Ketamine-diazepam anaesthesia for abdominal surgery. A review of 1000 cases with continuous "Micro-Mini" drip administration technique.

During a twenty-one month period, over 1000 patients who underwent various types of intra-abdominal operations were anaesthetized with a combination of diazepam and ketamine using a continuous "Micro-Mini" drip administration technique. The results obtained from 1000 cases were eminently satisfactory. This paper also reviews all the previous literature on ketamine anaesthesia related to abdominal surgery. All the undesirable adverse side effects associated with ketamine anaesthesia in adult patients have been almost completely eliminated using this technique. Overall, the excellent results clearly indicate that ketamine has a definite place in anaesthesia for intra-abdominal operations.

A simple method for the isolation of actin from myxomycete plasmodia.

A new, simple method for the isolation of actin from myxomycete plasmodia has been developed. Plasmodium myosin B was incubated at 55 degrees C for 15 min in the presence of ATP or was treated with 90% acetone. By this treatment myosin was denatured completely. Actin was then extracted with a dilute ATP and cysteine solution from the heat- or acetone-treated myosin B. The method is simple and almost pure actin was obtained in high yield. The purified G-actin polymerized to F-actin on addition of 0.1 M KCl or 2 mM MgCl2. The viscosity of the purified F-actin was 8-10 dl/g. The F-actin activated muscle myosin ATPase, and actomyosin synthesized from the F-actin and muscle myosin showed superprecipitation on addition of ATP.

Diazepam-ketamine anaesthesia for open heart surgery a "micro-mini" drip administration technique.

Two hundred open-heart cases were anaesthetized with a diazepam-ketamine combination. The results were excellent. A "Micro-Mini" drip technique insured low, even, but adequate dose levels of ketamine and less drug was used. Induction and maintenance are simple and smooth. Effects on the cardiovascular system and respiratory system are minimal. The margin of safety is wide and 100% oxygen can be used whenever needed.

Beta-actinin-like protein from plasmodium.

A beta-actinin-like protein was isolated from plasmodia of the slime mold. The chain weight was the same as that of actin (43,000), but the amino acid composition was significantly different. The action of plasmodium beta-actinin on actin was the same as that of beta-actinin from rabbit skeletal muscle: inhibition of the recombination of F-actin fragments; formation of Mg polymer; inhibition of interfilamental interaction of F-actin and retardation of depolymerization of F-actin. The only difference observed was its sensitivity to trypsin: plasmodium actinin was less quickly digested by trypsin than rabbit beta-actinin.

Variability of the diagnosis of stroke by clinical judgement and by a scoring method.

Existing criteria for the diagnosis of acute cerebrovascular disease (stroke) have not been satisfactory in epidemiological studies. Variability of the diagnosis of stroke, which had not been studied before, was investigated in a WHO collaborative study. Intra-observer and inter-observer variation of the diagnosis of stroke was studied by means of 45 case reports drawn at random from among those included in the study. Diagnosis of stroke and of the type of stroke was made by clinical judgement and by a scoring method. The clinical diagnosis of stroke was more consistent and more comparable than the diagnosis of the type of stroke. Inter-observer agreement in clinical diagnosis was improved by using the score method.

Experience from a multicentre stroke register: a preliminary report.

In collaboration with 15 centres in 10 countries of Africa, Asia, and Europe, WHO started a pilot study of a community-based stroke register, with standardized methods. Preliminary data were obtained on 6395 new cases of stroke in defined study communities, from May 1971 to September 1974. Information on incidence rates, clinical profiles, diagnosis, management, and course and prognosis for these patients is given.

Conformational changes induced in plasmodium actin polymer by Ca2+ in the presence of muscle native tropomysin.

The flexible polymer of plasmodium actin (Mg-polymer) combines with muscle native tropomyosin up to a weight ratio of actin: tropomyosin:troponin = 6:1:1. Mg-polymer which combined with muscle native tropomyosin undergoes a conformational change when the Ca2+ concentration of the solution is decreased by adding 0.5 mM ethylene glycol bis(beta-aminoethylether)-N,N'-tetraacetic acid (EGTA): the viscosity of the solution increases by 50% and its ATPase activity decreases to about 20% of the original value, which suggests that the structure of Mg-polymer becomes more rigid. This change is reversible with respect to the Ca2+ concentration in the solution. The threshold concentration of Ca2+ for the conformational change is about 10(-6) M. The possible role of this transformation in vivo is discussed.

