Calculations of H2O microwave line broadening in collisions with He atoms: sensitivity to potential energy surfaces.

Broadening parameters for three microwave lines of water at 22.2, 183.3, and 380.2 GHz, in a bath of helium atoms, are calculated using accurate molecular scattering S matrices obtained from two theoretical potentials presented by Palma et al., J. Chem. Phys. 89, 1401 (1988). For the 22 GHz line results are in substantial agreement with values presented in that work, indicating the accuracy of approximate methods used there. The present work improves the potential energy surfaces, computed from perturbation theory (MP4) and variational interacting correlated fragments (ICF1) wave functions, by correcting them for basis set superposition error (BSSE), and recomputes the line broadening using a different procedure for fitting computed energy points. In addition, the entire set of calculations are repeated with a quite different basis set for orbital expansion to establish the reliability of the potential energy surface. We show that adjustments for superposition error are essential, and that broadening cross sections computed from the new surfaces are changed 10%-30% from the old, significantly improving agreement with experiment. The MP4 BSSE adjusted surface appears to be the most accurate, giving room temperature broadenings of 8.9, 11.8, and 10.0 angstroms2 compared with experimental determinations of 12.2 +/- 1.2, 11.9, and 11.2 angstroms2 for the 22, 183, and 380 GHz lines, respectively. Thus, computed line to line variation is larger than observed. The ICF1 BSSE adjusted results for pressure broadening cross section parallel those from the MP4 BSSE calculations but are about 10% smaller. We believe our computed results are stable with respect to basis set for orbital expansion and that the scattering calculations are accurate. Any theoretical inadequacy has been pinpointed to too few points on the potential energy surface resulting in an inadequate description of the angle dependence. It is not clear whether the present discrepancy between computation and experiment stems from this or from errors in the experimental values, although we show some indication that additional information on the surface might decrease the computed broadenings, worsening agreement with experiment.

Esorubicin in advanced endometrial cancer: an ineffective and potentially toxic therapy. A Southwest Oncology Group study.

The Southwest Oncology Group conducted a phase II study of esorubicin treatment in patients with advanced endometrial cancer who had received no prior chemotherapy. Twenty of 31 patients were fully evaluable for response and toxicity. There were no clinical responses to treatment and 60% (12/20) of the patients developed severe or life threatening leukopenia on therapy. One evaluable patient was removed from study after a cumulative dose of 150 mg/M2 due to a reduction in left ventricular ejection fraction on MUGA scan and another developed congestive heart failure several months after discontinuation of treatment. Esorubicin has significant toxicity and limited clinical activity in patients with advanced endometrial cancer.

The pharmacokinetics of ethynylestradiol in the presence and absence of gestodene and desogestrel.

Single doses of ethynylestradiol (30 micrograms) were given alone and in combination with either gestodene (75 micrograms) or desogestrel (150 micrograms) to 10 healthy female volunteers. The doses of steroids were given both orally and by i.v. infusion over 5-7 minutes. Blood samples were taken at regular intervals over 24 hours. The area under the plasma concentration versus time curve (AUC) for oral EE2 alone was 867 +/- 338 pg/ml x h, for oral EE2 in the presence of gestodene it was 795 +/- 206 pg/ml x h and for oral EE2 in the presence of desogestrel it was 614 +/- 132 pg/ml x h. With either gestodene or desogestrel present, the AUC of EE2 was not significantly different from that found when EE2 was given alone. In addition, there was no significant difference between EE2 + gestodene and EE2 + desogestrel. Comparing the relative oral and iv doses, the bioavailability of EE2 (alone) was 59.0 +/- 13% (n = 6), for EE2 plus gestodene it was 62.1 +/- 10% and for EE2 in the presence of desogestrel it was 62.1 +/- 4.4%. The clearance of EE2 (alone) was 19.9 +/- 5.5 l/h and in the presence of gestodene it was 19.4 +/- 9.6 l/h. The clearance of EE2 in the presence of desogestrel appeared slightly greater at 27.7 +/- 8.9 l/h but none of these clearance values were significantly different from each other. The urinary excretion of 6-beta-hydroxy cortisol was similar after all 6 doses of EE2. These data strongly suggest that following single dose administration, neither gestodene nor desogestrel have any inhibitory effect on the metabolism of EE2 or alter its kinetics to any clinically significant extent.

