Role of DNA probes in characterization of pathogenic and non-pathogenic E. histolytica.

Various tests have been described to differentiate the pathogenic and non-pathogenic types of E. histolytica. Recently DNA hybridization has been described to differentiate between the two subtypes. Using common HMC probe the presence of E. histolytica in stool was confirmed. Then on the basis of hybridization with DNA probe P 145 (pathogenic) and B 133 (non-pathogenic) E. histolytica was characterized as being pathogenic and non pathogenic respectively. Out of 137 patients studied 88 were symptomatic and 49 asymtomatic. 65 patients harboured E. histolytica as proved by microscopic examination of stool. Sixty-eight stool samples tested positive for DNA hybridization with common HMC probe, this included 65 microscopy positive samples and 3 microscopy negative samples. This gives a sensitivity of 100% and 96% specificity. All the 68 samples were then subjected to hybridization with P 145 and B 133 DNA probes. Out of 88 symptomatic patients stool samples of 57 patients were microscopy positive, however 58 were positive by common HMC probe and all of these were P 145 (pathogenic) positive and B 133 (non-pathogenic) negative. Of the 49 asymptomatic cases 8 were E. histolytica positive on microscopy and 10 positive on hybridization with common HMC probe and all 10 were P 145 negative and B 133 positive. It can be thus concluded that DNA hybridization is a reliable way to differentiate between pathogenic and nonpathogenic E. histolytica.

Senior-Loken syndrome with unusual manifestations.

The Senior-Loken syndrome, also called Hereditary Renal Retinal Dystrophy is a rare disorder that combines a juvenile nephronophthisis-like disease with tapetoretinal degeneration. We are reporting a family in which two, possibly three siblings are affected by this disorder. In addition to these two documented components of the disease, our patients also have unusual manifestations: short stature; skeletal abnormalities in the form of kyphoscoliosis, short metacarpals; and cutis laxa-unreported so far.

Management of teeth with open apices and necrotic pulps with single visit apexification: 3 representative cases.

Apexification is the most widely accepted procedure for the treatment of nonvital immature teeth. Single visit apexification is less time consuming, more economical and an easily acceptable technique with the aim of inducing the formation of a hard tissue barrier, thus allowing proper condensation of gutta percha in the root canal. Factors considered during this procedure are: i. Debridement of root canal minimizing the infection and filling the root canal space with resorbable calcification inducing material i.e. Ca(OH)2. ii. Minimal mechanical intervention to maintain the integrity and vascularity of apical region. Once filled, Ca(OH)2 was not changed again and the treated tooth was examined only radiographically till the desired apical closure was achieved. Three representative cases treated with this single visit apexification technique are discussed here. In all the cases successful apical closure of different types was achieved. Treatment was then concluded with gutta percha obturation in all the cases with successful follow up.

Partial sequence analysis of a Plasmodium vivax cation transporting ATPase gene homologue & a putative pseudohomologue.

Molecular characterization of P. vivax is essential to develop suitable antimalarial drugs and vaccines. We describe here isolation and sequence analysis of a partial cDNA of a calcium ATPase as well as a putative pseudogene from this parasite. The immunoscreening of lambda gtll- P. vivax DNA library with patients serum has earlier resulted in the isolation of several seroreactive clones including Pv14. This clone contains a 299 bp insert having 18 amino acids (aa) reading frame fused with beta galactosidase. A larger fragment of approximately 15 kb was isolated from the EMBL3 library for Pv14 but it had only 2 extra aa in its reading frame. The far upstream region of Pv14 revealed a 101aa long putative open reading frame (ORF) showing homology to a variety of calcium ATPases in the M8 and M9 transmembrane region. But in the absence of a transcript in the parasite could indicate that it represents a pseudogene. However, the real gene for calcium ATPase in P. vivax was detected by RT-PCR using degenerate primers, designed from the conserved sequences of energy transduction and phosphorylation domains. The amplified cDNA-PCR product of 550 bp was cloned and sequenced which showed a significant aa homology to the calcium ATPase4 of P. falciparum. The present study, therefore, establishes the existence of calcium ion pumps in P. vivax which will be useful in drug development.

Predictors of outcome in fulminant hepatic failure in children.