Diazepam-ketamine anaesthesia for open heart surgery "micro-mini" drip administration technique.

Two hundred open heart cases anaesthetized with a combination of diazepam-ketamine using "Micro-Mini" drip administration technique were presented. The results were eminently desirable, and in the opinion of the authors are a notable improvement over other methods for all types of cardiovascular surgery including most advanced heart diseases. The advantages of this anaesthetic method for cardiovascular surgery are as follows: 1. Effects on cardiovascular system are minimal.--2. Respiratory depression is negligible.--3. There is no increase in salivation or muscle tone.--4. Induction and maintenance of anaesthesia are simple and smooth.--5. Anaesthesia can be maintained, using a high concentration of oxygen alone, by a slight increase in rate of ketamine administration whenever necessary.--6. Post-anaesthetic psychotomimetic effects are negligible.--7. Nearly 100% of patients have excellent amnesia.--8. It has a wide margin of safety.--9. This technique has proved highly acceptable to patients and surgeons. We feel ketamine should be used in small dosages continuously administered with either "Micro-Mini" drip infusion or infusion pump. Ketamine given in this fashion should be regarded as an analgesic.

Inducation of antibody against actin from myxomycete plasmodium and its properties.

Plasmodium actin was highly purified by gel filtration of crude G-actin on Sephadex G-100 followed by ultracentrifugation after polymerization in the presence of 1 M urea and 1 mM ATP. Purified actin showed a single band in the sodium dodecyl sulfate gel electrophoretic pattern. Antibody against this purified actin was induced in rabbits. The antibody obtained was immunologically monospecific for plasmodium actin, judging from the following results. (1) The addition of the antibody to a plasmodium F-action solution increased the turbidity of the mixed solution, showing the formation of the antibody-action complex. (2) In immunodiffusion and immunoelectrophoresis, the antibody formed single preciptin lines with the purified actin preparation and with the crude actin extract from the acetone-dried powder of plasmodium. (3) The antibody inhibited polymerization of plasmodium G-actin. (4) Plasmodium F-actin filaments were decorated with antibody in electron micrographs. The antibody reacted not only with plasmodium F- and G-actin, but also reacted with sea urchin egg actin, but it did not react with actin from rabbit striated muscle.

The polymerization of actin: its role in the generation of the acrosomal process of certain echinoderm sperm.

When Asterias or Thyone sperm come in contact with egg jelly, a long process which in Thyone measures up to 90 microm in length is formed from the acrosomal region. This process can be generated in less than 30 s. Within this process is a bundle of microfilaments. Water extracts prepared from acetone powders of Asterias sperm contain a protein which binds rabbit skeletal muscle myosin forming a complex whose viscosity is reduced by ATP. Within this extract is a protein with the same molecular weight as muscle actin. It can be purified either by collecting the pellet produced after the addition of Mg(++) or by reextracting an acetone powder of actomyosin prepared by the addition of highly purified muscle myosin to the extract. The sperm actin can be polymerized and by electron microscopy the polymer is indistinguishable from muscle F-actin. The sperm actin was shown to be localized in the microfilaments in the acrosomal processes by: (a) heavy meromyosin binding in situ, (b) sodium dodecyl sulfate (SDS) gel electrophoresis of the isolated acrosomal processes and a comparison to gels of flagella which contain no band corresponding to the molecular weight of actin, and (c) SDS gel electrophoresis of the extract from isolated acrosomal caps. Since the precursor for the microfilaments in the unreacted sperm appears amorphous, we suspected that the force for the generation of the acrosomal process is brought about by the polymerization of the sperm actin. This supposition was confirmed, for when unreacted sperm were lysed with the detergent Triton X-100 and the state of the actin in the sperm extract was analyzed by centrifugation, we determined that at least 80% of the actin in the unreacted sperm was in the monomeric state.
