Removal of mesenteric gastrinoma: a case report.

A 37-year-old man with a primary mesenteric gastrinoma is reported. A parathyroid adenoma had been removed 13 years ago. Six years earlier the patient underwent an emergency distal gastrectomy because of massive bleeding from a duodenal ulcer. Two months later stomal ulcers developed associated with hypergastrinemia. No gastrinoma was detected and total gastrectomy was performed. The fasting plasma level of gastrin was stable in the range from 580 to 920 pg/ml for the following 5 years. However, the level was found to abruptly increase to 4125 pg/ml. The level increased progressively to 11383 pg/ml within 1 year. A gastrinoma was identified in the jejunal mesenterium, and it was completely removed. After surgery the plasma level of gastrin decreased below the limit of the assay, and the paradoxical response to secretin was no longer observed.

Changes in serum levels of pancreatic isoamylase, lipase, trypsin, and elastase 1 after endoscopic retrograde pancreatography.

To compare the behavior of pancreatic enzymes in the circulation, serum levels of pancreatic isoamylase activity, lipase activity, immunoreactive trypsin (IRT), and immunoreactive elastase 1 (IRE) were measured in the same serum samples taken serially from 29 subjects undergoing endoscopic retrograde pancreatography (ERP). A striking correlation between the maximal increments of serum pancreatic enzyme levels after ERP and the degrees of opacification of pancreatic duct system was observed, except in 6 subjects with chronic pancreatitis. In 13 out of 19 subjects whose main pancreatic duct (MPD) and branches were opacified, serum pancreatic enzyme levels reached a peak within 2 hours after ERP and decreased thereafter. The mean maximal rise of serum levels of lipase, IRT, pancreatic isoamylase, and IRE in the 13 subjects was 32, 21, 10, and 4 times the basal value, respectively. A delay of peaking in serum levels of pancreatic isoamylase and IRE as compared with those of lipase and IRT was observed in the 13 subjects. The mean disappearance half-time of serum lipase, IRT, pancreatic isoamylase, and IRE in the 13 subjects was 2.8, 4.6, 8.0 and 16.0 hours, respectively.

Marked prolongation in disappearance half-time of plasma cholecystokinin-octapeptide in patients with hepatic cirrhosis.

For exploration on the elimination of cholecystokinin from the circulation, the disappearance half-time of cholecystokinin-octapeptide was estimated with cholecystokinin specific radioimmunoassay in normal subjects and patients with chronic renal failure and with hepatic cirrhosis. With a brief infusion of 30.4 ng/kg of cholecystokinin-octapeptide for 2 min, plasma cholecystokinin level rose from 16.1 +/- 3.6 pg/ml (mean +/- SE) to 216.5 +/- 6.1 pg/ml at 3 min after starting infusion, and decreased rapidly in a single exponential fashion for 10 min in hepatic cirrhosis. The disappearance half-time of cholecystokinin-octapeptide in patients with hepatic cirrhosis was 2.45 +/- 0.07 min, and it was significantly longer than that in normal subjects (1.30 +/- 0.07) or patients with chronic renal failure (1.70 +/- 0.11). These findings suggest that the liver plays a major role in cholecystokinin-octapeptide elimination in humans.

Release of vasoactive intestinal peptide by intraduodenal infusion of HCl or fat and intramuscular injection of neostigmine in man.

A highly sensitive radioimmunoassay system for plasma vasoactive intestinal peptide (VIP) was developed to examine the effect of intraduodenal infusion of HCl or fat on the plasma VIP levels in healthy subjects, and the effect of intramuscular injection of neostigmine in patients with irritable bowel syndrome (IBS). Intraduodenal infusion of 100 ml of 0.1 N HCl or 10 g fat caused a significant rise in plasma VIP level. Neostigmine (12.5 micrograms/kg) produced a significant rise in plasma VIP level, and the plasma VIP response to neostigmine was significantly greater in the IBS group than in the normal group. These results suggest that VIP might play a role in the pathophysiology of IBS.

Molecular cloning of the human gastrin gene.

A genomic clone that contains the human gastrin gene was isolated from a human gene library. Restriction endonuclease mapping and DNA sequencing analysis revealed that this gene is about 0.7 kb long, and has an intron. The intron is located at a position that separates the coding region into the peptide region essential for biological activities of gastrin and the non-essential, N-terminal peptide region.

Forskolin-induced cyclic AMP production and gastric acid secretion in dispersed rabbit parietal cells: novel evidence for a major role of cyclic AMP in acid release.

Forskolin, a unique diterpine which is a direct activator of cyclic AMP-generating systems, stimulated both cyclic AMP and acid production in dispersed rabbit parietal cells. This agent was also capable of augmenting the action of histamine on both cyclic AMP and acid production at a low concentration. These findings provided novel evidence for a major role of cyclic AMP in gastric acid secretion.

Toxic effects of sodium selenite on pregnant mice and modification of the effects by vitamin E or reduced glutathione.

