[Adjusted relative survival rate and causes of death after resection of gastric cancer].

To estimate how many of 1,302 patients with curative resection of gastric cancer died of recurrence or other diseases, the cumulative survival rate (act), relative survival rate (rl) and adjusted relative survival rate (adj) were analyzed. Adj was computed excluding gastric cancer death, because the death rate of this disease can not be ignored in the Japanese population. The death rate from gastric cancer can be estimated by [1.0-(adj)] and that from other causes by [(adj)-(act)]. It is more appropriate to calculate adj for the patients with mucosal or submucosal cancer, and rl can be substituted for adj for those with deeper invasion.

[A case of leiomyosarcoma of the rectum with widespread lymph node metastases].

A case of leiomyosarcoma of is reported. A 78-year-old man was admitted with a history of constipation and deformation of the stools. Barium contrast study and sigmoidoscopy showed a submucosal tumor of the rectum. On computed tomography and angiography, the tumor was suspected of being leiomyosarcoma. Abdominoperineal resection was performed. Microscopic examination revealed a leiomyosarcoma of the rectum and widespread lymph node metastases were found. Leiomyosarcoma of the rectum is an uncommon tumor. In our country, 89 cases have been reported. Six had lymph node metastases initially and another six subsequently. We have referred to the literature on leiomyosarcoma of the rectum.

Simultaneous early carcinomas of the ampulla of Vater and the stomach. Report of a case involving chronic inactive hepatitis.

Early carcinoma occurred simultaneously in both the stomach and the ampulla of Vater of a 69-year-old Japanese woman. A tentative diagnosis of early carcinoma of the stomach and acute hepatitis was made and needle biopsy of the liver revealed chronic inactive hepatitis. A carcinoma in the ampulla of Vater was detected by duodenoscopy. Cholangiography did not reveal the biliary tract. Pancreatoduodenectomy was performed and histological study of the resected specimen showed an early carcinoma restricted to the mucosa and an early carcinoma of the ampulla of Vater invading the sphincter of Oddi. There was no apparent metastasis or invasion of surrounding tissues.

The natural history of hepatocellular carcinoma. A study of 100 untreated cases.

The natural course of hepatocellular carcinoma (HCC) was evaluated in 100 untreated patients who were seen during recent 13 years. There were 80 males and 20 females. The macroscopic type of tumor was massive in 58, nodular in 35, and diffuse in 7 cases. Liver cirrhosis was associated with in 80% of patients. The mean survival time of overall patients was 4 months from the onset of initial symptoms to death and 2 months from admission to death. Occult and cirrhotic types of HCC showed a longer survival than frank and febrile types that had a mean survival time of all patients. The prognosis of icteric and metastatic types was poorest. Sex, tumor type, or associated cirrhosis did not substantially affect the natural course of HCC. The tumor stage and clinical stage were grossly correlated to the prognosis. In terms of symptoms, severe jaundice and ascites were the most dismal prognostic signs.

Structure of angiotensin II-binding site probed with diaminoalkanes.

The nature of the hormone-binding site of detergent-solubilized angiotensin II receptor from the bovine adrenal cortex has been characterized using diaminoalkanes. Their affinity for the hormone-binding site showed a dramatic alteration depending on their chain length; at the hexamethylene stage, a maximum and competitive inhibition was observed. These results indicate that the angiotensin II receptors have a definite hydrophobic binding groove limbed with diagonally located two negative charges, and those hydrophobic and electrostatic forces may play an important role in the proper register between the receptor and hormone.

Necessity of polyamines for maximum in vivo synthesis of beta beta' subunits of RNA polymerase.

The possibility that polyamines can stimulate the in vivo synthesis of beta beta' subunits of RNA polymerase has been examined through the use of a polyamine-requiring mutant of Escherichia coli. Results from autoradiographic estimation and activity measurement of RNA polymerase in cell extracts prepared from polyamine starved and unstarved bacteria showed that polyamines stimulate the synthesis of beta beta' subunits of RNA polymerase 2- to 3.6-fold.

Quantitative determination of immunoglobulins in the cerebrospinal fluid based on their light chain properties using a solid-phase enzyme immunoassay.

