The effects of hange-shashin-to on gastric function in comparison with sho-saiko-to.

The effects of "Hange-shashin-to (TJ-14)" on gastric function were examined in comparison with "Sho-saiko-to (TJ-9)". Oral treatment with TJ-14 (125-500 mg/kg) caused dose-dependent suppression of ethanol-induced gastric injury, while it did not suppress gastric lesions induced by water-immersion stress. TJ-9 (125-500 mg/kg, p.o.) suppressed both water-immersion stress-induced gastric lesions and ethanol-induced gastric injury in a dose-dependent manner. Intraduodenal administration of TJ-14 even at 500 mg/kg did not affect gastric juice secretion, while TJ-9 at 125 to 500 mg/kg dose-dependently suppressed gastric juice secretion. TJ-14 (125-500 mg/kg, p.o.) accelerated gastric emptying in normal rats and improved the delayed gastric emptying induced by BaCl2 in a dose-dependent manner, whereas such effect was not noted with TJ-9. Oral treatment with TJ-14 at 500 mg/kg significantly suppressed apomorphine-induced vomiting, but it did not affect copper sulfate-induced vomiting. These results suggest that TJ-14 exhibits an anti-ulcer action (probably based on its ability to protect the gastric mucosa), improvement of gastric emptying and an anti-emetic action. TJ-9 also showed anti-ulcer effects, probably based on its ability to suppress gastric secretion and to protect the gastric mucosa. Thus, the present study demonstrated the effectiveness of TJ-14 and TJ-9 against gastric disease, and provided basic data which explain the differences in clinical application between these two kampo medicines.

Subcutaneous fat distribution of the abdomen and buttocks in Japanese women aged 20 to 58 years.

Subcutaneous fat is an essential element in shaping the body of human beings. In this research, skinfold thickness was measured specifically in 33 regions of the human body, including the abdomen and buttocks. Based on our measurements, the subcutaneous fat distribution was assessed for several age groups. The subjects were healthy Japanese women aged 20 to 58 years. Skinfold thickness was measured using the B-mode ultrasound methods, together with anthropometric measurement. A comparison was made between the following five age groups: early 20's, late 20's, 30's, 40's and 50's. The measured values for the early 20's group were used as the standard and the relationship between increase ratio of subcutaneous fat and age was studied. Through our research, we obtained data on the subcutaneous fat distribution in each age group. The largest change was observed between the ages of the early 20's and late 20's. The skinfold thickness measurements of the abdomen and buttocks was consistently around 10 mm for the early 20's, and increased up to 23.8 mm on the rear side section for the late 20's. This result indicates that the increase ratio varied depending on the part of body. Furthermore, the changes in skinfold thickness were different in specific parts of the abdomen and buttocks among different age groups. The difference in skinfold thickness between upper and lower sections of the abdomen also becomes more pronounced with age. Skinfold thickness increased significantly between the early 20's and late 20's. Among the body regions, measurements at the rear side showed the largest change with age; averaging 11.3 mm for the early 20's compared to 33.6 mm for the 50's. The subcutaneous fat distribution on the buttock also showed the differences with age, indicating changes in body shape. Using careful measurements of the abdomen and buttocks, subcutaneous fat distribution among each age group was determined as well as the variation in changes with the aging process.

The effects of air mattress upon sleep and bed climate.

The purpose of this study was to investigate the effects of an air mattress upon sleep and bed climate. This air mattress, which employs a pump and timer to increase or decrease the inflation pressure in order to cure and prevent decubitus was tested. Six healthy female volunteers, aged 18 to 23, served as subjects. The experiments were carried out under three conditions: using regular Futon (Futon), the air mattress with pump and timer activated (Air+) and the same mattress without pump and timer activated (Air-). Room temperature and relative humidity were controlled at 22-23 degrees C and RH 50-60% respectively. Subjects' sleep was monitored by using EEG machine throughout the night, and subject's body temperature and bed climate were also continuously checked. Subjective estimation of bed and sleep were obtained before and after the recording sessions. Sleep onset latency and wake after sleep onset tended to be reduced in Air+ compared to Futon and Air-. The time and percentage of Stage 3 was increased significantly in the middle one third of the night in Air+. A significant difference was observed in bed climate of the waist area. Temperature tended to be higher in Futon than in Air+ and Air-, while relative and absolute humidity were significantly higher in Air+ and Air-. Significant difference between Air+ and Air- was observed only during one hour after sleep recordings started. Thermal sensation in the morning was cooler and comfort sensation tended to be better in Air+ and Air-. Subjective sleep estimation was somewhat good under all conditions. These results suggest that although these air mattresses do not affect sleep, we have to be cautious in using these mattresses as relative and absolute humidity were kept higher than with Futon. Further study on materials and construction of these air mattresses to decrease the humidity is needed.

