Crystal structures and magnetic properties of new cyano-bridged two-dimensional grid-like bimetallic assemblies [Ni(tn)2]2[Cr(CN)5((NO)]OH*H2O and [NI(tn)2]2[Co(CN)6]NO3*2H2O (tn=1,3-propanediamine).

Two bimetallic assemblies, [Ni(tn)(2)](2)[Cr(CN)(5)(NO)]OH.H(2)O (1) and [Ni(tn)(2)](2)[Co(CN)(6)]NO(3).2H(2)O (2) (tn = 1,3-diaminopropane), have been prepared and structurally and magnetically characterized. Crystal data for 1 (2): space group P1 (P1), a = 8.698(3) (8.937(2)) A, b = 10.001(2) (9.863(1)) A, c = 10.158(2) (10.064(1)) A, alpha = 87.40(2) (86.064(10)) degrees, beta = 65.10(2) (65.489(10)) degrees, gamma = 81.63(2) (81.572(12)) degrees and Z = 1 (1). Both structures consist of two-dimensional grid-like polycations containing Ni-N triple bond C-M linkages (M = Cr or Co) and counteranions (OH, NO(3)). Magnetic studies of 1 showed that the complex displays a metamagnetic behavior originating from intralayer ferromagnetic and interlayer antiferromagnetic interactions. Long-range antiferromagnetic ordering was observed at T(N) = 3.3 K. Complex 2 exhibits intramolecular ferromagnetic interactions through the diamagnetic N triple bond C-Co-N triple bond C bridges, owing to superexchange involving the empty d(sigma) orbital of the diamagnetic Co(III) ion.

Methods for testing equality of means of health care costs in a paired design study.

In this paper we propose five new tests for the equality of paired means of health care costs. The first two tests are the parametric tests, a Z-score test and a likelihood ratio test, both derived under the bivariate normality assumption for the log-transformed costs. The third test (Z-score with jack-knife) is a semi-parametric Z-score method, which only requires marginal log-normal assumptions. The last two tests are the non-parametric bootstrap tests: one is based on a t-test statistic, and the other is based on Johnson's modified t-test statistic. We conduct a simulation study to compare the performance of these tests, along with some commonly used tests when the sample size is small to moderate. The simulation results demonstrate that the commonly used paired t-test on the log-scale and the Wilcoxon signed rank for differences of the two original scales can yield type I error rates larger than the preset nominal levels. The commonly used paired t-test on the original data performs well with slightly skewed data, but can yield inaccurate results when two populations have different skewness. The likelihood ratio test, the parametric and semi-parametric Z-score tests all have very good type I error control with the likelihood ratio test being the best. However, the semi-parametric Z-score test requires less distributional assumptions than the two parametric tests. The percentile-t bootstrap test and bootstrapped Johnson's modified t-test have better type I error control than the paired t-test on the original-scale and Johnson's modified t-test, respectively. Combining with the propensity-score method, we can also apply the proposed methods to test the mean equality of two cost outcomes in the presence of confounders. Our two applications are from health services research. In the first one, we want to know the effect of Medicaid reimbursement policy change on outpatient health care costs. The second one is to evaluate the effect of a hospitalist programme on health care costs in an observational study, and the imbalanced covariates between intervention and control patients are taken into account using a propensity score approach.

Dissipative quantum dynamics in 2D: anisotropic dissipation and selective bond breaking in surface photochemistry.

The dissipative quantum dynamics of a model system, O2 at a Pt(111) surface, has been solved in two dimensions using a stochastic wave packet approach and parallel-computing techniques. It is found that, upon excitation, the dissipation anisotropy creates nonequilibrium and anisotropic energy storage between different reaction channels. The latter determines decisively the short-time reaction dynamics and, in particular, the branching ratio between desorption and dissociation, in agreement with recent experimental findings.

Tyrosine sulfation of glycoprotein I(b)alpha. Role of electrostatic interactions in von Willebrand factor binding.