A method for quantitative determination of immunoglobulins in the cerebrospinal fluid (CSF) on the basis of their light chain properties is described. The method is a solid-phase enzyme immunoassay, based upon competitive binding of immunoglobulins in unknown and labeled immunoglobulin for antibody adsorbed to the wells of a microplate. The values for immunoglobulins kappa (Ig-k), lambda (Ig-l) and k/l ratio obtained from 38 subjects without neurological disorders were 8.6 +/- 4.0 micrograms/ml, 7.0 +/- 2.6 micrograms/ml and 1.21 +/- 0.21, respectively (mean +/- SD). Our method is considered to be less influenced by the molecular size and weight of immunoglobulins than radial immunodiffusion, and thus may provide more precise values of Ig-k and Ig-l.

Fluorescence determination of 5-fluorouracil and 1-(tetrahydro-2-furanyl)-5-fluorouracil in blood serum by high-performance liquid chromatography.

Fluorescence derivatization of 5-fluorouracil (5-FU) and 1-(tetrahydro-2-furanyl)-5-fluorouracil (ftorafur, FT) with 4-bromomethyl-7-methoxycoumarin using 18-crown-6 as a catalyst is studied with aim of developing a sensitive and selective liquid chromatographic method. 5-FU and FT form virtually substituted derivatives which possess maxima in their fluorescence emission spectra near 400 nm. These derivatives are separated by thin-layer chromatography and high-performance liquid chromatography to confirm the completion of reaction. For the determination of 5-FU and FT in serum, the reversed-phase high-performance liquid chromatographic separation of the derivatives is studied with a C13 column. This chromatography is of importance for the accurate determination of 5-FU and FT, which are, respectively, an important antitumour agent for the treatment of solid tumours in clinical medicine and a masked form of 5-FU generated in vivo.

Preferential stimulation of the in vivo synthesis of a protein by polyamines in Escherichia coli: purification and properties of the specific protein.

The possibility that polyamines can stimulate the synthesis of special kinds of proteins has been examined by using a polyamine-requiring mutant of Escherichia coli. It was found that the synthesis of some proteins, particularly one with a molecular weight (Mr) of 62K, was significantly stimulated following polyamine supplementation of polyamine-starved cells. The preferential stimulation of the synthesis of this polyamine-induced protein of Mr 62K (PI protein) was followed by the stimulation of overall protein synthesis by polyamines. PI protein was purified to homogeneity and some of its properties were examined. From studies on the effect of PI protein on MS2 RNA directed protein synthesis, it was shown that this protein stimulated the synthesis of RNA replicase by 2.2-fold in the presence of 1 mM spermidine.

An improved method for determination of active and total renin concentration in human plasma using an excess of sheep substrate.

A method for measurement of renin concentration (PRC) in human plasma has been developed and validated experimentally and theoretically. Like most of the previous methods, the present method is based on a radioimmunoassay of angiotensin I generated during incubation of plasma with an excess of renin substrate. In the present study we used, as the substrate, sheep angiotensinogen partially purified from anephric sheep plasma by ammonium sulfate fractionation and pepstatin-aminohexyl-agarose chromatography. The advantage of using sheep substrate is that it has an exceptionally high affinity for human renin. The partially purified substrate, which contained no detectable renin activity, improved the sensitivity, allowing quantification of renin in low-renin plasma. Possible interfering influences of plasma proteins on the radioimmunoassay were eliminated by the introduction of a simple deproteinization step. For determination of total renin concentration (TRC), inactive renin was activated by exposing plasma to low pH or trypsin. Normal values of PRC and TRC after careful selection of assay conditions were 2.9 +/- 0.4 and 33.9 +/- 6.1 ng angiotensin I X ml-1 X h-1, respectively. As an illustrative example of usefulness of the assay, PRC determination of a patient with a ectopic renin-secreting tumor is presented.

In monkey COS cells only the TATA box and the cap site region are required for faithful and efficient initiation of the fibroin gene transcription.

The DNA sequences necessary for faithful and efficient initiation of transcription of Bombyx mori fibroin gene have been studied in vivo using monkey COS cells and a SV40 origin vector. Transcriptional analysis of 5' deletion genes and of exact substitution genes including a series of single base substitution mutants indicates that the TATA box and the cap region are required for faithful and efficient initiation of the fibroin gene transcription in vivo, whereas far upstream sequences and the CAAT-like sequences are not required. In addition, single base substitution genes of the cap site region have clearly shown that there exists a strict sequence requirement of the cap region of the fibroin gene. These results define the basic promoter of the fibroin gene to the region spanning the TATA box and the cap site region.

Immunoprophylaxis of hepatitis A of the children by immune serum globulin.