Benzodiazepines, putative anxiolytics and animal models of anxiety.

Anxiety is a complex state that includes a broad range of classified symptoms. Anxiolytic treatment has been dominated by the use of benzodiazepine drugs (BZs), which is often prolonged. However, the multiplicity of anxiety states and the lack of BZ specificity preclude the potent targeting of the drugs to different states, and point to the problem of drug dependence. Furthermore, animal models of anxiety give different results when challenged with apparently similar drugs. The search for alternative drugs with a higher specificity has led to 5-HT receptor subtypes. In this review, Simon Green describes how recent research might have dual benefits: in improving drug therapy of anxiety in humans, and in analysing and categorizing more clearly the concept of anxiety itself.

Second-line treatment of advanced measurable ovarian cancer with iproplatin: a Southwest Oncology Group study.

105 patients with advanced ovarian cancer previously treated with cisplatin or carboplatin were entered into a study of iproplatin as second-line therapy. Patients were either clinically resistant to cisplatin or carboplatin, or had relapsed after complete response to these agents. Patients were treated intravenously at an initial dosage of 270 mg/m2 with dosage adjustments to 340, 200 or 135 based on observed toxicity. Of 101 eligible patients, 7 responses (3 complete, 4 partial; 12%) were observed in 60 patients resistant to cisplatin. 2 partial responses (11%) occurred in 18 patients resistant to carboplatin. 2 complete and 3 partial responses were observed in 19 patients (26%) previously treated with but not resistant to cisplatin. Response durations were 2-20 months. Toxicities of iproplatin included thrombocytopenia in 93% of patients, leukopenia in 76% of patients, anaemia in 68% of patients, and diarrhoea in 40% of patients. Thus iproplatin shares cross-resistance with cisplatin and carboplatin in the treatment of ovarian cancer and is not recommended as an effective second-line agent for platinum-resistant ovarian cancer.

Cloning of novel members of the steroid hormone receptor superfamily.

We describe the cloning of four novel members of the steroid hormone receptor superfamily of ligand-activated transcription factors. Using chimaeric receptors we demonstrate that one of the receptor homologues is activated by the diverse peroxisome proliferator class of rodent hepatocarcinogens that includes hypolipidaemic drugs and industrial plasticisers. The identification of this receptor homologue should help elucidate the hypolipidaemic mechanism of these compounds and aid our evaluation of their potential carcinogenic risk to man.

Paradoxical response to nebulized ipratropium bromide in pre-term infants asymptomatic at follow-up.

Lung function measurements were performed before and after bronchodilator, nebulized ipratropium bromide (10 micrograms kg-1), in 20 pre-term infants [median gestational age 28 weeks (range 23-32 weeks) at a median postnatal age of 10 months (range 6-16 months)]. Eight of the infants had recurrent respiratory symptoms. Thoracic gas volume (TGV) and airways resistance (Raw) were measured by a plethysmographic technique and functional residual capacity (FRC) by a helium gas dilution technique. There was no significant change in either TGV or FRC following bronchodilator in the symptomatic and asymptomatic infants. Nebulized bronchodilator resulted in a significant improvement in Raw amongst the symptomatic infants (P less than 0.05), but a paradoxical response, that is, a deterioration (P less than 0.05) in Raw amongst the asymptomatic infants. In three asymptomatic infants, lung function measurements were repeated before and after nebulized saline and a similar deterioration in Raw was noted. We conclude the demonstration of respiratory symptoms at follow-up is useful in predicting infants who would have a beneficial response to nebulized ipratropium bromide.

In situ neutron spectrometry to 60 MeV in a water phantom exposed to a cancer therapy beam.

In-air and in-phantom neutron spectra have been measured between 10 and 60 MeV for two field sizes on the Clatterbridge cyclotron by unfolding the response of a specially built NE213 scintillator. The in-phantom measurements show distinct spectral hardening with depth, which is reflected in changes in the spectrum-averaged mean neutron energy. These findings are confirmed using Monte Carlo calculations.

A new method of estimating prevalence of injecting drug use in an urban population: results from a Scottish city.

