Treatment of Dunning R3327-AT rat prostate tumors with photodynamic therapy in combination with misonidazole.

Fischer X Copenhagen rats bearing Dunning R3327-AT tumors were treated with hematoporphyrin derivative and red light (630 nm from an argon-driven dye laser) alone or in combination with the hypoxic cell radiosensitizer, misonidazole (MISO). In vitro studies had suggested that hypoxia might significantly decrease the cytotoxicity of photodynamic therapy (PDT) and labeling with [14C]MISO had revealed a significant fraction of viable hypoxic cells in this tumor. PDT alone resulted in a growth delay of 8.8 days but no tumor cures were observed. The administration of MISO (i.p. at 0.5 mg/g) 33 min prior to PDT resulted in an average growth delay of 15.2 days and tumor cures (local control at 33 days) in 20% of animals treated. If MISO at a similar dosage was administered 30 min after PDT an average growth delay of 16.3 days was measured and tumor cure was observed in 70% of the animals treated. These results suggest that the hypoxic cell fraction in R3327-AT tumors might be a limitation to their curability by PDT. The addition of MISO and/or other hypoxic cell cytotoxic agents to PDT procedures may provide an effective means of treating PDT-resistant hypoxic cells in solid tumors.

Benign fibrous histiocytoma of the skin. An immunohistochemical analysis of 30 cases.

In this study the immunohistochemical analysis of distinct morphologic variants of benign fibrous histiocytoma (BFH) of the skin was performed with immunoperoxidase technique for both lysozyme and alpha-1-antitrypsin (A1AT). Thirty cases including cellular, fibrous and xanthomatous variants of BFH were selected. Out of the total 6 cases (20%) showed positive staining only for A1AT, 3 cases (10%) only for lysozyme and 10 (33.3%) for both markers. Thus, 19 cases (63.3%) showed positive staining for one of both markers. Positive staining was higher in the cellular variant than the fibrous and xanthomatous types. Negative staining of tumors of definite histiocytic morphology may be interpreted as a variable enzymatic expression of different histiocytic activation and/or undetectable enzymatic content by the current techniques. These results are in accordance with our previous evolutional hypothesis of BFH which considered the cellular tumors as functionally more active variants evolving to less cellular, more fibrous and less active types. Current histogenetic concepts about this controversial group of skin neoplasms are discussed.

Rudimentary cilia in hyperplastic, metaplastic and neoplastic cells of the lung and pleura.

Rudimentary cilia are cilium-like processes of the cell usually found within non-ciliated cells. The incidental electron microscopic finding of rudimentary cilia in 10 out 50 cases of hyperplastic, metaplastic and neoplastic lesions of the lung and pleura is reported. The fine structure of these peculiar structures is fully described. Reference is made to their significance and it is suggested that they represent deviations of the differentiation patterns of both normal and neoplastic cells.

Effect of asphyxia on free thyroid hormone levels in full term newborns.

Alterations in the thyroid metabolism of hypoxic, fasted, and chronically ill adults and older children have been described. We evaluated the effects of asphyxia on thyroidal indices of term newborns and compared them to those of a control population. Blood was drawn from the cord and then serially at 5 min and 3, 24, and 48 h after delivery in all patients. Seven term healthy newborns (group 1)increased their free thyroxine (FT4) concentrations significantly after delivery from a mean +/- SD baseline of 0.94 +/- 0.13 ng/dl in cord blood to a mean +/- SD peak of 2.6 +/- 0.6 ng/dl 48 h after delivery (p less than 0.001 at 3, 24, and 48 h), while their free triiodothyronine (FT3) levels increased from a mean +/- SD baseline level of 2.3 +/- 0.5 pg/ml in cord blood to a mean +/- SD peak of 3.7 baseline level of 2.3 +/- 0.5 pg/ml 48 h after delivery (p less than 0.001 at 24 and 48 h). Seven term newborns with transient low Apgar scores at birth (group 2) and seven term neonates born to mothers with toxemia or hypertension (group 3) failed to increase their FT4 and FT3 concentrations above baseline during the first 48 h of life. FT4 and FT3 values at 3, 24, and 48 h were significantly higher in the control group than in groups 2 and 3. Cord blood thyroid-stimulating hormone, FT4, and FT3 levels were not statistically different in the three groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Atypical cilia in hyperplastic, metaplastic, and dysplastic human bronchial mucosa.

