High lethality of F1 (Dh/+) male mice from the cross between DDD female and DH (Dh/+) male.

The present study demonstrates the incidence of high lethality of F1 (Dh/+) male mice, the offspring of DDD females and inbred DH (Dh/+) males. Among the newborn pups, the numbers of F1 (Dh/+) males are comparable to those of mice having other genotypes (+/+ male, Dh/+ female and +/+ female), but many F1 (Dh/+) males died before weaning. On the other hand, among the F1 mice from the cross between DH (Dh/+) females and DDD males, there are no biased numbers of Dh genotype pups. These results suggest the existence of defects in the interaction between the male derived Dh gene and the genetic traits of the DDD female.

Isolation of a germline-transmissible embryonic stem (ES) cell line from C3H/He mice.

We have isolated three embryonic stem (ES) cell lines from C3H/He mice using mouse STO cells as a feeder layer. One ES cell line (H-1) was male, and two (H-2 and H-3) were female, as determined by polymerase chain reaction, in situ hybridization, and karyotype analyses. All were immunocytochemically reactive with a C3H strain-specific antibody. Injection of cells from the female ES H-3 line into C57BL/6 blastocysts yielded four chimeras with slight coat color chimerism. All chimeras were male, and as expected, no germline-transmission was observed. By contrast, when male ES H-1 cells were injected into the perivitelline space of 8-cell C57BL/6 embryos, one male mouse with overt coat color chimerism was recovered, and it produced ES H-1-derived offspring exclusively. This germline-transmissible C3H/He cell line represents a novel addition to those ES lines currently employed for gene manipulation studies of development.

Impairment of antioxidants in colonic epithelial cells isolated from trinitrobenzene sulphonic acid-induced colitis rats. Protective effect of rebamipide.

BACKGROUND: The functional status of glutathione, its related enzymes, and Cu,Zn-superoxide dismutase in colonocytes isolated from rats with trinitrobenzene sulphonic acid-induced colitis, and the effect of rebamipide (a scavenger) on these antioxidants and on colitis, were studied. METHODS: The rats were treated with rebamipide, killed on day 14, and compared with controls. Glutathione and enzymatic activities were spectrophotometrically estimated. The effect of rebamipide on colitis was also assessed histologically and by myeloperoxidase activity. RESULTS: In controls the concentration of glutathione and the activities of glutathione S-transferase and Cu,Zn-superoxide dismutase were decreased, but that of glutathione peroxidase was increased. Rebamipide prevented the impairment of these enzymes, restored the concentration of glutathione, and attenuated (low damage scores and myeloperoxidase activity) the severity of the colitis. CONCLUSIONS: These features suggest that the impairment of antioxidants is closely related to the development of the inflammatory lesions, and rebamipide has a beneficial effect on experimental colitis.

Potentiation of nitric oxide-mediated vasorelaxation by xanthine oxidase inhibitors.

Nitric oxide (NO), now almost synonymous with endothelium-derived relaxing factor (EDRF), reacts with superoxide anion radical (O2-) and forms a potentially toxic molecular species, peroxynitrite (ONOO-). Because xanthine oxidase (XO) seems to be a major O2- -producing enzyme in the vascular system, it is important to clarify the mechanism of XO regulation of NO/EDRF. We first characterized the inhibition of XO in vitro by three types of pyrazolopyrimidine derivatives. Kinetic studies indicated that 4-amino-6-hydroxpyrazolo[3,4-d]pyrimidine (AHPP) and allopurinol competitively inhibited the conversion of xanthine to uric acid catalyzed by XO, with apparent Ki values of 0.17 +/- 0.02 and 0.50 +/- 0.03 micro M respectively; alloxanthine inhibited this conversion in a noncompetitive manner with an apparent Ki value of 3.54 +/- 1.12 microM. O2- generation in the xanthine/XO system assayed by lucigenin-dependent chemiluminescence was suppressed most strongly by AHPP in a dose-dependent fashion; allopurinol itself appears to reduce the enzyme by transfer of an electron to O2, thus generating O(2-). AHPP significantly augmented EDRF-mediated relaxation of aortic rings from both rabbits and spontaneously hypertensive rats (SHR) in a dose-dependent manner, whereas allopurinol did not affect the relaxation and only marginal potentiation of the vasorelaxation was observed with alloxanthine. Finally, iv injection of AHPP (50.4 mg/kg; 100 micromol/300 g rat) reduced the blood pressure of SHR rats to 70% of the initial pressure; this pressure is almost the blood pressure of normal rats. Allopurinol (100 micromol/300 g rat; iv) showed transient decrease in blood pressure and moderate reduction of hypertension of SHR (10%) was observed with iv injection of alloxanthine (100 mumol/300 g rat). On the basis of these results, it seems that XO regulates EDRF/NO via production of O2-.

