Immunocytochemical localization of human papilloma virus and cytomorphologic correlation in smears and biopsies of cervical flat condylomata.

The cytomorphologic features in cervical biopsies and smears associated with human papilloma virus antigen (Ag) expression as demonstrated by immunoperoxidase staining techniques are presented. There was good concordance between cytology and biopsy results with respect to immunoperoxidase staining for human papilloma virus Ag. Cytomorphologic scoring of low-grade lesions (cervical intraepithelial neoplasias, grades O and I) showed atypical mitoses, macronucleated koilocytes, superficial cell keratohyaline granules, and multinucleation to be more frequent in Ag+ biopsy specimens. Chronic inflammation was more evident in Ag- biopsy specimens (P less than 0.05). Parakeratosis was more frequent in Ag+ cytologic specimens. Koilocytosis was not a reliable indicator of Ag expression in either preparation. Human papilloma virus (HPV) Ag testing by immunoperoxidase techniques appears to be a useful adjunct for screening low-grade atypias of the cervix.

Endometrial aspiration studies on Isaacs cell sampler with cytohistologic correlation.

Endometrial aspiration material obtained with the Isaacs cell sampler from 220 patients was studied and correlated with tissue biopsy from 172 of these patients. Of 52 endometrial carcinomas, there were two false-negative readings. In addition, five cases thought to be malignancies by cytologic criteria were not confirmed by histology. Therefore, the endometrial cell sampler showed a specificity of 96.85%, sensitivity of 96.15% and predictive value of 90.90%. Only nine cases of unsatisfactory cellular samples were obtained as compared to 13 cases of insufficient material by dilatation and curettage.

Cytodiagnosis of endometrial malignant mixed mesodermal tumor.

The accuracy of endometrial aspiration smears obtained with the Isaacs cell sampler in the diagnosis of malignant mixed mesodermal tumors (MMMT) was compared to the results obtained with routine cervical and vaginal smears in five cases of MMMT found in a series of 220 endometrial aspirations. Cervical and vaginal smears previously taken on these patients were positive for adenocarcinoma or MMMT in two cases and suspicious for adenocarcinoma in the remaining three cases. Endometrial aspirates were positive for MMMT in three cases and positive for adenocarcinoma or MMMT in two cases. The endometrial aspiration smears contained a variety of cells: malignant glandular, squamous, spindly stromal, undifferentiated, osteoid and tumor giant cells; chondrocytes and free psammoma bodies were also observed. These cases indicated that endometrial aspiration can accurately detect the heterologous cellular elements found in MMMT and is an effective technique in its diagnosis.

Cytodiagnosis of malignant lesions in cerebrospinal fluid. Review and cytohistologic correlation.

In an evaluation of the efficacy of cerebrospinal fluid (CSF) examination in our laboratory, 1,635 CSF specimens received between March 1, 1975, and December 31, 1982, were reviewed. All 111 positive samples (from 77 cases) and all 141 suspicious samples (from 83 cases) were reviewed microscopically, and the clinical records in the cases were rechecked. The 77 positive cases were confirmed by tissue and other studies: 23 as primary brain tumors, 26 as secondary carcinomas, 14 as leukemias, 12 as lymphomas and 2 as multiple myelomas. The 83 suspicious cases were similarly confirmed: 31 as primary brain tumors, 14 as secondary carcinomas, 22 as leukemias and 7 as lymphomas-with 9 found to be "benign." There were no false positives in the positive group. This study suggests that the identification of malignant cells in CSF samples constitutes a reliable method of diagnosis and lessens the need for further biopsies.

Cytodiagnosis of gastric carcinoma, linitis plastica type (diffuse, infiltrating, poorly differentiated adenocarcinoma).

Two hundred seventy cases, with a total of 359 gastric samples, were reviewed. Fifteen cases of linitis plastica (diffuse, infiltrating, poorly differentiated adenocarcinoma) and 25 cases of other types of gastric cancer were identified. Four false-negative cases in the limits plastica group and nine false positives in the other group were studied. The sensitivity of cytologic evaluation in limits plastica was 73.3% and in other groups was 98%. Predictive value of positive results in linitis plastica was 100% but in the other group was 72.7%. The reasons for these findings are discussed.

Cytodiagnosis of lesions of the pancreas and related areas.

Our experience of six and one-half years with cyto-diagnosis of pancreatic and related lesions is presented. We studied 22 cases of direct (perioperative and endoscopic) aspiration (group A) and 14 cases of percutaneous aspiration using ultrasound guidance (group B). Cytohistologic correlation in the direct aspiration cases and cytoclinical correlation in the percutaneous aspiration cases showed a specificity of 90% in group A and 100% in group B. Sensitivity was 100% in group A and 72.7% in group B. Predictive value of positive results was 92% in group A and 100% in group B. Eight of ten positive cancer cases in group A were confirmed by initial endoscopic tissue biopsies. One periampullary duodenal aspiration and one bile aspirate were cytologically positive, with negative endoscopic biopsy; final clinical evaluation with laparotomy confirmed these two cases as cancer. Thus, cytodiagnosis was more sensitive than tissue biopsy.
