Histological lesions in the testis of infertile men with varicocele.

Varicocele frequently causes male infertility and histological lesions at the contralateral testis. The most frequent lesions found in this study included maturation arrest in the spermatidic phase, cellular and acellular thickening of the tubular wall, and degeneration of the Leydig cells. These lesions were typical of varicocele and their simultaneous presence suggests that scrotal temperature and modified endocrine secretion of the interstitial testis play a role in the pathogenesis of this type of infertility.

Circulating levels of immunoreactive peptides and steroid hormones in bronchogenic carcinoma.

Plasma levels of immunoreactive parathormone (iPTH), immunoreactive calcitonin (iCT) and prostaglandins (PGE2) were measured by RIA in 115 patients with bronchogenic carcinoma. In 37 of these cases the following hormones were also assayed: adrenocorticotropic hormone (ACTH), cortisol, plasma renin activity (PRA), aldosterone, prolactin, human growth hormone (HGH), thyroid stimulating hormone (TSH), luteinizing hormone (LH), follicle stimulating hormone (FSH) human chorionic gonadotropin (HCG), progesterone (P), androstenedione (A), testosterone (T), estradiol (E2) and dehydroepiandrosterone sulphate (DHAS). High serum levels of many hormone-like substances and hormones were found and the levels of certain hormones varied in some cases according to the clinical evolution of the disease and the response to therapy.

The influence of air pollution on the phyllosphere microflora composition of Tillandsia leaves (Bromeliaceae).

The effect of air pollution on total phyllospheric microflora from two species of the epiphytic neotropical genus Tillandsia (Bromeliaceae) was studied by comparing unpolluted plants living in a forest (Escazú, San José) with polluted ones from an urban site of Costa Rica (San José city). Dilutions of homogenized leaf samples were plated on media suitable for each microbial group. For each microorganism group, total counts were performed and purified strains of randomly chosen colonies were identified. There was a global reduction in the number of living microorganisms due to pollution effects, especially yeasts and bacteria, while nitrogen-fixing microorganisms and fungi were less affected. Our results showed that the phyllosphere microflora of Tillandsia plants living in a tropical urban environment changes in terms of number and species composition of yeasts and bacteria with respect to plants living in unpolluted environment.

[Ophthalmoparesis as a presentation form of subacute euthyroid thyroiditis].

The present study describes a case of subacute thyroiditis characterized by involvement of ocular motility that appeared many months before bioumoral findings of the disease. The authors believe that when the etiological diagnosis of an ophthalmoparesis, especially when isolated, is uncertain, one must carefully evaluate the function of the thyroid and levels of the specific antibodies, in order to start the appropriate treatment.

[Effects on the bones of nandrolone decanoate therapy in postmenopausal osteoporosis].

In many patients with involutional osteoporosis, anabolic steroids may produce a rapid subjective improvement and pronounced reduction of complaints. In animal experiments it has been demonstrated that anabolic steroids can also have objective effect on bone tissue. Twenty postmenopausal osteoporotic patients have been randomly assigned to two different treatments: 10 patients were treated with 50 mg i.m. of nandrolone decanoate every 3 weeks for 12 months; 10 patients were treated with placebo. Both groups received an oral calcium supplement (1 g/day). Bone mineral content (BMC) was measured by dual photon absorptiometry before and after 1, 3, 6 and 12 months. Plasma alkaline phosphatase (A.Ph.) and urinary hydroxyproline/creatinine ratio (HOP) were measured at the same times. Intestinal calcium absorption was measured by the 47Ca oral test before and after treatment. In 4 patients of both groups a transiliac bone biopsy was performed before and after treatment. After 1 year there was a significant increase in the BMC of the lumbar spine in the group receiving calcium plus nandrolone decanoate. A progressive but not significant increase of A.Ph. was observed in the group treated with nandrolone decanoate. Radiocalcium absorption significantly increased in nandrolone treated patients. The histomorphometric study of bone demonstrated a significant increases in trabecular bone volume and in active osteoid surfaces in patients treated with nandrolone decanoate. Because the plasma A.Ph. tendes to increase with no change in bone resorption (as measured by urinary HOP) and the active osteoid surfaces significantly augment, we conclude that nandrolone therapy increases the bone formation rate.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical efficacy of treatment with salmon calcitonin, administered intranasally for 1 year, in stabilized postmenopausal osteoporosis].