OBJECTIVE: To identify the predictors of outcome in fulminant hepatic failure (FHF) in children. STUDY DESIGN: Prospective cohort study. METHODS: 41 children with FHF were studied. Patient characteristics and findings on examination at the time of hospitalization were noted. Serum biochemistry and screening for hepatotropic viruses (A, B and C) were done in each patient. Patients were treated using a predefined protocol and followed up till death or discharge. Univariate and multivariate analysis was done to find the predictors of outcome. RESULTS: Hepatitis B was the commonest cause of FHF (11 children; 26.9%). Markers for hepatitis A and C viruses were present in one and two patients, respectively. Serology was negative in 27 children (65.9%), of whom two had history of ingestion of hepatotoxins (antitubercular drugs). The overall mortality was 61%. Irrespective of etiology, the following factors were associated with poor outcome on univariate analysis: presence of gastrointestinal (GI) hemorrhage, serum bilirubin more than 10 mg/dL, age 6 years or less, coma of grade 3 or more, presence of infection, prolongation of prothrombin time > 8 s over control, prothrombin concentration < 50%, hypoglycemia (blood glucose < 45 mg/dL), hyponatremia (serum sodium < 125 mEq/L) and hyperkalemia (serum potassium > 5.5 mEq/L). On multiple logistic regression analysis, presence of GI hemorrhage (p = 0.005), degree of coma (p = 0.02) and serum bilirubin level (p = 0.025) were identified as independent predictors of mortality.

Reliability and validity of the SKAMP rating scale in a laboratory school setting.

In children with attention deficit hyperactivity disorder (ADHD), the effects of methylphenidate were investigated in a pharmacodynamic comparison of placebo and the standard b.i.d. administration of methylphenidate. In each of these conditions, teachers completed ratings in classroom settings at times chosen to coincide with expected "peaks" and "troughs" of serum concentrations in the b.i.d. condition. Analyses of variance (ANOVAs) revealed the expected differences between the two conditions in the laboratory classroom setting using standard rating scales (Conners and the IOWA Conners) and a new rating scale (the SKAMP), which specifically measures the classroom manifestation of ADHD. The psychometric properties of the SKAMP were evaluated by calculating test-retest reliability and by calculating correlations with the standard rating scales to establish concurrent validity.

An alternative approach to bandage contact lenses.

PURPOSE: Therapeutic bandage contact lenses have been used in the management of a variety of corneal and anterior segment disorders. However, clinical experience with the therapeutic use of disposable contact lenses is limited. The present study was designed to evaluate the efficacy of disposable contact lenses as bandage lenses in anterior segment disorders. METHODS: We fit 45 patients who had various corneal and anterior segment disorders with disposable contact lenses (42% etafilcon A and 58% water content). We used this lens as a healing and surgical adjunct and for structural support and symptomatic relief. Patients were followed for a minimum period of 3 months. Therapeutic efficacy and complications were evaluated. RESULTS: Eighty-two percent of patients were fit successfully. Ninety-one percent of the successfully fit patients were therapeutic successes. Disposable lenses were found to be very useful in the management of epithelial abnormalities and as surgical adjuncts. Dry eye was the most frequent cause of contact lens associated therapeutic failure. Complications included contact lens deposits, lens loss, and corneal neovascularization. CONCLUSIONS: The use of disposable lenses was most effective in conditions requiring short-term lens use where one to two lens applications were sufficient to allow healing, making disposable lenses a very convenient and cost-effective alternative to conventional therapeutic bandage contact lenses. For reasons of cost, disposable lenses were not a viable alternative in conditions requiring therapeutic lens use for long periods of time.

Proteinase activity & virulence of Entamoeba histolytica on passage through hamster liver.

This study was undertaken to determine the enzymatic differences in the process of increasing the degree of virulence in E. histolytica successively passaged in hamster liver. Substrate gel electrophoresis was used to compare proteinase banding patterns under reducing conditions from whole cell lysates of one axenic E. histolytica (strain HM1:IMSS) and five xenic isolates of the same strain of E. histolytica passaged five times through hamster liver. Trophozoites successively passaged in hamster liver showed in supernatants, major bands in the 56-97 kDa region whereas only the axenic strain produced additional band at 34 KDa. Inoculation of amoebic trophozoites into hamster led to progressive increase in proteinase activity of supernatants as well as increased virulence of amoebae; proteinase activity of amoebae showed an excellent correlation with their virulence. All the infected animals died when activity of proteinase was 0.152 mg of protein.

