Phorbol ester inhibition of hormonal induction of tyrosine aminotransferase.

The liver specific enzyme, tyrosine aminotransferase, can be induced by glucocorticoids, cAMP analogs, or insulin. Each of these different inducing agents is believed to act through a separate pathway. The tumor promoting phorbol esters have been reported to stimulate phosphorylation of the insulin receptor and thereby decrease the ability of insulin to induce tyrosine aminotransferase. Our results demonstrate that TPA will not only inhibit the insulin stimulated increase in tyrosine aminotransferase, but will also inhibit induction of the enzyme by glucocorticoids or by cAMP.

Reduction in intraventricular hemorrhage by elimination of fluctuating cerebral blood-flow velocity in preterm infants with respiratory distress syndrome.

In a previous study of preterm infants requiring mechanical ventilation for the respiratory distress syndrome, we demonstrated a striking association of fluctuating cerebral blood-flow velocity in the first day of life with the subsequent occurrence of intraventricular hemorrhage. Because this fluctuating pattern could be eliminated by muscle paralysis, we conducted a prospective study of preterm infants receiving mechanical ventilation for the respiratory distress syndrome in which we evaluated the effect of paralysis and this flow-velocity pattern on the incidence and severity of intraventricular hemorrhage. Twenty-four infants with the fluctuating pattern in the first hours of life were identified and randomly selected to serve as controls (10) or to be subjected to muscle paralysis (14). Intraventricular hemorrhage developed in all 10 control infants but in only 5 of the 14 infants subjected to muscle paralysis. Moreover, in 4 of the 5 paralyzed infants in whom hemorrhage developed, it did so after cessation of the paralysis. Seven of the 10 control infants had Grade III hemorrhage, the most severe variety of intraventricular hemorrhage, whereas none of the paralyzed infants had Grade III hemorrhage. We conclude that elimination of fluctuating cerebral blood-flow velocity in preterm infants with respiratory distress syndrome markedly reduces the incidence and severity of intraventricular hemorrhage.

Murine monoclonal antibodies to the myelin-associated glycoprotein react with large granular lymphocytes of human blood.

Monoclonal antibodies prepared to human myelin-associated glycoprotein were shown to react with a population of human peripheral blood mononuclear cells. The population is similar to the large granular lymphocytes or natural killer cells defined by antibody Leu 7 (also called HNK-1). The population also includes cells exhibiting the Leu 2 marker for suppressor/cytotoxic T cells. The results indicate a shared antigenicity between the nervous system and the immune system and may be relevant to the pathogenesis of demyelinating diseases.

Cataracts in childhood leukaemia.

The incidence and severity of cataract were studied in 37 children who had completed treatment for acute lymphoblastic leukaemia. Twelve (32%) had posterior subcapsular lens opacities. Treatment had included corticosteroids and cranial irradiation, but no dose relationship was evident. The boys were more severely affected than the girls. Although none of the lens opacities was optically significant, clinicians should be aware of the potential risk to vision.

Analysis of lymphocyte subsets in patients with aplastic anaemia.

Lymphocyte subsets have been measured in the blood of 28 patients with aplastic anaemia. The mean helper/inducer:suppressor/cytotoxic T lymphocyte ratio (1.24 +/- 0.74) was significantly decreased in the total population in comparison to mean ratios in a normal population (1.78 +/- 0.57) and in patients with other haematological diseases (1.82 +/- 0.92). A reversed ratio (less than or equal to 1) was present in a large proportion (53%) of aplastic patients, due both to an absolute deficiency of helper/inducer lymphocytes and an increase in suppressor/cytotoxic lymphocytes. Increased HLA-DR expression, evidence of lymphocyte activation, was present in seven of 12 patients evaluated and was confined to the suppressor/cytotoxic lymphocyte population. In the bone marrow, the percentage of lymphocytes was increased two-fold compared to normal bone marrow but the ratio of T cell subsets was not abnormal. Two of four patients evaluated after haematopoietic recovery following ATG treatment showed a return to normal of the T4/T8 ratio; in two others a low T4/T8 ratio persisted despite recovery. These results indicate that lymphocyte subset imbalance and lymphocyte activation are present in many patients with aplastic anaemia, either as a contributing factor or as a result of bone marrow failure.

The response of articular cartilage to the in vivo replacement of synovial fluid with saline.

Experiments on rabbits were performed to determine whether continuous irrigation of a synovial joint produced irreversible degenerative changes in the articular cartilage. Continuous irrigation of a synovial joint permits observations of the effect on articular cartilage of altering the intra-articular environment. The right knee joints were irrigated with normal saline for varying periods, up to 14 days, and the left knees served as nonirrigated controls. One group of animals (short-term) was killed immediately following irrigation, while another group (long-term) was assessed after six months of cage activity following the irrigation period. Within each group, half were skeletally immature and half were mature. No differences in cell density, nuclei-to-lacunae ratio, or the ratio of calcified layer to total cartilage thickness were found in any group. Irrigation did produce a significant reduction in matrix staining with safranin-O. This was first detectable following three days of irrigation and was severe by seven to 14 days. Staining properties returned to normal after six months of cage activity. No obvious difference was found between immature and mature animals.

