Lower serum concentrations of cytokines in elderly patients with pneumonia and the impaired production of cytokines by peripheral blood monocytes in the elderly.

It has been well documented that the immune function declines with age; however, little is known about the monocyte/macrophage function of age. In the present study, we measured the concentrations of granulocyte colony-stimulating factor (G-CSF), granulocyte-macrophage colony-stimulating factor (GM-CSF), IL-1 beta, tumour necrosis factor-alpha (TNF-alpha), IL-8 and monocyte inflammatory protein-1 alpha (MIP-1 alpha) in sera from 15 elderly patients and 22 young patients with pneumonia, in the acute phase and after recovery, by ELISA. In addition, we measured the concentrations of these cytokines in culture supernatants from lipopolysaccharide (LPS)-stimulated peripheral blood monocytes from normal healthy elderly subjects and young subjects in order to clarify the ability of the elderly to produce these cytokines. The concentrations of these cytokines in sera from old patients and in those from young patients obtained in the acute phase were higher than those in sera obtained after recovery phase. However, the concentrations of these cytokines in the acute phase were lower in elderly patients compared with those in young patients. Serum concentrations of cytokines did not appear to be associated with clinical outcome. In the production of these cytokines by monocytes, LPS-stimulated monocytes from healthy normal elderly subjects produced smaller amounts of G-CSF, GM-CSF, IL-1 beta, TNF-alpha, IL-8 and MIP-1 alpha than those from healthy normal young subjects. These results with the impaired production of these cytokines in the elderly may prove, at least in part, the characteristic features of host defence mechanisms of the elderly.

[Optimal time of the day for measuring peak expiratory flow rates in patients with asthma].

Regular measurement of peak expiratory flow rate (PEF) has been recommended as an aid in the management of asthma, but the time of the day when PEF should be measured remains unclear. We therefore sought to determine the optimal time for measuring PEF in untreated patients with mild to moderate asthma. PEF was measured in 11 patients four times daily: on waking, around noon, in the evening, and at bedtime. Significant (p < 0.05) rhythms were detected by single cosinor analysis in all patients. Analysis by the population mean cosinor method showed that the mesor was 381 +/- 120 l/m, the amplitude was 52 +/- 21 l/m, and the acrophase was at 16:28 +/- 0:55. These data indicate that, in general, PEF is lowest at 4:30 a.m. and highest at 4:30 p.m. in patients with mild to moderate asthma. We conclude that PEF should be measured early in the morning and at 4:30 p.m. in such patients.

[Tuberculous mediastinal lymphadenitis presenting as hoarseness].

A 70 year-old woman was admitted to our hospital complaining of hoarseness. Bronchoscopic examination revealed that the left vocal cord was fixed in the paramedian position, and therefore left recurrent nerve paralysis was suspected. Although a chest X-ray film showed no abnormal findings, a chest computed tomogram showed that pretracheal (#3) and subaortic (#5) lymph nodes were swollen (1 to 2 cm in diameter). An open biopsy was done. Histological examination of the resected mediastinal lymph nodes revealed an accumulation of Langhans giant cells and epithelioid cells in caseous necrosis. An acid-fast bacillus was seen after Ziehl-Neelsen staining. This was a rare case of mediastinal tuberculous lymphadenitis in which hoarseness was useful for early diagnosis. Fifty-three cases of tuberculous mediastinal lymphadenitis have been reported in Japan.

[Age-related changes in morphological studies in rat and human kidney].

There have been many reports on changes in renal morphology with aging. In this study, morphological comparisons were made on the influence of aging in humans and rats. Kidneys were obtained from 63 autopsies (except those from cases of tumor, severe cicatrization, and cysts 10 mm or more in diameter) performed on humans aged from 0 to 92 years, including 7 cases with a past history of hypertension; the findings were compared with those from 201 Wistar male rats aged three to 115 weeks. First, changes in renal weight and renal cortex width were investigated. Next, changes in renal morphology were examined by microscopy. Finally, after performing alcian-blue-PAS staining, the ratio of the intima/outer diameter (I/OD ratio) of renal blood vessel was measured under a microscope with an image analyzer, and the results compared. A decrease in renal weight was observed in humans from about 50 years of age, but there was no decrease in weight with aging among rats. While significant thickening in the vascular intima was observed with aging in humans, there was no such thickening among rats. Furthermore, the presence of hypertension caused renal cortex width to be significantly decreased in humans (p < 0.04). Findings of sclerogenous changes of the glomeruli, infiltration of chronic inflammatory cells and fibrosis in the stroma, and tubular casts were observed with aging in both humans and rats. In conclusion, arteriosclerosis apparently develops with aging in humans, suggesting that arteriosclerosis greatly influences the reduction of human renal weight.