[Retroperitoneal abscess formation accompanied by intraabdominal free air, a rare complication of transcatheter arterial embolization of renal tumor: a case report].

Transcatheter arterial embolization (TAE) has been commonly used in the treatment of renal cancer. Postembolization syndrome consisting of fever, flank pain and gastrointestinal symptoms is encountered in almost every case. We report a case of E. coli-induced retroperitoneal abscess accompanied by intraabdominal free air which developed three weeks after TAE in a 59-year-old woman with T4N0M0 renal cancer. No fistula between abscess and gastrointestinal tract could be demonstrated by gastrofiberscopy and barium enema. Urine culture just before TAE should be examined to avoid such a complication.

[Combined hormone-chemotherapy for stage D2 prostatic cancer: starting time of chemotherapy in relation to hormone therapy].

We retrospectively analyzed 29 patients with stage D2 prostatic cancer who had been treated with chemotherapy at the urological clinic of Tokyo Medical and Dental University Hospital between 1983 and 1992 to evaluate the efficacy of chemotherapy for advanced prostatic cancer. The patients were divided into three groups according to the starting time of chemotherapy in relation to hormone therapy; 9 patients who received chemotherapy more than four weeks after the initial hormone therapy (group H), 9 patients who received chemotherapy only or in combination with the hormone therapy more than four weeks previously (group C), and 11 patients in whom both therapies were started within four weeks (group HC). Follow-up period ranged from four to 108 months averaging 35. Combination chemotherapy including cisplatin was administered one to 17 times with the median of five. The five-year survival rates estimated by Kaplan-Meier method were 18% in group H, 28% in group C, and 78% in group HC, respectively (HC vs. C: p < 0.05, HC vs. H: p = 0.059). These findings indicate that a combination chemotherapy including cisplatin may improve the prognosis of patients with advanced prostatic cancer when it is started with the initial hormone therapy simultaneously.

[In vitro and in vivo activities of sulopenem compared with those of imipenem and cephalosporins].

The in vitro and in vivo antibacterial activities of sulopenem (CP-70,429),a new parenteral penem antibiotic, were compared with those of imipenem (IPM), flomoxef, cefuzonam (CZON) and cefotaxime. Sulopenem possessed broad-spectrum activities against Gram-positive bacteria and Gram-negative bacteria. Antibacterial activities of sulopenem against methicillin-sensitive Staphylococcus aureus, Staphylococcus epidermidis, Enterococcus faecalis, Streptococcus pyogenes and Streptococcus pneumoniae were equivalent to or somewhat superior to those of IPM. Against members of the family Enterobacteriaceae, sulopenem was 4- to 260-fold more active than reference antibiotics with broad-spectra. In a killing kinetics study for Haemophilus influenzae, sulopenem showed a 99.9% decrease of viable cells after 8 hours at a concentration of 0.20 micrograms/ml. This effect was obtained at a concentration 8-fold lower than that of IPM. The protective effects of sulopenem in murine experimental systemic infections were superior to those of imipenem/cilastatin. In murine experimental mixed infection with Escherichia coli and Bacteroides fragilis, sulopenem had lower ED50, in other words stronger antimicrobial activities than IPM. The therapeutic effect of sulopenem are related well with its MIC value. In guinea pigs experimental lung infection with Klebsiella pneumoniae, sulopenem was more effective than CZON or cefotiam.

[Two patients with acute promyelocytic leukemia whose relapse was noted by cytodiagnosis of middle ear discharge].

