Measurement of blood flow in skin with an intact vascular pedicle.

An experimental model was devised to confirm directly the validity of total skin blood flow measurement, as postulated by Sejrsen. A section of skin in the groin of the rat was prepared, which contained an intact artery and vein. Both of these were direct branches of the femoral vessels. When 133Xe was injected intradermally in saline solution, it was demonstrated that a sham isolated vascular pedicle cleared the isotope, and that the clearance was biexpoential, suggesting two compartments. When the vascualr pedicle was ligated, thereby removing direct blood flow to the piece of skin, the 133Xe clearance curve was monexponential. It is concluded that the rapid portion of the biexponential clearance curve, as analyzed by Sejrsen, does represent the dermal vascular components of isotope clearance, and that measurement of cutaneous blood flow by this method is a valid and useful technique. Results using this technique to follow rejection in a new skin transplant model are presented.

Treatment of posttransplant lymphoceles: internal versus external drainage.

Six lymphoceles developed in 93 renal transplant recipients. Four were drained externally and two were drained internally. All patients with external drainage of lymphoceles experienced prolonged wound drainage with eventual contamination of the wound with pathogenic organisms. A technique of internal lymphocele drainage and its advantages was discussed.

Post-transplant function of cadaver kidneys preserved with plasma perfusate containing antidonor antibody.

Two cadaver kidneys were preserved by the perfusion method with AB cryoprecipitated plasma which contained antidonor lymphocytotoxic antibody. The kidneys were transplanted subsequently and no demonstrable immunologic damage to the kidneys was observed in the preservation and post-transplantation periods. In one patient renal function was immediate, and function has remained satisfactory in the 3 month follow-up period. The other patient had an initial period (2 weeks) of acute tubular necrosis, but renal function became satisfactory and the patient's condition has remained stable in the 3 month follow-up period. The significance of antidonor lymphocytotoxic antibody in the plasma perfusate on subsequent kidney function has been discussed.

Graft survival of perfused vs nonperfused cadaver kidneys.

No significant difference in the late survival rate of primary cadaver kidney transplants was demonstrated between perfused and nonperfused kidneys in the New England area. It is felt that the advantages of continuous perfusion to clinical renal transplantation continue to make it the preferred method of renal preservation.

Cyclosporine blood concentrations determined by specific versus nonspecific assay methods.

Cyclosporine blood concentrations were simultaneously determined by radioimmunoassay (RIA) and high-performance liquid chromatography (HPLC) at multiple points in time in two patients receiving cyclosporine for immunosuppression following liver transplantation. Radioimmunoassay levels always exceed those determined by HPLC; however, the divergence between the two methods increased as serum bilirubin concentration increased, with HPLC:RIA ratios generally less than 0.3 when serum bilirubin concentrations exceeded 10.0 mg/dL. These preliminary results suggest that retention of immunoactive cyclosporine metabolites due to imparied liver function may account for RIA-determined cyclosporine concentrations that greatly exceed those measured by HPLC.

Drinking water source and spontaneous abortion: A cross-sectional study in a rural Chinese population.

The authors examined the association between the risk of spontaneous abortion and the type of drinking water source in a rural Chinese population. Information about pregnancy outcomes and various exposures was collected by means of a detailed interview questionnaire. 2,876 pregnancies occurring in 1989-1993 among 2,201 nonsmoking and non-alcohol-drinking women were analyzed by logistic regression. Generalized estimating equations were used to adjust for correlations between multiple pregnancies in the same woman. The use of pond water as a drinking water source was associated with a higher risk of spontaneous abortion than the use of well or river water (odds ratio, 1.63; 95% confidence interval, 1.11 to 2.39), adjusting for potential confounders. More studies are needed to identify the agents responsible for the observed association. The results of this study emphasize the importance of monitoring potential drinking-water contamination in communities.