Physiological and psychological effects of head heating during rest and work.

The purpose of this study is to obtain some knowledge related to physiological and psychological effects of head heating by radiation on the vital body under varying conditions of the environmental temperature and physical activity. The study was proceeded with experiments 1 and 2 using 8 women, all in good health as subjects. In experiment 1, firstly the subject at rest was kept at an ambient temperature of 28 degrees C for 40 minutes. The, moving to a room kept at an ambient temperature of 18 degrees C or 28 degrees C, the state at rest was maintained with head heating at 3 radiant intensities (without radiation, 35 degrees C and 40 degrees C of globe temperature) for a further 40 minutes. In experiment 2, continued from the same procedure of experiment 1, work on the ergometer was performed for 30 minutes with head heating at 2 radiant intensities (without radiation and 35 degrees C of globe temperature). Items measured were tympanic temperature, skin temperatures, heart rate, and subjective sensations. The principal results obtained are as follows: 1) Tympanic temperature, forehead skin temperature and score of thermal sensation of head increased, while the others decreased at 18 degrees C. 2) Decrease of scores was inhibited by head heating in finger skin temperature, toe skin temperature and heart rate. 3) When radiant intensities at 35 degrees C and 40 degrees C were compared, statistical significant difference was noted in forehead skin temperature only, the difference of radiant conditions gives a little effect to the vital body. 4) Effects brought by head heating during rest decreased owing to the addition of work.

[Childhood acute promyelocytic leukemia treated with all-trans retinoic acid].

We treated two children with acute promyelocytic leukemia (APL) in whom complete remission was successfully induced by oral administration of all-trans retinoic acid (ATRA). We followed these patients with conventional chemotherapy. The first patient has remained in continuous complete remission. However, the other patient relapsed during the maintenance therapy and died of progressive disease in spite of a second treatment with ATRA and chemotherapy. From a clinical point of view, the latter case had a hyperleukocytosis on admission. Also morphologically speaking, this patient had a different M3 variant than the first case. There are two major isoforms of PML/RAR alpha transcripts, so called short and long type transcripts, according to the breakpoints in the PML genes. In the first case the "long type' isoform was detected by reverse transcriptase polymerase chain reaction (RT/PCR) amplification. On the other hand the "short type' isoform was observed in the latter case. Also the second case became PCR positive at relapse, although the detectable isoform was negative during remission. The "short type' isoform may be related to the poor prognosis and RT/PCR analyses may be a powerful to detect early relapse.

Characterization of measles viruses isolated after measles vaccination.

Seven measles virus (MV) strains were isolated from children who developed clinical signs of fever and rash 3-9 days after measles vaccination. The nucleotide sequence of the H gene, the molecular size of the H protein, the haemadsorption activity on African green monkey red blood cells, and antigenicity as determined by virus neutralization revealed that one strain was of the vaccine type and the remaining six were the wild virus type. Isolation of the virus directly from patients suspected of a vaccine-induced side-reaction and subsequent characterization of such isolated virus may be useful in differentiation between vaccine-induced side-reactions and natural measles.

Topical tacalcitol (1,24-(R)-dihydroxyvitamin D3) induces a transient increase in thymidine incorporation and calmodulin content in pig epidermis following tape stripping in vivo.

Tape stripping induces transient increase in keratinocyte proliferation in vivo. The effects of tacalcitol (1,24-(R)-dihydroxyvitamin D3) ointment on the cell kinetics of pig epidermis after the tape stripping were investigated. The tacalcitol ointment (2 micrograms/g) was applied once to the back of pigs immediately after the tape stripping. The pig epidermal cell kinetics were analyzed at various times following the treatment. Tape stripping transiently increased thymidine incorporation of keratinocytes; the maximal effect was observed at 24 h. Tape stripping-induced increase in thymidine incorporation was markedly augmented by tacalcitol treatment. At 24 h following the tape stripping DNA-flow cytometry revealed an accelerated transition from G0/1 to S phase of cell cycle in tacalcitol treated epidermis. There was no significant difference, however, in mitotic counts and G2/M phase fractions between tape stripping-treated and tape stripping plus tacalcitol ointment-treated epidermis. We also measured calmodulin content of pig epidermis following the treatments. Although tape stripping slightly increased calmodulin content of pig epidermis, this was statistically not significant. Tape stripping plus tacalcitol ointment treatment resulted in a significant increase in calmodulin content at 24 h following the treatment. There was no significant difference in calmodulin content between tape stripping treated- and tape stripping plus tacalcitol-treated epidermis.