Patient 1 was a 36-year-old male and diagnosed as APL in April 1989, and treated with BHAC-DMP and BHAC-AMP. In January 1990, a diagnosis of exudative otitis media was made, but intractable. In June, left facial paralysis appeared and cytodiagnosis of the discharge from the middle ear confirmed leukemic cells. Otitis media and facial paralysis improved after high dose Ara-C, but developed again 5 months later. The condition improved after high dose Ara-C and irradiation of the temporal bones. In September 1992, he died of recurrence but no aggravation in facial paralysis or otitis media. Patient 2 was a 24-year-old female and diagnosed as APL in July 1989, and treated with BHAC-DMP. In May 1990, exudative otitis media was appeared. In July, recurrence was observed but improved by high dose Ara-C. In October, otitis media was aggravated again, and cytodiagnosis confirmed leukemic cell infiltration. She was treated with high dose Ara-C and irradiation of the temporal bones, then achieved complete remission. Maintenance therapy was continued until August 1992, she has been alive. When exudative otitis media developed during the course of leukemia, cytodiagnosis of the discharge from the middle ear should be performed. High dose Ara-C and irradiation of the temporal bone were effective.

[Central alveolar hypoventilation syndrome with compression of the medulla oblongata by tortuous and elongated vertebral arteries].

A 43-year-old woman was admitted to the hospital with hypoventilation, which developed when she took sedatives because of an operation on an adrenal gland tumor. Central alveolar hypoventilation syndrome was diagnosed after neurophysiological and pulmonary-function tests. Magnetic resonance imaging and vertebral angiography revealed that tortuous and elongated vertebral arteries compressed the ventral sides of the medulla oblongata, which may have caused the hypoventilation. Mechanical ventilation during sleep improved her condition. Home mechanical ventilation was prescribed and she has been well during follow up as an outpatient.

[Therapeutic keratoplasty using corneas obtained from keratoconus patients].

Therapeutic lamellar keratoplasty was performed using corneas obtained from keratoconus patients undergoing penetrating keratoplasty. The corneas used in this series were stored in preservation solution for 7 to 59 days (average, 27.8 days) and submitted to surgery. The recipients were three patients with recurrent pterygium, one with primary pterygium, one with corneal perforation and iris prolapse due to fungal corneal ulcer, and one with limbal dermoid. Graft rejection developed in two cases postoperatively, but they were successfully treated with steroid therapy. During the entire period of clinical observation, there was no sign of recurrence of pterygium. In the case of the fungal corneal ulcer, the site of perforation healed quickly and the donor cornea maintained its transparency. A marked cosmetic improvement was achieved in the case of the limbal dermoid. Obtaining corneas from keratoconus patients and storing them for a short period is a potentially useful application for therapeutic lamellar keratoplasty.

Powerful immunosuppression by a new 40-kd serum protein (KX-5) induced by liver transplantation.

The presence of novel immunosuppressive proteins in the serum of liver grafted rats is associated with the induction of tolerance. This may provide part of the explanation for the induction or maintenance of tolerance in this liver graft mode, although their relationship to the antigen-specific aspects of liver graft survival (clonal deletion of T cells, release of soluble class I antigen, production of anti-class II antibodies) remains to be investigated. The KX-5 protein was nonspecifically immunosuppressive in vivo, suggesting the possibility of harnessing the tolerogenic properties of liver grafts in a soluble form for clinical application.

Cervical myelopathy caused by pseudoarthrosis between the atlas and axis associated with diffuse idiopathic skeletal hyperostosis.

STUDY DESIGN: This is a report of a patient with severe cervical myelopathy due to pseudoarthrosis between the posterior tubercle of the atlas and the spinous process of the axis, associated with diffuse idiopathic skeletal hyperostosis. Radiographs of 170 patients with neck pain were reviewed to identify lesions involving abnormal contact between the atlas and axis. OBJECTIVE: Based on an analysis of 170 radiographs, the prevalence of the reported condition was estimated. SUMMARY OF BACKGROUND DATA: A number of histologic studies on pseudoarthrosis involving the lumbar spine have been reported. In contrast, lesions between the posterior tubercle of the atlas and the spinous process of the axis have not been reported in association with cervical myelopathy. METHODS: Clinical and pathologic features of a patient with pseudoarthrosis between the posterior tubercle of the atlas and the spinous process of the axis were investigated. Radiographs of 170 Japanese patients over 40 years old were examined, and abnormal contact between the atlas and axis was classified into two groups based on the degree of spinal hyperostosis. RESULTS: Of 170 patients, 53 showed abnormal contact between the atlas and axis. The prevalence of abnormalities in the pronounced hyperostosis group (Forestier's stages II and III) was much higher than in the group with normal or slight hyperostosis. Two men had radiographic patterns showing osteophytes projecting into the spinal canal and associated with marked cervical myelopathy. CONCLUSION: In the group with pronounced hyperostosis, pseudoarthrosis or a variant can cause serious problems in the upper cervical spine that should not be overlooked.