Glycoprotein I(b)alpha (GP I(b)alpha), the ligand binding subunit of the platelet glycoprotein Ib-IX-V complex, is sulfated on three tyrosine residues (Tyr-276, Tyr-278, and Tyr-279). This posttranslational modification is known to be critical for von Willebrand factor (vWF) binding; yet it remains unclear whether it provides a specific structure or merely contributes negative charges. To investigate this issue, we constructed cell lines expressing GP I(b)alpha polypeptides with the three tyrosine residues converted to either Glu or Phe and studied the ability of these mutants to bind vWF in the presence of modulators or shear stress. The mutants were expressed normally on the cell surface as GP Ib-IX complexes, with the conformation of the ligand-binding domain preserved, as judged by the binding of conformation-sensitive monoclonal antibodies. In contrast to their normal expression, both mutants were functionally abnormal. Cells expressing the Phe mutant failed to bind vWF in the presence of either ristocetin or botrocetin. These cells adhered to and rolled on immobilized vWF only when their surface receptor density was increased to twice the level that supported adhesion of cells expressing the wild-type receptor and even then only 20% as many rolled and rolled significantly faster than wild-type cells. Cells expressing the Glu mutant, on the other hand, were normal with respect to ristocetin-induced vWF binding and adhesion to immobilized vWF but were markedly defective in botrocetin-induced vWF binding. These results indicate that GP I(b)alpha tyrosine sulfation influences the interaction of this polypeptide with vWF primarily by contributing negative charges under physiological conditions and when the interaction is induced by ristocetin but contributes a specific structure to the botrocetin-induced interaction.

Incidence of dementia and Alzheimer disease in 2 communities: Yoruba residing in Ibadan, Nigeria, and African Americans residing in Indianapolis, Indiana.

CONTEXT: Alzheimer disease (AD) represents a major and increasing public health problem. If populations were identified with significantly lower or higher incidence rates of AD, the search for risk factors in the genesis of AD could be greatly enhanced. OBJECTIVE: To compare incidence rates of dementia and AD in 2 diverse, elderly community-dwelling populations. DESIGN: The Indianapolis-Ibadan Dementia Project, a longitudinal, prospective population-based study consisting of a baseline survey (1992-1993) and 2 subsequent follow-up waves after 2 years (1994-1995) and 5 years (1997-1998). Each wave followed a 2-stage design, with an in-home screening interview followed by a full diagnostic workup of a subsample of participants based on screening performance. SETTING AND PARTICIPANTS: A total of 2459 community-dwelling Yoruba residents of Ibadan, Nigeria, without dementia, and 2147 community-dwelling African American residents of Indianapolis, Ind, without dementia (all aged 65 years or older). The cohorts were followed up for a mean of 5.1 years and 4.7 years, respectively. MAIN OUTCOME MEASURES: Incident cases of dementia and AD in each of the 2 populations. RESULTS: The age-standardized annual incidence rates were significantly lower among Yoruba than among African Americans for dementia (Yoruba, 1.35% [95% confidence interval [CI], 1.13%-1.56%]; African Americans, 3.24% [95% CI, 2.11%-4.38%]) and for AD (Yoruba, 1.15% [95% CI, 0.96%-1.35%]; African Americans, 2.52% [95% CI, 1.40%-3.64%]). CONCLUSION: This is the first report of incidence rate differences for dementia and AD in studies of 2 populations from nonindustrialized and industrialized countries using identical methods and the same group of investigators in both sites. Further explorations of these population differences may identify potentially modifiable environmental or genetic factors to account for site differences in dementia and AD.

Development of the fragment constant method for estimating the partition coefficients of nonionic organic mixtures.

The fragment constant model for estimating partition coefficient of nonionic organic mixtures is developed based on group contribution theory. For 60 mixtures randomly composed of eight substituted benzenes, the C(18)-Empore disks/water partition coefficients (K(MD)) are determined. Establishment of a model between these K(MD) and the mole fraction of the fragments (Cl, Br, I, NO(2), CH(3)) acquires the fragments constants for K(MD). The significant linear regression equation, between these fragment constants and that of Hansch, provides an approach for calculating K(MD) of untested nonionic organic mixtures by using the fragment constants of Hansch.

Validation of germination rate and root elongation as indicator to assess phytotoxicity with Cucumis sativus.

