Association between pectus excavatum and segmental bronchomalacia.

Five children presented with recurrent respiratory illnesses and were noted to have pectus excavatum. In four patients subsequent investigation demonstrated a collapsible segment in the middle third of the left main bronchus, which appeared to lack cartilage support and caused gas trapping in the left lung. The remainder of the bronchial tree was normal. In the remaining child the clinical and physiologic pattern was very similar. In three children there were no other anomalities, but two had congenital heart disease. The association of bronchial anomalies and pectus excavatum may occur more often than suspected.

The role of nutritional status, airway obstruction, hypoxia, and abnormalities in serum lipid composition in limiting exercise tolerance in children with cystic fibrosis.

Previous work has shown that impaired exercise tolerance in children with cystic fibrosis (C.F.) is related to the severity of airway obstruction without elucidating the possible roles of hypoxia or malnutrition. It has been suggested that poor nutrition leads to abnormalities in serum fatty acids composition, which may lead to tissue hypoxia. We investigated the roles of hypoxia, pulmonary mechanics, nutritional status, and serum fatty acid composition in limiting exercise tolerance in C.F. In 20 children with C.F., exercise tolerance, while breathing air and while breathing oxygen, was evaluated on a cycle ergometer and compared to pulmonary function tests, anthropometric data, serum lipid compostition, and clinical condition. The mean percent work expected from height (Wmax) was 75, and was unchanged by O2. Wmax correlated significantly with the degree of respiratory impairement, the discrepancy between height and weight, and the clinical score but not serum fatty acid composition. Where measured, no child at any time had an elevated end tidal CO2(PetCO2). We conclude that nutritional status and airway obstruction are closely correlated with exercise tolerance in C.F. and that, unlike the case in adults with chronic obstructive pulmonary disease, exercise-limiting dyspnea occurs in the presence of a normal PetCO2.

Unilateral hyperlucent lung: the case for investigation.

Seventeen children with unilateral hyperlucent lungs were referred for investigation. Of the 11 who had a referring diagnosis of possible Macleod's syndrome only two were shown to have post-viral bronchiolitis. Three of the 11 had conditions that required surgical treatment and a further two with brochiectasis were treated medically. To avoid confusion we suggest that Macleod's syndrome is reserved exclusively for children with post-viral bronchiolitis. Radioisotopic regional lung function studies were useful in the investigation of the subjects from three points of view. Firstly, they distinguished children with primary perfusion abnormalities and normal ventilation, secondly, they defined the extent of altered respiratory function, and thirdly, they were able to distinguish compensatory emphysema from congenital lobar emphysema. As bronchography and bronchoscopy may be hazardous in small children with poor respiratory reserve, such regional studies may be useful in indicating which patients do not require further invasive investigation.

Human macromolecular insoluble cold globulin (MICG). I. T-cell origin of T-MICG and null cell origin of N-MICG.

Although surface immunoglobulin characterizes B cells in man, there are few surface markers that distinguish T cells. We have described a new protein synthesized in human T cells, termed T-MICG. This protein is a macromolecule of 225,000 daltons, is insoluble in the cold, and migrates as a beta-globulin on electrophoresis. Separation of human peripheral blood lymphocytes into T and B-cell populations by rosette sedimentation and anti-human-Fab columns clearly demonstrated the T-cell origin of the 225,000 dalton component. Furthermore, null cells were shown to synthesize a protein of 185,000 daltons, termed N-MICG, with physical properties similar to T-MICG, T-MICG and N-MICG were shown to be antigenically dissimilar, employing antiserum to each of these proteins. The present studies demonstrate two novel cell surface markers, T-MICG and N-MICG, which characterize T cells and null cells, respectively.

A clinical comparison of beclomethasone dipropionate delivered by pressurised aerosol and as a powder from a rotahaler.

14 children with severe asthma were studied for 2 months in a double-blind cross-over trial to compare the efficacy of beclomethasone dipropionate delivered as an aerosol and as a powder from a rotahaler. Inhalation via the aerosol was satisfactory in 13 patients, each of whom used the rotahaler correctly. Younger children preferred to use the rotahaler. Comparison of daily symptoms, twice daily peak expiratory flow rate (PEFR), and use of salbutamol during the 2 months of the trial showed that control of asthma was equally good, irrespective of the device used to deliver active beclomethasone.

New technique for servo-control of arterial oxygen tension in preterm infants.

Equipment has been developed for the servo-control of arterial oxygen tension in sick, newborn babies. Using an indwelling umbilical arterial oxygen electrode as sensor, the equipment successfully regulated the administration of oxygen to 12 newborn babies with respiratory distress syndrome, significantly improving the stability of arterial oxygen tension and lessening the duration of episodes of hypoxia and hyperoxia.

Status asthmaticus in children: a one-year study.

