Supercritical fluids are superior media for catalysis by cross-linked enzyme microcrystals of subtilisin Carlsberg.

We report on the performance of cross-linked enzyme microcrystals (CLECs) of subtilisin Carlsberg in supercritical fluids (SC-fluids). The catalytic activity of CLECs in SC-ethane was found to be 2- to 10-fold greater than in hexane under the same conditions, using CLECs dried by propanol washing. Air-dried CLECs and lyophilized powders showed much lower activities, reflecting the same hydration hysteresis effects as in organic solvents. Reaction rates were much lower in SC-CO(2), especially at higher water activity, probably as a result of acid-base effects of carbonic acid on the enzyme.

Ethical and legal issues associated with substance abuse by pregnant and parenting women.

The many pressing medical, psychological, legal, and ethical problems associated with substance abuse by pregnant and parenting women present daunting challenges. One approach is to adopt a legal perspective where the rights of mothers, fetuses, and children are pitted against one another in courtrooms and state legislatures. Another approach is to embrace a public health perspective, characterized by treating pregnant and postpartum substance abusers, protecting at-risk fetuses and children, and strengthening broken families. Both ethical concepts and legal tenets must be considered to develop new paradigms designed to help at-risk mothers and children and to protect the rights claimed by all involved.

Preoperative and intraoperative assessment of myometrial invasion and histologic grade in endometrial cancer: role of magnetic resonance imaging and frozen section.

The objective of this study was to determine the value of myometrial invasion by magnetic resonance imaging (MRI), histologic typing and grading by endometrial biopsy, and the intraoperative evaluation of both parameters by frozen section in the evaluation of endometrial cancer. The preoperative and intraoperative records of 180 patients with endometrial cancer were used to compare the preoperative endometrial biopsy, the myometrial invasion by MRI, and the intraoperative frozen sections, with the final histopathologic findings. The preoperative endometrial biopsy gave us the tumor histologic type and grade. MRI gave us the depth of myometrial invasion. The evaluation of intraoperative frozen sections gave us the tumor histologic type, the tumor grade, and also the myometrial invasion. Patients were classified as low risk (grade 1 and 2, and myometrial invasion <50%) and high risk (grade 3 or myometrial invasion >50%). Standard statistical calculations were used. Evaluation of the tumor grade by preoperative biopsy has a sensitivity and a specificity of 75% and 95%, respectively. Evaluation of the tumor grade by intraoperative biopsy has a sensitivity and a specificity of 40% and 98%, respectively. Evaluation of the depth of myometrial invasion with MRI has a sensitivity and a specificity of 79% and 82%, respectively. Evaluation of the depth of myometrial invasion with intraoperative frozen sections has a sensitivity and a specificity of 74% and 95%, respectively. Evaluation of all four of the parameters together has a sensitivity and a specificity of 80% and 82%, respectively with a kappa of 0.621. In our opinion, the combination of preoperative biopsy and intraoperative frozen section is the best way to decide whether a lymphadenectomy is necessary with a low rate of understaging patients. MRI would have a fringe benefit in these patients.

Human cutaneous leishmaniasis caused by Leishmania (Viannia) braziliensis in Santiago del Estero, Argentina: identification of parasites by monoclonal antibodies and isoenzymes.

Diagnostic and parasite characterization and identification studies were carried out in human patients with cutaneous leishmaniasis lesions in Santiago del Estero, Northern Province of Argentina. Diagnostic procedures were biopsies of lesions for smears and inoculations in hamster, needle aspirations of material from ulcers for "in vitro" cultures. Immunodiagnostic techniques applied were IFAT-IgG and Montenegro skin test. Primary isolation of eight stocks of leishmanial parasites was achieved from patients with active lesions. All stocks were biologically characterized by their behaviour in hamster, measurements of amastigote and promastigotes and growth "in vitro". Eight stocks were characterized and identified at species level by their reactivity to a cross-panel of sub-genus and species-specific Monoclonal Antibodies through an Indirect Immunofluorescence technique and a Dot-ELISA. We conclude from the serodeme analysis of Argentina stocks that: stocks MHOM/AR/92/SE-1; SE-2; SE-4; SE-8; SE-8-I; SE-30; SE-34 and SE-36 are Leishmania (Viannia) braziliensis. Three Leishmania stocks (SE-1; SE-2 and SE-30) did not react with one highly species-specific Monoclonal Antibody (Clone: B-18, Leishmania-(Viannia) braziliensis marker) disclosing two serodeme group patterns. Five out of eight soluble extracts of leishmanial promastigotes were electrophoresed on thin-layer starch gels and examined for the enzyme MPI, Mannose Phosphate Isomerase; MDH, Malate Dehydrogenase; 6PGD, 6 Phosphogluconate Dehydrogenase; NH, Nucleoside Hydrolase, 2-deoxyinosine as substrate; SOD, Superoxide Dismutase; GPI, Glucose Phosphate Isomerase and ES, Esterase. From the isoenzyme studies we concluded that stocks: MHOM/AR/92/SE-1; SE-2; SE-4; SE-8 and SE-8-I are isoenzymatically Leishmania (Viannia) braziliensis. We need to analyze more enzymes before assigning them to a braziliensis zymodeme.

