.o., 3-30 micrograms/kg, i.v.) exhibited a similar potency to AE0047, but these maximal effects were produced at 1-2 hr and 0.5-1 min in the case of oral and intravenous administration, respectively, with a rapid recovery in the above hypertensive rats. These results indicate that AE0047 exhibits an antihypertensive effect with a slow onset and long-lasting profile.

[New medicinal action of native hormone].

Estrogen is involved in the growth and development of female organs such as uterus and mammary gland. On the other hand, from clinical point of view, it is recently suggested that estrogen is effective to protect postmenopausal women from osteoporosis, coronary heart disease and Alzheimer disease. In order to study the molecular mechanism of estrogen action, we have identified an estrogen responsive gene, efp (estrogen-responsive finger protein), which might mediate estrogen action in various target organs at diverse stages and targeted mutagenesis of efp gene could help clarify physiologic actions of estrogen.

Characterization of the bovine prion protein gene: the expression requires interaction between the promoter and intron.

We cloned the part of the bovine PrP gene which contains the 5'-flanking region, exon 1, exon 2 and intron 1 to analyze its promoter region. The 5' non-coding region of the bovine PrP gene consisted of three exons and two introns, and its organization was similar to that of the mouse, rat and sheep PrP genes. The 5'-flanking region of the bovine PrP gene from the transcription start site to nucleotide position -88 was (G + C)-rich (78%) and contained three potential binding sites for the transcription factor Sp1, but no CCAAT-box or TATA-box. This region showed high homology (89%) with that of the sheep PrP gene, but relatively low homology (approximately 46-62%) with the same region of the mouse, rat, hamster and human PrP genes. The position from -88 to -30 within the 5'-flanking region of the bovine PrP gene showed major promoter activity. However, this region was able to function properly only in collaboration with the region at +123 to +891 of intron 1 of the bovine PrP gene.

Association of bone mineral density with apolipoprotein E phenotype.

The phenotypes of apolipoprotein E (Apo E) and their relationship with the bone mineral density (BMD) were examined in 284 unrelated postmenopausal Japanese women aged 47-82 years (64.0 +/- 1.0 years, mean +/- SE). The Apo E phenotype was analyzed by the isoelectric focusing method, followed by immunoblotting. The relationship between the Apo E phenotype and the vitamin D receptor (VDR) gene or estrogen receptor (ER) gene genotypes was also studied in the same population. The Apo E phenotypic frequencies in our population were 9.9% for E3/2, 66.5% for E3/3, 1.8% for E4/2, 19.7% for E4/3, and 2.1% for E4/4. We classified these phenotypes into three categories: Apo E4-/- (E3/2 and E3/3, n = 217, Apo E4 +/- (E4/3 and E4/2, n = 61), and Apo E4+/+ (E4/4, n = 6). The age, body weight, body height, and years since menopause were not significantly different among these three categories. The lumbar BMD values in these three groups were significantly different in the order of E4-/- (0.91 +/- 0.01 g/cm2), E4 +/- (0.85 +/- 0.02 g/cm2), and E4+/+ (0.83 +/- 0.06 g/cm2) (p = 0.031). The same trend was also observed for the Z score of the total BMD (p = 0.022). The serum level of intact osteocalcin in E4+/+ (15.2 +/- 5.7 ng/ml) was higher than in E4-/- (7.7 +/- 0.3 ng/ml) or E4 +/- (7.7 +/- 0.7 ng/ml) (p = 0.004 by analysis of variance). However, there were no other significant differences in the serum or urinary levels of bone turnover markers. Serum cholesterol in the E4+/+ group tended to be higher than in the other two groups (p = 0.05). There were no significant associations of the VDR and ER genotypes with the Apo E4 phenotype. A multivariate linear regression analysis revealed Apo E4 to be a significant, independent predictor of the Z score of the lumbar BMD. The effect of the Apo E4 allele on the Z score of the lumbar BMD (-0.493 +/- 0.152) was not significantly different from that in the AAB of VDR (-0.616 +/- 0.225) or PPxx of ER (-0.785 +/- 0.314). In conclusion, the Apo E4 allele is associated with a low bone mass in postmenopausal Japanese.

Follow-up after the Hanshin-Awaji earthquake: diverse influences on pneumonia, bronchial asthma, peptic ulcer and diabetes mellitus.

We evaluated the influence of the Hanshin-Awaji earthquake on the patients in our internal medicine department. After the initial rush of patients with injury, the number of respiratory diseases, largely pneumonia, increased within one month. This same event, however, seemed to decrease attacks among asthma patients. During the following three months, the number of peptic ulcer patients increased: 39.5% had a giant gastric ulcer and 34.8% had bleeding complications. Diabetic control of outpatients became worse after the earthquake. It is important to recognize that various disorders involving physical and psychological problems develop at different stages after a large-scale disaster.

