Histochemical study of the epithelia of nasal polyps by biotinylated lectins and neoglycoprotein. A comparison with the normal human respiratory epithelium.

Formalin-fixed, paraffin-embedded sections from human nasal polyps and the normal respiratory epithelium were glycohistochemically investigated. Three biotinylated lectins were used: peanut (Arachis hypogaea) agglutinin (PNA) which, binds to terminal galactose (beta 1-3) N-acetylgalactosamine residues that can be unmasked by a neuraminidase digestion; wheat germ (Triticum vulgare) agglutinin (WGA), which binds to N-acetylglucosamine and N-acetylneuraminic acids; and gorse seed (Ulex europaeus) agglutinin (UEA-1), which binds to L-fucose. In addition, the presence of accessible galactose (beta 1-3) N-acetylgalactosamine (T-antigen) glycan receptors (endolectins) was also assessed. The avidin-biotin-peroxidase complex (ABC) and diaminobenzidine (DAB) were used as chromogen. The ciliated cells of the normal respiratory epithelium and those of the pseudostratified epithelium of nasal polyps possess similar glycohistochemical characteristics suggesting no major alterations on the level of lectin-reactive carbohydrate epitopes as indicators of cellular glycosylations. Notably, this parameter can respond sensitively to changes in cell differentiation or activation. The basal and mucus-secreting cells in the two epithelia display different reactivity patterns emphasizing the presence of dissimilar sugar residues. Similarly, the dysplasia reflecting squamous epithelium of nasal polyps shows a distinct staining behaviour, indicative for disparate glycoconjugate display. Thus, quantitative differences in the lectin-selective staining of various cell types are detectable. The expression of T-antigen-bearing neoglycoprotein binding is weak and similar in both the normal epithelium and the pseudostratified epithelium lining nasal polyps. Only the most superficial cells of the squamous epithelium disclose a moderate labelling with this probe. These results indicate that further studies in this field are warranted, employing neoglycoproteins and also endolectins from human tissues to correlate glycobiological properties of the epithelium of the conducting airways and its diseased forms with functional features.

Determination of proliferative activity in nasal polyps.

AIMS: To determine the level of proliferative activity in 39 nasal polyps with clear cut distinct clinical behaviour patterns. METHODS: The 39 nasal polyps included 11 polyps labelled as "single" and taken from the lateral nasal wall and the middle turbinate; 12 polyps labelled as "massive" and relating to diffuse polyposis involving the entire nasal cavity; six polyps labelled as "ASA" and relating to nasal polyps from patients with acetylsalicylic acid intolerance and asthma; and 10 polyps from cystic fibrosis related polyposis. Cell proliferation was determined by two independent methods: first, the computer assisted microscope analysis of isolated Feulgen stained nuclei for the measurement of the percentage of cells in the S phase of the cell cycle; and second, the immunohistochemical evaluation of a proliferation associated protein by means of the MIB 1 monoclonal antibody. RESULTS: The cystic fibrosis related polyposis exhibited the highest proliferative activity of all the clinically identified nasal polyp groups. Acute inflammatory nasal polyps exhibited a higher cell proliferation than chronic ones. The results also show that while the immunohistochemical determination of cell proliferation by means of the MIB 1 monoclonal antibody is a valuable tool in determining cell proliferation in nasal polyps, the cytometrical image analysis of Feulgen stained nuclei is not useful for this purpose. CONCLUSION: Cell proliferation activity identifies cystic fibrosis as being distinct from the other nasal polyp groups.

Algorithm analysis of lectin glycohistochemistry and Feulgen cytometry for a new classification of nasal polyposis.

The aim of this study is to present a new classification of nasal polyps. This classification is based both on morphologic criteria relating to morphonuclear features from isolated Feulgen-stained nuclei and on glycohistochemical characteristics from histologic slides submitted to three lectins (peanut, wheat germ, and gorse seed agglutinins) and one neoglycoconjugate glycohistochemical stain. While the morphonuclear features (including 30 variables) relate essentially to chromatin pattern, the glycohistochemical stains (including 16 variables) are linked to the presence of specific carbohydrate moieties in cell membranes and cytoplasm. Forty-nine nasal polyps, including single polyps, diffuse polyposis, cystic fibrosis-related polyposis, and aspirin idiosyncracy-related polyposis associated with asthma, were thus characterized. All the variables were obtained quantitatively by means of computer-assisted microscopy. Two complementary methods of data classification were used to determine the actual diagnostic value contributed by each quantitative variable, namely, discriminant analysis, which forms part of multifactorial statistical analysis, and the decision tree technique, which is an artificial intelligence-related algorithm. The data so obtained show that our morphologic classification of nasal polyps fits in with the classification of nasal polyps defined on the basis of clinical criteria.

Sinusonasal polyposis.

The histological aspects and general disorders related to nasal polyps are reviewed. The origin and physiopathology of nasal polyposis are discussed. The influence of morphology in polyp classification and the contribution of glycohistochemistry are emphasized.

Histochemical detection of expression of binding sites for labelled hyaluronic acid and carrier-immobilized synthetic (histo-blood group trisaccharides) or biochemically purified (ganglioside GM1) glycoligands in nasal polyps and other human lesions including neoplasms.

