Epidemiology of scapular fractures. Incidence and classification of 338 fractures.

We have studied scapular fractures, with special reference to intraarticular glenoid fractures, during a 10-year period in 2 Swedish counties. There were 338 scapular fractures in 322 patients. The annual incidence was 10/10(5) inhabitants, of which 30 percent affected the glenoid cavity. Out of 100 intraarticular glenoid fractures, 55 occurred in men and 45 in women. The mean age of the women at the time of fracture (64 years) was significantly higher than the mean age of men (49 years). The most common intraarticular glenoid fracture type was the anterior chip fragment fracture which, in about two-thirds of the cases, was associated with shoulder dislocation.

Lethal horse-riding injuries.

Riding accidents can be of a serious nature. Knowledge of risk factors is of essential value in the prevention of injuries. From the years 1969 through 1982 a series of 53 lethal riding injuries is analysed with reference to the rider, the horse, and the environment. Craniocerebral injuries dominate in this series, indicating the importance of adequately protecting helmets. Among the victims the female sex is dominating before the age of 25 and the males above this age. Older horses are less frequently involved in these accidents than younger ones. A long training period for riders under surveillance of a teacher is of outmost importance. Outdoor riding should be recommended only to experienced riders.

Surgical decision in the treatment of extradural haematoma.

Extradural haematomas (EH) do not always present with the classical picture of skull trauma followed by a lucid interval, deepening coma, unilateral enlarging pupil and hemiplegia. Symptoms and signs can be puzzling and complex in many cases of EH and may thus lead to dangerous therapeutic delay. Uncertainties also arise concerning the length of the postoperative intensive care in unsuccessful cases. In this study the variables which from clinical experience seemed to be of main prognostic importance were related to the outcome in 44 patients with EH. A surgical decision-making table was compiled from these relationships in order to facilitate the decision process related to the diagnosis and therapy of EH.

Surgical decision analysis. Methodological aspects of statistical evaluations in therapeutic trials.

This report aims at discussing some of the sources of error and pitfalls involved in evaluating the efficiency of alternative modes of therapy in clinical materials. One of the main obstacles in such analysis is that the therapeutic groups are basically different due to the original selection of patients. In a previous paper (Pellettieri, 1980) a special method was worked out to circumvent this problem. The method requires that the variables of main prognostic importance can be identified. Patients in the different therapeutic groups who have identical patterns of combination of variables (risk profiles) can then be compared and the efficiency of different treatment programs can be evaluated. We believe that this method can be used in other fields of medicine.

Tendinography for diagnosing injuries to tendons and ligaments.

A radiographic method of tendinography is described. In rabbits no inflammatory reaction in the Achilles tendon was observed 12 to 15 days after injection of contrast medium. Effects of examination of two healthy subjects and a patient with a traumatic condition are described. Suitable amounts and concentrations of contrast medium for examinations of Achilles tendon and cruciate ligaments are discussed.

Hip joint instability after the neonatal period. II. The acetabular growth potential.

In 26 girls with idiopathic hip joint instability, diagnosed after the neonatal period, the acetabular angle is measured before and after treatment. A mathematical analysis of the acetabular growth potential in these hip joints and normal ones is presented. During treatment the monthly reduction of the acetabular angle is greater than in normal hip joints. The age limit for sole treatment in immobilisation without operative procedures is discussed.

Intussusception into the enteroanastomosis of Billroth II gastric resection.

A case of intussusception into the enteroanastomosis associated with a Billroth II gastric resection is presented. The case is discussed in the light of jejunogastric intussusceptions, described for the first time in 1917, about 30 years after the first gastrojejunostomy. For 30 years enteroanastomosis has been a general procedure in connection with the Billroth II type of gastric resection. The aetiology is discussed.

Ideas on the suspension of the below-knee prosthesis.

Comparative studies with the PTB-suction prosthesis and the ordinary PTB prosthesis have provided opportunities to analyze and understand some functional differences in the suspension of below-knee prostheses. In a roentgenological study of the stump-socket contact signficantly less vertical displacement in the PTB-suction prosthesis has been shown. A study of the pressure variations in the suction socket verified the theory of the necessary pressure gradient in the PTB-suction socket. Added negative pressure by muscle action was also observed. An electromyographic study suggested a muscular activity pattern in the suction prosthesis similar to that of a normal leg, as a contrast to the ordinary PTB prosthesis where simultaneous contractions of antagonistic muscles seemed to be the usual pattern. The latter is interpreted as a defence reaction. An optimal prosthesis has been deduced. It shall have a soft prosthetic socket with a perfect fit, a suction, adhesion and friction fixation and a pressure gradient between the skin and the socket wall with minimum compression distally. The plaster casting shall be done in a downward modelling manner making available space for soft tissue without compression.

Non-occlusive enteric gangrene associated with severe upper gastrointestinal bleeding.

Six cases of non-occlusive enteric gangrene in association with severe upper gastro-intestinal haemorrhage are described, the incidence of which corresponds to approximately one per cent of the patients with serious gastric bleedings. A brief discussion of the pathophysiology of ischaemic gut lesions is given. In order to reduce the incidence of this disease rapid and adequate treatment of all massive gastro-intestinal bleedings is emphasized. Patients with unexplained abdominal pain in association with haemorrhagic shock should especially be regarded as being at risk of developing the enteric gangrene.

A roentgenological study of the stump-socket contact and skeletal displacement in the PTB-Suciton Prosthesis.

The PTB-suction prosthesis has been studied by a roentgenological technique. There is complete contact between the skin of the stump and the plastic inner surface of the socket. The movement of the stump in the socket is minimal. The displacement of the skeletal stump is reduced to half in the PTB-suction prosthesis, compared with the strap-suspended PTB prosthesis. As a result, the stability between stump and socket is increased, provided that the stump is sufficiently long. Skin sores are avoided. The prosthesis, when adequately fitted, guarantees a good circulation within the stump and a cosmetically favourable fit with a feeling of walking with "a normal leg".

Electromyographic study of muscular activity in the amputation stump while walking with PTB- and PTB-suction prosthesis.

A comparative electromyographic study of the muscular activity while waling with PTB and PTB-suction prosthesis is presented. The PTB-suction prosthesis has been used clinically since 1969 with generally promising results. This prosthesis is mainly of value for patients with skin problems (poor sensibility and contact sores) and also has a cosmetic advantage. The present investigation has demonstrated that there is frequently a muscular activity pattern when walking with the PTB-suction prosthesis which is similar to that of a normal leg. The PTB prosthesis often shows a pattern of simultaneous contractions of antagonistic muscles. Furthermore, when the EMG-pattern is more "normal" and the amplitude higher with the PTB-suction prosthesis, it is probably the more suitable prosthesis. These EMG investigations can thus be a possible adjunct in the selection between the PTB-suction prosthesis and PTB prosthesis.
