The encircling mesh in abdominal wall surgery: rationale and technical details of a dorso-ventral physiological restoration.

We describe a new, functional surgical technique, known as 'encircling mesh', designed to address abdominal wall problems. The rationale of the procedure is to connect the anterior and posterior trunk muscle compartments by means of a purpose built polypropylene mesh that encloses a belt shifted posteriorly across the spine subcutaneously, through use of a disposable introducer. The technical details and a case presentation, with specific references to the cosmetic and functional outcomes are described.

Reconstruction of the retroauricular fold: a personal technique.

In auricular reconstruction emphasis is placed on carving the rib-cartilage framework. The three-dimensional frame is very important in obtaining a good anatomical shape but often a good shape is not complemented by a good projection of the auricle. In order to avoid obliteration of the retroauricular fold we use a cartilage wedge covered by a local fascial flap. We have treated 17 ears in 16 patients with this technique and have obtained satisfactory results in all cases, achieving a mean projection of 1.7cm between the mastoid plane and the helical rim.

Macrocephaly with multiple soft tissue and visceral hamartomas ("Bannayan-Zonana" syndrome). Case report.

Since its first description few cases of the rare association of macrocephaly with multiple soft tissue and visceral hamartomas (Bannayan-Zonana syndrome) have been reported. In this paper we report a case of a 7 year old child in whom lipomas of the abdomen and right flank have been treated with liposuction. We could find no previous evidence that this surgical technique has been used in the treatment of this syndrome.

[Surgical treatment of gynecomastia. Indications and methods].

In this paper the authors present their experience in the surgical treatment of gynecomastia; the different surgical techniques used (liposuction, adenomammectomy, liposuction associated with adenomammectomy) are compared bilaterally 42 patients (37 patients) were affected of bilateral gynecomastia, 5 patients monolateral only (age 18-38 years) were treated in the Department of Plastic and Reconstructive Surgery-Ca' Granda Hospital of Milan, from 1985 to 1991. 12 patients were treated by the adenomammectomy technique; plain liposuction alone was used in 4 patients only. The associated method (liposuction and adenomammectomy) was performed on 26 patients. The results obtained suggest that the associated method is usually the most effective, because esthetic results are excellent and post-operative complications are very low. In selected cases however the other techniques should be preferred, and the criteria of selection are discussed.

Updated evaluation of the activity of antibiotics in a burn centre.

The results of microbiological tests performed in a burns unit between January 1989 and December 1990 have been analysed. Burn wound swabs and biopsies, blood cultures, central venous and urinary catheters, bronchial aspirates, pharyngeal swabs and faecal cultures for a total of 7950 examinations were considered. Staph. aureus was the most frequently isolated bacterium, followed by Staph. epidermidis, Pseud. aeruginosa and E. coli. The antibiograms have shown a low efficacy rate of cephalosporins, even of the latest generation, while with Gram-positive isolates the highest rates of activity were recorded by vancomycin and teicoplanin (100 per cent sensitivity). The situation seems better with Gram-negative organisms since they appear to be sensitive to a larger number of antimicrobial agents.

Legionnaire's pneumonia complicating a thermal burn.

The report describes a patient with 45 per cent BSA burns who developed Legionnaire's disease 3 days after the acute injury. The diagnosis of this life-threatening complication was late because most of its signs and symptoms can be encountered in the burned patient. This delay could have been fatal to the patient and required the evacuation of the burn centre for disinfection.