An outbreak of hepatitis A occurred in Kyoto. Immune serum globulin was administered prophylactically to children who had not yet developed the disease. However 52 children in succession contracted hepatitis A. Late injections of immune serum globulin had no effect on the clinical course. Middle injections suppressed only jaundice. Early or preexposure injection suppressed the disease remarkably. All who had overt hepatitis A seroconverted to positive antibody to hepatitis A virus. In inapparent infection, those who had slight elevation of transaminase developed passive active immunization irrespective of pre or postexposure administration of immune serum globulin. Three children who might block hepatitis A virus completely had no change of transaminase and failed to establish passive active immunization.

Transport of acetylcholine in a membrane. Laminate model of the neuromuscular junction.

A laminate model of the cleft-plus-postsynaptic membrane structure of the neuromuscular junction was studied. In order to prepare a model of the postsynaptic membrane, the properties of acetylcholine (Ach) receptor-rich vesicles purified from Torpedo fish were measured. Immobilization of vesicles was demonstrated by various methods, in particular, by investigating collagen and carrageenan matrices as models of the fluid-filled fibrous matrix of the cleft. It was found that a laminated system employing a liquid membrane-containing vesicle suspension, together with a swollen collagen membrane, is an appropriate model for examining important transport/reception aspects of the cleft-plus-postsynaptic membrane structure. Combined transport with immobilization of Ach in the liquid membrane system was elucidated and effective diffusivities in the vesicle suspension layer were calculated. Effective diffusivities of the composite system simulating the cleft and the postsynaptic membrane were evaluated as well. These data illustrate the importance of penetrant immobilization in retarding the diffusion process during neurotransmission.

The negative effect of splenectomy on the prognosis of gastric cancer.

During the 11 year period from 1970 through 1980, 103 patients underwent total gastrectomy for gastric cancer. Splenectomy was performed in 53 of the patients because of perioperative accidental injury or an operative finding of tumor metastasis to the lymph nodes or direct invasion to the splenic hilus. Retrospective comparisons were carried out among the 53 splenectomized and 50 nonsplenectomized patients in terms of the long-term survival rate and early and late complications. The two groups of patients were quite similar with regard to rates of curative resection and stages of disease. The 5 year cumulative survival rates overall and in the splenectomized and nonsplenectomized groups were 23.1, 21.8 and 23.8 percent, respectively. Splenectomy did not affect the prognosis of gastric cancer. In addition, there were no differences between the two groups in the incidences of early surgical complications and infectious disease of late onset. Our results suggest that, in the treatment of gastric cancer, splenectomy may not be avoided, either when the spleen is injured perioperatively or when cancer involvement is suspected in the splenic hilus.

The use of xanthine oxidase for a specific and sensitive fluorimetric determination of 6-mercaptopurine in serum.

A specific and highly sensitive fluorimetric method was developed for the determination of 6-mercaptopurine (6MP) in serum. The method is based on the enzymatic oxidation of 6MP with xanthine oxidase to the oxypurine, followed by oxidation with acidic chromate to the corresponding 6-sulfonate. The fluorescent product has excitation and emission maxima at 330 and 400 nm, respectively. The limit of sensitivity was approximately 22 pg/ml for 6MP in water. The sensitivity limit for 6MP in serum containing azathioprine was approximately 2.2 ng/ml. The rate constants for conversion of 6MP into the final product (6-thiouric acid) and the apparent Michaelis constant were also determined by a nonlinear regression analysis based on the integrated Michaelis-Menten equation using the ultraviolet absorbance data and the simplified complementary tristimulus colorimetry.

Quantitation of IgG, IgA and IgM in the cerebrospinal fluid by a solid-phase enzyme-immunoassay. Establishment of normal control values.

A sensitive, specific and simple solid-phase enzyme-immunoassay for quantitative determination of IgG, IgA and IgM in the cerebrospinal fluid (CSF) has been developed. Using this technique, we have established the control values for CSF immunoglobulins from 30 neurologically normal subjects. The value of IgG determined by our method was closely correlated with that found by single radial immunodiffusion although the latter was found 1.5 times higher. The values of IgG and IgA showed significant positive correlations with CSF total protein values, while the value of IgM did not. This finding suggested that the concentration of CSF IgM might be less affected by transudation from the serum than those of IgG and IgA in neurologically normal subjects. Our method needs no complicated technique nor special apparatus other than microplate photometer, and offers more facility and convenience for routine laboratory use.