Using modelling techniques derived from behavioural ecology, unnamed identifier data from a variety of partial samples of injecting drug users [IUDs] were used to estimate the prevalence of this population in Glasgow during 1989. The model yielded an estimate of 9424 [95% confidence interval +/- 2460] which represents a prevalence rate of 15 per 1000 population aged 15-55. The estimated male:female ratio was 2.64:1 and the modal IDU aged 20-24. These figures should facilitate assessment of the service requirements of this group and provide a basis from which the number of IDUs infected with HIV and the number likely to progress to AIDS can be determined. As the methodology required to facilitate reliable prevalence estimates utilizes relatively easy to obtain information it could be repeated in other urban centres where drug injecting is known to occur.

Phase II study of fludarabine phosphate (NSC-312887) in patients with advanced endometrial cancer. A Southwest Oncology Group Study.

Nineteen evaluable patients with advanced endometrial cancer refractory to one prior chemotherapeutic regimen were treated with a 5-day schedule of fludarabine phosphate. No responses were noted. The major toxicity was grade 2 or greater leukopenia in 45% of patients. Fludarabine phosphate at this dose and schedule does not appear to be an active agent for patients with refractory endometrial cancer.

Development and use of statistical survey criteria for release of materials at a former uranium processing facility.

To minimize the amount of waste and to recover reusable resources during demolition of radioactively contaminated facilities, building construction material and equipment can be efficiently and successfully surveyed using statistical methods described in this article. A radioactive contamination survey strategy aimed at reducing survey technician labor while still providing assurance that only uncontaminated material is released for salvage is discussed. Provided is the survey method, rationale, and results of three case studies where this method was successfully employed at a former uranium processing facility.

Lung function abnormalities at 6 months of age after neonatal intensive care.

Respiratory function was studied in 46 preterm infants (median gestational age 28 weeks) at 6 months of age. Respiratory function was assessed by measurement of thoracic gas volume, airways resistance (from which specific conductance was calculated), and functional residual capacity. To determine the effect of the duration of neonatal intermittent positive pressure ventilation (IPPV) on respiratory function, the infants' results were divided into three groups: group A, no IPPV; group B, IPPV for less than seven days; and group C, IPPV for more than seven days. Measurements of thoracic gas volume and functional residual capacity were similar to published reference ranges and there were no significant differences between the three groups. Measurements of airways resistance and specific conductance were higher than published reference ranges (mean airways resistance of group A, 34 cm H2O/l/sec; B, 34.1 cm H2O/l/sec, and C, 30.4 cm H2O/l/sec) but there was no significant difference between the three groups. The 95% confidence intervals of the difference of the means between groups A and B were -6.94 to 6.73 cm H2O/l/sec and between groups A and C -3.8 to 11.0 cm H2O/l/sec. Our results suggest that factors other than neonatal ventilation may be more important in the pathogenesis of chronic lung function abnormalities in preterm infants.

Management of supracondylar fractures of the femur with the GSH intramedullary nail: preliminary report.

The GSH supracondylar nail is a closed section rigid intramedullary device that combines the biomechanical advantages of intramedullary fixation with the stability of rigid internal fixation for the management of supracondylar fractures of the femur. The use of this implant is indicated in the management of distal comminuted nonarticular fractures and fractures with intraarticular extension. Intramedullary fixation provides improved fracture stabilization in both elderly patients with osteoporotic metaphyseal bone and in younger patients with extensively comminuted fractures. The experience with the GSH supracondylar nail has shown that it provides better results than supracondylar plating for the stabilization of acute fractures or revision of failed internal fixation.

Ilizarov technique in treatment of congenital hand anomalies. Two case reports.

An Ilizarov apparatus was successfully used in the treatment of a six-year-old child with a radially deviated hand caused by congenital pseudoarthrosis of the distal radius after previous traditional surgery failed. The limb length was restored, the pseudoarthrosis healed, and the deviated hand corrected. A second child, five years old, with Poland's syndrome, had a 90 degrees flexion contracture of the wrist that was treated with the Ilizarov apparatus. The flexion contracture was gradually corrected. It seems that the Ilizarov apparatus can be an important tool in the treatment of complex limb deformities.