[Commencement and discontinuance of receiving financial aid for intractable disease treatment].

Data concerning all patients receiving financial aid for 26 types of intractable diseases in 1984, 1988 and 1992 were individually linked. Proportions of patients who discontinued financial aid in 1988 or 1992 among patients who were receiving aid in 1984 (ratio of discontinuance per patients); and ratios of the numbers of patients newly commencing aid in 1988 or 1992 to the numbers of patients who were receiving aid in 1984 (ratio of commencement per patients) were calculated. The results were as follows; 1. Among patients receiving financial aid in 1984, ratios of discontinuance per 100 patients were 28 in 1985-88 and 43 in 1985-92. Ratios of commencement per 100 patients were 96 in 1985-88 and 203 in 1985-92. 2. Ratios of discontinuance and of commencement per 100 patients in 1985-92 among 26 intractable diseases were 27-96 and 31-1,068, respectively. 3. Ratios of discontinuance per patients were higher an age groups 0-19 year and 60 year and over than in other age groups. Ratios of commencement per patients were higher in age groups 20-29 year and 70 year and over than in other age groups. Both ratios were higher among males than among females. 4. Ranges of ratios of discontinuance per patients and of ratios of commencement per patients were smaller among 47 prefectures than among diseases and age groups. The number of patients receiving financial aid in 1984 per population among prefectures was inversely correlated to ratios of commencement per patients, and were not correlated with ratios of discontinuance per patients.

Cloning and expression of PCPTP1 encoding protein tyrosine phosphatase.

A novel cDNA encoding PTP (protein tyrosine phosphatase) was cloned from PC12h cells and designated as PCPTP1 (gene encoding PC12 protein Tyr phosphatase). The longest open reading frame (ORF) of this clone encodes a 656-amino-acid (aa) protein with a single PTP catalytic domain. Western blot analysis using a polyclonal Ab (antibody) raised against the cytoplasmic region of PCPTP1 detected two products, a major 65-kDa and minor 42-kDa protein, designated PCPTP1-MFI and PCPTP1-MVQ, respectively, in PC12h cells. These two proteins correspond to the products translated from the second and fifth methionine of PCPTP1, respectively. The bacterially expressed GST::PCPTP1-MVQ fusion protein had phosphatase activity with pNPP (p-nitrophenyl phosphate) as a substrate. Alignment of the aa sequence of PCPTP1-MVQ with those of other PTP showed the highest similarity to STEP and LC-PTP/HePTP, with 54 and 51% identity, respectively. Northern blot analysis showed only one 3.9-kb transcript in PC12h cells, indicating that PCPTP1 corresponds to this 3.9-kb transcript. The 3.9-kb PCPTP1 mRNA was detected in the brain and adrenal gland, but not in other non-neuronal tissues in adult rats. Two other transcripts of 3.3 and 1.7 kb were also detected in brain. NGF (nerve growth factor) and glucocorticoid are known to bimodally regulate the cell fate decision of sympathoadrenal precursors like PC12 cells, with NGF promoting the neuronal phenotype and glucocorticoid promoting the chromaffin phenotype. Still, both agents decreased the level of PCPTP1 mRNA in PC12h cells. Therefore, it is likely that the decrease in the level of PCPTP1 mRNA might be associated or correlated with cell differentiation in PC12h cells.

Structural studies on a cell wall polysaccharide from Bifidobacterium longum YIT4028.

The major fraction of rhamnogalactan was isolated from the cell wall of Bifidobacterium longum YIT4028 by treatment with N-acetylmuramidase. This polysaccharide was composed of rhamnose and galactose in a molar ratio of about 2:3. The 13C NMR spectrum indicated that it contained a pentasaccharides repeating unit. This observation, and the results of Smith degradation, partial acid hydrolysis and methylation analysis led to the conclusion that its structure is [formula: see text]

A new animal model of cerebral infarction: magnetic embolization with carbonyl iron particles.

