Low versus high pressure for in vitro determination of hydraulic conductance of human dentine.

The purpose of this study was to evaluate the effect of applied pressure and measurement time on the in vitro measurement of hydraulic conductance of human dentine. Dentine slices were prepared from 50 third molars. Water was forced through the slices under a constant hydrostatic pressure. Five pressures were tested: 1.3 kPa (n = 10), 13.3 kPa (n = 10), 26.6 kPa (n = 10), 40 kPa (n = 10) and 53.3 kPa (n = 10). The volume that went through the slices was recorded every 10 min for 3 h. The volume, the fluid flow rate and the hydraulic conductance under the five pressures were compared. The volume increased with time and pressure. The fluid flow and hydraulic conductance decreased with time under 13.3, 26.6 and 40 kPa, but remained constant under 1.3 and 53.3 kPa. Used of a low pressure (1.3 kPa) may permit water to pass through dentinal tubules without disturbing intratubular contents. Medium pressures (13.3, 26.6, 40 kPa) seemed to disturb tubule contents progressively, resulting in decreased fluid flow and therefore a decreased hydraulic conductance with time. Under these pressures, the calculated values of hydraulic conductance may be unreliable because they are time-dependent. High pressure (53.3 kPa) seemed to pack the tubule contents against intratubular resistances immediately, resulting in low fluid flows and low hydraulic conductances.

[Heparin-induced thrombocytopenia: review of the literature and description of 2 clinical cases].

We present a literary review regarding heparin-induced thrombocytopenia and we report two cases. The first case regards a female patient with a suspect pulmonary embolism and deep vein thrombosis. Low platelet count was observed already during sub-cutaneous heparin therapy but was aggravated by subsequent intra-venous treatment. The second patient developed thrombocytopenia only after intra-venous heparin infusion while previous sub-cutaneous heparin prophylaxis had not caused low platelet count. In both patients platelet counts normalized rapidly only a few hours after withdrawal of heparin treatment. We therefore point out the importance of the correct monitoring of platelet counts for a timely detection and prevention of heparin-induced thrombocytopenia, and possible consequent paradoxical thrombosis.

[Arrhythmogenic right ventricular myocardiopathy. A study of a family group].

In this paper, we report the cases of 4 male patients (mean age 32.7 yr) with right-ventricular dysplasia, that occurred in familial form. Diagnosis was possible according to the clinical features and to the electrocardiographic, echocardiographic and scintigraphic criteria; at Holter monitoring recorded ventricular arrhythmias with left bundle branch block configuration, associated to sinus arrhythmias (sino-auricular block). In two patients a sustained ventricular tachycardia was recorded. The patients are living and a fourteen-year-old subject is without heart disease. From our data we conclude that: the most serious clinical features occur in extensive form with right atrial and left-ventricle involvement; the clinical features were manifested at a young age and then got worse. A dominant autosomic inheritance is observed (only males were affected).

Hyperplastic gingivitis during diltiazem therapy.

This report concerns a patient with ischemic heart disease, who developed hyperplastic gingivitis with verapamil. The gingivitis resolved completely when the drug was discontinued, but recurred when another calcium antagonist, diltiazem, was administered. It is suggested that it is the mechanism of action of calcium antagonists which is responsible for the gingival changes.

[Hypertrophic gingivitis caused by verapamil].

We observed gingival hyperplasia, similar clinically to the hyperplasia caused by diphenylhydantoin during the last 11 months in two patients treated by verapamil, suffering from angina pectoris pains. In Case I discontinuation of the calcium antagonist resulted in marked regression of the gingival hyperplasia. Evidence points to a strong relationship between the gingival hyperplasia and the administration of verapamil. Further studies are required to confirm definitively the relationship between the administration of verapamil and gingival hyperplasia, its incidence, and the correlation with local factors.

[Systolic anterior motion of the mitral valve in mitral valve prolapse: report of a case and etiopathogenetic considerations (author's transl)].

The Authors describe a case of a patient suffering from mitral prolapse whose echocardiogram shows in addition to typical findings of this syndrome an annual systolic anterior motion (SAM) of the anterior mitral leaflet, generally present in the idiopathic hypertrophic obstructive cardiomyopathy and more seldom in some others disease, however not present in this present. The Authors suggest that this finding is correlated to the mitral prolapse and express some hypothesis regarding this possibility.

[Mitral valve prolapse in patients with coronary artery insufficiency. Report of two unusual cases (author's transl)].

The authors illustrate two peculiar cases of mitral valve prolapse in patients with coronary artery disease. The first case reports a mitral valve prolapse that, absent in basal conditions, is unmasked by a premature ventricular beat. In the second case concerns about the manifestation of a mitral valve prolapse developed during the course of retrosternal chest pain.