Duplication cyst of the antrum: a case report.

Gastrointestinal duplication is a rare congenital anomaly. Although it usually presents within the first few years of life, it may appear much later as described in this report of a 19-year-old man who had symptoms of gastric outlet obstruction. He was found to have a noncommunicating antral duplication cyst. The cyst was managed by antrectomy with excision of the cyst and several centimetres of duodenum. Microscopically the duplication cyst contained a mucosa, submucosa and muscularis. There was no evidence of ulceration or malignant cells. His recovery was smooth. The etiology, presentation and management of antral duplication cysts causing gastric outlet obstruction are discussed.

Sialadenosis of parotid gland: a cytomorphologic and morphometric study of four cases.

BACKGROUND: Sialadenosis is recurrent, noninflammatory, nonneoplastic enlargement of salivary glands usually associated with an underlying systemic disorder. CASES: Fine needle aspiration was performed on four patients with bilateral, painless parotid swellings. The cytologic picture was distinctive and identical in all the cases. Smears revealed clusters of swollen acini and numerous naked nuclei of acinar origin in the background. There was absence of inflammatory cells. Diagnosis of sialadenosis was made in each case. Morphometric measurements were performed using an ocular micrometer. Results indicated a significant increase in mean acinar diameter in a sialadenotic gland as compared to a normal gland (76.03 microns vs. 53.79 microns). CONCLUSION: Cytomorphologic features of sialadenosis are distinctive enough to enable its diagnosis on fine needle aspiration. It is important to be aware of this entity as most cases do not require surgical intervention.

Hepatosplenic tuberculosis: a cause of persistent fever during recovery from prolonged neutropenia.

SETTING: Hepatosplenic abscesses in neutropenic patients, especially during the recovery phase, are almost always attributed to fungal infections. We report similar lesions due to Mycobacterium tuberculosis in neutropenic patients in a tertiary care centre in India. OBJECTIVE: To characterize the features of hepatosplenic tuberculosis in neutropenic patients. DESIGN: Retrospective comparison of disease pattern and response to treatment of hepatosplenic tuberculosis in febrile neutropenia patients (four of 30 with severe prolonged neutropenia) and in non neutropenic patients diagnosed during the same 12-month period (n = 4, control group). RESULTS: The disease in the neutropenic patients typically presented during the recovery phase of neutropenia, with ultrasonic abnormalities similar to those seen in hepatosplenic fungal infections. In contrast to the marked organomegaly and typical granulomatous response found in the control group, the disease in the neutropenic patients was characterised by an absence of organomegaly, non-involvement of other sites, poor inflammatory response and a high bacillary load. The initial response to therapy was satisfactory in both groups. CONCLUSION: Tuberculosis needs to be considered in the diagnostic work-up of hepatosplenic abscesses that occur during the recovery phase of neutropenia.

Effect of CDRI-85/287, a potent antiimplantation agent on uterine endometrial dimensions and peroxidase levels in rat.

CDRI compound 85/287 a potent estrogen antagonist and antiimplantation agent in rat was studied to elucidate its mechanism of action. In ovariectomized rats 85/287 treatment antagonized estrogen stimulated uterine volume density, eosinophil leucocyte infiltration, stromal mitotic cell number and peroxidase activity. In parallel experiments in pregnant rats, uterine peroxidase activity also decreased significantly as compared to controls on day 5 post-coitum. The results show that 85/287 exerts its antiimplantation activity by inhibition of responses to estradiol action.

Intestinal amoebiasis: antibody-secreting cells and humoral antibodies.

Splenic plasma cell response and systemic antibody response to intestinal amoebiasis were studied in C3H/HeJ mice from 5 to 60 days post-inoculation with Entamoeba histolytica. At various time intervals specific antibody-secreting cells (ASC) in the spleen were measured in infected mice and non-infected control mice by enzyme-linked immunospot (ELISPOT) assay. Serum antibodies were measured by enzyme-linked immunosorbent assay (ELISA). The infected animals showed high IgA ASC from 30 to 50 days post-inoculation as compared to IgM and IgG ASC. However, class-specific serum antibody showed high IgG titre from 30 to 60 days post-inoculation as compared to IgM and IgA serum titres. Our results suggest that E. histolytica trophozoites can induce a plasma cell response in the spleen that is different from anti-amoebic antibody response in serum.