A new animal model of cerebral infarction was developed by magnetic embolization with carbonyl iron particles. An electromagnet was placed upon the lateral portion outside the orbit of Mongolian gerbils and charged with a current intensity of 2 A, 3 V (400 G) for 10 min. By intracardiac injection of iron particles at the beginning of the electric charge, infarction was selectively produced in the cerebral cortex. Two days after the operation, microhemorrhaging was seen in the necrotic region. Multiple scattered infarctions accompanied with microhemorrhagic signs closely resembled embolic infarction encountered in the human brain.

Somatic diversification and selection of immunoglobulin heavy and light chain variable region genes in IgG+ CD5+ chronic lymphocytic leukemia B cells.

Chronic lymphocytic leukemia (CLL) is characterized by the clonal expansion of CD5-expressing B lymphocytes. Most studies have found that these leukemic CD5+ B cells, like their normal counterparts, use immunoglobulin (Ig) variable (V) region genes that exhibit minimal, if any, somatic diversity. These and other observations have suggested that CD5+ B cells may be incapable of generating Ig V gene diversity, and therefore may not be able to develop higher affinity binding sites that could be selected by antigen. However, most of the studies of CLL and normal CD5+ B cells have focused on IgM-producing cells. Since somatic mutations are most often seen in B cells that have undergone an isotype class switch, we analyzed the Ig heavy (H) and light (L) chain variable region genes of seven IgG+CD5+ CLL B cells to determine if somatic diversification and antigen selection had occurred. The data derived provide evidence for skewed use, somatic diversification, and antigenic selection of the Ig V region genes. Nonrandom use of both H and L chain V region genes was manifested by an overrepresentation of VH4 and VKI family genes and the underrepresentation of the JH4 gene segment. Furthermore, VH4 gene use was restricted to only two family members (4.21 and 4.18). In four of the seven cases, the VH and VL genes displayed > or = 5% difference from the most homologous known germline counterparts. Polymerase chain reaction and Southern blot analyses performed in two of these patients demonstrated that their unique VH CDR2 and adjacent sequences were not present in their germline DNA. In addition, a significant level of diversity was seen in the rearranged DJH segments and at the VL-JL junctions of every patient that occurred both at the time of recombination and subsequently. The localization of replacement changes to complementarity determining regions of some patients suggested that antigen selection had occurred. Furthermore, the mutations identified in the VH and VL genes of each individual patient were strikingly similar, both in number and location. Collectively, the data indicate that a subset of CD5+ CLL B cells can display Ig V region gene mutations. In addition, they are consistent with the notions that in some cases antigen selection of these mutations may have occurred, and that antigen stimulation may be a promoting factor in the evolution of certain CLL clones.

Intermediate TCR cells with self-reactive clones are effector cells which induce syngeneic graft-versus-host disease in mice.

It has been established that, even after syngeneic bone marrow transplantation, animals treated with immunosuppressive drugs may suffer from graft-versus-host disease, showing autoimmune-like symptoms. Although the major effector cells are known to be T-cell subsets, detailed characterization of such T cells remains to be investigated. In the present study, we characterized them, especially as to whether they are thymus-derived T cells or extrathymic T cells, and how self-reactive clones were distributed among the above T-cell subsets. BALB/c mice (Mls-1b2a) were irradiated (9 Gy), subjected to bone marrow transplantation, and then treated with cyclosporin A (CsA) for 6 weeks. From 2 weeks after the cessation of CsA, these mice displayed signs of GVH disease. The major target organs included the liver and colon. Two-color staining for CD3 and IL-2R beta was applied to identify CD3-IL-2R beta+ NK cells, CD3-intermediate +IL-2R beta+ cells (i.e., intermediate CD3 or TCR cells of extrathymic origin) and CD3-high+IL-2R beta- cells (i.e., high CD3 cells of thymic origin). It was demonstrated that the major expanding lymphocytes were intermediate TCR cells and that self-reactive clones (V beta 3+ and V beta 11+ cells in this strain of mice) were confined to this population. Interestingly, these self-reactive clones had ability to respond to immobilized anti-V beta 3 and anti-V beta 11 mAbs. Liver MNC in mice with GVH disease which contained the highest proportion of intermediate TCR cells were able to mediate the adoptive transfer of GVH disease to other irradiated (6.5 Gy) mice. Intermediate TCR cells also showed potent cytotoxic activity against syngeneic leukemia cells. These results suggest that intermediate TCR cells are the major effector cells for the induction of syngeneic GVH disease.