This study is intended to demonstrate the versatility and feasibility of custom-made oligosaccharide-exposing neoglycoconjugates including histo-blood group epitopes in various human lesions, including nasal polyps. The binding of the biotinylated probes was determined on formalin-fixed paraffin-embedded sections from archive materials. The general aspects of our results may be interpreted as follows: the neoglycoconjugates used here can readily detect differences in the ability of cells to bind glycan residues in tissue sections, thereby enabling the extent of the binding capacity of various types of human lesions to be compared. Furthermore, the reactivity to glycan may reflect characteristics of the cells and their environment. The investigation into pathological disorders with respect to the binding capacity of these carrier-immobilized mono- or oligosaccharide structures derived from custom-made synthesis or biochemical purification is based on the prospect of translating progress in this field into the establishment of potentially beneficial procedures for medical diagnosis and pathological classification.

[Neurinoma of the VIIIth cranial nerve with negative electrophysiological findings].

Electrophysiological features in acoustic neurinoma are reviewed. A case of an acoustic neurinoma with a normal BERA is reported. The authors conclude that no isolated electrophysiological sign is able to disclose a retrocochlear hearing loss. They insist on the necessity to interpret an unilateral hearing loss on basis of an integration of a complete clinical, paraclinical and electrophysiological testing.

[Proposal for an original method of high-frequency audiometry using mathematical equations].

Many studies have established high-frequency audiometry norms. Unfortunately the majority presents different averages for each decade, sex and frequency together with standard deviations. In order to solve these problems we established our norms on a widespread audiometer for each sex based on a strictly normal hearing sample. We calculated the linear regression between age and hearing loss for each frequency. We propose a set of equations to predict the "normal" threshold of a subject in function of age and sex.

[Otological manifestations of Wegener's granulomatosis].

The authors report an observation of Wegener's disease, clinically isolated as a secretory otitis media on the left side followed by a right sudden deafness. Resistance to the usual treatment and the evolution of the disease with association of pulmonary manifestation and fever have lead to the diagnostic of Wegener granulomatosis confirmed by ANCA test and renal punction.

[Development of the auditory threshold during the first months of life related to modifications of the physiology of the external auditory canal].

A study realised among 60 children by means of a probe tube microphone allowed us to determine the mean ear canal resonance by the newborn and the young children. This is noticeably less than the adult one. The ear canal reaches its adult physiology after one year, so it implies a somewhat different auditory sensibility by the newborns than by the adults, particularly in the 2000-4000 Hz range. We suggest that the A.B.R. wave I latency and thresholds modifications during children's first year of life are (at least partially and in the majority of cases) an ear canal physiological variation effect and that different calibrations should be used for newborns screening test and for adults purposes.

[Sleep apnea syndrome : multidisciplinary management. Apropos of a preliminary experience].

The authors describe a multidisciplinary approach for patients with sleep apnea syndrome. During the first year 40 patients with obstructive sleep apnea documented by polysomnographic recordings were managed. Therapy proposed to the patient included weight loss and abstention from sedatives at night as sole treatment (n = 12) or, more often, combined with nasal and/or throat surgery (n = 21) or nasal continuous positive airway pressure (NCPAP) during sleep (n = 7). NCPAP was by far the most effective in reversing apneas but in view of its cost for the patient and its constraining aspects was proposed solely to those patients with the most severe apnea syndromes.

[Otitis media caused by Mycoplasma pneumoniae].

Description of a case of a bilateral otitis media due to Mycoplasma Pneumoniae associated with a sudden deafness. The hearing function of the patient returned to normal after a treatment of ten days with Erythromycine.

Cervical thymic cyst: a case report.

This paper describes the case of a young man who presented with a mass in the left side of his neck. Surgical excision revealed an ectopic thymic cyst. Thymic cyst should be included in the differential diagnosis of neck masses, especially in children. The authors review the embryology, diagnosis, and management of this pathology.

Congenital laryngeal cyst: benefits of prenatal diagnosis and multidisciplinary perinatal management.

Congenital laryngeal cysts are rare and can cause upper airway obstruction if they are misdiagnosed. We describe a case of large vallecular cyst diagnosed at 33 weeks of gestation. The purpose of an early diagnosis is to establish a careful perinatal management. If the airway is compromised, a multidisciplinary medical team is necessary to perform emergency immediate tracheostomy. Elective delivery should be always carried out in a tertiary referral center.

[Recurrent retroperitoneal liposarcoma, diagnostic and therapeutic problems].

Retroperitoneal liposarcomes are characterised by their tendency to recurrence, Eight cases are related, from which no one get out of this rule. Late diagnosis is due to their silent growth and poor symptomatology. Ancillary procedures, particularly computerised tomography, and histological examination, allow to confirm diagnosis. The main treatment is surgery, it must be as aggressive as possible, it allows to confirm histologically the nature of the tumor. Postoperative radiotherapy is helpful and chemotherapy can be associated, if it seems that complete excision has not been possible or if metastasis are detected. Histologically, these sarcomes seem to derive from a pluripotential mesenchymatous cell, it explains their polymorphic differentiation. A clinical prognosis, based on histology, seems to be illusive.
