[Determination of plasma catecholamine as a quantitative index for the evaluation of sympathetic nervous system function in man during methacholine test (author's transl)].

For many years blood pressure has been only an index among a number of physiological conditions presenting sympathetic nervous systems activities in methacholine test due largely to the lack of specific and practical method for the determination of catecholamine. We found that methacholine induced a significant increase in plasma catecholamines with an alternation in hemodynamic change in the present study by the use of high speed liquid chromatography and automated THI method. The increments in plasma catecholamines were not correlated with apparent decrements in blood pressure. Therefore the increments in plasma catecholamines induced by methacholine were not attributable to reflex responses to hemodynamic changes and rather could be the direct effect of methacholine on sympathetic neuron and adrenal medullae. Besides there was no correlation between the increments in plasma catecholamines and methacholine indices. These data question the suitability of blood pressure as an index for the evaluation of sympathetic nervous system function during methacholine test and suggest that plasma catecholamines might be a proper parameter for the sympathetic nervous activities response to methacholine.

Skin test with Varicella-zoster virus antigen on herpes zoster patients.

Intracutaneous injection of Varicella-zoster (V-Z) viral antigen was performed on 55 patients with herpes zoster. All of them reacted positively after 10 days. The patients in the acute stage responded weakly wereas those in the convalescent phase developed a stronger reaction. The intensity of the reaction did not correlate with the number of days of illness. Positive reactions were observed in those patients who had suffered from herpes zoster several years ago. Complement fixing antibody titers did not parallel the intensity of skin reaction.

Chronic granulomatous disease associated with peculiar Aspergillus lesions. Patho-anatomical report based on two autopsy cases and a brief review of all autopsy cases reported in Japan.

Chronic granulomatous disease (CGD) is based on the dysfunction of phagocytes and characterized by a comparatively uniform granulomatous lesion caused by organisms which do not produce hydrogen peroxide and which are catalase-positive. This report describes two autopsy cases in children, a nine year-two month-old boy and a ten month-old girl, with the clinical manifestations and autopsy findings of CGD and a brief review of all autopsy cases consistent with CGD reported in Japan. In these cases, in addition to the usual CGD lesions, there was a markedly different type of granuloma due to Aspergillus sp., which consisted of multinuclear giant cells alone, or a caseous center surrounded by giant cells. This peculiar type of aspergillosis may correspond to a primary infection, described as pseudotuberculosis aspergillina. It appears that in some CGD patients, macrophages may function normally and sometimes be more activated by these infections than in the normal body. The histochemical and electron microscopic examination of the present cases and a review of the previous cases suggest that the yellowish brown pigment found seems to resemble a ceroid. It is a product of the degradation of leukocytes and tissue elements with subsequent accumulation in phagocytic histiocytes and is not necessarily peculiar to CGD.

Incontinentia pigmenti achromians (Ito).

A patient with incontinentia pigmenti achromians was reported. She displayed other abnormalities such as a saddle nose, malformed auricles, growth retardation, convulsions and abnormal EEG. Histopathologically, a partial decrease in melanin granules in the basal layer and the presence of melanocytes which showed a weak dopa reaction were the main findings. The hypofunction of melanin production was detected by electron microscopy.

Plasma cyclic nucleotide responses to insulin-induced hypoglycaemia and methacholine in patients with hyperthyroidism.