Germination rate and root elongation, as a rapid phytotoxicity test method, possess several advantages, such as sensitivity, simplicity, low cost and suitability for unstable chemicals or samples. These advantages made them suitable for developing a large-scale phytotoxicity database and especially applicable for developing quantitative structure-activity relationship (QSAR) to study mechanisms of phytotoxicity. In this paper, the comparative inhibition of germination rate and root elongation of Cucumis sativus by selected halogen-substituted phenols and anilines were determined. The suitability of germination rate and root elongation as phytotoxicity endpoints was evaluated. Excellent reproducibility and stability of germination rate and root elongation in the control test, relatively greater sensitivity and similar dose-response relations for all tested compounds were observed. These results together with those of a 2-day test were used to demonstrate the suitability of this phytotoxicity test method. A QSAR was developed for the phytotoxicity mode of action of the tested compounds to C sativus seeds. Models that combined the logarithm of 1-octanol/water partition coefficient (log Kow) and the energy of the lowest unoccupied molecular orbital (Elumo) were developed for both germination rate inhibition and root elongation inhibition. The results of these studies indicate that phytotoxicity of substituted phenols and anilines to C. sativus seeds could be explained by a polar narcosis mechanism. This paper will promote the application of germination rate and root elongation method and the development of large-scale phytotoxicity database, which will provide the fundamental data for QSAR and ecological risk assessment of organic pollutants.

A new nerve guide conduit material composed of a biodegradable poly(phosphoester).

There is a resurgence of interest in the development of degradable and biocompatible polymers for fabrication of nerve guide conduits (NGCs) in recent years. Poly(phosphoester) (PPE) polymers are among the attractive candidates in this context, in view of their high biocompatibility, adjustable biodegradability, flexibility in coupling fragile biomolecules under physiological conditions and a wide variety of physicochemical properties. The feasibility of using a biodegradable PPE, P(BHET-EOP/TC), as a novel NGC material was investigated. Two types of conduits were fabricated by using two batches of P(BHET-EOP/TC) with different weight-average molecular weights (Mw) and polydispersity indexes (PI). The polymers as well as conduits were non-toxic to all six types of cells tested, including primary neurones and neuronally differentiated PC12 cells. After in situ implantation in the sciatic nerve of the rat, two types of conduits triggered a similar tissue response, inducing the formation of a thin tissue capsule composed of approximately eight layers of fibroblasts surrounding the conduits at 3 months. Biological performances of the conduits were examined in the rat sciatic nerve model with a 10 mm gap. Although tube fragmentation, even tube breakage, was observed within less than 5 days post-implantation, successful regeneration through the gap occurred in both types of conduits, with four out of 10 in the Type I conduits (Mw 14,900 and PI 2.57) and 11 out of 12 in the Type II conduits (Mw 18,900 and PI 1.72). The degradation of conduits was further evidenced by increased roughness on the tube surface in vivo under scanning electron microscope and a mass decrease in a time-dependent manner in vitro. The Mw of the polymers dropped 33 and 24% in the Type I and II conduits, respectively, in vitro within 3 months. Among their advantages over other biodegradable NGCs, the PPE conduits showed negligible swelling and no crystallisation after implantation. Thus, these PPE conduits can be effective aids for nerve regeneration with potential to be further developed into more sophisticated NGCs that have better control of the conduit micro-environment for improved nerve regeneration.

DSPP mutation in dentinogenesis imperfecta Shields type II.

We identified a nonsense mutation (Gln45stop) in exon 3 of the dentin sialophosphoprotein (DSPP) gene in a Chinese family with dentinogenesis imperfecta Shields type II (DGI-II), in which the affected members showed discoloration and severe attrition of their teeth, with obliterated pulp chambers.

A pilot study of microembolic signals in patients with middle cerebral artery stenosis.

OBJECTIVE: There has been limited data on the frequency of microembolic signals in patients with middle cerebral artery (MCA) stenosis, especially during the acute phase of stroke. Using transcranial Doppler, the authors prospectively monitored the MCA segments distal to stenosis in 4 groups of patients for 30 minutes: (1) symptomatic patients with acute ischemic stroke and MCA stenosis, (2) asymptomatic group patients with asymptomatic MCA stenosis, (3) control patients with acute ischemic stroke of undetermined etiology, and (4) normal people. A total of 60 patients completed the study. There were no microembolic signals in the asymptomatic, control, and normal groups. Among 20 patients in the symptomatic group, microembolic signals were detected in 3 patients (15%). The number of emboli ranged from 1 to 6 per 30 minutes. This is the first report of the presence of microembolic signals in acute stroke patients with MCA stenosis.

Senescence represses the nuclear localization of the serum response factor and differentiation regulates its nuclear localization with lineage specificity.

