Karydakis operation for sacrococcygeal pilonidal sinus disease: experience in a district general hospital.

Asymmetrical excision of sacrococcygeal pilonidal sinus has been shown to be associated with low recurrence rates. We report our experience with an asymmetric technique--the Karydakis operation. Of 28 patients who had the operation over a 4-year period, no recurrences were observed in 27 patients available for follow-up (median follow-up 3 years). Three patients had complications requiring surgical intervention. The operation is easy to teach and learn and is worth considering by both specialist and non-specialist surgeons.

Health behavior, perceptions, practices, and decision making: sub-national and socio-demographic differentials in Bangladesh, 1995.

The study examines health behavior, perceptions, practices, and decision making using data from the 1995 Health and Demographic Survey conducted by the Bangladesh Bureau of Statistics. Knowledge about causes of diarrhea and food contamination were higher than knowledge about causes of nightblindness and worm infestation but this knowledge is inadequate. The household's economic condition and the respondent's education were found to be positively associated with disease knowledge and food contamination; the same is also true for health behavior and practices. Sub-national variation in disease knowledge and food contamination exists but did not correspond always with the health behavior and practices. Decisions regarding treatment of disease were made by the husband and wife together in two-thirds of the cases, however, it was not uniform across socio-demographic and sub-national categories.

Cryptosporidiosis: a cause of diarrhea in Bangladesh.

Fecal samples from diarrheal patients and non-diarrheal controls were examined for Cryptosporidium oocysts in a year-long prospective study at a diarrhea treatment center in Dhaka, Bangladesh. Cryptosporidium oocysts were detected in 42 (3%) of 1,382 diarrheal patients but in none of 235 non-diarrheal controls. In 32 (76%) of 42 patients, no other enteropathogens were detected. Children less than 5 years of age were more commonly infected than older children (4.8% vs. 1.6%, P greater than 0.05) and adults (4.8% vs. 0.2%, P less than 0.01). A higher number of cases were detected during hot and humid months (April--July). Nineteen children less than 5 years of age (index cases) and their 71 family members were followed for 3 weeks after the release of the index cases from the hospital. Diarrhea continued for greater than 14 days (persistent diarrhea) in 8 (32%) index children. Cryptosporidium oocysts were detected in 1 (12.5%) of 8 family members who developed diarrhea during the follow-up period. Index cases excreted oocysts for 3-28 days (12.37 +/- 8.35 days). Almost all the patients recovered with oral rehydration. Cryptosporidium ssp. cause self-limited diarrhea episodes in children less than 5 years of age in Bangladesh, with a low frequency of intrafamilial transmission.

Infrequent detection of Yersinia enterocolitica in childhood diarrhea in Bangladesh.

In a search for Yersinia infection in Bangladesh, one isolate of Y. enterocolitica serotype 0:3 was obtained from stools of 1,450 children with fever and diarrhea and one of Y. enterocolitica serotype 0:8 was recovered from intestinal contents of 80 fatal diarrheal cases during postmortem examination. These results suggest that Yersinia infection is an infrequent cause of tropical diarrhea.

Prolonged depression of serum zinc concentrations in children following post-measles diarrhoea.

Serum zinc levels were measured in three groups of children aged 6 months to 6 years. In group I, there were 25 children with diarrhoea, who had a history of measles within the 2 weeks preceding the zinc level tests (post-measles diarrhoea); in group II there were 17 children who had diarrhoea only; and in group III there were 20 children who were apparently healthy and had neither diarrhoea nor measles (controls). Serum zinc concentrations of less than 5 mumol/l were observed in 5 out of 16 children in group I, but none of the children in group II had such low levels. The serum zinc level in group II rose significantly during the recovery stage compared to the acute stage (P less than 0.01). Children with post-measles diarrhoea showed a continued and prolonged depression of serum zinc levels.

Age-related changes in platelet microviscosity and aggregation in rats.