Toxic effects of sodium selenite (SS) on dams and/or conceptuses and modification of the effects by vitamin E (VE) or reduced glutathione (GSH) were investigated in pregnant mice during late gestation. Dose-dependent fetocidal effects and fetal growth retardation were observed in mothers injected SC with SS on day 12 but not on day 16 of gestation. On day 12, abortion occurred only at a dose level of 58.8 mumol/kg SS, whereas on day 16 it occurred at two dose levels: 27.2 and 40.0 mumol/kg SS. Pregnant mice were treated with or without VE (50 mg/kg, SC, daily from day 7 to day 11) and injected with a single dose of SS (46.2 mumol/kg, SC) on day 12. VE prevented abortion and maternal death, but none of the other effects such as fetal loss, fetal growth retardation, and decrease in body weight gain of the dams. Pretreatment with GSH (0, 2, or 5 mM/kg, SC) 20 min before SS injection on day 12 exacerbated maternal death, abortion, and decrease in body weight gain of the dams. No effect were observed in the number of live offspring, and in body weight of offspring.

Molecular cloning of human gastrin precursor cDNA.

We cloned cDNA of gastrin mRNA from human gastric antrum. First we obtained a porcine gastrin precursor cDNA clone using a synthetic oligodeoxyribonucleotide, d(A-A-A-G-T-C-C-A-T-C-C-A-T-C-C-A-T) as a hybridization probe. Then, using this porcine clone as a hybridization probe, human gastrin precursor cDNA clones were obtained. Sequence analysis revealed 4, 303, and 98 nucleotides, respectively, in the 5' untranslated region, in the amino acid coding region, and in the 3' untranslated region. The deduced precursor molecule codes for big and small gastrin, surrounded by pairs of basic amino acids. When the sequences of porcine and human gastrin precursor are compared, a high degree of homology in the active peptide region and lower homology in other regions are observed.

Lack of dominant lethal effects in male CD-1 mice after short-term and long-term exposures to vinyl chloride monomer.

Dominant lethal studies were conducted with vinyl chloride monomer (VCM) in male CD-1 mice, using short-term (10 000 ppm, 4 h/day, for 5 days) and long-term (5000 ppm, 4 h/day, 5 day/week, for 10 weeks) exposures. There was no evidence of dominant lethal effects due to VCM in either case. Semen examination of male mice after long-term exposure failed to reveal any alteration in sperm motility and shape. Sporadic appearance of giant cells was observed in the testes of males exposed to VCM, but was not considered to affect spermatogenesis.

Effect of acid perfusion of the terminal ileum on plasma immunoreactive secretin and pancreatic secretion in the rat.

The distribution of immunoreactive secretin in acid extracts of the alimentary tract of the rat was determined. It was found to be contained in higher concentrations in the duodenum and in the terminal ileum than in other parts of the intestine. To elucidate the possible role of secretin contained in the terminal ileum, this part of the intestine was perfused with hydrochloric acid, a potent secretin secretagogue, and changes in plasma immunoreactive secretin levels and in pancreatic secretion were investigated in anaesthetized rats. Plasma immunoreactive secretin was increased from 65 +/- 5 pg/ml to 108 +/- 13 pg/ml during a 10 minutes perfusion with hydrochloric acid. This was significantly higher than that of saline perfusion but significantly lower than that of duodenal perfusion with hydrochloric acid. Acid perfusion of the terminal ileum also caused a three-fold increase in pancreatic flow rate and a twofold increase in bicarbonate concentration. These results suggest that the terminal ileum, through release of secretin, might play a role in regulating the pancreatic secretion of water and bicarbonate in response to changes in intraluminal fluids in the distal intestine in the rat.

Plasma cholecystokinin responses after ingestion of liquid meal and intraduodenal infusion of fat, amino acids, or hydrochloric acid in man: analysis with region specific radioimmunoassay.

A highly sensitive and precise radioimmunoassay system for plasma cholecystokinin (CCK) was developed with the anti-CCK-8 specific antiserum which raised against N-terminal amino acids residue of sulfated CCK-8 and reacted with CCK-8, CCK-33, and CCK-39 but not with gastrin and its related peptides. Mean concentration of the fasting plasma CCK determined with this method using CCK-8 as standard was 12.9 +/- 5.9 pg/ml in normal subjects (n = 26), and in patients with hepatic cirrhosis it was significantly higher (36.7 +/- 16.9 pg/ml, n = 9, p less than 0.01) than in normal subjects. In six young healthy volunteers, intraduodenal infusion of fat caused a significant increase (p less than 0.05) of plasma CCK from a basal level of 8.0 pg/ml to a peak of 43.0 +/- 12.0 pg/ml at 20 min after starting of infusion. In the same subjects, a significant increase of plasma CCK was also observed by amino acids infusion, but no elevation of plasma CCK level was found during intraduodenal acidification.

Effect of intraduodenal load of endogenous acid on secretin release in patients with peptic ulcer.

In order to explore secretory mechanisms in peptic ulcer, the plasma secretin response to an intraduodenal load of gastric acid stimulated with tetragastrin was studied in 10 patients with duodenal ulcer, nine with gastric ulcer, and five young healthy volunteers. After the injection of tetragastrin plasma secretin level was significantly increased in all subjects. The integrated incremental secretin output significantly correlated with the incremental acid output in the duodenal ulcer group as well as the gastric ulcer group. THere was no significant difference in the integrated incremental secretin output among the three groups. However, the integrated incremental secretin output per unit amount of gastric acid loaded in the duodenum was significantly lower in the duodenal ulcer group than in the other two groups. These results suggest that in patients with duodenal ulcer the secretin release in response to an intraduodenal load of endogenous acid is impaired.
