Atypical pityriasis rosea.

Six cases of pityriasis rosea with atypical morphology and distribution of the eruption are reported. The eruption did not show a typical 'Christmas-tree' arrangement, confined to the trunk and proximal parts of the extremities. However, the histology of the eruption revealed dyskeratotic cells in the epidermis and extravasated erythrocytes in the dermis, which were recently reported as rather characteristic findings of this disease. Prodromal symptoms, course and response to therapy were compatible with pityriasis rosea. Histological examination is important and helpful for the diagnosis of atypical cases.

[Lectin immunoradiometric assay versus improved radioimmunoassay: a comparison of methods for determination of desialylated forms of human chorionic gonadotropin].

Two different assay systems have been developed for the measurement of asialo hCG(ashCG). One of them is an immunoradiometric assay which utilizes peanut lectin (PNA) to selectively extract ashCG and then directly quantify it with purified radiolabeled antiserum which is generated against carboxy terminal peptide of hCG beta (beta-CTP). PNA is highly specific for the terminal carbohydrate linkage Gal beta-(1----3)-Gal NAc which is only exposed to hCG when the O-serine linked oligosaccharide of its unique beta-CTP region is deficient in sialic acid. The other is an improved RIA using a specific antiserum to as beta-CTP, in which extraction of hCG with monoclonal antibody-Sepharose 4B conjugate is used to obtain increased sensitivity with retention of specificity. The ashCG concentration can be measured as low as 0.01 and 0.05 pmoles/ml in the presence of an excess amount of hCG in each of these assays. These assays were applied to detect ashCG in the urine of patients with trophoblastic tumors and normal pregnant women. The proportion of ashCG to total hCG in choriocarcinoma was significantly higher than that in normal pregnancy in both assays. However, the proportion was dependent upon the amount of total hCG and it decreased as total hCG increased.

[Monitoring of follicles by ultrasonography during induction with HMG-HCG treatment--especially on the prevention of side effects].

Estrogen levels are a useful indicator to use in predicting of ovarian hyperstimulation syndrome (OHSS) which is one of the side effects of HMG-HCG therapy. However, the quantitative assay of estrogens entails cumbersome time-consuming procedures. The present study represents our attempt to establish criteria for predicting the occurrence of OHSS by the use of ultrasonography (USG), a diagnostic procedure that can be performed quickly and conveniently. The subjects were 40 anovulatory women (79 cycles) receiving HMG-HCG therapy. Each patient had USG performed at the time of switching to HCG in a regimen of sequentially administered gonadotropins and was measured for maximum follicular diameter (FD) and total of vertical follicular area (FA) to correlate measurements of these parameters with simultaneously determined serum estradiol (E2) levels. A study was also made of relationships of FD and FA with ovulation and OHSS. The results are summarized as follows: No distinct correlation was observed between FD and E2 (r = 0.3794). It should be noted, however, that the therapy was successful in inducing ovulation in those cases in which the patient was switched to HCG from HMG when FD was 18mm or above. There was a significant correlation between FA and E2 (r = 0.8113, p less than 0.001). FA was thus proven to well reflect E2 levels and hence to be a parameter of the predictive value for OHSS. All but one (with moderate OHSS) of 26 cases showing evidence of OHSS had FA values of more than 6.0cm2, while those developing severe OHSS invariably.(ABSTRACT TRUNCATED AT 250 WORDS)

Ultrastructural observations of skin lesions in MRL mice--dermoepidermal junction.

The MRL-lpr/lpr (MRL/l) mouse, a new animal model for the study of human systemic lupus erythematosus (SLE), shows characteristic skin manifestations in addition to several systemic autoimmune phenomena. The ultrastructural changes observed in the dermoepidermal junction (DEJ) and in the uppermost dermis were: infolding of the DEJ; deformities of the basal lamina--partial disappearance, thickening, hanging down, duplication, and separation from the basal cell membrane; basal laminalike dense material in the uppermost dermis and increased anchoring fibrils; particles composed of circulated half-desmosomes between the basal cells and the basal lamina, and in the uppermost dermis with or without an enclosing basal lamina; cell processes of the basal cells; and invagination of the basal lamina in the basal cells. Most of these findings were similar to the ultrastructural changes observed in the skin lesions of human SLE. The skin eruptions of MRL/l mice might be a new aid in the investigation of the pathogenesis of the skin lesions of human SLE.

Lipodystrophia centrifugalis abdominalis infantilis. A follow-up study.