Reaction mechanism of chitosanase from Streptomyces sp. N174.

Chitosanase was produced by the strain of Streptomyces lividans TK24 bearing the csn gene from Streptomyces sp. N174, and purified by S-Sepharose and Bio-Gel A column chromatography. Partially (25-35%) N-acetylated chitosan was digested by the purified chitosanase, and structures of the products were analysed by NMR spectroscopy. The chitosanase produced heterooligosaccharides consisting of D-GlcN and GlcNAc in addition to glucosamine oligosaccharides [(GlcN)n, n = 1, 2 and 3]. The reducing- and non-reducing-end residues of the heterooligosaccharide products were GlcNAc and GlcN respectively, indicating that the chitosanase can split the GlcNAc-GlcN linkage in addition to that of GlcN-GlcN. Time-dependent 1H-NMR spectra showing hydrolysis of (GlcN)6 by the chitosanase were obtained in order to determine the anomeric form of the reaction products. The chitosanase was found to produce only the alpha-form; therefore it is an inverting enzyme. Separation and quantification of (GlcN)n was achieved by HPLC, and the time course of the reaction catalysed by the chitosanase was studied using (GlcN)n (n = 4, 5 and 6) as the substrate. The chitosanase hydrolysed (GlcN)6 in an endo-splitting manner producing (GlcN)2, (GlcN)3 and (GlcN)4, and did not catalyse transglycosylation. Product distribution was (GlcN)3 >> (GlcN)2 > (GlcN)4. Cleavage to (GlcN)3 + (GlcN)3 predominated over that to (GlcN)2 + (GlcN)4. Time courses showed a decrease in rate of substrate degradation from (GlcN)6 to (GlcN)5 to (GlcN)4. It is most likely that the substrate-binding cleft of the chitosanase can accommodate at least six GlcN residues, and that the cleavage point is located at the midpoint of the binding cleft.

Long-term follow-up evaluation of surgery for ossification of the posterior longitudinal ligament.

STUDY DESIGN: We compared anterior and posterior surgery for cervical myelopathy resulting from ossification of the posterior longitudinal ligament. Surgical techniques, based on shape and distribution of ossification of the posterior longitudinal ligament, were divided into four technical phases. OBJECTIVES: Long-term follow-up data on anterior and posterior surgery were analyzed to establish guidelines for surgical treatment. SUMMARY OF BACKGROUND DATA: Comparison of anterior and posterior surgery is difficult because surgical techniques, ossification of the posterior longitudinal ligament shape classifications, and surgical criteria varied. No reports have accurately assessed spinal changes over a 10-year follow-up period. METHODS: Fifty patients received anterior surgery and 65 received posterior surgery between 1968 and 1993. Assessment after surgery was based on the recovery rate using the scoring system of the Japanese Orthopaedic Association. Spinal changes in the anterior group were assessed radiographically. RESULTS: Recovery and final results improved with phase after anterior, but not posterior, surgery. Neurologic deterioration after initial recovery was lower for the anterior group. One third of patients in the anterior group followed for more than 7 years exhibited neurologic deterioration, with most showing these changes within 10 years. Worsening was attributed to insufficient removal of lateral, superior, or inferior ossification of the posterior longitudinal ligament, reossification at the excision site, kyphotic malalignment, growth of ossification at upper cervical levels, or untreated complicated hypertrophy of the posterior longitudinal ligament. Many patients showed a good outcome after surgery. Accurate alignment and long-range fusion improved results. If the cord was compressed in a canal narrowed to under 3 mm, anterior surgery was considered "too risky." CONCLUSIONS: Complete extirpation of ossification of the posterior longitudinal ligament as confirmed by ultrasonography during surgery and long-range fusion with fibular grafts is advocated in the management of ossification of the posterior longitudinal ligament.