Effects of mass screening for neuroblastoma and the presumptive natural history of this tumor.

1) By March 1992, 166,959 infants were screened in Sapporo City, and 35 patients were found to have this tumor. Incidence of this tumor in our screening was one among 5,059 babies examined. 2) Eleven infants among the group with negative screening at 6 months developed neuroblastoma later. The patients developed tumors one to several years after mass screening, and 2/3 of these cases showed high urinary HVA compared to VMA. 3) We postulate that some of the adrenal steroids, possibly cortisol, may play a role for the malignant progression of this tumor.

Ultrastructural localization of superoxide dismutase in human skin.

The ultrastructural localization of copper and zinc superoxide dismutase (Cu,Zn-SOD) and manganese superoxide dismutase (Mn-SOD) was investigated in normal human skin by the post-embedding immunogold staining method. Epidermal keratinocytes were positive for both Cu,Zn-SOD and Mn-SOD. The Cu,Zn-SOD was predominantly located in the cytoplasm with a slight labelling on the mitochondria. The basal cells were remarkably positive for Cu,Zn-SOD and the upper spinous or granular cells faintly positive for this enzyme. In contrast to Cu,Zn-SOD labelling, Mn-SOD labelling was mostly restricted to the mitochondria. A similar labelling pattern was observed in dermal fibroblasts, endothelial cells, melanocytes, and in various epidermal appendageal cells (i.e. outer root sheath cells, sweat ducts and eccrine sweat gland cells). Thus the subcellular localization of Cu,Zn-SOD and Mn-SOD in the skin was similar among various cell types. Although no evidence for the peroxisomal localization of Cu,Zn-SOD was obtained in the present study, this could be due to a marked fragility of peroxisome that has not been identified in epidermal keratinocytes.

[Primary myelofibrosis in an infant--a case report and review of the literature].

Primary myelofibrosis (PMF) is regarded as a chronic myeloproliferative disorder. It is characterized by marrow fibrosis, leukoerythroblastosis, tear drop erythrocytes and extramedullary hematopoiesis. Most patients are in their late 50s when first diagnosed. Pediatric PMF is said to be quite rare. Here describe a female infant with PMF. The patient was born on Aug. 7, 1991. The pregnancy and delivery were uneventful. Hepatomegaly was noted soon after birth. Combined blood counts showed polycythemia and leukocytosis. It was thought to be extramedullary hematopoiesis due to intrauterine infection. She was followed up in another hospital, but since her condition was unchanged she was admitted to our hospital for further medical examinations at age 7 months. On the peripheral blood smear, there were tear drop erythrocytes, normoblasts and early myeloid elements. Repeated bone marrow aspirations were dry taps. This case presented the classical findings of fibrosis of the bone marrow on bone marrow biopsy. She is in good health without any therapy until now. A review of 7 cases of PMF, including our case, in Japanese children was made and discussed in comparison to adult cases.

Elevated serum manganese superoxide dismutase in acute leukemias.

We measured the serum levels of manganese-superoxide dismutase (Mn-SOD) in acute myeloid leukemia (AML) and acute lymphoid leukemia (ALL). Serum Mn-SOD level for normal subjects was 94.1 +/- 23.5 ng/ml (mean +/- S.D.), the levels for AML and ALL patients were 159.6 +/- 77.1 ng/ml and 154.4 +/- 77.0 ng/ml, respectively. The serum Mn-SOD levels were unrelated to individual intracellular Mn-SOD levels, but correlated well with serum lactate dehydrogenase values. Regression of the leukemia was accompanied by decrease in the serum level of Mn-SOD. Serum Mn-SOD may thus serve as a measure of the activity of the disease.

High levels of Mn-superoxide dismutase in serum of patients with neuroblastoma and in human neuroblastoma cell lines.

