[Induction of ovulation with subcutaneous pulsatile administration of human menopausal gonadotropin].

Induction of ovulation with subcutaneous pulsatile (every 90 min.) administration of HMG (Pergonal) 75 or 150 IU/day using a portable pump (Nipro SP-3I) was performed in 3 PCO patients (6 cycles), 4 first grade amenorrhea (Am-I) patients (7 cycles) and 4 Am-II patients (4 cycles). All patients ovulated except one cycle of Am-I patients and one PCO woman conceived. In regard to the duration of administration and the total dose of HMG until ovulation, the administration of 150 IU/day (M +/- SD=15.2 +/- 5.0 days, 2280 +/- 774 IU) is superior to 75 IU/day (39.5 +/- 11.4 days, 3900 +/- 1357 IU), and there was no significant difference between this method and the daily intramuscular injection of HMG. The group treated with HCG in the luteal phase revealed a longer luteal phase (14.0 +/- 2.3 days) than the nontreated group (12.6 +/- 1.5 days). Ovarian hyperstimulation was observed in one case and subsided spontaneously after admission. There were no other side effects. In conclusion, this method has the following advantages: A high ovulation rate, comparable with daily intramuscular administration. It is a less painful procedure than daily intramuscular injection. It is possible for the patient to lead normal life, insertion and removal being easily done by herself.

[Clomiphene-HMG ovarian stimulation in human in vitro fertilization and embryo transfer].

In a program for in vitro fertilization and embryo transfer, laparoscopies for oocyte aspiration were performed in 40 cycles in 36 normally menstruating women with irreparable tubal diseases (IVF patients) who received clomiphene citrate (CC) and human menopausal gonadotropin (hMG). An intramuscular injection of human chorionic gonadotropin (hCG) was given to all patients after completion of follicular maturation. Fourteen cycles in 13 spontaneously ovulating women (control patients), also stimulated with CC and hMG, were adequately monitored to identify the appearance of the spontaneous luteinizing hormone (LH) surge. The follicular maturation was followed by daily ovarian ultrasonographic examination and serum estradiol estimations. Just before the LH surge the diameter of the leading follicle was 20.2 +/- 0.7 (mean +/- S.E.) mm and the serum estradiol concentration per follicle was 384.1 +/- 16.3pg/ml in the control patients. In the IVF patients the former was 20.6 +/- 0.3mm and the latter was 305.8 +/- 13.3pg/ml prior to hCG administration. When the relationship of follicular size to the rates of oocytes recovery, maturation, fertilization and cleavage was examined, larger follicles (3ml less than or equal to follicular fluid volume) showed good results. Of the 152 oocytes that were recovered from these IVF patients, 96 (63.2%) were fertilized and 79 (52.0%) cleaved. Three pregnancies resulted from 35 embryo transfers.

Phagocytosis of polystyrene spheres in the rabbit corneal endothelium: contribution of lysosomal enzymes to the endothelial degeneration.

The rabbit corneal endothelium phagocytized polystyrene spheres 0.5 micron in diameter. After phagocytizing spheres, the endothelium degenerated, and lost from the Descemet's membrane. Lysosomal enzyme activities of the endothelium-Descemet's membrane complex, such as acid phosphatase, beta-glucuronidase and N-acetyl-beta-D-glucosaminidase, were assayed and the total activities per microgram DNA were almost constant. The unsedimentable activities in the complex, however, increased by phagocytosis of polystyrene spheres, which indicated an extralysosomal release of lysosomal enzymes. Released lysosomal enzymes probably would have accounted for the degeneration of the corneal endothelium.

Primary structure of heat-stable enterotoxin produced by Yersinia enterocolitica.

A heat-stable enterotoxin was isolated and purified from the culture supernatant of Yersinia enterocolitica by reversed-phase high-performance liquid chromatography. The amino acid sequence of the purified toxin was determined to be as follows: (sequence; see text) (X: not determined). The C-terminal sequence containing 6 half-cystine residues was highly homologous to that of heat-stable enterotoxin of enterotoxigenic Escherichia coli.

Role of mobile phase components for multi-functional solutes in silica gel liquid-solid chromatography. Resolution of protected deoxyribonucleotide derivatives oligomer synthesis.

