The haemodynamic effects of subcutaneous sumatriptan, a 5HT1-receptor agonist.

AIMS: Previous in vivo studies with sumatriptan, a 5HT1-receptor agonist, have demonstrated vasopressor responses in the pulmonary and systemic arterial circulation. Pulmonary artery wedge pressure (PAWP) also increases after sumatriptan injection, raising the possibility of an additional venoconstrictive action or a negative inotropic effect. The mechanism for the rise in PAWP was investigated in the study. METHODS: Ten patients undergoing diagnostic coronary arteriography underwent haemodynamic monitoring. RESULTS: There was a significant rise (P < 0.05) in systemic and pulmonary arterial pressure and total systemic and pulmonary vascular resistance. There was a similar rise (P < 0.05) in PAWP and left ventricular end diastolic pressure (LVEDP). There was no change in cardiac output nor in peak rate of left ventricular pressure rise (dP/dt). CONCLUSIONS: The sumatriptan induced rise in PAWP and LVEDP appears consequent upon increased afterload although a negative inotropic effect cannot be excluded.

Selective screening versus presumptive treatment criteria for identification of women with chlamydial infection in public clinics: New Jersey.

OBJECTIVE: Our goals were to determine the prevalence of chlamydial infection, to identify criteria for selective screening, and to compare the sensitivity of selective screening to presumptive treatment criteria in different clinical settings. STUDY DESIGN: A total of 5128 women enrolled in a cross-sectional study in public clinics in New Jersey. Univariate and multivariate methods of statistical analysis were used. RESULTS: The prevalence of chlamydia varied across type of public clinic and ranged from 8% to 15%. Selective screening criteria were developed for women attending each type of public clinic by use of risk factors significant in the multivariate analyses. A combination of young age and attending an urban clinic was highly predictive of chlamydia infection and identified a minimum of 85% of infected women in all public clinic settings. The use of presumptive treatment criteria identified 78% of infected women in sexually transmitted disease clinics but only 4% to 9% of infected women in other clinical settings. CONCLUSIONS: A chlamydia program that includes presumptive treatment of women attending sexually transmitted disease clinics and selective screening of women in other clinical settings where women are more likely to asymptomatic is a clinically appropriate and economically feasible approach to directing treatment of women with chlamydial infection.

Immunosuppression with chlorambucil, versus placebo, for scleroderma. Results of a three-year, parallel, randomized, double-blind study.

Placebo and chlorambucil (0.05-0.1 mg/kg/day) were compared as treatments for scleroderma in a 3-year, randomized, double-blind, parallel study of 65 patients. Slopes of change over time, calculated for each organ system of each patient, failed to show significant differences between treatment groups. An immunosuppressive effect in the chlorambucil-treated group was shown by decreases in the white blood cell counts (P = 0.02), platelet counts (P = 0.04), lymphocyte counts (P = 0.001), IgA/IgM concentrations (P less than or equal to 0.05), and wheal size on mumps skin tests (P = 0.02). Three years of immunosuppressive treatment with chlorambucil did not benefit this scleroderma population.

Carcinoma of the bladder in patients less than 40 years old.

The records were reviewed of 22 patients with bladder carcinoma before they were 40 years old to determine whether age alone influenced the course of the disease. There were 19 men and 3 women, reflecting a male predominance of 6 to 1. Of the 9 patients with superficial stage A/O disease 8 are alive 6 months to 28 years after diagnosis, the remaining patient being lost to followup. The other 13 patients with tumors invading muscle are dead, 11 as a direct result of the disease. No evidence was found to substantiate earlier reports that bladder carcinoma in young patients has a more benign behavior.
