Immunohistochemical localization of glomerular basement membrane antigens in various renal diseases.

The immunofluorescent localization of glomerular basement membrane (GBM) antigens was examined in 52 specimens from normal kidneys and in various renal diseases using antisera to human GBM (HGBM), IV type collagen (IV Col) and P3 antigen, a rat nephritogen. Anti-HGBM serum normally stained the GBM and the mesangium in a restrictive pattern, anti-IV Col serum stained the GBM and the mesangium in a wider pattern and anti-P3 serum stained only the GBM. In mesangial proliferative glomerulonephritis, including IgA nephropathy and Henoch-Schönlein nephritis, the widened mesangial areas were stained with anti-HGBM and anti-IV Col sera. In membranous nephropathy, the punched-out lesions of thickened GBM were demonstrated with the three antisera in moderate cases and a double linear distribution with fine granulation with anti-HGBM and anti-IV Col sera were revealed in one severe case. In membranoproliferative glomerulonephritis, the expanded mesangium and thickened capillary walls were stained with anti-HGBM and anti-IV Col sera, while the outer line of glomerular capillary walls was only positive with anti-P3 serum. In crescentic glomerulonephritis, the collapsed glomerular tufts were stained normally with anti-HGBM and anti-P3 sera and weakly with anti-IV Col serum. In diabetic nephropathy, anti-HGBM serum stained the GBM in a double linear distribution without reacting with the expanded mesangium; anti-IV Col serum stained the mesangium and the GBM in a less clear double linear fashion while anti-P3 serum stained the GBM as single line. Thin membrane disease and Alport's syndrome had normal reactivity with all antisera. However, in one case of Alport's syndrome anti-HGBM and anti-P3 sera stained the GBM in a focal and segmental pattern, while normal staining with anti-IV Col serum was found. In lesions with adhesions and crescents the staining was positive for HGBM and IV Col and negative for P3; obsolescent glomeruli were stained with anti-HGBM and anti-P3 sera, and had diminished staining with anti-IV Col serum. The identification of the various structural glomerular antigens is useful in the classification of certain types of glomerular diseases. Further insight into the mechanisms underlying these conditions may be obtained in this way.

Antigens

Glomerulonephritis with focal and segmental distribution of glomerular basement membrane antigen(s).

In this report, the authors describe a case of a 2-year, 11-month-old girl with glomerulonephritis and no family history of renal diseases and deafness. Immunofluorescent studies in the renal biopsy specimens with the use of anti-sera against human glomerular basement membrane (GBM) and P3 antigen (prepared from bovine GBM and inducible of Steblay's type nephritis in rats) demonstrated focal and segmental distribution of the GBM antigen(s). Electron microscopic examination revealed splitting and thinning of the GBM. Indirect immunofluorescence showed that there was no binding of Goodpasture's anti-GBM antibodies to the glomeruli. These findings are similar to those in patients with hereditary nephritis. The immunofluorescent examination of the fixation of the various anti-sera, including anti-types IV and V collagens, laminin, fibronectin, and actomyosin sera on the GBM, revealed normal reactivity. The abnormalities observed in this case may be a part of the spectrum of primary GBM defects.

Antigens

[VEPA therapy in B-lymphoma].

48 previously untreated adult patients with B-lymphoma were treated with VEPA therapy. The complete remission rate was 79.2%. 48% of the patients were still alive after 4 years of the therapy. About 90% of the patients of stage II was expected to be cured by the therapy. VEPA therapy is greatly effective to B-lymphoma.

[The effects of serum from tumor-bearing mice on mitogen-induced blastogenesis of normal spleen cells].

In order to detect the serum immunosuppressive factors of tumor bearer, the effects of normal C3H/He mice serum (NMS), serum from methylcholanthrene induced fibrosarcoma bearing mice (F4w serum) and sera fractionated by high performance liquid chromatography (HPLC) using anion exchange column were examined on mitogen-induced blastogenesis of normal murine spleen cells (Nspc). The addition of 5% of NMS suppressed the blastogenesis of Nspc. Less than 0.6% of F4w serum suppressed the blastogenesis. NMS was applied to HPLC and was separated to four peaks (A,B,C,D). In HPLC pattern of F4w serum, peak B was higher than that of NMS and new peak (E) was observed next to peak D. Peak D of NMS suppressed the blastogenesis. Not peak D but peak E of F4w serum suppressed the blastogenesis. The properties of those blastogenesis-suppressive fractions were discussed.

Animals

Acute inhalation of cigarette smoke augments hypoxic chemosensitivity in humans.

Hypoxic and hypercapnic ventilatory responses were measured after two levels of acute inhalation of cigarette smoke, minimum-level nicotine smoke (smoke 1) and nicotine-containing smoke (smoke 2), in 10 normal men. Chemosensitivity to hypoxia and hypercapnia was assessed both in terms of slope factors for ventilation-alveolar PO2 curve (A) and ventilation-alveolar PCO2 line (S) and of absolute levels of minute ventilation (VE) at hypoxia or hypercapnia. Ventilatory response to hypoxia and absolute level of VE at hypoxia significantly increased from 23.5 +/- 22.6 (SD) to 38.6 +/- 31.3 l . min-1 . Torr and from 10.6 +/- 2.5 to 12.6 +/- 3.5 l . min-1, respectively, during inhalation of cigarette smoke 2 (P less than 0.05). Inhalation of cigarette smoke 2 tended to increase the ventilatory response to hypercapnia, and the absolute level of VE at hypercapnia rose from 1.42 +/- 0.75 to 1.65 +/- 0.58 l . min-1 . Torr-1 and from 23.7 +/- 4.9 to 25.5 +/- 5.9 l . min-1, respectively, but these changes did not attain significant levels. Cigarette smoke 2 inhalation induced an increase in heart rate from 64.7 +/- 5.7 to 66.4 +/- 6.3 beats . min-1 (P less than 0.05) during room air breathing, whereas resting ventilation and specific airway conductance did not change significantly. On the other hand, acute inhalation of cigarette smoke 1 changed none of these variables. These results indicate that hypoxic chemosensitivity is augmented after cigarette smoke and that nicotine is presumed to act on peripheral chemoreceptors.

Adult

[An analysis of 103 patients with renal injuries].

Between 1971 and 1983, 103 patients with renal injury were admitted to the Department of Urology, Tatebayashi Kosei Hospital. There were 87 males and 16 females. The most frequent age group seen (27.2% of patients) was the 10 to 19 year group. The various causes included traffic accidents (55 patients), falls (16 patients), athletic injuries (8 patients), labor accidents (7 patients), blows (6 patients) and others (11 patients). Associated injuries were present in 52.4% of the patients, including thoracic injuries such as rib fracture (20 patients), lumbar vertebra and transverse process fractures (18 patients), and head injuries (12 patients). Evaluation of the renal injuries was made by excretory urogram (IVP) and conservative management was for most, contusion and rupture, and operation for laceration as a principle: 95 patients (97 renal injuries) were treated by conservative management and 7 patients were treated surgically, 5 patients by nephrectomy and 2 patients by perirenal drainage. Nine patients who had associated injuries were treated surgically. In the follow-up study of 41 patients who had received conservative management and operation (perirenal drainage), followed for more than 6 months and had IVP, urinalysis and blood pressure measurement, representative changes were calyceal deformity (3 patients), renal calculus formation (2 patients) and non-functioning kidney (2 patients) in urograms.