[The clinical study of radical cystectomy for lymph node positive bladder cancer].

BACKGROUND: The impact of pelvic lymph node dissection on the survival of patients with lymph node positive bladder cancer is controversial. We analyzed the records of the bladder cancer patients treated with radical cystectomy. METHODS: Between 1976 and 1993, 84 patients with transitional cell carcinoma of the bladder were treated with radical cystectomy at Tokyo Metropolitan Komagome General Hospital. Sixteen patients (19.0%) had pathologically proved nodal metastases. These patients' records were reviewed with regard to pathological status, and the outcome compared with negative lymph node patients. The two patients with pN3, which we could not completely undergo lymph node dissection of, were excluded from the analyses for recurrence and survival. RESULTS: There were 10 males and 6 females, aged between 47 and 83 with a mean of 68.3 years. There were significant differences between the patients with positive lymph node and negative in the rate of grade 3 and the rate of infiltration of cancer to the vein and lymphatics of the bladder walls. The frequency of the patients of positive lymph node was 5.3% in pT1, 10.0% in pT2, 28.6% in pT3a, 32.0% in pT3b, 50.0% in pT4, and none in pTis, pTa and pT4is. The five-year survival rates estimated by the Kaplan-Meier method were 68.6% over all 82 patients, 75.7% for the patients with negative lymph node and 30.7% for positive. Of the patients with positive lymph node, two-year survival was 80.0% for pT2-3a, versus 0% for pT3b-4. This difference was highly significant (p < 0.01). But survival analysis for pT2-3a showed no significant difference between the lymph node negative patients and positive. The recurrence rate was 57.1% (8 of 14) in patients with positive lymph node and seven patients suffered from distant metastases. All the patients which had recurrent tumors died by bladder cancer. CONCLUSION: Radical cystectomy with lymph node dissection can provide favorable outcome in lymph node positive patients with low stage (pT2-3a). But the patients with high stage (> or = pT3b) were poor prognosis. Most of the patients died by distant metastases.

Mode of antifungal action of benanomicin A in Saccharomyces cerevisiae.

The mechanism of fungitoxic action of an antifungal antibiotic benanomicin A was studied with intact cells and protoplasts of Saccharomyces cerevisiae as well as with its enzymic preparations. The results obtained are summarized as follows: (1) benanomicin A at relatively high concentrations (almost equal to MIC) was fungicidal and disrupted the cell permeability barrier, inducing leakage of intracellular K+ and ATP in growing cells, while the antibiotic had none of these effects in non-growing cells; (2) no biosynthesis of any of several major cellular constituents in yeast cells was inhibited markedly or selectively enough to explain its fungitoxic activity; (3) whereas benanomicin A induced lysis of metabolically active yeast protoplasts incubated in the presence of glucose, inactive yeast protoplasts incubated without glucose were refractory to the lytic action of the antibiotic; (4) osmotically shocked yeast cells became feasible to the cidal action of benanomicin A; (5) benanomicin A substantially inhibited uptake of 6-deoxy-glucose by yeast cells; (6) liposomes composed of phospholipids and cholesterol were not susceptible to benanomicin A; and (7) benanomicin A inhibited in vitro activity of H(+)-ATPase from yeast cell membranes to a greater extent than that for H(+)-ATPase from yeast mitochondria or H(+)-ATPase from yeast vacuolar membranes. Based on these and our previous data that benanomicin A preferentially binds to mannan or mannoproteins constituting the cell wall and cell membrane of yeasts, such binding of the antibiotic is suggested to deteriorate the normal structure and function of those cell membranes of yeasts which are in a growing or metabolically active state, ultimately leading to cell death.

Distribution of Rift Valley fever among cattle in Zambia.

In the present study, 1,421 cattle in 32 herds within nine districts, which are important cattle-producing centers in the nine provinces of Zambia, were tested for Rift Valley fever by the indirect immunofluorescence assay. One hundred and forty-seven cattle (10.5%) in 28 herds (88.9%) in the nine districts tested were positive for Rift Valley fever implying a country-wide distribution. In districts associated with flood plains and/or "dambos" (low lying areas of perpetual flooding), high herd and individual positive rates (100% and > 10%, respectively) were found, suggesting a significance of these features in the distribution of the disease.

[A case of isolated ACTH deficiency with neuromuscular symptoms].

