Strategic psychotherapy in psychiatric consultations.

Inpatient psychiatric consultation work often involves the need to quickly bring about behavioral changes in distressed patients who are not interested in participating in psychotherapy. The behavior of such patients often impinges upon other individuals in the general hospital milieu and can be viewed within a systems framework. The strategic psychotherapeutic approach is summarized and two cases are described that illustrate the benefits that such an approach can have in effective consultations. Reframing, pacing and leading, and confusion techniques are among the specific tactics that are discussed.

Neurohumoral control mechanisms in congestive heart failure.

Neurohumoral vasoconstrictor systems may play an important role in the hemodynamic derangement and natural history of congestive heart failure (CHF) by raising impedance to left ventricular ejection and shifting blood centrally to augment cardiac filling. Activation of the sympathetic nervous system, and renin-angiotensin system, and the antidiuretic hormone-vasopressin system can be demonstrated in clinical CHF by increased plasma levels of norepinephrine, renin activity, and arginine vasopressin. Because the magnitude of increase in each of these hormones varies widely from patient to patient, profiling of the neurohumoral response might provide new insight into the mechanisms of regulation of the circulation in CHF and into specific management with drugs to inhibit or reverse the vasoconstrictor process. Preliminary encouraging experience with converting-enzyme inhibitors to block formation of angiotensin II and alpha-receptor blockers to inhibit norepinephrine-induced vasoconstriction raise the possibility that selective therapy may eventually have a place in long-term management of CHF. Controlled trials in a larger patient population are now required.

Resection arthroplasty following infected total hip arthroplasty.

We evaluated twenty-one patients, one to eight years after removal of total hip-arthroplasty components because of infection. Sixteen of the patients had moderate to severe pain with either weight-bearing or sitting, and the other five had continuous moderate to severe pain. Twelve patients had an Iowa hip rating of less than 50 points, and the mean for the entire group was 49 points. All but three patients required a walker or crutches for walking. Only three patients were satisfied with the result of the resection arthroplasty. There was a suggestion that the patients with a smooth intertrochanteric line of resection of the proximal end of the femur had better results than those who did not. Patients whose wounds never healed had worse results than those whose wounds healed, but neither the initial treatment of the wound (open packing versus closed-tube irrigation) nor retained cement determined whether healing would occur.

The impact of midtrimester abortion techniques on patients and staff.

We studied 250 midtrimester abortions by dilatation and extraction (D and E) under general anesthesia and compared them with abortions by the intra-amniotic injection of prostaglandin (amnio) in order to assess the physical and emotional changes experienced by patients and staff under the different circumstances represented by each procedure. We found patients undergoing D and E abortions had fewer physical complications and described the procedure as minor surgery which went smoothly. The patients who had amnio abortions had more pain and reacted with more anger and depression afterward. Nurses were more disturbed by amnio abortions in which they played major roles in supporting the patient as well as in her abortion. Physicians reported the D and E procedures to be emotionally difficult which may limit the adoption of this procedure despite its value to patients.
