[Surgical therapeutic planning for non-small cell lung cancer].

The survival rates of 380 resected cases of lung cancer in our hospital were analyzed according to curability and histological cell type. The overall 5-year survival rate for stage I a cases was 64.5%, that for stage I b 52.3%, and that for stage II 26.7%. However, there were distinct differences in survival rates between stages I a-II with mediastinal lymph node dissection and those without mediastinal lymph node dissection. Of these 380 tumors, many were advanced (for instance, stage III tumors comprised 180 cases). T3 tumors had better prognosis (40.7% showing 5-year survival) than N2 tumors (26.7% showing 5-year survival). Among stage III tumors, squamous cell carcinoma (T3: 41%, N2: 36.7% showing 5-year survival) had a better prognosis than adenocarcinoma (T3: 16.1%, N2: 21.4%). T3N2 tumors, however, had such a poor prognosis that the value of surgery in these cases seemed questionable. Adjuvant therapy should therefore be evaluated accurately in future to improve prognosis. It was stressed that a randomized controlled study would be needed to evaluate the effectiveness of adjuvant therapy.

[A clinical study of radiotherapy with HCFU for advanced and recurrent breast cancers. Radiation and HCFU Study Group for breast cancer].

We have investigated the usefulness of combination therapy with radiation and HCFU for advanced and recurrent breast cancer according to a clinical controlled multicenter trial from 1982 to 1984. One hundred cases were registered and 82 of them were available. Treatment sites were the lymph nodes, skin, bone and lung, and the overall response rate was 58% in CR and 19% in PR, while the duration of remission was 18 weeks in CR. Side effects were found in 10% of the patients. Combination therapy with radiation and HCFU may be useful in multimodal treatment for advanced recurrent breast cancer.

Acute effects of the new oral angiotensin converting enzyme inhibitor 1-(D-3-acetylthio-2-methylpropanoyl)-L-prolyl-L-phenylalanine (Alacepril) in essential hypertension.

In 15 patients with mild-to-moderate essential hypertension a new orally active angiotensin converting enzyme inhibitor, 1-(D-3-acetylthio-2-methylpropanoyl)-L-prolyl-L-phenylalanine (Alacepril), was administered with a single oral dose of 50 mg to evaluate its antihypertensive effect. Following Alacepril plasma angiotensin converting enzyme (ACE) activity was inhibited by approximately 75% within 2 h. Plasma renin activity increased slightly whereas plasma levels of aldosterone decreased significantly. Blood pressure fell markedly not only in patients with high renin levels but also in those with low renin levels. Nevertheless, the magnitude of blood pressure reduction was correlated with the pre-treatment plasma renin values (r = -0.602, p less than 0.05 systolic, r = -0.667, p less than 0.01 diastolic). No relevant changes in pulse rate was observed. These findings demonstrate that in essential hypertension the novel orally active ACE inhibitor Alacepril exerts marked antihypertensive effect, which may offer a new effective approach to treatment of hypertension.

Biochemical identification of renin in human pheochromocytoma.

A high activity of renin was demonstrated in human pheochromocytoma tissue. This activity was inhibited by specific antibody raised against human renal renin, indicating that it was not due to the nonspecific actions of proteases such as cathepsin D. The specific renin shared some biochemical features with well-known kidney renin, such as molecular weight (47,000 daltons), optimum pH (6.0), the presence of trypsin-activatable inactive renin, and glycoprotein nature. However, the isoelectrofocusing pattern of renin from the pheochromocytoma differed from that of kidney and plasma renins hitherto reported, a discrepancy which could be interpreted as evidence for endogenous synthesis of the enzyme. Furthermore, angiotensin converting enzyme activity was found in the tissue. Since pheochromocytoma is considered to be of neural crest origin, these results provide biochemical and immunological evidence for the presence of the renin-angiotensin cycle within human neuronal cells.

[Late diastolic mitral regurgitation: pulsed Doppler echocardiographic study].

Since the description of late diastolic mitral regurgitation (MR) detected by left ventriculography, the risk of overestimating systolic MR has been pointed out. However, it has not been well studied by pulsed Doppler echocardiography, though this method is thought to be extremely useful to analyze systolic MR. The purpose of this study was to evaluate the usefulness of pulsed Doppler echocardiography in assessing late diastolic MR and to clarify the pathophysiology of this MR. With the probe in the apical position, the blood flow pattern at the left atrial outflow tract was investigated in 226 consecutive cases who underwent left ventriculography. Late diastolic MR, which could not be attributed to ventricular premature contraction, was observed in 10 cases (4.4%) by left ventriculography. Meanwhile, by pulsed Doppler echocardiography, late diastolic disturbed flow directing towards the left atrial cavity in late diastole was detected in 15 cases (6.6%) including 10 cases with positive left ventriculographic MR. Among these 15 cases, 14 (93%) had atrial fibrillation and exhibited this MR only in the cardiac cycle with prolonged R-R intervals. The limitation in number of cardiac cycles which could be analyzed and the rapid heart rate eliminating the appearance of the beat with prolonged R-R intervals may be the reasons for the paucity of late diastolic MR by left ventriculography. Ten cases (66%) of late diastolic MR, including one with sinus rhythm, had aortic regurgitation, three cases showed high-grade systolic MR, and two had atrial septal defect. The simultaneous recording of pulmonary arterial wedge pressure and left ventricular pressure in three patients show a reversal of pressure gradient across the mitral valve in late diastole when the R-R interval was prolonged. In conclusion, pulsed Doppler echocardiography is useful in detecting late diastolic MR, and in reducing overestimates of systolic MR by ventriculography which was induced by erroneous addition of late diastolic MR, because Doppler method has better time resolution in phase analysis. We believe that incomplete premature mitral valve closure caused by a delay in left ventricular contraction following prolonged R-R intervals, and the reversal of left atrial-left ventricular pressure gradient induced by left ventricular over-filling played important roles in the occurrence of late diastolic MR.

Augmentation of host resistance to Listeria monocytogenes infection by Lactobacillus casei.

The effect of heat-killed Lactobacillus casei YIT 9018 (LC 9018), a potent antitumor agent as well as a potent macrophage-activating agent, on the resistance of mice to Listeria monocytogenes was studied. Mice pretreated intravenously (i.v.) with 0.5 mg of LC 9018 showed enhanced resistance to subsequent i.v. infection with L. monocytogenes. Antigenic cross-reaction between L. monocytogenes and LC 9018 was not seen in the delayed footpad reaction in mice, and LC 9018 inhibited the growth of L. monocytogenes in the liver and spleen of athymic nude mice. Therefore the antilisterial activity of LC 9018 was considered to be expressed nonspecifically and independently of the thymus. Carrageenan, a selective macrophage blocker, diminished the antilisterial effect of LC 9018. Peritoneal cells induced by intraperitoneal administration of LC 9018, which were composed mainly of macrophages and polymorphonuclear cells, showed enhanced phagocytosis and marked intracellular killing activity against L. monocytogenes in vitro as compared with those of the control. Kupffer cells of mice pretreated i.v. with LC 9018 produced a high oxygen radical level. The antilisterial effect of LC 9018 was maintained even at 21 days after its injection.