A female case of isolated ACTH deficiency associated with neuromuscular symptoms was reported. Although her initial development was delayed with perinatal troubles, developmental catch up was seen. The patient complained of general fatigue with weakness and poor school performance at the age of eleven. Muscle weakness predominant to the proximal portion and mental dullness were suspected from neurological examination. Her laboratory data were as follows; mild elevation of CK in serum, subclinical thyroidal dysfunction, abnormal electrocardiogram, slow wave activity on electroencephalogram, delayed nerve conduction velocity, and localized hypoperfusion of cerebral blood flow on single photon emission computed tomogram. Histological examination of muscle biopsy demonstrated only mild fiber size variation. During two years' follow-up, her intelligence quotient fell down, while muscle weakness did not progress significantly. At the age of fourteen, a low level of plasma ACTH was pointed out by chance and a definitive diagnosis was obtained by endocrinological examinations; no response of ACTH and cortisol on insulin stimulation, delayed response of cortisol on continuous ACTH stimulation, and no response of plasma ACTH on corticotropin releasing hormone stimulation. Other signs of adrenocortical insufficiency, such as hypoglycemia and abnormal serum electrolytes, were not observed during the clinical course. This case suggested that isolated ACTH deficiency should be considered for differential diagnosis of neuromuscular disorders.

[Prostagrandin E1-induced hypotension well maintains cerebral circulation and carbon dioxide reactivity in non-neurosurgical patients under sevoflurane-anesthesia].

To evaluate the effect of prostagrandin E1 (PGE1)-induced hypotension on cerebral blood flow (CBF) and carbon dioxide (CO2) reactivity of CBF, regional cerebral hemoglobin oxygen saturation (rSo2) was measured in non-neurosurgical patients (n = 10) under sevoflurane-anesthesia using near infrared spectroscopy. PGE1 was infused intravenously to maintain arterial pressure at a level of about 75% of the MAP (hypotensive group) under sevoflurane-anesthesia alone (normotensive group). Ventilation was controlled to adjust PaCO2 to hypocapnia (25-30 mmHg), normocapnia (35-40 mmHg) and hypercapnia (45-50 mmHg) in both normotensive and hypotensive groups. rSo2 during hypotension did not change by hypocapnia and normocapnia, but significantly increased by hypercapnia, compared with rSo2 during normotension. Significant correlations between rSo2 and PaCO2 during both normotensive and hypotensive groups were observed. Slope of the regression line of rSo2 and PaCO2 did not differ between the normotensive and hypotensive groups. When arterial oxygen content and cerebral metabolic rate of oxygen are constant, changes in rSo2 correlate with those of CBF. Therefore, CBF and CO2 reactivity of CBF that indicates autoregulation in response to changes in CO2 during hypotension were maintained as those during normotension. The results show that PGE2-induced hypotension maintains CBF and CO2 reactivity well in non-neurosurgical patients under sevoflurane anesthesia.

[Pulmonary infection with Mycobacterium kansasii presenting as solitary nodule shadow in the left anterior basal segment].

A 71-year-old man was admitted to our hospital for evaluation of a solitary pulmonary nodule in the left lower lung field. He had been treated for Parkinson's disease for two years, and had no respiratory symptoms on admission. No abnormal findings were detected in laboratory tests on admission, and Mantoux's skin test was negative. A postero-anterior chest roentgenogram, a conventional tomogram, and a computed tomogram showed that the nodule was located in the left anterior basal segment (S8). The nodule was not calcified, the contour was clear but irregular, and pleural indentation could be seen, so the nodule was strongly suspected to be a primary malignant lung tumor. Because two attempts at transbronchial lung biopsy and bronchial brushing and washing were of no diagnostic value, a thoracotomy was done. The lesion was found in the subpleural region of the left S8 with pleural indentation, and it was partially resected. The nodule was elastic, soft, and filled with suppurative fluid. Histopathologic examination of the nodule revealed epithelioid cell granuloma. A smear test of the fluid was negative but a culture was positive for mycobacteria; which were identified as Mycobacterium kansasii. Rifampicin and isoniazid were administrated to the patient for 1 year. Two years after the operation, the patient was asymptomatic.

[Encephalitis located at bilateral corpus callosum on MRI--a case report].

A case of viral encephalitis is described. A 38 year-old female was admitted because of high fever accompanied by cough and headaches. Two days after admission the patient began to exhibit abnormal behavior. Cerebrospinal fluid examination revealed pleocytosis. T2-weighted MRI revealed a high-intensity area located in the corpus callosum bilaterally that gradually increased in intensity within several days the patient's behavior returned to normal. The high-intensity area on MRI persisted for several months but diminished in intensity. Rubella may have been the etiology of the encephalitis.