Several controlled clinical studies have confirmed the rationale of calcitonin therapy in postmenopausal osteoporosis. However, administration by injection and side-effects reduced patient compliance and flexibility of dosage. Recently, evidence has been given that an intranasal spray may provide an effective alternative administration route for calcitonin. Forty women with established postmenopausal osteoporosis (at least on vertebral crush fracture) divided into three groups, entered and completed a one-year controlled study on the effects of treatment with synthetic salmon calcitonin nasal spray on bone mass and mineral metabolism. The first group (n = 20) received a daily treatment with 100 I.U. of salmon calcitonin (sCT) nasal spray; the second group (n = 10) received 100 I.U. of sCT by subcutaneous injection every second day; the control group (n = 10) received an oral calcium supplement, 1 g per day. Bone mineral content (BMC), evaluated by dual photon absorptiometry, was measured at the distal radius before and after 6 and 12 months of treatment. Every three months, throughout the year, an evaluation of some parameters of bone remodeling was made. BMC increased (p less than 0.01) in the treatment groups, whereas at the end of treatment, a decrease (p less than 0.05) was observed in the control group. Biochemical estimates of bone resorption, such as urinary hydroxyproline excretion showed a significant decrease in the calcitonin groups. No changes in markers of bone formation, serum alkaline phosphatase and osteocalcin were observed in all patients. This study demonstrates that one-year treatment with sCT nasal spray is able to increase bone mass in osteoporotic patients without important local side-effects.

Effects of pravastatin treatment on vitamin D metabolites.

Hyperlipidemias, and notably hypercholesterolemia, represent important risk factors for atherosclerotic vascular disease. The enzymatic inhibitors of 3-hydroxy-3-methylglutaryl coenzyme A reductase, a selective and specific key enzyme involved in endogenous cholesterol synthesis, cause a significant mean reduction in low-density lipoprotein (LDL) cholesterol, both in familial and nonfamilial hypercholesterolemic forms. It has been hypothesized that these compounds might interfere with vitamin D endogenous synthesis secondarily to their effects on cholesterol. To verify this hypothesis, we studied 14 hypercholesterolemic patients treated as follows: 4 weeks of low-lipid, fiber-rich diet followed by 8 weeks of pravastatin treatment at the oral evening dose of 20 mg/d and by a 1-month washout period. No significant changes in serum calcium, 25-hydroxyvitamin D, and 1,25-dihydroxyvitamin D were noticed; on the contrary, significant (P < 0.01) reductions in total cholesterol and LDL cholesterol and a significant (P < 0.05) increase in high-density lipoprotein cholesterol were observed. After the final 1-month washout period, all values returned to baseline levels. In conclusion, our study confirms the clinical efficacy of pravastatin on lipid fractions and demonstrates the absence of any interference on the circulating levels of the main vitamin D metabolites.

Procollagen type I carboxy-terminal propeptide as a marker of osteoblastic bone metastases.

Collagen type I is the sole collagen type found in bones and tendons. Carboxyterminal propeptide, deriving and cleaved from procollagen type I (PICP) during collagen synthesis, is delivered into the blood, where it can be measured. According to current knowledge, PICP correlates with bone collagen synthesis and bone formation rate. Elevated serum levels of PICP in patients with Paget's disease, compared with normal subjects and correlated with serum alkaline phosphatase (Alk.Ph.), have been previously described. Thus, PICP may be a valuable marker of bone formation. PICP, serum Alk.Ph., serum bone Gla protein and 24-h urinary hydroxyproline:creatinine ratio have been measured in 47 cancer patients: 27 with predominantly osteolytic lesions (5 myeloma, 15 breast, 3 lung, 2 kidney, 1 bladder, 1 thyroid) and 20 with predominantly osteoblastic lesions (18 prostate and 2 breast). The higher levels of PICP were noted in patients with osteoblastic or mixed metastases. In the entire group of patients, a statistically significant correlation between PICP and bone Gla protein (r = 0.57; P < 0.001), PICP and Alk.Ph. (r = 0.80; P < 0.001), and bone Gla protein and Alk.Ph. (r = 0.44; P < 0.01) was noted. In those patients with osteoblastic metastases we observed a significant correlation only between PICP and Alk.Ph. (r = 0.62; P < 0.003). During chemotherapy, 13 of 20 patients with osteoblastic metastases who achieved objective response or stable disease showed a more rapid and significant decrease in PICP with respect to the other bone markers. Serum PICP level could be considered a good marker of osteoblastic activity.