Apoptosis is induced in BHK cells by the tsBN462/13 mutation in the CCG1/TAFII250 subunit of the TFIID basal transcription factor.

A temperature-sensitive (ts) mutant of the BHK21 cell line derived from golden hamsters, tsBN462 has a mutation in the gene encoding the largest subunit of the TFIID complex, TAFII250/p230/CCG1, and arrests in the G1 phase at the nonpermissive temperature, 39.5 degrees C. We found that tsBN462 cells underwent apoptosis following growth arrest at 39.5 degrees C, suggesting a role for CCG1 as a repressor of apoptosis. By electron microscopic observation, tsBN462 cells at 39.5 degrees C showed characteristic features of apoptosis. Apoptosis was not suppressed by expression of Bc1-2 or the adenovirus E1B 19 kDa protein. Cell death was suppressed completely by expression of wild-type CCG1 and partially by wild-type p53, a growth suppressor protein. Cell cycle arrest induced by p53 may help survival of tsBN462 cells at 39.5 degrees C. Apoptosis was accelerated in SV40 large T antigen-transformed tsBN462 cells at 39.5 degrees C where SV40 large T antigen formed a complex with p53, implying that the apoptosis of tsBN462 cells at 39.5 degrees C occurred in a p53-independent manner. Our results suggest that CCG1/TAFII250 is required for the expression of factors regulating apoptosis.

Transesophageal echocardiography in the diagnosis of thoracic saccular aortic aneurysm. The value of transesophageal echocardiography during the course of treatment.

Transesophageal echocardiography (TEE) was performed in 17 cases of aortic aneurysms referred to our hospital for further examination and treatment. All 17 cases were treated surgically and TEE was performed as a preoperative examination. In nine of the 17 cases, there were already some signs of bleeding upon admission and in all of these nine cases, rupture of the aneurysm was confirmed during surgery. Measurement on cross-sectional TEE imaging disclosed large aneurysmal diameters in eight of these nine cases, suggesting a close relationship between diameter and rupture. Moreover, observation of the lesions by TEE suggested a relationship between the risk of rupture and morphological characteristics of the thrombus. In seven of the nine bleeding cases, TEE imaging revealed destructive features of the aneurysmal thrombus, such as exfoliation from the aortic wall and/or tearing-off, suggesting expansion of the aortic diameter. Detailed findings of the aneurysm and thrombus on TEE corresponded with surgical findings. Thus, we concluded that TEE is a useful method of obtaining information about aortic aneurysms not only as a preoperative examination but also as an independent examination to determine treatment options and prognosis.

Autonomic hyperreflexia during labour.

We present two cases of automatic hyperreflexia (AH) during labour in women with spinal cord damage, in whom AH developed before and after delivery. The AH was successfully controlled using epidural anaesthesia in Case #1, but failed in Case #2. The blood pressure was controlled with nicardipine. However, overdose of nicardipine produces vasodilation and its side effects include headache, flushing and palpitation similar to AH. Considering these effects, we recommend epidural anaesthesia to control AH, because epidural anaesthesia does not only reduce BP, but also blocks the noxious stimuli and relieves the symptoms of AH. Our experience suggests that the epidural catheter can be placed two to three weeks before the date of predicted childbirth, because the onset of labour in a patient with spinal cord damage is difficult to predict and can proceed very rapidly. Also, the epidural catheter is available after the delivery. We recommended the epidural catheter is maintained for 24-48 hr postpartum.

PCDD, PCDF, planar and other PCB levels in human milk in Japan. off.

26 human milk samples in Japan were individually analyzed for PCDDs, planar and other PCB. Mean total concentrations of PCDDs planar and other PCBs were 8.5 pg/g, 8.8 pg/g and 5.5 ng/g of whole milk, respectively. TEQs were calculated using International-TEFs(I-TEFs) for PCDDs/Fs and TEFs proposed by the WHO for planar PCBs. Mean total TEQs of PCDDs and planar PCBs were 0.48, 0.63 and 0.39 pg/g of whole milk, respectively. These values were similar to those of other industrialized countries.