1. To study possible changes in platelet microviscosity in aged animals, 1,6-diphenyl-1,3,5-hexatriene (DPH) was used as a nonpolar probe embedded in thrice-washed platelets from young, adult and aged rats. With the known values of maximum limiting anisotropy and the structural parameter of DPH and by estimating the steady state of fluorescence anisotropy and the average fluorescence of lifetime, we applied the Perrin equation to calculate the microviscosity. 2. We measured platelet aggregation, platelet lipids and platelet polyunsaturated fatty acids to determine any causal relationship between these parameters. Platelet aggregation, the platelet molar ratio of cholesterol to phospholipid ([C]/[PL]) and platelet microviscosity increased with age (P < 0.05) and were correlated with one another (P < 0.05). 3. Age-dependent increases in the steady state of fluorescence anisotropy, order parameters and the short component of fluorescence lifetime of the fluorophore were expressed as functions of variables, such as microviscosity or the [C]/[PL] ratio. 4. Platelet concentrations of arachidonic and eicosapentaenoic acids increased with age, but were not associated with aggregation. Age-related changes in microviscosity and the [C]/[PL] ratio seemed to be determinants affecting biophysical properties of platelet aggregation.

The reliability and validity of the impact on lifestyle questionnaire in patients with acromegaly.

OBJECTIVES: Treatments for acromegaly, a growth hormone disorder, can be burdensome to patients, often requiring multiple self-administered injections daily. We developed the Impact on Lifestyle Questionnaire (ILQ) to measure the impact on patient's lifestyle imposed by the burden of injectable treatments for acromegaly. The primary objective of this study was to establish the reliability and validity of the ILQ. METHODS: The ILQ consists of the SF-12 and 30 additional questions. Thirty-four patients, from two sites, completed the ILQ and scales measuring related concepts. Fourteen patients also completed a retest survey 4 weeks later. Survey sample data were combined with ILQ data from another 56 patients with acromegaly for a factor analysis. Reliability was assessed with Cronbach's alpha and test-retest. Zero-order correlations were examined between ILQ subscales and symptoms, depression, SF-12 mental and physical components, a measure of self-care burden, appraisal of illness, and single-item measures of quality of life and satisfaction. RESULTS: The preconceived subscale structure was supported by factor analysis. These factors were internally consistent and stable over time. Good convergent validity was demonstrated between the Burden and Disruption scales with other measures of the burden of treatment. Patients indicated that they were generally compliant with therapy, and that treatment was not particularly burdensome or disruptive. Results based on the ILQ were consistent with other scales and qualitative responses. CONCLUSIONS: The ILQ has three subscales, Burden, Lifestyle Disruption, and Compliance, that are reliable and demonstrate preliminary evidence of construct validity.

Quality of analgesia when air versus saline is used for identification of the epidural space in the parturient.

BACKGROUND AND OBJECTIVES: Identification of the epidural space is often performed using the loss-of-resistance technique (LOR), commonly with air or saline. The effect of air or saline on the quality of labor epidural analgesia has not been adequately studied. METHODS: Women who requested labor epidural analgesia were randomly assigned to 1 of 2 groups depending on the syringe contents used for the LOR technique and injected into the epidural space. In the air group (n = 80) the anesthesiologist used 2 mL of air, and in the saline group (n = 80) the anesthesiologist used 2 mL of 0.9% saline. After LOR was obtained, a multiorifice epidural catheter was threaded 5 cm into the epidural space and 13 mL of bupivacaine 0.25% was administered in divided doses. The success of the epidural block was determined by asking the patient if she required additional medication 15 minutes later. The occurrence of paresthesias and intravascular or subarachnoid catheters was noted. RESULTS: In the air group, 36% of patients requested additional pain medication, and in the saline group 19% requested additional medication (P =.022). We were not able to find a statistically significant difference between groups in the incidence of paresthesias (42% air v 51% saline), intravascular catheters (5% air v 8% saline), or subarachnoid catheters (0 in both groups). CONCLUSIONS: Using 0.9% saline for the LOR technique is associated with better analgesia as compared with air for labor analgesia, and this advantage should be considered when selecting the syringe contents for the LOR technique.