50 children were admitted on 72 occasions during one year in status asthmaticus, defined as wheezing not relieved by two doses of bronchodilator 4 hours apart. At least one-third of children were hypercapnoeic on admission. They were managed with either oral prednisolone and nebulised salbutamol. Those with peak expiratory flow rates of greater than 25% expected for height were satisfactorily managed on the oral regimen. One child needed assisted ventilation. 73% were fit for discharge within 4 days and more than half of them needed a change in maintanance treatment.

Macromolecular insoluble cold globulin (MICG): a novel protein from mouse lymphocytes. IV. Evidence for the plasma membrane distribution of MICG.

Macromolecular insoluble cold globulin is a glycoprotein synthesized predominantly by T lymphocytes in the mouse. The present report details experiments demonstrating the plasma membrane distribution of MICG on T lymphocytes. By utilizing immunofluorescent techniques it was shown that MICG was located in the external cell surface of 98% of thymic lymphocytes and 60% of splenic lymphocytes. Furthermore, in spleen cells, it was demonstrated that T cells and not B cells were surface MICG positive. Antibody to MICG was able to cap all of the immunofluorescent-positive (60%) spleen cells. In contrast, anti-MICG antibody did not induce cap formation on thymus cells. Only when dilute solutions of antibody were used did MICG cap on the thymus cells. Employing limited proteolysis of thymus and spleen cells MICG was shown to be regenerated on the surface of T cells with a half-life of 3.5 hr. The distribution and cell surface characteristics of MICG are discussed in terms of a "receptor-like" function for this protein.

A comparison of oral choline theophyllinate and beclomethasone in severe perennial asthma in children.

Fourteen children with perennial asthma previously treated with beclomethasone dipropionate were studied for four months in a crossover trial to compare the efficacy of choline theophyllinate and beclomethasone dipropionate aerosol in the control of their symptoms. Assessments were made using twice daily peak flow measurements, together with daily records of symptom scores and use of extra salbutamol. Beclomethasone and oral choline theophyllinate together improved control of asthma in comparison with beclomethasone alone. Reduction of the dose of beclomethasone from 400 microgram/day caused worsening of asthma. Oral choline theophyllinate did not prevent this deterioration and therefore cannot replace beclomethasone for the long-term treatment of such asthmatic patients.

Nasal resistance during infancy.

Nasal Resistance (Rn) was measured in 30 Caucasian and 13 Negro infants during the 1st year of life, using an adaptation of the posterior rhinomanometric method. Concurrent measurements of Thoracic Gas Volume (TGV) and Airway Resistance during nose breathing (Raw (n) were made using the plethysmographic technique. The percentage contribution of Rn to Raw (n) was significantly higher in the Caucasian infants (mean 49.2 +/- 7.5 (SD)%), than in the Negro infants (mean 31.1 +/- 6.8 (SD)%), which probably resulted from anatomical differences in nasal structure. In each infant, Rn was subtracted from Raw (n) in order to assess resistance, and its reciprocal, conductance (Gaw), during mouth breathing. A strong linear relationship was found to exist between Gaw (m) and TGV throughout the first year of life (r = 0.92), with no significant difference between Negro and Caucasian infants. Specific Airway Conductance during mouth breathing (SGaw (m) = Gaw (m)/TGV) was found to be considerably higher during infancy than at any other time during life, which may help to at least partially compensate for the fact that newborn infants are obligatory nose breathers.

A three- to five-year follow-up of the use of the aerosol steroid, beclomethasone dipropionate, in childhood asthma.

Nineteen asthmatic children treated with the aerosol steroid, beclomethasone dipropionate, were followed 3 to 5 yr. Good control was maintained in all but one child throughout, although 73% have needed 1 or 2 wk of supplementary oral steroids per year for exacerbations. Growth has been along the percentile on which the child entered the study. No serious side effects have been encountered among 41 children treated between 1 and 5 yr with beclomethasone dipropionate. Seventeen percent needed prolonged alternate-day oral steroids, although all but one child did eventually return to good control with beclomethasone dipropionate.

Serial lung function studies in cystic fibrosis in the first 5 years of life.

Lung mechanics were studied in 8 infants with cystic fibrosis at 6 months of life and radiosotopic lung function was measured in 5 of them at 5 years of age. The children who were initially asymptomatic had normal lung mechanics in infancy but the 2 restudied later had abnormal radioisotopic lung function. The symptomatic children showed abnormalities in infancy and more marked changes later. It is concluded that the lungs in cystic fibrosis are probably normal initially and that damage occurs later even in the absence of symptoms.

Heart failure apparently due to overfeeding in a neonate.

Severe heart failure in a one-month-old infant is described, apparently due to the administration of cows' milk in the region of 300 ml/kg per day. There was also pronounced hypocalcaemia. The infant responded rapidly to treatment with no evidence of intrinsic heart disease.
