Ursodeoxycholic acid oral tolerance test in patients with constitutional hyperbilirubinemias and effect of phenobarbital.

An oral tolerance test using a 25-mg dose of ursodeoxycholic acid was carried out in 46 patients with constitutional hyperbilirubinemias: 26 with Gilbert's syndrome, 9 with Dubin-Johnson syndrome, 7 with Rotor Syndrome, 3 with constitutional indocyanine green intolerance, and 1 with Crigler-Najjar syndrome type II; they were compared with 14 healthy control subjects. The integrated area under the curve of increments of serum UDCA above the base line between 0 and 2 h after oral administration was significantly increased in patients with Gilbert's syndrome and Dubin-Johnson syndrome, whereas it was normal in patients with Rotor syndrome and constitutional indocyanine green intolerance. As the area under the curve reflects the hepatic clearance of intestinally absorbed ursodeoxycholic acid and therefore gives a quantitative estimate of hepatic ursodeoxycholic acid clearance, the hepatic clearance of ursodeoxycholic acid seems to be impaired in patients with Gilbert's syndrome and Dubin-Johnson syndrome. Administration of phenobarbital, 100 mg/day for 2 wk, improved the ursodeoxycholic acid tolerance test in concordance with the reduction in serum bilirubin in 9 patients with Gilbert's syndrome studied. If the turnover of ursodeoxycholic acid is representative of that of other bile acids, these findings suggest that the hepatic transport of bile acid is impaired like other anions in patients with certain constitutional hyperbilirubinemias.

[Studies of solid ovarian tumors in Tokai District (author's transl)].

The prognosis of patients with ovarian carcinoma is believed as worst among malignant diseases in the field of gynecology. Multiple factors including clinical stage, histologic type and grade may play an important role in the prognosis of patients. Because of these factors and the few cases of ovarian carcinoma seen in any one center over a short period, the ability to make valid evaluation of these factors has been limited. We organized ovarian tumor studying group in Tokai district with eleven member hospital and analyzed 453 cases of solid ovarian tumor retrospectively. 1) Cumulative survival rate was calculated according to the clinical stages. Five year survival rate of stage 1 was about 80%, that of stage 2 was about 30% and that of stage 3 and stage 4 were around 10% respectively. 2) Cumulative survival rate was calculated according to histologic types. There was minor variation of survival by histologic types. When we compared with different histologic types by same clinical stages such as stage 1 and stage 3 there was no definite differences of survival rate except slightly poor prognosis in case of mucinous cystadnocarcinoma. 3) Cumulative survival rate was calculated according to histologic grading. Histologic grade affects the prognosis of patients with ovarian carcinoma.

ATP dependent reversible conformational change of Na+,K+-ATPase modified with N-(p-(2-benzimidazoly)phenyl)maleimide.

Na+,K+-ATPase [EC 3.6.1.3] from pig kidneys was treated with the fluorescent reagent N-(p-(2-benzimidazolyl)phenyl)maleimide (BIPM) in the presence of CKl. The resultant preparation showed 70% of the activity with only a small change in the apparent affinity for ligands of the enzyme. The addition of Na+ to the treated preparation induced a -2.1 +/- 0.1% change of the total fluorescence intensity observed in the absence of Na+. Further addition of both Mg2+ and ATP transiently increased the fluorescence to +0.5 +/- 0.1%. After the exhaustion of ATP, the fluorescence decreased to -3.1 +/- 0.1%. This cycle can be repeated by the readdition of ATP but not by ADP. Ouabain inhibits the fluorescence change. The ligands used reduced the fluorescence intensity as follows: Mg2+ + Na+ + ATP approximately K+, none, Mg2+ approximately ATP, Na+ + ATP, Na+ approximately Na+ + Mg2+, Na+ + Mg2+ + ADP approximately Na+ + ADP. The data indicate the presence of multiple conformational states of the enzyme.