The differentiation of cultured 3T3T mesenchymal stem cells into adipocytes represses growth factor responsiveness by limiting the nuclear localization of the serum response factor (SRF) that binds to and activates the promoters of growth control genes that contain the serum response elements (SRE), such as junB and c-fos. The regulation of SRF nuclear localization by adipocyte differentiation is specific, because we show that adipocyte differentiation does not repress the nuclear localization of six other transacting factors. To determine if repression of growth factor responsiveness that occurs during senescence also represses the nuclear localization of SRF, we studied normal human WI-38 fibroblasts at low versus high population doublings. The results show that SRF localizes to the nucleus of proliferative cells whereas in senescent cells SRF can not be detected in the nucleus. This result is apparent in both immunofluorescence assays and in western blot analysis. We next evaluated the cellular distribution of SRF in selected human tissues to determine whether the loss of proliferative potential in vivo could have a different effect on SRF nuclear localization. We found that in cells of the small bowel mucosa, differentiation modulates SRF nuclear localization in an opposite manner. Minimal SRF expression and nuclear localization is evident in undifferentiated cells at the base of crypts whereas increased SRF expression and nuclear localization is evident in differentiated cells at the surface tip of the villus. These results together establish that regulation of SRF expression and nuclear localization is important in senescence and differentiation in a lineage specific manner.

Effect of long-term exposure to fluoride in drinking water on risks of bone fractures.

Findings on the risk of bone fractures associated with long-term fluoride exposure from drinking water have been contradictory. The purpose of this study was to determine the prevalence of bone fracture, including hip fracture, in six Chinese populations with water fluoride concentrations ranging from 0.25 to 7.97 parts per million (ppm). A total of 8266 male and female subjects > or =50 years of age were enrolled. Parameters evaluated included fluoride exposure, prevalence of bone fractures, demographics, medical history, physical activity, cigarette smoking, and alcohol consumption. The results confirmed that drinking water was the only major source of fluoride exposure in the study populations. A U-shaped pattern was detected for the relationship between the prevalence of bone fracture and water fluoride level. The prevalence of overall bone fracture was lowest in the population of 1.00-1.06 ppm fluoride in drinking water, which was significantly lower (p < 0.05) than that of the groups exposed to water fluoride levels > or =4.32 and < or =0.34 ppm. The prevalence of hip fractures was highest in the group with the highest water fluoride (4.32-7.97 ppm). The value is significantly higher than the population with 1.00-1.06 ppm water fluoride, which had the lowest prevalence rate. It is concluded that long-term fluoride exposure from drinking water containing > or =4.32 ppm increases the risk of overall fractures as well as hip fractures. Water fluoride levels at 1.00-1.06 ppm decrease the risk of overall fractures relative to negligible fluoride in water; however, there does not appear to be similar protective benefits for the risk of hip fractures.

[The standard for electronic data exchange in healthcare environments--HL7].

With the development of HIS/RIS(Hospital/Radiology Information System), it is very important to study the HL7(Health Level Seven) Standard for electronic data exchange in healthcare environments. Firstly, in this paper, the history, development goal and conceptions of HL7 are introduced. Secondly, its applications and auto-realization technologies are presented. Then, the problems which exist in the standard and the difficulties of HL7 in China are analyzed. Finally, some helpful suggestions about the development of HL7 in China are proposed.

Cerebrovascular disease among Chinese populations--recent epidemiological and neuroimaging studies.

Stroke mortality in individuals of Chinese descent has declined during the past decades, although the absolute number of patients with first-ever stroke is escalating. This review summarises recent epidemiological and imaging studies conducted among Chinese populations. Data indicating differences between Chinese and Caucasian ethnic groups in the pathophysiological mechanisms of stroke are highlighted, and the potential implications of these findings for prevention and management of stroke are discussed.

X-ray crystallographic and (13)C NMR investigations of the effects of electron-withdrawing groups on a series of pyrroles

A series of pyrroles substituted with various electron-withdrawing groups (EWGs) on the N atom have been synthesized and full characterization including X-ray crystal structures obtained. Analysis of (13)C chemical shifts and X-ray crystal structures reveals that a trend between decreased aromaticity and the strength of the EWG exists. Experimental results regarding alternative mechanisms of nucleophilic substitution reactions can thus be rationalized.
