Diet and high density lipoproteins.

The acute and subacute effects of different diets on serum high density lipoprotein (HDL) cholesterol concentration and/or HDL composition are described. In obese normolipemic, hypercholesterolemic (type II) and hypertriglyceridemic (type IV) patients, low HDL cholesterol values remained low during total starvation for 2 weeks. Prudent diets in patients with type II and IV hyperlipoproteinemia had no significant effects on HDL cholesterol within 3 weeks, whereas in normal individuals, a high carbohydrate diet given for 10 days caused significant decreases in HDL cholesterol with concomitant increases in HDL triglycerides as compared to a high-fat diet. The HDL triglyceride fatty acid composition changed in healthy volunteers during the day, depending on the type of ingested fat. The data demonstrate the possibility of acute manipulations of HDL in normal patients and the difficulties of normalizing low HDL cholesterol levels in patients by dietary means. Additional information on the function of HDL is desirable before the therapeutic manipulation of HDL cholesterol or other constituents of HDL can be evaluated regarding their effectiveness in the prevention of ischemic vascular disease.

[Internal medicine and dactyloscopic examinations of mothers with malformed children].

A retrospective study concerning anamnestic data as well as an internal and dactyloscopic examination were caried out on 75 woman who were delivered from a deformed child. Apart from the well-known causes of a deformity the chronic arterial hypertension is emphasized as teratogenetic factor. The dactyloscopic examination did not result in an accumulation of stigmata which refer to a genetic condition of the deformities. Differences concerning the distribution of patterns on the finger-tips need further investigations; at present they are not yet to be explained.

[Tilidine as an analgesic in combination anesthesia].

In 2446 "insufflation anesthesias" the usefulness of Tilidine as an analgesic has been established. The preferred combinations were with Methohexital, Dehydrobenzperidol, Nitrous oxide, Succinyl and Diallyl-Nortoxiferine.

[The extent to which ethanol absorption is influenced by an alteration of gastric emptying (author's transl)].

Gastric emptying of a 500ml liquid test meal containing 6.2% ethanol and phenol red was studied on two consecutive days in a group of persons without gastroduodenal disease. On the first day an aqueous ethanol solution was administered, whilst on the second day a solution of ethanol in 10% glucose was given in order to delay gastric emptying. Twenty minutes after introduction of the test meal by tube, the gastric contents were recovered by aspiration and a gastroscopic examination was performed. Simultaneously, the amount of ethanol absorbed by the stomach, as well as the ethanol level in the peripheral venous blood was determined over a period of 10 to 90 minutes. Considerable individual variations were observed both in respect to the gastric emptying rate and the level of blood ethanol during the absorptive phase. The absorption rate and blood ethanol curvetion rate and blood ethanol during the absorptive phase. The absorption rate and blood ethanol curve were markedly reduced as a result of a delay in gastric emptying. The tolerance to alcohol may be greatly influenced by the rate of gastric emptying and, consequently, by the composition of alcholic beverages, drinking habits, and food intake.

[The efficacy of spironolacton in cardiopulmonary insufficiency (author's transl)].

In 17 patients with cardiopulmonary insufficiency the authors found a significant positive inotropic heart effect and a moderate improvement of ventilation after one injection of 400 mg Spirolacton. 2. The positive inotropic effect is experimentally confirmed in 8 dwarf pigs. 3. In 13 patients with severe cardiac and cardiopulmonary insufficiency a tendency to normalization of arterial blood gases, of actual bicarbonate and of vital capacity was found. There may be an indirect effect on the respiratory center by alteration of acid-base-status and by diuresis causing diminuation of pulmonary compliance. Spironolacton seems to be a valuable therpeutical completion in cardiac and cardiopulmonary insufficency in its intravenous quick application and its oral time application.

[Clinical significance of hyperuricaemia in medically-hospitalized women over 50 years of age (author's transl)].

Hyperuricaemia was detected in 150 female patients over 50 years of age in a medical ward and was studied in detail. Serum uric acid levels over 6.2 mg/100 ml were found in 29.1% of the females of this age group, the incidence of idiopathic hyperuricaemia being 6.4%. The most common causes of urate elevation were the ingestion of diurectics (32%), pulmonary diseases (20%) and idiopathic hyperuricaemia (24%). These findings suggest that treatment with saliuretics, especially in the case of hypertensive cardiovascular disease, requires the control of serum uric acid levels. When serum uric acid levels are found to be elevated, treatment with allopurinol is essential.