The effect of insulin-induced hypoglycaemia and methacholine on plasma cAMP and cGMP levels was studied in normal volunteers, hyperthyroid and hypothyroid patients. A significant positive correlation existed between the maximal increase in plasma cAMP and the maximal decrease in plasma glucose in normals during insulin-induced hypoglycaemia. Therefore, the plasma cAMP response is considered to be dependent on the degree of hypoglycaemia, rather than the dose of insulin. The cAMP response to hypoglycaemia was significantly higher in hyperthyroid patient, and was lower in patients with hypothyroidism than in normals. The cAMP response of the hyperthyroid patients was normalized when their hyperthyroidism was controlled after 3 months of treatment. The plasma level of cGMP was slightly elevated during hypoglycaemia, but there was no significant difference between controls and hyperthyroid patients. The cGMP response to methacholine, which is probably mediated by cholinergic receptors, was significantly potentiated in hyperthyroid patients. The cAMP response, which is presumably dependent on endogenous catecholamines secreted during methacholine-induced hypotension, was also enhanced in hyperthyroid patients. It is likely that beta-adrenergic receptor responses and cholinergic receptor responses are both enhanced in hyperthyroidism.

Biosynthesis of ergosterol in cell-free system of yeast.

We previously proposed the occurrence of multiple pathways in the ergosterol biosynthesis of yeast, based on the results of examination of 14C-incorporation into sterols from L-[methyl-14C]methionine which was given to the intact cells of yeast. This led us to investigate the validity of the pathways by experiments with the cell-free system. Highly active cell-free extracts could be prepared by disruption of yeast cells with a Vibrogen Cell Mill in the presence of 0.1 mM dithiothreitol. This preparation catalyzed 14C-incorporation from [14C]methionine into ergosterol with a high yield. This preparation was found to be favorable for elucidation of ergosterol synthesis, since only a small amount of radioactivity was incorporated into fatty acid ester form of sterols which were reported to be inactive as a substrate for sterol synthesis reaction. Time course experiment of 14C-incorporation from [14C]methionine into various sterols under aerobic conditions showed that ergosta-8,24(28)-dien-3 beta-ol was a precursor for ergosta-7,24(28)-dien-3 beta-ol and that radioactivities were converted through ergosta-5,7,24(28)-trien-3 beta-ol and ergosta-5,7,22,24(28)-tetraen-3 beta-ol into ergosterol with time. In contrast, similar experiments under anaerobic conditions showed that ergosta-7,24(28)-dien-3 beta-ol accumulated and very little conversion of radioactivity into ergosterol occurred. In addition, the results indicated that oxygen was required for the introduction of double bond into 22 position as well as into 5 position. The results obtained with the cell-free system supported the validity of the proposal of multiple pathways of ergosterol synthesis in the intact cells.

[Clinical observation of 12 cases of primary aldosteronism, special reference to the diagnostic problem (author's transl)].

Twelve cases of patients with primary aldosteronism (PA) who admitted to Hokkaido University Hospital from 1965 to 1975 were studied. 1. Attention should be paid for the existence of PA as a frequent cause of juvenile hypertension in female, since female were frequently than in male (11 cases of female to 1 case of male) and 10 of 12 patients with PA were pointed out to have high blood pressure at the age of under 40. 2. Periodic paralysis as a main clinical symptom with PA were more frequent in our study than in the reports of other countries, which might be due to a large amount of salt intake in Japanese. 3. In accordance with the rapid progress of hormonal estimation for plasma renin activity (PRA) and plasma aldosterone concentration (PAC), the usefulness of potassium clearance test by the use of sodium thiosulfate became less worth. 4. It was impossible to differentiate primary aldosteronism from low renin essential hypertension by means of PRA after the stimuli of intravenous injection of furosemide and walking, but it could be differentiated after 3 days of sodium restriction, 3 days of administration of hydrochrolothiazide (75 mg/day) and 3 hours of walking. 5. With the long-term use of diuretics and various kinds of antihypertensive drugs in the treatment of high blood pressure in patients with PA, normal basal level and responsiveness of PRA were observed. Therefore no patient should be put through renin and aldosterone studies while on diuretics and antihypertensive drugs. 6. The 131I-19-iodocholesterol adrenal scintigraphy is a first choice method for the localizing diagnosis of PA because of inherent disadvantages in selective adrenal venography or in the determination of PAC in adrenal venous blood obtained against by catheterization of the adrenal veins.