To improve the resolution of a given sample mixture in liquid-solid chromatography, the characteristics of various components in the mobile phase were determined with a mixture of closely related compounds associated with DNA oligomer synthesis. Water (S0) was added so as to reduce peak tailing efficiently, but in amounts that would not significantly diminish retentivity in the silica gel column. The role of the stronger solvents, such as 1-propanol (S1) and dioxane (S2), in n-hexane (W) as diluent was quantitatively evaluated to control the retentivity. A linear correlation was found between the logarithms of the capacity ratios and molar concentration of the stronger component in the ternary solvent mobile phase. The selectivity of two stronger solvents was incorporated into the phase system to increase the resolution function for four given samples. A quaternary solvent system (W + S1 + S2 + S0) was prepared by integrating the characteristics of all the components, as determined by regression analysis. The multi-component phase system led to a high degree of resolution. This effective procedure was found suitable for analytical and preparative purposes in the resolution of various complex mixtures.

In vitro effect of hyaluronate on lysosomal enzyme activities in the bovine ciliary body and iris.

The effect of hyaluronate on lysosomal enzyme activities in bovine ciliary body and iris was studied in vitro. Hyaluronates from both the human umbilical cord and bovine vitreous inhibited the activities of cathepsin B and acid phosphatase. Cathepsin D, beta-N-acetyl-glucosaminidase, beta-D-glucuronidase and alpha-L-fucosidase activities were not affected by the hyaluronate.

Preinjection with sodium fluorescein aids discrimination in vitreous surgery.

To discriminate formed and opacified vitreous from lacunae and irrigating solution during closed vitrectomy, sodium fluorescein solution was injected intravenously 18 to 20 hours prior to the operation. Vitreous gel became yellowish-green and helped in the resection of opacified vitreous, especially when ultraviolet light was used. The higher the total amount of fluorescein in the vitreous of patients with impaired blood-retinal barriers, the stronger the vitreous gel was stained. Neurosensory retina and hard proliferative vitreous membrane, however, did not stain well. This method can be useful to the beginning vitreous surgeon and in teaching surgical procedures.

[Effect of KC-404 on allergic reactions types I-IV].

The effects of 3-isobutyryl-2-isopropylpyrazolo [1,5-a] pyridine (KC-404), a new anti-allergic agent, on type I to IV allergic reactions were investigated. KC-404 administered orally inhibited heterologous and homologous passive cutaneous anaphylaxis (PCA) reactions in guinea pigs and homologous PCA in rats; the minimum effective doses were 50, 12.5 and 3 mg/kg, respectively. However, the inhibition of PCAs by KC-404 was incomplete so that only 50 to 60% inhibitions were obtainable even at the highest doses used. KC-404 had no effect on increased vascular permeability by chemical mediators other than SRS-A and hardly affected antigen-induced degranulation of the sensitized mesenteric mast cells in vitro. These results suggest that KC-404 exerts its effect conceivably through inhibition of the SRS-A-mediated component of PCA. KC-404 had no effect on type II allergic reaction as estimated by its failure to inhibit reversed cutaneous anaphylaxis in rats and the Forssman systemic reaction in guinea pigs. Also, no influence on complement activity was observed in vitro and in vivo. KC-404 (100 approximately 200 mg/kg, p.o.) produced a marked inhibition of passive and active Arthus reactions in guinea pigs and rabbits, respectively. The tuberculin reaction in guinea pigs was not affected by KC-404. These results suggest that KC-404 inhibits PCAs mediated by IgE as well as IgG antibodies probably through a unique mechanism of action. KC-404 was shown to be effective also on type III allergic reaction.

Acidic lens-protein degrading activity: I. Distribution in bovine ocular tissues.

Bovine lens-protein degradation by ocular tissue homogenates was studied biochemically. Acidic lens-protein degrading activity was widespread in ocular tissues. The ciliary body enzyme, which showed the highest activity at pH 3.8, was inhibited by pepstatin and leupeptin. Lens-protein degrading activity may result from a combination of cathepsins B, D, H and L.

[A study of 65 cases of urinary tract injury].

Since 1968, we experienced 65 cases of urinary tract injury. These include a rare case of traumatic renal artery thrombosis which was later followed by renovascular hypertension, a rare case of complete ureteral disruption, and 2 cases of renal injury which were first treated conservatively but were nephrectomized later because of rebleeding. These cases are reported and discussion is made about the relationship between the clinical findings and the indication of nephrectomy. We used endoscopic management (Yasuda method) in the cases of complete disruption of posterior urethra accompanied by pelvic fracture and obtained good results.

[A case of myocardial infarction related to total cystectomy and ileal conduit diversion under general anaesthesia].

Prognosis is extremely poor when an aged patient is attacked by myocardial infarction during or after a major operation. This paper reports that a 63-year-old male patient who had not been specifically diagnosed to have any coronary artery disease before operation recovered from a myocardial infarction which occurred during total cystectomy and ileal conduit diversion under general anesthesia. The continuous electrocardiogram monitoring carried out during general anesthesia enabled us to discover and treat the disease early and save the patient's life. References to the myocardial infarction and the general anesthesia are also discussed.
