Late metastases (beyond ten years) of cutaneous malignant melanoma. Literature review and case report.

A case of late metastasis of a primary malignant melanoma of the sole of the foot after a disease-free interval of 13 years in a premenopausal woman is presented. The clinicopathologic findings are described, and the literature concerning late metastases (beyond 10 years) of melanoma is reviewed. Only seventeen documented cases have been previously reported, to which we add our own case. Sixteen of these eighteen cases concerned premenopausal women. This phenomenon suggests that female gonadal hormones may exert an inhibitory effect on cutaneous melanoma, hence lengthening the disease-free interval.

Primary malignant melanoma of the vermilion of the lower lip.

A 30-year-old man with primary nodular, rapidly growing malignant melanoma of the vermilion of the lower lip is described. The tumor was excised with wide tumor-free margins and an Abbe-Estlander reconstruction was carried out. The literature on oral melanomas has also been reviewed with special attention given to melanoma of the lip which is extremely rare (31 reported cases). This case is highly unusual in two respects: the melanoma was a nodular type, level 5, while nearly all melanomas reported previously have been superficial with slow radial growth; it was a small lesion arising on the vermilion and not an extension from the cutaneous junction or oral mucosa.

Immunodiagnostic test for the early detection of carcinoma of the breast.

A cytotoxic assay for the identification of circulating lymphocytes bearing surface ferritin was performed on 100 patients with malignant disease of the breast and in 50 healthy women. Forty-five patients were found to have a subpopulation of ferritin bearing lymphocytes, and all of them proved to have Stages I and II carcinoma of the breast. No such lymphocytes were identified in normal women, and in the remaining 55 patients, 33 had benign disease of the breast, 20 had Stage III carcinoma of the breast and two had Stages I and II carc-noma of the breast. It appears that this immunodiagnostic test may provide an additional tool for the early detection of carcinoma of the breast and may have a prognostic implication.

Use of tolonium chloride in the diagnosis of malignant gastric ulcers.

Peroral staining with tolonium chloride (toluidine blue) was performed in 45 patients with suspected gastric ulcer disease. During endoscopy, 19 of 21 malignant ulcers and one of 15 benign ulcers were stained. Following surgery, 18 of 21 malignant ulcers found in the surgical specimens were stained. Eleven patients with benign ulcers underwent surgery and none of these ulcers were found to be stained in the surgical specimens. Normal gastric mucosa and areas of gastritis appeared unchanged. The data suggest that tolonium chloride staining prior to endoscopy or surgery seems to be helful in differentiating between minute benign and malignant gastric ulcers.

Urine xanthine oxidase activity in urinary tract infection.

Xanthine oxidase (XO) activity was found to be negligible in sterile human urines (less than 480 units, as presently defined, per litre). Significant XO activity was found in all urines containing more than 10(5) bacteria/ml, except for urines infected with Staphylococcus aureus, in which XO activity ranged from 347 to 714 units per litre. Plasma XO is not transferred to the urine, as demonstrated by the negligible XO activity found in sterile urines from patients with raised plasma XO activity. Determination of urinary XO activity is a suitable procedure for the detection of urinary tract infection.