Expression of proliferating cell nuclear antigen in follicular thyroid tumors: correlation with clinicopathological findings.

The purpose of this study was to correlate proliferating cell nuclear antigen (PCNA) immunostaining with clinicopathological findings in 60 follicular thyroid tumors. Mean proliferative index (PI) was significantly higher for carcinoma (73%) than microfollicular adenoma (19.6%) (p<0.0001). For atypical and oncocytic adenomas, intermediate PI values (64% and 50%) correlated with size and cytological atypia but not with sex and age. PCNA immunostaining is of limited value for differential diagnosis of malignant and atypical tumors but could be useful for identification of actively growing atypical follicular thyroid nodules on fine needle aspiration biopsies.

[Indirect ultramicroElisa for the detection of total antibodies against cytomegaloviruses in human blood].

We have standardized an indirect ultramicro ELISA assay for detecting antibodies to human Cytomegalovirus (CMV) using human serum samples (UMELISA CMV). The optimal concentration of coating antigen (30 ug/ml), serum dilution (1:40) and anti-human conjugate working dilution (1:1500), were determined by a check board titration method. The UMELISA CMV was compared with the latex agglutination test for antibodies to CMV (Dupont de Nemours) and with an indirect immunofluorescent method. The results have showed the high coincidence, sensitivity and especificity of the proposed assay regarding the two methods compared with, and supporting its use either for a blood donors screening or in the serological diagnosis of this infection by paired serum samples.

[Detection of cytomegalovirus by the immunoperoxidase technique and viral isolation].

An Immunoperoxidase assay was applied to detect early antigens of cytomegalovirus (CMV) in 150 urine samples from immunocompromised patients, using the commercial available monoclonal antibody against CMV E13. The detection of early antigen by IP (IPCMV) is compared to the conventional cell culture isolation regarding specificity and sensitivity in order to evaluate is usefulness in the diagnostic of CMV infections. The IPCMV showed a sensitivity of 89.8% and a specificity of 91.3% when compared to the isolation method. The great advantage of the IPCMV is based on the shorter time results are achieved, since 48-72 Hs can be enough to provide evidence of CMV infection, while in the isolation technique cytopathogenic effect was present around 14 days after sample inoculation.

In vitro assay for protein digestibility: interlaboratory study.

True protein digestibilities of 17 protein sources were estimated by 6 laboratories using an in vitro, 3-enzyme digestion system in a pH stat. Samples from animal, vegetable, and mixed food sources were freeze-dried (if not already dried), ground, mixed, and shipped to each collaborator along with a sodium caseinate standard and trypsin, chymotrypsin, and peptidase. The uptake of titrant during enzymatic digestion was used to calculate estimates of digestibility. Digestibilities ranged from 100% for casein to 89.9% for whole wheat cereal. Mean relative standard deviations for repeatability were 1.4% for rolled oats and less than 1% for the remaining 16 samples. Mean relative standard deviations for reproducibility ranged from 5.0 to 0.8%; values were less than 2.5% for 13 of the 17 samples.

[Clinical value of a new method of therapeutic control of heparin].

A modification of thrombosis time (TT) for therapeutic control of heparin (TTDT), is described. A useful dose of heparin increases plasma TT, which on dilution with fresh plasma rich in platelets (250 +/- 50 x 10(9)/1) at a dilution of 1:4 to 1:8 (+/-20%), causes the increased plasma TT to return to normal values. Three coagulation tests used simultaneously were tried: plasma recalcification time, partially activated thromboplastin time, and TT, used simultaneously in 30 samples of platelet rich fresh normal plasma with different concentrations of heparin (0.05 to 1.0 mu/ml). Results show that modified TT is the most sensitive and reproducible test to detect varying heparin dosage. Fifty two patients without coagulation deficiencies and with various clinical thrombi, and 22 with generalized intravascular coagulation (CID) treatment with 100mu of heparin XK of body weight every 4 to 6 hours, were studied. Results are described. Advantages of TRDT for therapeutic control of heparin are discussed.