Severe hyperbilirubinemia and coma in chronic congestive heart failure.

A 64-year-old man developed severe hyperbilirubinemia of predominantly conjugated fraction in 1978, eight years after a myocardial infarction and development of congestive heart failure. In 1975, he was admitted elsewhere for symptoms suggestive of chronic hepatitis, but liver biopsy revealed replacement of hepatocytes by red blood cells which was interpreted as a result of left-sided cardiac failure. In 1978, liver biopsy showed congestive liver disease with cardiac sclerosis. Despite initial improvement, his condition deteriorated, he became encephalopathic, and died in a coma. This case is reported to illustrate that chronic congestive heart failure can present with severe jaundice and terminate in hepatic coma.

Automated enzyme-linked fluorescence immunoassays for antiviral antibodies: a test for anti-HBs.

A solid-phase enzyme-linked fluorescence immunoassay (ELFA) for antiviral antibodies using antigen covalently linked to Escherichia coli beta-galactosidase and a fluorogenic substrate (4-methyl-umbelliferyl-beta-D-galactopyranoside) is described. Antibodies to hepatitis B surface antigen (HBsAg) from test samples were adsorbed to wells of 96-well aminopolystyrene plates precoated with HBsAg. The antibodies were subsequently detected using enzyme-conjugated HBsAg. The measurement of fluorescence was automated. The sensitivity of the test was approximately 50 times higher than that of a commercial radioimmunoassay.

Interrelation of the renin system and erythropoietin in rats.

Evidence is presented implicating the renin-angiotensin system in the regulation of plasma erythropoietin concentration. Male and female rats were exposed to hypoxia at 0.43 atm for 8 hr. In male rats, individual erythropoietin values showed a positive correlation with renin, renin substrate, and angiotensin I after 8 hr of hypoxia. In female rats, renin was not elevated during hypoxia. However, after renin was injected subcutaneously, plasma renin values became similar to those in male rats and erythropoietin doubled. Individual erythropoietin values of the combined groups showed a positive correlation with plasma renin. A single oral dose of SQ 14225, an angiotensin I-converting enzyme inhibitor, reduced erythropoietin to undetectable levels in renin-injected female rats. Angiotensin II, in subpressor amounts, prevented the suppression of erythropoietin by SQ 14225. SQ 14225 also reduced renin substrate concentration at ambient pressure and during hypoxic exposure. The correlation coefficient between renin substrate and erythropoietin in rats given SQ 14225 to lower substrate or angiotensin II to increase substrate was 0.85; p < 0.0005. The increase in renin substrate that occurred during hypoxia in female rats given angiotensin II correlated positively with erythropoietin, r = 0.86; p < 0.0005.

Dimensions and determinants of judgements of colour samples and a simulated interior space by architects and non-architects.

Architects and non-architects made Semantic Differential ratings of colour samples (chips) and a simulated interior space (a model). In analyses of the total samples' ratings (architects and non-architects) of (a) colour chips and (b) models, and individual sample analyses, (c) architects' chip judgements, (d) architects' model judgements, (e) non-architects' chip judgements, and (f) non-architects' model judgements, five factors occurred, though not necessarily all in any one analysis. These were: (i) dynamism; (ii) spatial quality; (iii) emotional tone; (iv) evaluation; (v) complexity. Linear correlations between parameters of the Munsell Color System and the above factors in the various analyses were calculated, while parallel analyses were carried out employing a graphical technique described by Sivik (1974a) involving isosemantic maps. In all analyses, linear correlations between the colour parameters and judgements were found for the dynamism factor, spatial quality factor, and emotional tone factor. They were associated respectively with chroma, value, and hue. Inspection of the isosemantic maps indicated subsidiary effects of the non-dominant dimensions of a non-linear sort, though the maps also exhibited the linear relations. Linear correlations were low or non-existent for the evaluation and complexity factors, and the complex nature of their determinants was clear from the isosemantic maps. The dterminants of judgements were similar for architects and non-architects, with the exception of evaluative judgements for the models in which markedly different determinants were noted. Comparability of the present findings with other studies carried out in a variety of countries over a 20 year period was high for dynamism, spatial quality, and emotional tone, and it is suggested that there may be something inherent in the response to colour in relation to such judgements. Recent physiological work is discussed, and its limitations in terms of colours sampled and an overconcentration on the hue dimension noted. In contrast, it is suggested that dimensions of judgement, such as evaluation or complexity, reflect to a greater extent culture or training, and are hence independent of the basic colour attributes.