To determine the course and prognosis and also to reexamine the clinical and histologic features of lipodystrophia centrifugalis abdominalis infantilis, originally described by Imamura et al in 1971, a questionnaire was sent to 94 dermatology and pediatric departments in Japan. Data on 86 cases collected from 44 departments confirmed the unique and distinct features of this disease: onset by age 3, development of the lesion in the groin, axillae, or their surroundings, presence of slightly inflammatory areas around the depressed one, and histologically, loss of subcutaneous fat in the depressed area with panniculitis in the surrounding area. The depressed lesions were found to enlarge centrifugally for several years and to involve a large part of the abdominal and/or chest wall. However, the enlargement was found to stop spontaneously by age 13, and the depressed area tended to undergo recovery thereafter.

Cutaneous metastasis from papillary carcinoma of the thyroid gland. Report of two cases.

Two cases of metastatic skin tumors on the scalp appearing several years after thyroidectomy for thyroid papillary carcinoma are reported. Histologic examination of biopsy specimens from both patients revealed glandular-forming masses with intricate papillary folds in the dermis. The tumor epithelium was composed of single-layered cylindrical and cuboidal cells without atypia. Using the immunoperoxidase technic, many of the proliferating cells were shown to produce triiodothyronine (T3) and thyroglobulin (Tg).

Role of oxygen intermediates in UV-induced epidermal cell injury.

To investigate the role of oxygen intermediates (OIs) in sunburn cell (SC) formation and development of UV-inflammation in vivo, groups of mice were injected intravenously with OI scavengers, including bovine blood superoxide dismutase (SOD), bovine liver catalase, L-histidine, D-mannitol, and saline (controls) before and/or after UV irradiation with sunlamp tubes (mainly 280-320 nm; 300 mJ/cm2; UVR). Ear thickness was measured before and 6 and 24 h after UVR. Ears were removed 24 h after UVR and the number of SCs per unit length of ear epidermis was counted using hematoxylineosin stained sections. The number of SCs was significantly decreased (p less than 0.02) by a single injection of SOD (10-30 units/g body weight) given either just before or immediately after (less than 15 min) UVR, while SC formation was no longer suppressed by injections given more than 2 h before or after UVR. Four repeated injections of SOD (10 units/g) also reduced SC counts but did not significantly alter ear-swelling responses (ESR). Neither SC counts nor ESR were remarkably suppressed by 4 injections of any of the other active OI scavengers, inactivated SOD, or bovine serum albumin. A single injection of diethyldithiocarbamate, an SOD inactivator, significantly augmented SC formation (p less than 0.05), but did not change ESR. These findings suggest that OIs generated by UVR participate in SC formation but are not apparently involved in UV-edema.

The ultrastructural observation of a case of lymphomatoid papulosis.

Electron microscopic studies of biopsy specimens from a patient with lymphomatoid papulosis were performed. In the epidermis, infiltrating lymphoid cells, many of which had highly convoluted nuclei, were revealed to come into close contact to increased Langerhans' cells but did not accumulate to form microabscesses. In the dermis, Langerhans' cells with rather small numbers of Birbeck granules were scattered among the thick infiltration of mononuclear cells showing close apposition with lymphoid cells. Some macrophages contacted the Langerhans' cells and lymphoid cells were also observed. On one occasion, a Langerhans' cell clinged by a lymphoid cell in the uppermost dermis was revealed to expand the cytoplasm through the ruptured basal lamina touching another lymphoid cell in the epidermis.

Bullous pemphigoid-like lesions induced by phenacetin. Report of a case and an immunopathologic study.

An 84-year-old man with chronic eczema and ischemic heart disease had bullous pemphigoid-like lesions develop following the oral administration of phenacetin. The diagnosis was confirmed by routine histopathologic studies and by immunofluorescent and electron microscopy. Since rechallenge with phenacetin resulted in bulla formation, phenacetin was considered to be the causative agent. This patient showed several atypical clinical features, compared with patients with idiopathic bullous pemphigoid, including no associated erythematous macules, a positive Nikolsky's sign, painful superficial ulcers, and nonpruritic bullae. Histopathologically, there was lack of polymorphonuclear leukocyte infiltrate. The immunopathologic characteristics, however, were quite typical for those found in bullous pemphigoid. The antigenic sites for the autoantibodies from our patient were, at least in part, different from those of idiopathic bullous pemphigoid.

Evaluation of circulating immune complexes and antinuclear antibodies in Japanese patients with leprosy.

In 79 patients with leprosy a significant increase of anti-extractable nuclear antigen (ENA) antibodies and circulating immune complexes (CIC) was found. No correlation between CIC and anti-ENA antibodies was demonstrable. Since such a correlation is known from antinuclear antibodies and CIC in patients with systemic lupus erythematosus, it appears likely that anti-ENA antibodies do not play a causative role in CIC-mediated pathogenesis of leprosy.