Twenty-two cases displaying potentially precancerous epithelial changes of the bronchial mucosa--including basal and goblet cell hyperplasia, epidermoid metaplasia, and dysplasia--showed a wide variety of cilial abnormalities on the ultrastructural level. The changes comprised abnormal configurations of the ciliary plasma membrane, variations in the amount of ciliary matrix, disorganization of basal bodies, and diverse alterations of the axonemal microtubular pattern. More than one such alteration was observed in one and the same case, and there was no correlation with the actual type of epithelial lesion nor the degree of dysplasia. These changes, interpreted as abnormal ciliary regeneration in severely damaged ciliated cells, are potentially reversible and seem to represent a nonspecific response of the affected cells to an ample spectrum of noxes. Their diagnostic significance is poor, especially with regard to the modification of the axonemal microtubular pattern, unless the entire clinical picture suggests an immotile cilia syndrome.

Reference intervals for serum lipids, lipoproteins, and apoproteins in the elderly.

We measured total cholesterol, high-density-lipoprotein cholesterol, low-density-lipoprotein cholesterol, triglycerides, apoprotein A, and apoprotein B in serum. The subjects were a volunteer group of 145 white people (50 men and 95 women) ranging in age between 65 and 95 years, who were not receiving medical treatment and had no disease that could influence lipid metabolism. Before further categorization, we saw no significant sex-related difference in any of these lipid constituents. The mean age of the group was about 80 years, and we compared results for those older and younger. For the women, the only significant difference was a decrease in low-density-lipoprotein cholesterol for those older than 80 years. In men over 80 there was a significant decrease in triglycerides and in apoprotein B and an increase in high-density-lipoprotein cholesterol. The only sex-related difference for persons under and over 80 was in values for high-density-lipoprotein cholesterol, which were higher for men over 80, whereas triglycerides were higher for women over 80.

Discussion groups for chronic hemodialysis patients and their families.

The family as well as the patient undergoes many psychological stresses in the adjustment to end stage renal disease and maintenance hemodialysis. Despite the apparent need for psychological support services for families, few are offered. Renal social workers have found it difficult to elicit interest in discussion groups for families dealing with problems associated with dialysis. This report presents two discussion groups that were recruited and conducted for dialysis patients and their families. The process and development of the two groups are described, and recommendations are given for conducting multiple-family groups as an adjunct to the biologic treatment of severe long-term illness.

Benign fibrous histiocytoma of the skin. A morphologic study of 290 cases.

Two hundred and ninety cases of benign fibrous histiocytoma of the skin are reviewed. Their frequency, distribution according to age, sex and sites, and the architectural and cytological features of the tumors are analyzed. Ninety two cases (31,7%) were male and 198 cases (68,3%) were female. The tumors were most frequently found between second and fourth decades and located mainly in the extremities. Twenty three cases were clinically diagnosed as malignant tumors. Five cytological patterns of benign fibrous histiocytoma of the skin are described: fibrous, vascular, xanthomatous, pleomorphic and giant cell histiocytoma. Their probable histogenesis and the spectrum of morphological variants are discussed.

Adrenal cortical carcinoma in a 4-year-old child: temporal changes in steroid secretion.

A case is reported of a 4-year-old female with an adrenal cortical carcinoma that developed at the site of a previously removed adrenal gland. Initially, the tumor was only virilizing, but with time, it also became the source of a large secretion of glucocorticoids and, to a lesser degree, mineralocorticoids. It is one of the largest and most functional tumors described in a child.

Endometrial findings in postmenopausal women receiving long-term estrogen therapy.

An histological study of the endometrium of 43 postmenopausal women under the chronic effect of cyclically administered conjugated estrogens was performed. Four atypical hyperplasias and one adenocarcinoma in situ were found. The frequency of these findings is similar to the one found in a sample of the population consulting the gynecologic clinic of our Hospital that never received estrogen therapy.

A new instrument for uncomplicated emergency cricothyrotomy.

A new device for establishing an emergency airway provides sterility, anesthesia, control of bleeding, safe and quick entrance into the cricothyroid space, and adequate air exchange. In addition, the device is compatible with standard anesthetic and resuscitation machines, inexpensive, and small enough to be carried in a pocket.