Levels of serum manganese superoxide dismutase (Mn-SOD) in normal children aged from 1 to 14 years and children with various hematological and malignant diseases were determined by enzyme-linked immunosorbent assay (ELISA). In the normal children, the serum Mn-SOD levels gradually increased in proportion to age. By 8 years of age, the Mn-SOD level was nearly at the adult level. The normal values of serum Mn-SOD (mean +/- SD) of children below 4 and above 8 years old were 48 +/- 10.2 ng/ml and 84 +/- 22.5 ng/ml, respectively. Assuming the upper limit of normal Mn-SOD level in serum to be the mean value +/- 2 SD of children at each age, high serum levels of Mn-SOD were found for 8 of 12 patients with neuroblastoma, three of four patients with Wilms tumor, and four of five patients with acute myeloid leukemia. The patients with neuroblastoma exhibited a transient increase in Mn-SOD following chemotherapy, but after 1 week the levels decreased markedly to the control levels. The changes in serum Mn-SOD levels in the patients with neuroblastoma correlated well with the levels of neuron-specific enolase. Mn-SOD was intensely stained in bone marrow cells of patients whose cancer cells had moved into the bone marrow. High levels of Mn-SOD were also found in cultured human neuroblastoma cells. These data indicate that Mn-SOD is expressed in neuroblastoma cells, may serve as one of the diagnostic and prognostic markers for the neuroblastoma, and may be useful to predict the effectiveness of chemotherapy for neuroblastoma and the recurrence of this disease.

Protective effects of TJ-960 herbal mixture on hippocampal neuron damage induced by cobalt focus in the cerebral cortex of rats.

In the cobalt focus experimental epilepsy model, severe hippocampal neuron damage occurs with marked EEG changes. The effects of TJ-960, a herbal medicine formulation, were studied on neuron damage in the CA1 area of rat hippocampus. Continuous oral administration of TJ-960 from one month prior to the cobalt application showed almost complete protection against hippocampal neuron damage induced by cobalt application to the cerebral cortex. TJ-960 also completely inhibited the EEG changes as well as the brain edema induced by cobalt application.

[Ultrasonic observation on distribution of subcutaneous fat in Japanese young adults with reference to sexual difference].

Utilizing an ultrasonic device (B-mode, 7.5MHz), the thickness of subcutaneous fat was measured at 54 different points in the bodies of 59 males (aged 20-25) and 66 females (aged 20-23). The results were as follows: 1. The females had significantly thicker subcutaneous fat than those of the males at every point measured. 2. The thick subcutaneous fats were observed at the abdomen, buttocks, and the anterior and medial parts of the thigh in the males, and at the breast, buttocks, abdomen, the posterior, medial, and anterior parts of the thigh, and the anterior, medial, and posterior parts of the upperarm in the females, respectively. 3. The subcutaneous fat of both sexes was more likely to be thicker in body parts closer to the trunk than the extremities. 4. The correlation coefficient between the percent body fat and the mean thickness of subcutaneous fat obtained from data was 0.91 for the males and 0.83 for the females.

[Clinical evaluation of severe toxicity in two patients with acute lymphoblastic leukemia receiving outpatient methotrexate therapy].

Methotrexate (MTX) is now widely used for the treatment of acute leukemia and non-Hodgkin lymphoma in the pediatric oncology field and is thought to be one of the key drugs for this treatment. A regimen utilizing high dose MTX (HD-MTX) with leucovorin rescue is being investigated as effective chemotherapy in the patients with these kinds of cancer. Relatively large amounts of MTX (225 mg/m2) are given to such outpatients by intravenous push as a course of maintenance therapy. It is said that those amounts will infuse safely. However, we experienced two serious cases-patients T.H. and M.Y.--which developed into severe side effects after this treatment. Both patients showed acute renal failure, severe myelosuppression, erosion around the oral and anal region, and continuous diarrhea. Judging from the serum concentration of MTX, patient T. H. was exposed to more than the maximum allowance serum MTX level for 9.6 days, patient M. Y. for 6.5 days. This suggests physicians must pay attention to the clinical symptoms even after treatment using MTX without HD-MTX.

Effects of cycloheximide on delayed neuronal death in rat hippocampus.

The effect of cycloheximide, a protein synthesis inhibitor, on hippocampal selective neuronal death was morphologically studied in rats subjected to 10 min forebrain ischemia using a 4-vessel occlusion model. Neuronal damage in the hippocampal CA1 subfield 72 h after ischemic insult was dramatically decreased by the lasting inhibition of protein synthesis through consecutive administration of cycloheximide. Cycloheximide, which was administered once within the first 24 h of recirculation, showed protective action on ischemic cell necrosis and its most potent effect was observed when injected at 12 h of post-ischemia. After 36 h of recirculation, however, treatment with cycloheximide could no longer prevent cell death. The possibility is considered that hippocampal delayed neuronal death following transient ischemia is caused by abnormal protein(s).
